[Dehiscence and disinsertion of the aponeurotic bundle of the upper eyelid levator in acquired ptosis].

Aponeurotic defects and disinsertion of the levator aponeurosis are responsible for many cases of acquired ptosis. The typical clinical findings in aponeurotic defects are: history of prior orbital swelling, injury, ocular surgery, blepharochalasis; good to excellent levator function, thinning of the eyelid above the tarsus, high to absent lid crease, and normal Müller's muscle function. The purpose of the treatment is to repair a defect or advance the aponeurosis onto the tarsus. Levator aponeurosis surgery was used to treat 18 upper eyelids with acquired ptosis. Local anesthesia and surgery

Factors Influencing the Lagophthalmos and Lid Lag After Simple Congenital Blepharoptosis Correction.

BACKGROUND: Lagophthalmos and lid lag is the most common complication after ptosis correction. Great efforts had been made to control it, but little was known about the factors influencing it. So we ran this research to explore the possible mechanisms underlying it. This would be beneficial to solving this problem. MATERIALS AND METHODS: In this cohort study, patients treated with the forked frontalis muscle aponeurosis suspension and levator aponeurosis-Muller's muscle complex resection were identified. Lagophthalmos height and lid lag was measured at the postoperative week 1 visits. The Spe

Ultrasound-guided minimally invasive release of the proximal medial gastrocnemius: A cadaveric study.

BACKGROUND: Posterior chain contractures contribute to functional impairment in foot and ankle disorders. Open gastrocnemius lengthening techniques are effective but invasive and associated with soft tissue morbidity. Ultrasound-guided percutaneous aponeurotic release represents a minimally invasive alternative. This study aimed to assess the feasibility, safety, and efficacy of an ultrasound-guided, single-operator medial gastrocnemius aponeurosis release technique. METHODS: Six fresh-frozen cadavers (12 lower limbs) underwent ultrasound-guided proximal medial gastrocnemius aponeurosis relea

Recurrent ptosis in a patient with blepharochalasis: clinical and histopathologic findings.

A 37-year-old woman presented with right upper eyelid blepharochalasis with ptosis. Right upper eyelid edema had occurred 2 to 3 times per year by 30 years old, although the frequency decreased with age. The edema occurred spontaneously and resolved within 1 to 2 days. She underwent a right levator tucking surgery at 22 years old, and the ptosis recurred 2 years postoperatively. She again underwent ptosis surgery with skin excision at 37 years old. The intraoperative findings showed a thin levator aponeurosis. The white line was therefore advanced to the upper tarsal edge, resulting in an appr

Distal ligamentous restraints of the first metatarsal. An in vitro biomechanical study.

BACKGROUND: Hypermobility or instability of the first metatarsal is associated with increased hallux valgus deformity and greater risk of recurrence after surgery. The objective of this in vitro study was to determine the ranges of movement, under directional loading, of the normal first metatarsal and the effect of the longitudinal plantar aponeurosis and the transverse tie-bar ligamentous system. METHODS: A model has been developed to quantify the influence of the first intermetatarsal ligament and plantar aponeurosis on movements of the first metatarsal. All muscular structures controlling

Middle-aged adults exhibit altered spatial variations in Achilles tendon wave speed.

The purpose of this study was to investigate spatial variations in measured wave speed in the relaxed and stretched Achilles tendons of young and middle-aged adults. Wave speed was measured from the distal Achilles tendon, soleus aponeurosis, medial gastrocnemius aponeurosis and medial gastrocnemius muscle in healthy young (n = 15, aged 25   ±   4 years) and middle-aged (n = 10, aged 49   ±   4 years) adults in resting, dorsiflexed and plantarflexed postures. In both age groups, Achilles tendon wave speed decreased proximally, with the lowest wave speed measured in the gastrocnemius aponeurosi

Dupuytren's cord involving the septa of Legueu and Juvara: a case report.

A patient with Dupuytren's disease with involvement of the palmar fascial complex and digital contracture is described. A vertical cord had developed in the transverse ligament of the palmar aponeurosis fibers and the underlying septa of Legueu and Juvara. The cord was composed of a pretendinous band, transverse ligament of the palmar aponeurosis, and septum of Legueu and Juvara. The cord was attached deeply in the soft tissue confluence of the sagittal band, palmar plate, and interpalmar plate ligament. Involvement of the transverse ligament of the palmar aponeurosis and septa of Legueu and J

Distal hamstring lengthening in cerebral palsy: the influence of the proximal aponeurotic band of the semimembranosus.

We undertook a prospective review of 24 children with spastic diplegia treated by distal hamstring fractional lengthening at the Sheffield Children's Hospital. In 14 children (24 limbs) the correction achieved (popliteal angle) was inadequate and in these patients in the operating room a further correction was attained by dividing a tight band palpated in the substance of the semimembranosus muscle. Under general anaesthetic (preoperatively) the popliteal angle (a degrees ) was measured using a goniometer, then again (b degrees ) following distal hamstring fractional lengthening and finally (c

Palmar fascial complex anatomy and pathology in Dupuytren's disease.

Familiarity with the normal palmar fascial anatomy of the hand is necessary for understanding the convoluted pathologic changes that take place in Dupuytren's disease. This article includes a literature review and the findings of a study by the author of the fascial anatomy and pathology as related to Dupuytren's disease. Gross and microdissection of the palmar fascial structures were carried out with the aid of the operative microscope and an arthroscope, which allowed examination of the fine and undisturbed retinacular anatomy. The palmar fascial complex of the hand has five components: the

Ultrasound classification of medial gastrocnemious injuries.

High-resolution ultrasound (US) has helped to characterize the "tennis leg injury" (TL). However, no specific classifications with prognostic value exist. This study proposes a medial head of the gastrocnemius injury classification based on sonographic findings and relates this to the time to return to work (RTW) and return to sports (RTS) to evaluate the prognostic value of the classification. 115 subjects (64 athletes and 51 workers) were retrospectively reviewed to asses specific injury location according to medial head of the gastrocnemius anatomy (myoaponeurotic junction; gastrocnemius ap

Micro-Computed Tomography-Based Three-Dimensional Anatomical Structure of the Region Around the Pterygoid Hamulus.

OBJECTIVE: Palatoplasty would involve the structures around the pterygoid hamulus. However, clinicians hold different opinions on the optimal approach for the muscles and palatine aponeurosis around the pterygoid hamulus. The absence of a consensus regarding this point can be attributed to the lack of investigations on the exact anatomy of this region. Therefore, we used micro-computed tomography to examine the anatomical structure of the region surrounding the pterygoid hamulus. DESIGN: Cadaveric specimens were stained with iodine-potassium iodide and scanned by micro-computed tomography to

Quantitative ultrasound mapping of regional variations in shear wave speeds of the aging Achilles tendon.

OBJECTIVES: Evaluate the effects of aging on healthy Achilles tendon and aponeurosis shear wave speed (SWS), a quantitative metric which reflects tissue elasticity. METHODS: Shear wave elastography was used to measure spatial variations in Achilles tendon SWS in healthy young (n = 15, 25 ± 4 years), middle-aged (n = 10, 49 ± 4 years) and older (n = 10, 68 ± 5 years) adults. SWS was separately measured in the free Achilles tendon, soleus aponeurosis and gastrocnemius aponeurosis in resting (R), stretched (dorsiflexed 15° from R) and slack (plantarflexed 15° from R) postures. RESULTS: SWS sign

Presence of multiple tendinous insertions of palmaris longus: a unique variation of a retrogressive muscle.

BACKGROUND: Palmaris longus, being the most variable muscle of the upper extremity is generally considered important morphologically rather than functionally. The palmar aponeurosis represents the distal part of the tendon of Palmaris longus. CASE DETAILS: We report here a unique case of palmaris longus presenting variation distally as its tendon divides to gain multiple attachments with the fascia covering the abductor pollicis brevis, flexor retinaculum and the tendon of flexor carpi ulnaris. In addition, it also continues as palmar aponeurosis as its normal course. CONCLUSION: Presence of

[Dupuytren disease--formal pathogenesis without contraction and a new surgery concept].

The palmar fibromatosis holds the finger in the flexed position--extension inhibition rather than contraction. According to this extrinsic concept, only a passive, secondarily reactive effect can be attributed to the palmar aponeurosis. Out of 72 hands examined 5 or more years after an operation in which the aponeurosis had been left intact, 47% proved to be clear of disease.

Tendinous tissue properties after short- and long-term functional overload: Differences between controls, 12 weeks and 4 years of resistance training.

AIM: The potential for tendinous tissues to adapt to functional overload, especially after several years of exposure to heavy-resistance training, is largely unexplored. This study compared the morphological and mechanical characteristics of the patellar tendon and knee extensor tendon-aponeurosis complex between young men exposed to long-term (4 years; n = 16), short-term (12 weeks; n = 15) and no (untrained controls; n = 39) functional overload in the form of heavy-resistance training. METHODS: Patellar tendon cross-sectional area, vastus lateralis aponeurosis area and quadriceps femoris vo

Early changes in Achilles tendon behaviour in vivo following downhill backwards walking.

Downhill backwards walking causes repeated, cyclical loading of the muscle-tendon unit. The effect this type of repeated loading has on the mechanical behaviour of the Achilles tendon is presently unknown. This study aimed to investigate the biomechanical response of the Achilles tendon aponeurosis complex following a downhill backwards walking protocol. Twenty active males (age: 22.3 ± 3.0 years; mass: 74.7 ± 5.6 kg; height: 1.8 ± 0.7 m) performed 60 min of downhill (8.5°), backwards walking on a treadmill at -0.67 m · s(-1). Data were collected before, immediately post, and 24-, 48- and 168-

The iliopubic tract. The key to inguinal herniorrhaphy?

The iliopubic tract exists. It is not illusory. It is the small inferior tongue of the transversus abdominis aponeurosis; separated from its parent by the spermatic cord at the internal ring. The tract may be reinforced by a variable number of fibers from the transversalis fascia, however the entire structure is of significant strength in less than 25% of cases. Since the iliopubic tract inserts into the pubic ramus, a significant gap is created between it and the main transversus aponeurosis which inserts some distance above into the rectus sheath. Despite the use of a relaxing incision and i

Biochemistry and histology of the connective tissue of Dupuytren's disease lesions.

When compared to age-matched control aponeurosis, lesions of Dupuytren's disease contain higher contents of water, collagen and chondroitin-sulphate, as well as increased proportions of soluble collagens and of reducible cross-links; these indicate synthesis of new collagen. The lesions show also increased amounts of type III collagen and an increased hydroxylation and glycosylation of the reducible cross-links. All these parameters are characteristic of granulation and scar tissues. Type III collagen was located by means of immunofluorescence on thin argyrophilic fibres and also within the la
