Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, affecting millions of individuals worldwide. The rapid, irregular, and disordered electrical activity in the atria gives rise to palpitations, fatigue, dyspnea, chest pain and dizziness with or without syncope. Patients with AF have a five-fold higher risk of stroke. Oral anticoagulation (OAC) with warfarin is commonly used for stroke prevention in patients with AF and has been shown to reduce the risk of stroke by 64%. Warfarin therapy has several major disadvantages, however, including bleeding, non-tolerance, interact

A 7-year-old child was noted to have dextrojuxtaposition of the left atrial appendage at the time of surgical atrial septal defect repair. Given the favourable anatomic location and size of the atrial appendage, it was inverted and used to close the atrial defect. This is the first report of atrial septal defect repair using a juxtaposed atrial appendage. The cardiac anatomy and theoretical benefits of this repair are discussed.

Focal atrial tachycardias arising from the atrial appendages and the aortic sinuses of Valsalva are less frequently encountered in clinical practice. This review article describes the clinical presentation, surface P wave morphology, electrophysiologic characteristics and treatment of these arrhythmias. Catheter ablation of these focal tachycardias has a high success rate. It is however important to be aware of specific anatomic considerations in these locations for optimal treatment outcomes with low complication rates.

A 69-year-old man with a history of paroxysmal atrial fibrillation (PAF) and subsequent cerebral infarction was referred to our hospital because of an abnormal accumulation of 18F-fluorodeoxyglucose (FDG) in the left atrial appendage (LAA) that was detected on positron emission tomography-computed tomography (PET-CT) imaging during a health screening. Transesophageal echocardiography (TEE) demonstrated thrombus formation in the LAA. Even after the thrombus disappeared by strictly guided oral anticoagulant therapy, intense abnormal FDG uptake in the LAA on PET-CT imaging persisted. In low-risk

We report a case with paroxysmal atrial fibrillation, in whom multiple foci were identified in the left atrial appendage after pulmonary vein isolation. The patient was successfully treated by catheter ablation to disconnect this structure.

BACKGROUND: Intussusception of the left atrial appendage (LAA) is an uncommon and often underrecognized finding; poor knowledge of this condition is still widespread in clinical practice. A systematic review was carried out to provide a comprehensive and updated overview on this condition.
METHODS: A bibliographic search using major databases resulted in 25 case reports meeting the criteria. Included records reported data from patients aged > 18 years with a diagnosis of LAA intussusception, determined either intraoperatively or incidentally by imaging tools, and findings on baseline demograp

Juxtaposition of the atrial appendages is known to occur in specific congenital heart lesions. Recognition of these variants is critical in balloon atrial septostomy and atrial switch operations. There remains little clinical data on the prevalence and associations of these lesions. This is the largest echocardiographic study of juxtaposition of the atrial appendages. This is a retrospective study using the Mt. Sinai echocardiogram database (EchoLAN), which contains echocardiogram reports performed or reviewed at Mt. Sinai Hospital (New York, NY) between 1992 and 2019. Each report was reviewed

PURPOSE OF REVIEW: In patients with atrial fibrillation who are unable to take novel oral anticoagulants for stroke prophylaxis due to bleeding risk or other contraindications, left atrial appendage (LAA) occlusion and exclusion devices have shown benefit. In this review, we highlight the risks and benefits associated with LAA removal.
RECENT FINDINGS: LAA, once considered a vestigial organ, has been shown to have physiological, anatomical, and arrhythmogenic properties. Device-related complications such as pericardial effusion, device embolization, device-related thrombus, while uncommon, ar

As we have tried to demonstrate in this chapter, extensive understanding of morphology is necessary for successful definitive repair in individual patients with isomeric atrial appendages, as well as for the further evolution of optimal surgical strategies. Obviously different from patients with the usual atrial arrangement, those with isomeric atrial appendages have specific morphologic features that may underscore the clinical problems frequently found in this setting. Surgical strategies as well as individual operative procedures should be established on the basis of knowledge regarding tho

As part of a prospective study into the diagnostic role of transoesophageal echocardiography in children with complex congenital heart disease, the atrial morphology was assessed in 62 children. Using the direct visualization of atrial appendage morphology, 58 were shown to have usual atrial arrangement, two were documented to have isomerism of the right and two isomerism of the left appendages. Of those with usual arrangement, four children were demonstrated to have left juxtaposition of the atrial appendages. Only two of these patients were identified during praecordial echocardiographic re-

It is well known, that in complex congenital heart disease, the bronchial anatomy reflects the atrial situs in the majority of patients. Few exceptions to this rule have been reported in the literature. We report four patients in whom we found discordance between the anatomy of the bronchial tree and the external aspect of the atrial appendages. Two patients had bronchial situs solitus with atrial levoisomerism, the third patient had bronchial levoisomerism with atrial situs solitus and in the fourth one we found inverted bronchial anatomy with levoisomerism of the atrial appendages. In three

A case of the uncommon right-sided juxtaposition of the atrial appendages is presented--to our knowledge the 17th instance reported so far. The interest of this case resides in the fact that there were no associated major cardiac anomalies, and that it occurred in an anencephalic female. It is the second reported case of this association. Both cases had in common the presence of ventricular D-looping, normal relationships of the great arteries and only minor cardiac defects. The absence of major cardiac anomalies, in contrast to the more common left-sided juxtaposition, and the association wit

BACKGROUND: Performing atrial fibrillation (AF) catheter ablation and percutaneous left atrial appendage occlusion (LAAO) during a single procedure may improve efficiency and reduce cumulative procedural risk. However, concerns remain regarding potential increases in periprocedural and longer-term complications, including stroke, pericardial effusion, device leak, and device-related thrombus. Evidence evaluating the safety of this concomitant strategy remains limited.
OBJECTIVES: The objectives of the study was to compare the safety and short- and long-term outcomes of concomitant AF ablation

OBJECTIVE: To compare outcomes after left atrial appendage occlusion (LAAO) via implanted device vs no LAAO in a matched cohort of patients with atrial fibrillation (AF).
METHODS: This longitudinal retrospective cohort study was based on the national database covering hospital care for the entire French population. Adults (≥18 years of age) who had been hospitalized with AF (January 1, 2015, to January 1, 2020) who underwent LAAO were identified. Propensity score matching was used to control for potential confounders of the treatment-outcome relationship. The primary outcome was a composite o

OBJECTIVES: The clinical efficacy and safety of a novel left atrial appendage (LAA) occluder of the SeaLA closure system in patients with nonvalvular atrial fibrillation (NVAF) were reported.
BACKGROUND: Patients with NVAF are at a higher risk of stroke compared to healthy individuals. Left atrial appendage closure (LAAC) has emerged as a prominent strategy for reducing the risk of thrombosis in individuals with NVAF.
METHODS: A prospective, multicenter study was conducted in NVAF patients with a high risk of stroke.
RESULTS: The LAAC was successfully performed in 163 patients. The mean age
