A Norwalk-like virus outbreak on the Appalachian Trail.

In May and June 1999, an outbreak of acute gastrointestinal illness occurred among long-distance hikers on the Appalachian Trail between Catawba and Troutville, Virginia. An investigation found that 45 out of 70 hikers had become ill within two days of arriving in Catawba, Virginia. Water samples were collected from a general store frequented by the hikers and from several nearby buildings and a popular all-you-can-eat restaurant. Symptoms were consistent with those caused by Norwalk-like viruses, and laboratory diagnosis detected Norwalk-like viruses in stool and serum specimens. People who c

Contrasting Patterns of Raccoon (Procyon lotor) Spatial Population Genomics Throughout a Rabies Management Area in Eastern North America.

Wide-ranging, generalist species provide both interesting and challenging opportunities for research questions focused on population structure. Their continuous distributions and ability to occupy diverse habitat types can obscure genetic signals of ancestry and geographic clustering. However, spatially informed population genetic approaches are notable for high-resolution identification of geographic clusters that often elude more classical clustering models. The northern raccoon (Procyon lotor) is a broadly distributed species in North America, with populations in diverse habitats ranging fr

Can umbilical cord testing add to maternal urine drug screen for evaluation of infants at risk of neonatal opioid withdrawal syndrome?

OBJECTIVE: This study evaluated maternal urine drug screen (UDS) at delivery and umbilical cord drug testing and its association with neonatal opioid withdrawal syndrome (NOWS) diagnosis and severity following opioid exposed pregnancy. METHODS: A retrospective chart review of 770 mother-infant dyads at five birthing hospitals in the United States Appalachian region for a five-year period was performed. Variables of interest included dyad demographics, results of maternal UDS at delivery and umbilical cord drug testing, and three neonatal outcomes: NOWS diagnosis, pharmacologic treatment admin

Partner Relationships and Injection Sharing Practices among Rural Appalachian Women.

BACKGROUND: The role of relationships in initiating and maintaining women's risk behaviors has been established. However, understanding factors that may underlie partner relationships and women's risky drug use, particularly in rural contexts, is limited. This study is the first to examine the association between injecting partners and women's risky injection practices as a function of relationship power perception. METHODS: Female participants were recruited from three rural jails in the Appalachian region. Women were selected randomly, provided informed consent, and screened for study eligi

Identifying spatiotemporal patterns in opioid vulnerability: investigating the links between disability, prescription opioids and opioid-related mortality.

BACKGROUND: The opioid crisis remains one of the most daunting and complex public health problems in the United States. This study investigates the national epidemic by analyzing vulnerability profiles of three key factors: opioid-related mortality rates, opioid prescription dispensing rates, and disability rank ordered rates. METHODS: This study utilizes county level data, spanning the years 2014 through 2020, on the rates of opioid-related mortality, opioid prescription dispensing, and disability. To successfully estimate and predict trends in these opioid-related factors, we augment the Ka

Loneliness and Alcohol use among College Students During the COVID-19 Pandemic in Rural Appalachia.

OBJECTIVE: Binge drinking and heavy alcohol use are highly prevalent among college students. During the COVID-19 pandemic, due to lockdown restrictions and other challenges, many college students were burdened with loneliness, which can contribute to chronic stress, and substance use. The current study explores the association between loneliness and various levels of alcohol use among college students in the rural, underserved region of Central Appalachia, USA. METHODS: Data were collected from a regional sample (n = 320) of college age adults, age 18-25 in the Central Appalachian region. The

Willingness to use a mobile syringe service program among people who inject drugs in rural Appalachia.

BACKGROUND: Rural people who use drugs face a litany of structural and psychosocial barriers to SSP utilization (e.g., unreliable transportation, stigma). Mobile SSPs may mitigate some barriers, but little is known about acceptability among rural people who use drugs and their preferences for mobile SSP design. This study examines (1) factors associated with willingness to use a mobile SSP among rural people who use drugs, and (2) their desired attributes and services. METHODS: Data were collected from 259 participants who inject drugs that were recruited through outreach and respondent-drive

Gender and power dynamics in three types of injection partnerships among young women in rural Appalachia.

BACKGROUND: Young women who inject drugs (YWID) face heightened risks for HIV, hepatitis C, and overdose that are shaped by the relational contexts in which they inject. Injection partnerships, defined as dyadic relationships between people who inject together, remain understudied beyond male-female intimate pairs. METHODS: We conducted 26 semi-structured interviews with YWID (ages 18-35) in rural Appalachian Ohio to explore dynamics within three partnerships types: dual partnerships with male intimate partners, family partnerships, and friend partnerships, with attention to female peer partn

Drinking water sources, quality, and associated health outcomes in Appalachian Virginia: A risk characterization study in two counties.

OBJECTIVES: In the US, violations of drinking water regulations are highest in lower-income rural areas overall, and particularly in Central Appalachia. However, data on drinking water use, quality, and associated health outcomes in rural Appalachia are limited. We sought to assess public and private drinking water sources and associated risk factors for waterborne pathogen exposures for individuals living in rural regions of Appalachian Virginia. METHODS: We administered surveys and collected tap water, bottled water, and saliva samples in lower-income households in two adjacent rural counti

Implementing a telemedicine-led heart failure medication regimen optimization clinic in medically underserved heart failure populations.

BACKGROUND: Suboptimal guideline-directed medical therapy (GDMT) management for heart failure (HF) is a critical issue in rural communities. Most patients with HF in rural communities are treated in primary care settings. Multidisciplinary telemedicine-led HF medication optimization clinics were implemented to improve access to specialty care and address health disparities in HF care in rural Appalachian areas. OBJECTIVES: The project aimed to evaluate the effect of a multidisciplinary telemedicine HF medication optimization clinic on the use of GDMT in cardiology and primary care services.

Prenatal substance use: Religious women report lower use rates, but do they use less?

An inverse relationship exists between self-reported religiousness and substance use. However, we question whether religious culture impacts the veracity of self-reported substance use. The primary aim of this study of low-income pregnant women in South Central Appalachia was to determine the accuracy of self-reported substance use in pregnant women as well as to determine whether there were differences in use rates and/or differences in the degree to which women would accurately report substance use depending on their religiousness. Self-reported use and toxicology screening results taken fro

Pregnancy Status, Risk Factors, and Opportunities for Referral to Care Among Reproductive-Aged Women With Newly Reported Chronic Hepatitis C Virus Infection in Tennessee.

OBJECTIVES: From 2006 through 2012, the number of acute hepatitis C virus (HCV) infections increased 364% among persons aged ≤30, including reproductive-aged women, in Central Appalachian states. Outreach to reproductive-aged women with newly diagnosed HCV infection affords a unique opportunity to provide counseling, further testing, and linkage to treatment. We modeled a centrally located statewide effort to reach this population and their health care providers to ascertain pregnancy status, HCV risk factors, and opportunities for linkage to additional services. METHODS: Using the Tennessee

Examining HIV pre-exposure prophylaxis (PrEP) acceptability among rural people who inject drugs: predictors of PrEP interest among syringe service program clients.

Rural communities in the US have increasing HIV burden tied to injection drug use, yet engagement in pre-exposure prophylaxis (PrEP) care has been low among people who inject drugs (PWID). Syringe service programs (SSPs) are widely implemented in Kentucky's Appalachian region, presenting an important opportunity to scale PrEP services. This paper examines PrEP awareness, interest and preferences among PWID attending community-based SSPs in Appalachia. Eighty participants were enrolled from two SSP locations. Eligibility included: ≥ 18 years old, current injection drug use and SSP use, and an i

A new genetic variant of La Crosse virus (bunyaviridae) isolated from New England.

La Crosse virus (LACV) is found primarily in the Midwestern and Appalachian regions of the United States where it is a leading cause of mosquito-borne encephalitis in children. To determine whether the distribution of this virus extends further east into New England, we analyzed a bunyavirus that was isolated from a pool of eastern tree-hole mosquitoes, Ochlerotatus triseriatus (= Aedes triseriatus), collected from Fairfield, Connecticut (CT) in 2005. Nucleotide and encoded amino acid sequences from portions of the S, M, and L segments were more similar to the prototype strain of La Crosse vir

Mental Health Emergency Department Use among Treatment-Seeking Adults in a Rural Northern Appalachian Clinic: The Role of Homelessness and Alcohol Use.

Emergency departments are vital safety-net resources for adults seeking mental health treatment. Frequent mental health emergency department use has been associated with homelessness and alcohol use. However, little of this research has been conducted in rural settings, particularly in Northern Appalachia. Therefore, this program-level study aimed to describe the relationship between recent mental health emergency department use, homelessness, and alcohol use in intake survey data from one outpatient mental health clinic. Participants were 1,293 adults seeking treatment between June 2020 and A
