MRI findings of obstructive azoospermia: lesions in and out of pelvic cavity.

Obstructive azoospermia (OA) is caused by excurrent duct obstruction, which can occur anywhere along the course of the male reproductive tract and is classically characterized by normal spermatogenesis. To be familiar with the imaging anatomy of normal and abnormal male genital ducts is essential to the diagnosis of OA. In some circumstances, OA can also be related to some specific syndromes; thus, making an accurate diagnosis may require an integral view of the whole abdomen and pelvis. MR is a great complementary imaging modality either for the detection of obstructive factors, especially fo

MicroRNAs association with azoospermia, oligospermia, asthenozoospermia, and teratozoospermia: a systematic review.

Infertility is a major health problem across the world. One of the main reasons for male infertility are defects in sperm. Semen analysis is the most common test utilized to evaluate male fertility and since it suffers from multiple drawbacks, reproduction scientists have tried to find new molecular markers for detecting sperm defects. MicroRNAs (miRNAs) are small molecules in cells which take part in regulating gene expression. Various studies have confirmed miRNAs to have a role in defining multiple sperm characteristics, including sperm count, motility, and morphology. In this paper, we hav

[Surgical testicular extraction of spermatozoa in men with non-obstructive azoospermia].

Non-obstructive azoospermia (NOA) is a severe form of male infertility. The only option to help men with NOA to become biological fathers is to surgically extract spermatozoa from the testicles, and in this review different modalities are discussed. Microdissection testicular sperm extraction seems to achieve better sperm retrieval rates compared with both testicular sperm aspiration and testicular sperm extraction. However, there are significant limitations in the current literature, and without prospective randomised trials it is not possible to define the optimal sperm retrieval technique f

Reassessing reconstruction in the management of obstructive azoospermia: reconstruction or sperm acquisition?

Treatments for male factor infertility secondary to reconstructable obstructive azoospermia include either surgical reconstruction or direct sperm retrieval. We examine the risks and benefits of both types of therapies and discuss their respective medical and economic implications. Most male factor infertility studies comparing vasectomy reversal with sperm retrieval favor the former as the more cost-effective therapy for obstructive azoospermia. Analysis should include assessment of direct procedural costs and indirect costs, including the cost of complications, lost productivity, and multipl

ERCC1 and ERCC2 polymorphisms and risk of idiopathic azoospermia in a Chinese population.

The ERCC1 (excision repair cross complementation group 1) and ERCC2 (excision repair cross complementation group 2) genes are important in repairing DNA damage and genomic instability in germ cells, and are essential for normal spermatogenesis. It has been verified that polymorphisms of these two genes could alter DNA repair capacity in some phenotypic studies. However, little information is available on these polymorphisms in male infertility. This study was designed to examine whether ERCC1 polymorphisms 3 UTR (C8092A), Asn118Asn (G19007A) and ERCC2 polymorphisms Asp312Asn (G-->A), Lys751Gln

8.5 Mb deletion at distal 5p in a male ascertained for azoospermia.

We report on a 5p- azoospermic male not showing the clinical features diagnostic for the cri-du-chat syndrome but for a breathy, raspy voice. The 5p deletion breakpoint, determined by fluorescent in situ hybridization (FISH) analysis with BAC clones, maps 8.5 Mb far from the short arm telomere in 5p15.31. Genotype/phenotype correlations in this subject, including his neuropsychological assessment, led us to define that the gene for the cat-like cry and one gene responsible for mild mental retardation with speech delay map both in the distal 8.5 Mb of chromosome 5 short arm.

[Mutations and polymorphisms in CFTR genes in infertile men with oligospermia or azoospermia].

INTRODUCTION: Impaired infertility of the male partner is causative or contributOry to in up to one half of all couples unable to conceive spontaneously. A considerable number of genes are now known that have an essential function in human reproduction and which, when deleted or mutated, can cause pathologic changes in the male reproductive system. Congenital bilateral absence of the vas deferens (CBAVD) is an important cause of obstructive azoospermia in otherwise healthy men. It is also present in 95% of men with an autosomal recessive systematic disease--cystic fibrosis. However, clinically

The treatment of obstructive azoospermia in male infertility--past, present, and future.

Until recently, the primary treatment option for infertile men with obstructive azoospermia was the reconstruction of the male seminal tract when the causes of obstruction were reconstructable. For unreconstructable causes, such as congenital absence of the vas deferens, the primary treatment option involved implantation of an alloplastic artificial spermatocele for subsequent percutaneous retrieval of sperm. Retrieved sperm was then used for intrauterine insemination. The introduction of in vitro fertilization (IVF), performed together with microsurgical epididymal sperm aspiration (MESA), pr

Primary testicular disease presenting as azoospermia or oligozoospermia in an infertility clinic.

A group of 173 patients who made up 23% of a population of patients attending a male infertility clinic with primary testicular disease were examined; 63 patients (36%) were azoospermic and 110 (64%) were oligozoospermic. Their aetiology and clinical features are described, together with the endocrine findings, white blood cell karyotypes and, where available, the histology seen on a testicular biopsy. The relationship of these lesions with the presence or absence of a varicocele is discussed.

[Pregnancy by micromanipulation in a case of excretory azoospermia associated with a natural spermatocele].

Microinjection of capacitated sperm into the perivitelline space of oocytes has been performed in a case of male infertility attributed to excretory azoospermia. In this particular case, a spermatocele detected five years ago was observed, which provide motile spermatozoa. These sperm collected by direct puncture into the cyst, were capacitated using discontinuous Percoll gradient. After ovarian stimulation, seventeen oocytes were collected. Nine of them were used for classical IVF whereas the other eight were submitted to microinjection. Only one oocyte of the second group fertilized and clea

Temporary intraabdominal cryptorchidism in the weanling rat leads to irreversible azoospermia.

To study surgical reversibility of intraabdominal cryptorchidism, a study in which weanling rats were made artificially cryptorchid was undertaken. At the same time both sperm ducts were anastomosed into the urine bladder (vasocystostomy) to enable sperm output measurements, which were performed by putting the animals in metabolic cages for 24 hr. Group I animals were reoperated on 2 weeks after the initial cryptorchidism operation to reverse this intervention by an orchiopexy operation; in Group II orchiopexy was carried out after 4 weeks, in Group III after 8 weeks. In Group IV, a control gr

Gynaecomastia and azoospermia as sole presenting symptoms of feminizing adrenal tumor.

Adrenal feminizing tumours are rare but frequently malignant. A case of adrenal feminizing tumour in a 35-year-old man, which presented with gynaecomastia and infertility due to azoospermia, without loss of libido and sexual potency is reported in this paper. Our patient represents a relatively unusual case. The long free interval from the excision allows us to consider the patient cured as despite the pathological malignant features of the neoplasm.

Reversal of Radioiodine-Induced Azoospermia in Thyroid Cancer: A Case Report.

INTRODUCTION: Radioiodine-131 (131I) is a commonly used adjuvant therapy following surgery for differentiated thyroid cancer. The nonspecific radiation damage may affect the male reproductive system, impacting sperm quality. However, reports of azoospermia are rare. CASE PRESENTATION: A 26-year-old male patient with papillary thyroid carcinoma underwent cumulative 14.06 GBq 131I ablation therapy postoperatively. Eight months post-treatment, he was diagnosed with azoospermia due to planned conception. After ruling out congenital factors, 131I-related iatrogenic injury was considered. Sperm wer

Novel anoikis-related genes for the diagnosis of non-obstructive azoospermia.

BACKGROUND: Non-obstructive azoospermia (NOA) is a prevalent cause of male infertility, featured by the absence of sperm in the ejaculate due to impaired spermatogenesis. The involvement of anoikis in the pathogenesis of NOA remains inadequately understood. This research aims to identify anoikis-related genes as potential biomarkers for NOA diagnosis. METHODS: Based on the Gene Expression Omnibus (GEO) database and Limma R package, we identified differentially expressed genes of NOA and downloaded anoikis-related genes based on the GeneCards database. Subsequently, anoikis-related hub genes w

An integrated approach for improving clinical management of non-obstructive azoospermia.

BACKGROUND: Non-obstructive azoospermia is the most severe form of male infertility. A testicular biopsy is required for the diagnosis of non-obstructive azoospermia, and the causal factors for non-obstructive azoospermia remain unknown. OBJECTIVES: To reduce the risk of multiple biopsies and identify factors that contribute to non-obstructive azoospermia, we proposed an integrated approach for the preoperative diagnosis and clinical management of non-obstructive azoospermia by applying the chromosome-spreading technique and whole-exome sequencing. MATERIALS AND METHODS: Between July 2020 an

A homozygous missense variant in YTHDC2 induces azoospermia in two siblings.

Male infertility is a complex multifactorial reproductive disorder with highly heterogeneous phenotypic presentations. Azoospermia is a medically non-manageable cause of male infertility affecting ∼1% of men. Precise etiology of azoospermia is not known in approximately three-fourth of the cases. To explore the genetic basis of azoospermia, we performed whole exome sequencing in two non-obstructive azoospermia affected siblings from a consanguineous Pakistani family. Bioinformatic filtering and segregation analysis of whole exome sequencing data resulted in the identification of a rare homozyg

Primary mediastinal seminoma with azoospermia: case report and review of the literature.

INTRODUCTION: Since the first report, primary mediastinal seminoma has a low incidence in the population, and it mainly affects young and middle-aged men, is clinically rare, and accounts for a very small proportion of mediastinal tumors. In this study, we describe the first case of primary mediastinal seminoma with azoospermia and hypothesize that the coexistence of the two disorders may not be a coincidence. CASE REPORT: A 16-year-old man presented with chest tightness and chest pain, a mediastinal mass on chest CT, and abnormal 18F-fluoro-deoxyglucose uptake on a PET-CT scan. By biopsy of
