Development of a progressive audiologic tinnitus management program for Veterans with tinnitus.

Tinnitus is the most prevalent service-connected disability awarded to Veterans. However, clinical protocols for management of tinnitus have been inconsistent across Department of Veterans Affairs (VA) medical centers. A study was funded to develop and pilot test a protocol to provide tinnitus services consistently across VA audiology clinics. Drawing on a series of prior VA and external research projects, a clinical model was formulated, supporting materials in multimedia were developed, and a pilot study was conducted. Five hierarchical levels of care were defined and labeled the Progressive

Evaluating the physical fit of receiver-in-the-ear hearing aids in infants.

BACKGROUND: In spite of early identification and intervention efforts achieved by Early Hearing Detection and Intervention (EHDI) programs, many infants with hearing loss experience delays in early vocabulary development in comparison to peers with normal hearing (Mayne, Yoshinaga-Itano, Sedey, 2000a; Mayne, Yoshinaga-Itano, Sedey, Carey, 2000b; Moeller et al, 2007a, 2007b). One of the several factors that may contribute to individual differences in outcomes is inconsistent hearing aid use in this age group. This may be associated with the physical fit when using traditional behind-the-ear (BT

Large vestibular aqueduct syndrome: an overlooked etiology for progressive childhood hearing loss.

If a drastic change in hearing has occurred in a child following a minor head trauma, change in barometric pressure, or physical exertion, large vestibular aqueduct syndrome (LVAS) should be considered. Most audiologists are unaware of LVAS or do not suspect it, in part due to the presence of a conductive component. LVAS can be seen in conjunction with Mondini's dysplasia or may appear by itself and is easily identified by a computed tomography scan. We present five cases of LVAS and discuss the natural history, audiologic and imaging findings, and relevant literature.

The deafness of Beethoven: an audiologic and medical overview.

The hearing loss of Ludwig von Beethoven has fascinated physicians, audiologists, and musicologists for almost 200 years. In this review of the literature, the features, treatment, and differential diagnosis of the composer's malady are discussed. An original series of hypothetical audiograms and actual photomicrographs depicting cochlear otosclerosis and Paget's disease are also provided. The author concludes with the majority of otologists that cochlear otosclerosis is the most likely diagnosis for the hearing loss. In addition, a summary of Beethoven's complicated medical history and result

Outcome survey of auditory-verbal graduates: study of clinical efficacy.

Audiologists must be knowledgeable about the efficacy of aural habilitation practices because we are often the first professionals to inform parents about their child's hearing impairment. The purpose of this investigation was to document the status of graduates of one aural habilitation option; auditory-verbal. A consumer survey was completed by graduates from auditory-verbal programs in the United States and Canada. Graduates were queried regarding degree and etiology of hearing loss, age of onset, amplification, and educational and employment history, among other topics. Results indicated t

Cochlear Implant Training in Doctor of Audiology Programs: A Survey Study.

PURPOSE: As the population of cochlear implant (CI) candidates and recipients grows, there is a need for more audiologists to meet their clinical needs. Little information exists regarding the perception of recent doctorate of audiology (AuD) graduates and the training they received related to CI. METHOD: We surveyed audiologists who graduated from an accredited AuD program between 2017 and 2023 and asked them several questions about their level of satisfaction regarding their AuD studies. The survey included questions about current work setting, areas of rotation available, and instructor ch

Limitations of Conventional Hearing Aids: Examining Common Complaints and Issues that Can and Cannot Be Remedied.

This article examines and evaluates methods, from an audiologist's perspective, of reducing common complaints with conventional hearing aids and issues such as the occlusion effect, acoustic feedback, discomfort, and insufficient gain. Although often successful, reducing one problem may have the tradeoff of causing another issue. This article is meant to provide information to the reader regarding modern conventional hearing aids, the means to alleviate common problems in the clinic, and when middle ear implants and osseointegrated implants can be beneficial.

Meniere's disease: identifying classic symptoms and current treatments.

Meniere's disease affects 615,000 individuals in the United States. Therefore, occupational health nurses may encounter employees who express symptoms of Meniere's disease or have been diagnosed with this problem. When nurses have an understanding of the symptoms of Meniere's disease, they can validate clients' concerns by directing them to an otolaryngologist or audiologist for further evaluation. Awareness of Meniere's disease may aid in counseling individuals during their annual education and training that is required to maintain an effective hearing conservation program.

The Tinnitus Handicap Inventory as a screening test for psychiatric comorbidity in patients with tinnitus.

BACKGROUND: Psychiatric comorbidity is common in patients who seek help for tinnitus. The perceived severity of tinnitus correlates closer to psychological and general health factors than to audiometrical parameters. Audiologists need valid screening tools in order to identify patients with psychiatric disorders and to tailor treatment in a multidisciplinary setting. The tinnitus handicap inventory (THI) has gained widespread acceptance as a self-report measure of tinnitus handicap. In several studies, THI test score correlates with the level of psychopathologic distress. The aim of our study

Internet-Based Intervention for Tinnitus: Outcome of a Single-Group Open Trial.

BACKGROUND: Managing chronic tinnitus is challenging, and innovative ways to address the resulting health-care burden are required. Internet-based cognitive behavioral therapy (iCBT) for tinnitus shows promise as a cost-effective treatment option. The feasibility and effectiveness of iCBT in the United Kingdom are yet to be explored. Furthermore, it is not known if iCBT can be supported by an audiologist rather than a psychologist. PURPOSE: This study aimed to determine the feasibility of guided iCBT using audiological support on tinnitus distress and tinnitus-related comorbidities. Furthermo

Remote Learning for Children with Auditory Access Needs: What We Have Learned during COVID-19.

COVID-19 has challenged most everyone in every facet of life. In the beginning of the pandemic shutdown, schools had to make decisions quickly often with limited planning. For students who were deaf or hard of hearing, communication access during instruction was the urgent focus of teachers of the deaf/hard of hearing and educational audiologists. The move from the classroom to home resulted in both predictable and unpredictable challenges as well as some unexpected benefits. Based on numerous conversations with these professionals as well as parents, the challenges encountered with online lea

Neurofibromatosis type 2: The future holds hope.

Progress in molecular biology has provided us with new understanding of neurofibromatosis type 2. The diagnosis of the condition is based on a number of clinical and radiological findings, which include the use of the scanner and MRI. The treatment objective is to preserve neurological function for as long as possible, with a minimum of surgical intervention. The dilemma is particularly great as far as the hearing is concerned. The care of these patients and their families must be carried out in highly specialised multi-disciplinary centres which have available the services of neuro-otologists

[Current diagnostics and treatment options for tinnitus].

Tinnitus is a common condition with great variability in the intensity of symptomatology. In recent years, more and more insights have been gained into the mechanism of tinnitus and its relationship with hearing loss and other factors such as stress. Depending on the symptoms and clinical findings, a patient may be referred to an ENT specialist or audiologist. For the majority of patients, re-assurance and providing good information is sufficient. For others improving hearing and possibly cognitive behavioral therapy remains the most important pillars for treatment of tinnitus. A number of exp

Audiologic assessment of children with suspected hearing loss.

In assessing children with suspected hearing loss, audiologists are faced with the challenge of attempting to describe auditory function in a population that presents with a wide range developmental levels and abilities. Fortunately, there are a number of behavioral and physiologic tools available for the evaluation of auditory sensitivity and system integrity in children of all ages and developmental abilities. As powerful as many of these tools may be, however, it is important that the diagnosis of hearing loss be based on the outcome of independent measures of auditory function, which serve

Translation and validation of the Tinnitus Functional Index (TFI) into Kannada: a tool for clinical and research use in the Indian population.

PURPOSE: The Tinnitus Functional Index (TFI) is a widely recognized measure for evaluating tinnitus and is frequently used in both therapeutic and research contexts. Unfortunately, TFI is currently not available in any Indian languages. In a multilingual country like India, cross-cultural adaptation, translation, and validation of such scales in regional languages are critical to improving their use for clinical and research purposes. Therefore, this study focused on translating and validating the TFI in Kannada for use among the Indian population. MATERIALS AND METHODS: The original version
