Shifting of global aphasia to Wernicke's aphasia in a patient with intact motor function: a case report.

BACKGROUND: Global aphasia without hemiparesis (GAWH) is a rare stroke syndrome characterized by the dissociation of motor and language functions. Here, we present a case of GAWH with the patient later regaining speech fluency. CASE PRESENTATION: A 73-year-old man was admitted to our emergency department immediately after an episode of syncope. On arrival, we noted his global aphasia but without any focal neurologic signs. Computed tomography (CT) perfusion scans showed a large hypodense region over his left perisylvian area. Under the impression of acute ischaemic stroke, he received recombi

The effects of two teaching techniques on recognition and use of function words by aphasic stroke patients.

The study described in this article examined two different methods to reteach reading to stroke patients with Broca's aphasia. The first method was a multisensory technique that used visual, verbal, kinesthetic, and tactile input stimuli, while the alternate method used only a visual-verbal input procedure. A single-subject alternate-treatment design was used to compare the two approaches. It was found that word recognition improved in all patients regardless of the method used. The results of this study suggest that nurses who are aware of this problem may plan a simple approach that can help

Investigating changes in connected speech in nonfluent/agrammatic primary progressive aphasia following script training.

Script training is a speech-language intervention designed to promote fluent connected speech via repeated rehearsal of functional content. This type of treatment has proven beneficial for individuals with aphasia and apraxia of speech caused by stroke and, more recently, for individuals with primary progressive aphasia (PPA). In the largest study to-date evaluating the efficacy of script training in individuals with nonfluent/agrammatic primary progressive aphasia (nfvPPA; Henry et al., 2018), robust treatment effects were observed, with maintenance of gains up to one year post-treatment. How

Negotiation of identity in group therapy for aphasia: the Aphasia Café.

BACKGROUND: There has been increasing interest in ensuring that aphasia intervention includes attention to the negotiation of a robust identity after the life-altering changes that often accompany the onset of aphasia. But how does one go about simultaneously improving communication and positive identity development within aphasia therapy? Socially oriented group therapy for aphasia has been touted as one means of addressing both psychosocial and communicative goals in aphasia. AIMS: This article describes the results of a sociolinguistic analysis of group therapy for aphasia in which positiv

Mixed Nonfluent/Agrammatic Primary Progressive Aphasia and Behavioral Variant Frontotemporal Dementia: A Case Report From Tanzania.

Frontotemporal dementia (FTD) comprises neurodegenerative syndromes causing progressive deterioration in behavior, language, and executive function, with relative preservation of memory early on. Overlap between the behavioral variant (bvFTD) and nonfluent/agrammatic primary progressive aphasia (nfvPPA) reflects shared frontotemporal network pathology but remains rarely documented in Sub-Saharan Africa, where Alzheimer disease predominates. We describe a 61-year-old Tanzanian man who initially presented with effortful, halting speech, agrammatism, and impaired syntax consistent with nfvPPA. Ov

Contrasting speech patterns in apraxia of speech and phonemic paraphasia.

This study was designed to determine if perceptual phonological analysis would reveal distinctions between patients with apraxia of speech and patients with phonemic paraphasic speech. Test findings from 10 Broca's aphasics with apraxia of speech were compared to findings from 10 paraphasic speakers (5 conduction and 5 Wernicke's aphasics). Several marked differences were revealed. Predominant locus of errors and relative difficulty of different classes of phonemic segments were significant discriminators. There was a nonsignificant trend for substituted phonemes to be further from target phon

Axial movements in ideomotor apraxia.

Non-symbolic axial movements were examined and compared to oral and limb movements in a group of 60 aphasic patients (15 of each major subgroup) with exclusively left-sided brain damage. The contention in the literature that axial movements are preserved in patients with ideomotor limb apraxia was not confirmed.

Hemichorea as a Manifestation of Isolated Insular Infarction Successfully Treated with Mechanical Thrombectomy.

Stroke-associated hyperkinetic movements are most often linked to basal ganglia lesions, while cortical or insular involvement is rarely reported. Hemichorea is an uncommon manifestation of acute ischemic stroke and may pose diagnostic and therapeutic challenges. We report the case of an 81-year-old woman presenting with acute Broca-type aphasia and left hemibody choreic movements. Initial computed tomography (CT)/computed tomography angiography revealed an occlusion of the right M2 segment of the middle cerebral artery. Perfusion imaging showed a significant ischemic penumbra without establis

[A case of corticobasal degeneration that progressed from primary progressive aphasia].

A 65-year-old woman began to experience slowly progressive speech disturbance from 2001. She was admitted to our hospital for examination on May 2003. She had dysprosody, paragraphia, and mild disturbance in comprehension of spoken language. Repetition was preserved. No clear paraphasia was found. Calculation was disturbed, but there were no other neurological abnormalities including apraxia and agnosia. Brain MRI showed atrophy of the left frontal and parietal lobes. 99mTc-ECD SPECT showed decreased blood flow in the left frontal lobe and parietal lobe, especially in the Broca area and supple

Characterizing Verb Argument Structure in Aphasia Using Dependency Parsers.

PURPOSE: Verb argument structure (VAS) is often impaired in poststroke aphasia. Previous studies investigated VAS impairments using constrained experiments or less often narrative speech, which relies on time-consuming manual annotations of VAS in speech transcriptions. Here, we aimed to develop and validate an automated approach to quantify VAS use in narrative speech using natural language processing (a dependency parser) and to apply it to a new data set. METHOD: First, we validated this approach by comparing manually annotated measures of VAS from a previous study, with automatically anno

The Efficacy and Timing of Melodic Intonation Therapy in Subacute Aphasia.

Background Little is known about the efficacy of language production treatment in subacute severe nonfluent aphasia. Although Melodic Intonation Therapy (MIT) is a language production treatment for this disorder, until now MIT effect studies have focused on chronic aphasia. Purpose This study examines whether language production treatment with MIT is effective in subacute severe nonfluent aphasia. Methods A multicenter, randomized controlled trial was conducted in a waiting-list control design: patients were randomly allocated to the experimental group (MIT) or the control group (control inter

Serial processing and the "phonetic route:" lessons learned in the functional reorganization of deep dyslexia.

A patient with chronic agrammatic Broca's aphasia presented with symptoms of deep dyslexia, in which it is presumed that phonologic processing of the written message is disrupted. Functional reorganization of the "nonlexical" or phonetic route of reading was undertaken according to principles of A. R. Luria, whereby the patient must learn consciously to control behaviors that had formerly been unconscious and automatic. This patient's responses in treatment made it clear that the phonetic route encompasses at least two dissociable functions, grapheme-phoneme conversion and sequential analysis.

Music-based interventions for nonfluent aphasia: A systematic review of randomized control trials.

Music-based interventions (MBIs) offer promising strategies for addressing speech-language impairments in individuals with nonfluent aphasia. This systematic review summarizes the current literature of MBIs for nonfluent aphasia recovery by types of MBIs to determine the efficacy of MBIs and assesses the risk of bias to identify common methodological limitations. A systematic search was conducted of MEDLINE, PubMed, and APA PsycInfo for the 20 years preceding July 2024. Risk of bias assessment was performed using the revised Joanna Briggs Institute critical appraisal tool for randomized contro

Effects of melodic intonation therapy in patients with chronic nonfluent aphasia.

Patients with large left-hemisphere lesions and post-stroke aphasia often remain nonfluent. Melodic intonation therapy (MIT) may be an effective alternative to traditional speech therapy for facilitating recovery of fluency in those patients. In an open-label, proof-of-concept study, 14 subjects with nonfluent aphasia with large left-hemisphere lesions (171 ± 76 cc) underwent two speech/language assessments before, one at the midpoint, and two after the end of 75 sessions (1.5 h/session) of MIT. Functional MR imaging was done before and after therapy asking subjects to vocalize the same set of

A modality-specific mapping impairment: spoken versus written production.

A 29 year-old dysphasic woman (AF) presented with superior ability in written over spoken sentences. In contrast, her comprehension showed the reverse trend. Cognitive neuropsychological investigations revealed that her double dissociation was more apparent than real. AF's superior auditory comprehension was attributed to suspected dyslexic factors impeding written comprehension. However, an account of a strong dissociation between her written and spoken production was less obvious. The evidence suggested AF suffered from a procedural mapping deficit which had a disproportionate effect on spok

Averbia as a selective naming disorder: a single case report.

After a moderate head injury a 33-year-old woman presented a selective defect in finding verbs and name actions. Objects, colors, body parts, and qualities were named in a normal way. No other associated aphasic defects were observed. It was proposed that the selection and use of verbs in their correct forms implies two different aspects, which eventually can become dissociated: (1) the ability to retrieve the lexical item (the action name); and (2) the ability to use the correct affixation, that is, to select the morphological form according to the current context. Only the second would be as

Markedness analysis of consonant error productions in apraxia of speech.

This study applied a markedness analysis of consonant error productions to three adult aphasic patients with apraxia of speech. Single consonants and initial consonant clusters were investigated, employing both naming tasks and spontaneous narrative tasks. Results showed that phonemes of lower complexity values tended to substitute for phonemes of higher complexity values more than the reverse, and successive attempts at self-correction tended to reveal a progression toward the target segment in terms of markedness complexity. Theoretical and clinical implications are discussed.

Language disintegration in dementia: effects of etiology and severity.

The speech characteristics on a standardized picture description task of 26 subjects with presumed senile dementia of the Alzheimer type (SDAT) and 13 subjects with stroke-related dementia (SRD) were compared to 15 normal subjects over age 59 years. Compared to the normal subjects, the dementia subjects used fewer total words, fewer unique words, fewer prepositional phrases, fewer subordinate clauses, and more incomplete sentence fragments. Lexical deficits tended to be more severe than syntactic ones, confirming prior suggestions that lexicon is more vulnerable to disruption in dementia than
