Utility of a dedicated pediatric cardiac anticoagulation program: the Boston Children's Hospital experience.

Congenital heart disease is the leading cause of stroke in children. Warfarin therapy can be difficult to manage safely in this population because of its narrow therapeutic index, multiple drug and dietary interactions, small patient size, high-risk cardiac indications, and lack of data to support anticoagulation recommendations. We sought to describe our institution's effort to develop a dedicated cardiac anticoagulation service to address the special needs of this population and to review the literature. In 2009, in response to Joint Commission National Patient Safety Goals for Anticoagulati

Percutaneous Microwave Ablation of Hepatocellular Carcinoma As Bridge-to-Transplant Therapy in a High-Risk Patient With Alpha-1 Antitrypsin Deficiency and Factor V Leiden Mutation: A Case Report.

The use of percutaneous microwave ablation has been established as a treatment for early-stage hepatocellular carcinoma (HCC). HCC can often arise in the setting of cirrhosis and often requires locoregional therapy to ablate the area as a bridge for the ultimate treatment of liver transplantation. The challenge is its use in patients with decompensated cirrhosis and hypercoagulable states, as these high-risk populations can lead to other major complications, requiring anticoagulation, which remains challenging. In this case report, a 60-year-old woman with a history of alpha-1 antitrypsin defi

Evaluating the benefits of home-based management of atrial fibrillation: current perspectives.

Atrial fibrillation (AF) is the most common arrhythmia worldwide, leading to an extensive public health and economic burden. The increasing incidence and prevalence of AF is due to the advancing age of the population, structural heart disease, hypertension, diabetes, and thyroid disease. The majority of costs associated with AF have been attributed to the cost of hospitalization. In order to minimize costs and decrease hospitalizations, counseling on modifiable risk factors contributing to AF has been strongly emphasized. With the release of novel oral anticoagulants bypassing the need for ant

Anticoagulant serine fibrinogenases from Vipera lebetina venom: structure-function relationships.

Amino acid sequences of two anticoagulant serine fibrinogenases - alpha- and beta-fibrinogenase (VLAF and VLBF) from Vipera lebetina venom have been deduced from the cDNA sequences encoding the enzymes. The mature protein sequences of 234 amino acids (VLAF) and 233 amino acids (VLBF) exhibit significant similarity with other snake venom serine proteinases. Both enzymes contain the catalytic triad His57, Asp102, Ser195, and twelve conserved cysteines forming six disulfide bridges. Unlike typical trypsin-like serine proteinases, they lack the third aspartate, Asp189 which is replaced by Gly189.

Short-term dabigatran interruption before cardiac rhythm device implantation: multi-centre experience from the RE-LY trial.

AIMS: Cardiac implantable electronic device (CIED) surgery is commonly performed in patients with atrial fibrillation (AF). The current analysis was undertaken to compare peri-operative anticoagulation management, bleeding, and thrombotic events in AF patients treated with dabigatran vs. warfarin. METHODS AND RESULTS: This study included 611 patients treated with dabigatran vs. warfarin who underwent CIED surgery during the RE-LY trial. Among 201 warfarin-treated patients, warfarin was interrupted a median of 144 (inter-quartile range, IQR: 120-216) h, and 37 (18.4%) patients underwent hepari

Delayed Traumatic Subarachnoid Hemorrhage Following Initially Negative Computed Tomography (CT) Imaging in a Patient With a Mechanical Mitral Valve and Markedly Elevated International Normalized Ratio (INR).

Warfarin remains the anticoagulant of choice for patients with mechanical heart valves because of its effectiveness in preventing thromboembolic events. However, management becomes particularly challenging when severe supratherapeutic anticoagulation is complicated by intracranial hemorrhage. We present the case of a 76-year-old male patient with a mechanical mitral valve on chronic warfarin therapy who presented after a ground-level fall with head trauma and an admission international normalized ratio (INR) of 9.0. Initial computed tomography (CT) of the head demonstrated only a scalp hematom

Anticoagulation-Amplified Recurrent Esophageal Hemorrhage in Triple-Positive Antiphospholipid Syndrome: The Inescapable Anticoagulation Dilemma.

INTRODUCTION: Antiphospholipid syndrome (APS) is an acquired thrombophilia. Triple-positive APS, defined by lupus anticoagulant, anticardiolipin, and anti-β2-glycoprotein I antibody positivity, usually requires long-term vitamin K antagonist therapy; direct oral anticoagulants are discouraged, and low-molecular-weight heparin is reserved for selected situations. CASE PRESENTATION: A 52-year-old female with systemic lupus erythematosus and triple-positive APS on warfarin (International Normalized Ratio [INR] 2.9) presented with massive hematemesis requiring intubation. Therapeutic Plasma Exchange After Spontaneous Intracranial Hemorrhage for a Patient With Antiphospholipid Syndrome and Lupus Anticoagulant Hypoprothrombinemia.

Antiphospholipid syndrome (APS) is characterized by the presence of antiphospholipid antibodies (aPL), macro- and micro-vascular thromboembolic complications. Lupus anticoagulant-hypoprothrombinemia (LAHPS) may confound the diagnosis and management of bleeding. Catastrophic APS has a category 1 indication for therapeutic plasma exchange (TPE). However, in patients with APS, LAHPS, and intracranial hemorrhage (ICH), TPE is not well described. A 47-year-old man with known APS anticoagulated on warfarin was transferred for diffuse spontaneous subdural hemorrhages (SDH) with somnolence. aPL levels

Retrievable inferior vena cava filters: initial clinical results.

Anticoagulation is the accepted therapy for patients with thromboembolic disease. When contraindications to anticoagulant therapy are present, however, interruption of the inferior vena cava (IVC) may prevent pulmonary embolism (PE). The objective of this study was to report our early technical and clinical results with retrievable IVC filters (IVCFs) for the prevention of PE. One hundred and twenty-seven multitrauma patients between December 1, 2002, and December 31, 2004, underwent placement of Gunther-Tulip (n = 49), Recovery (n = 41), or OptEase (n = 37) retrievable IVCFs under real-time i

Differences among western European countries in anticoagulation management of atrial fibrillation. Data from the PREFER IN AF registry.

Due to improved implementation of guidelines, new scoring approaches to improve risk categorisation, and introduction of novel oral anticoagulants, medical management of patients with atrial fibrillation (AF) is continuously improving. The PREFER in AF registry enrolled 7,243 consecutive patients with ECG-confirmed AF in seven European countries in 2012-2013 (mean age: 71.5 ± 10.7 years; 60.1% males; mean CHA2DS2-VASc score: 3.4). While patient characteristics were generally homogeneous across countries, anticoagulation management showed important differences: the proportion of patients taking

Anticoagulation and antiplatelet therapy in implantation of electrophysiological devices.

The growing implantations of electrophysiological devices in the context of increasing rates of chronic antithrombotic therapy in cardiovascular disease patients underscore the importance of an effective periprocedural prophylactic strategy for prevention of bleeding complications. In this review, we provide a concise overview of the data regarding anticoagulation and antiplatelet therapy in arrhythmia device surgery. Also, we critically discuss risk factors and procedural parameters that are potentially associated with haemorrhagic untoward events in this setting. Of note, current evidence su

Thromboprophylactic management in the neurosurgical patient with high risk for both thrombosis and intracranial bleeding.

PURPOSE OF REVIEW: Pharmacologic thromboprophylaxis is indicated in neurosurgery patients having high risk for venous or arterial thrombosis. The pharmacologic thromboprophylaxis, as well as temporary interruption of antithrombotic drugs because of surgery, and possible use of substitutive medication ('bridging therapy') are reviewed. RECENT FINDINGS: Pharmacologic thromboprophylaxis is used for most neurosurgical patients, but clinical practices vary a lot. There are only few reports of the management of neurosurgery patients having mechanical prosthetic heart valves, atrial fibrillation wit

Guidance for the practical management of the direct oral anticoagulants (DOACs) in VTE treatment.

Venous thromboembolism (VTE) is a serious medical condition associated with significant morbidity and mortality, and an incidence that is expected to double in the next forty years. The advent of direct oral anticoagulants (DOACs) has catalyzed significant changes in the therapeutic landscape of VTE treatment. As such, it is imperative that clinicians become familiar with and appropriately implement new treatment paradigms. This manuscript, initiated by the Anticoagulation Forum, provides clinical guidance for VTE treatment with the DOACs. When possible, guidance statements are supported by ex

Management of bleeding in patients using antithrombotic agents: prediction, prevention, protection and problem-oriented intervention.

Antiplatelet agents and anticoagulants are effective in the prevention and treatment of a variety of thrombotic disorders. Several clinical settings require more intense antithrombotic regimens. These can be provided by combining (i) two antiplatelet drugs, (ii) antiplatelet monotherapy with an anticoagulant, or (iii) anticoagulation with dual antiplatelet treatment (triple therapy). A major side effect of all antithrombotic regimens, however, is the induction of a bleeding diathesis. This is especially true in patients with preexisting haemostatic defects of any kind that may remain compensat

Thrombin generation and the pharmacodynamics of oral anticoagulants.

Anticoagulants are essential for preventing and treating thromboembolism across a wide range of clinical settings. Yet, they share a narrow therapeutic index that demands a precise understanding of their pharmacodynamic behavior. Thrombin generation provides an integrated measure of anticoagulant effects by capturing the timing, velocity, and total amount of thrombin formed over time. We reviewed the literature to establish a unified theoretical framework, grounded in mechanistic rationale and supported by experimentation, to better define the pharmacodynamics of anticoagulation therapy using
