The 'Allan Johnson' voice prosthesis. A modification of the Bivona voice prosthesis for immediate post-fitting aphonia after secondary tracheo-oesophageal puncture.

The combination of a narrow oesophageal lumen and a hypertonic pharyngo-oesophageal (P-E) segment following laryngectomy and secondary tracheo-oesophageal puncture (TEP) can cause obstruction of a voice prosthesis and consequently prevent phonation. The 'Allan Johnson' modification of the Bivona voice prosthesis incorporates a stainless steel slide and has been successfully used to remedy such a circumstance.

[Analysis of the use of reflex therapy in functional disorders of the larynx].

The paper reviews data on effective use of reflex therapy in laryngeal dysfunction: hypo- and hypertonicity dysphonia, spastic dysphonia, functional aphonia. Besides classical methods of acupuncture, effective were, especially in functional aphonia, novocaine blockades of laryngeal tender zones.

A historical vignette (23). A voiceless poet at Bourbon - l'Archambault: Nicolas Boileau.

In 1687, the poet was stricken by an aphonia that would last six months. He underwent a cure at a spa in the Massif Central, but without result. The correspondence between Boileau and his friend Racine allows us to relive the atmosphere of being a spa guest in the 17th century (Figure 1). Boileau's aphonia seems to have been functional for the larger part.

[A rare complication of endotracheal intubation: posterior luxation of the left arytenoid cartilage].

The luxation of the arytenoid cartilage is a very uncommon complication of endotracheal intubation. Symptoms usually are aphonia, dysphonia, pain or discomfort in the throat, odynophagia and laryngeal stridor. It may be due to several factors: the pressure of the convex aspect of the tube on the arytenoid, traumatic or prolonged intubations, reintubations within a short period, or some systemic diseases. We report a case of posterior luxation of the left arytenoid in a patient who had been anesthetized for the radical cure of a left inguinal hernia. The clinical features developed when he reco

Acute palsy of the recurrent laryngeal nerve complicating Epstein-Barr virus infection.

We report an 11-year-old boy with active Epstein-Barr virus (EBV) infection who developed acute aphonia and had signs and symptoms of recurrent laryngeal nerve palsy. The association of isolated recurrent laryngeal nerve palsy and EBV infection has not previously been reported. This case report expands the spectrum of neurologic complications of EBV infection, and suggests that infectious mononucleosis should be considered in the differential diagnosis of acute aphonia in children.

Vocal cord paralysis secondary to impacted esophageal foreign bodies in young children.

Impacted foreign bodies in the esophagus can result in respiratory symptoms including stridor and aphonia. Several mechanisms have been proposed to explain these symptoms, but the possibility of vocal cord paralysis and its cause has not been adequately emphasized. Two cases of young children with esophageal foreign body are described; both presented with respiratory symptoms, 1 with aphonia and the other with stridor. In both cases, the symptoms were secondary to vocal cord paralysis. A possible mechanism of recurrent nerve injury is proposed.

An fMRI study of whispering: the role of human evolution in psychological dysphonia.

Humans are the only species known to use whispering for communication, and humans are also the only species known to suffer from functional (psychological) aphonia, a condition in which only whispered and not vocalized speech can be produced. Thus, whispering is uniquely a human function and psychological aphonia is uniquely a human dysfunction; both can be considered to result from human evolution. We hypothesize that the human brain has developed one or more identifiable switching mechanisms that overlay whispering onto the mechanisms of normal speech production. Thus, for whispering the spe

Bypassing shame and conversion disorder.

We report a case of conversion disorder (partial aphonia) that was successfully treated with speech therapy. During the one year duration of this illness, the patient regained transiently (minutes) her normal speech on a few occasions, independently of concomitant pharmacological interventions. One year after recovery she developed aphonia for the second time, which responded again to speech therapy, although the response rate was slower. Several mechanisms of conversion disorder are reviewed as specifically applied to this case. The possible role of pertinent shame mechanisms in the conversio

[Psychogenic aspects of functional dysphonia].

In order to estimate the significance of the psychogenic factor in the etiology of functional voice disorders on the basis of relevant psychological dimensions we compared patients with functional dysphonia, aphonia and a normal control group by means of an empirical-psychological test procedure. Psychogenic conditions proved to be a major factor in the etiology of aphonia, whereas such an interrelation turned out only partially in patients with functional dysphonia. The conclusion can be that psychogenic aspects are not necessarily of exceeding importance in the etiology of functional dysphon

Rehabilitation Therapy for Vocal Fold Paralysis Caused by Lung Cancer: A Case Report.

OBJECTIVE: Unilateral vocal fold paralysis (UVFP) can be caused by iatrogenic injury or tumor-induced damage to the recurrent laryngeal nerve. Studies of comprehensive rehabilitation therapies for patients suffering from severe UVFP are limited. The purpose of this case report is to describe an improvement in complete aphonia after comprehensive rehabilitation therapies in a patient with severe UVFP due to a lung tumor. METHODS: An 81-year-old woman with a history of bronchial adenoma had complete aphonia due to compression of the left recurrent laryngeal nerve by the tumor. Dynamic fibrolary

[Foreign accent syndrome in a case of dissociative (conversion) disorder].

Psychiatric symptoms are often manifested in verbal expression. Generally, the contents of such expression are pathological. The formal abnormalities of speech are also observed in various mental disorders, as far as quantitative abnormalities are concerned. However, disturbance of intonation, namely disprosody, is more commonly observed in organic disorders of the brain. When the accent of words and the intonation of sentences changes from that of a native speaker, the speech sounds like the broken language of untrained foreigners. Such foreign accent syndrome is usually an issue of neuropsyc

Correction of late glottic insufficiency.

Preservation of respiration, deglutition, and phonation, as well as tumor ablation, is the objective of conservation surgery of the larynx. Immediate reconstruction may fail, leading to aspiration and aphonia. Five extralaryngeal techniques for correction of late glottic insufficiency are described.

[Schwannoma of the larynx in Recklinghausen disease. Differential diagnostic considerations and immunohistochemical detection].

A case of schwannoma of the larynx in a 29-year-old woman with Recklinghausen's disease is reported. The tumor was located in the arytenoid region. The patient complained of disturbed swallowing, aphonia, and dyspnea. The tumor was endoscopically removed. The light-microscopic and immunohistochemical findings are presented.

CoblationTM-assisted excision of suprastomal granulation tissue.

BACKGROUND: Suprastomal granulation tissue is a common complication of long-term tracheostomy. It may be associated with bleeding, aphonia, airway obstruction and delayed decannulation. METHODS: This study describes the experience of a tertiary paediatric medical centre with CoblationTM-assisted suprastomal granulation tissue excision. RESULTS: Thirteen children (mean age, 5.7 years) who underwent the procedure from 2013 to 2019 because of delayed decannulation or aphonia were included. Lumen obstruction ranged from 50 to 90 per cent, with a mean of 68.8 per cent. After the procedure, decann
