De novo anorexia nervosa post-bariatric surgery.

Metabolic and bariatric surgery is generally known to decrease rates of eating disorders post-procedure. In the relatively uncommon event that an eating disorder develops de novo or recurs after bariatric surgery, most cases are associated with suboptimal weight loss or further weight gain. This case report presents the history of a 56-year-old woman without a prior eating disorder diagnosis who developed anorexia nervosa restricting type after bariatric surgery, leading to psychiatric hospitalization. Implications for surgical decision-making in tandem with psychiatric screening pre-surgicall

Metabolic Bariatric Surgery in Kidney Transplant Candidates.

Patients with obesity are the largest and fastest-growing demographic among individuals with renal failure in the United States. Transplant centers often reject patients with high body mass index due to the risk of posttransplant complications. Metabolic bariatric surgery, the most efficient and durable weight loss method, can facilitate a timely kidney transplant listing and improve obesity-related comorbidities in a relatively safe manner. In this review, we identify kidney transplant candidates who are likely to benefit from metabolic bariatric surgery, discuss the potential risks of the pr

Psychopathological profile before and after bariatric surgery.

Presurgical psychopathological assessment usually focuses on detecting severe mental disorders. However, mild intensity psychopathology and eating behaviour pattern may also influence postsurgical outcomes. The aim was to identify psychopathology and eating behaviour pattern in candidates prepared for bariatric surgery compared to a normative population before and after surgery. A cohort of 32 patients seeking bariatric surgery in a university hospital between March 2016 and March 2017 were evaluated with Personality Assessment Inventory (PAI), 36-item EDE-Q and BES before and after surgery. Vitamin D Concentration Changes after Bariatric Surgery.

INTRODUCTION: Bariatric surgery causes physiological and anatomical changes in the gastrointestinal tract that interfere with intestinal absorption and, consequently, with the nutritional status, especially about vitamin D. The aim of the study was to evaluate the vitamin D levels and body composition of these patients in the Roux-en-Y gastric bypass (RYGB) and vertical sleeve gastrectomy (VSG) types of surgery. METHODS: The retrospective cohort study included a population of 120 patients aged between 18 and 65 years, with class II or III obesity, undergoing bariatric surgery procedures (VSG

Alterations of digestive motility after bariatric surgery.

Bariatric surgery can induce changes in digestive motility that are de novo or secondary to an improvement or aggravation of previous disorders due to obesity. Alterations of digestive motility are frequently part of the mechanism of action and a result of surgery. They are not rare and they are not always associated with an increase in weight loss but can lead to the negative consequences on quality of life, which are more or less reversible as a real surgical complication. Knowledge of these complications has become essential, especially in this period when bariatric surgery often concerns p

Sleep Quality Before and After Bariatric Surgery.

PURPOSE: Sleep deprivation is associated with growth hormone deficiency and an elevated cortisol level, both of which have been linked to obesity. However, assessing sleep quality is often not established in the multidisciplinary peribariatric evaluation program. This study aimed to determine sleep quality in patients who are seeking or underwent bariatric surgery by using Pittsburgh Sleep Quality Index (PSQI) measurements. MATERIALS AND METHODS: In this observational study, patients who underwent or were seeking bariatric surgery between April and November 2021 were included. Self-reported p

Concomitant versus Delayed Cholecystectomy in Bariatric Surgery.

BACKGROUND: Obesity and weight loss after bariatric surgery have a close association with gallbladder disease. The performance and proper timing of laparoscopic cholecystectomy (LC) with bariatric surgery remain a clinical question. OBJECTIVE: Evaluation of the outcome of LC during bariatric surgery whether done concomitantly or delayed according to the level of intraoperative difficulty. METHODS: The prospective study included patients with morbid obesity between December 2018 and December 2019 with preoperatively detected gallbladder stones. According to the level of difficulty, patients w

Micronutrients deficiences in patients after bariatric surgery.

Bariatric surgery is an effective option for managing obesity and has gained general acceptance among patients in recent years. Generally, despite the high caloric intake, a bad nutritional habit of obese people results in the deficiency of several vitamins, minerals, and trace elements essential for body metabolism and normal physiological processes. Additionally, the current bariatric surgical approaches such as sleeve gastrectomy (SG), Roux-en-Y-gastric bypass (RYGB), laparoscopic adjustable gastric banding (LAGB), and jejunoileal bypass (JIB) can cause or exacerbate these deficiencies. Diet approach before and after bariatric surgery.

Bariatric surgery (BS) is today the most effective therapy for inducing long-term weight loss and for reducing comorbidity burden and mortality in patients with severe obesity. On the other hand, BS may be associated to new clinical problems, complications and side effects, in particular in the nutritional domain. Therefore, the nutritional management of the bariatric patients requires specific nutritional skills. In this paper, a brief overview of the nutritional management of the bariatric patients will be provided from pre-operative to post-operative phase. Patients with severe obesity ofte

Five-Year Outcomes in Bariatric Surgery Patients.

Background and objectives: Obesity presents as a multifactorial, pandemic disease that arises as a consequence of unequal energy intake and energy consumption. Obesity adversely affects the quality of life, leading not only to disability, but also to various other disorders. Bariatric surgery is the most effective method for achieving significant and sustained weight loss in individuals with extreme obesity. The aim of this study was to examine how well surgically induced weight loss is maintained after five years of follow-up and its effects on cardiovascular risk factors and outcome. Materia

Bone Health Following Bariatric Surgery: An Update.

Obesity rates are increasing rapidly, and bariatric surgery is currently the most effective tool for weight loss. Recently, bariatric surgery induced bone loss has gained attention. Such detrimental effect on bone is multifactorial and causes may include nutrient deficiencies, gut and gonadal hormonal changes, mechanical unloading, loss of lean mass, increased bone marrow fat, and increased risk of fall. This review describes the available evidence on bone loss and fracture risk following bariatric surgery and summarizes the guidelines on the topic. Increased bone resorption starts early posts

Reduction of intraocular pressure after bariatric surgery.

PURPOSE: To investigate the effects of bariatric surgery on intraocular pressure (IOP). METHODS: We prospectively enroled consecutive obese patients who underwent a laparoscopic sleeve gastrectomy procedure. We measured IOP prior to and 3-6 months following surgery and recorded medical and demographical parameters. RESULTS: Thirty-two patients completed all evaluations. Mean age was 40.5 ± 12 and 24 (75%) were men. Following surgery, mean body-mass index (BMI) decreased from 42 ± 6 to 31 ± 8 (p < 0.001). The mean IOP decreased from 16.9 ± 4 mmHg to 14.1 ± 3 mmHg (p < 0.001). The extent of IO

Managing Malnourishment in Pregnancy after Bariatric Surgery.

BACKGROUND: Little information exists to guide monitoring and treatment of malnourishment during pregnancy after bariatric surgery. Here we present a case with severe deficiencies and recommendations for testing and treatment. CASE: Our patient underwent a duodenal switch procedure resulting in significant weight loss and numerous deficiencies. She then experienced a neonatal demise with multiple congenital abnormalities, including diaphragmatic hernia, possibly related to severe vitamin A deficiency. After high doses of oral and parenteral replacement, pancreatic enzymes, and total parentera

Endoscopic management of post-bariatric surgery complications.

Understanding the technical constructs of bariatric surgery is important to the treating endoscopist to maximize effective endoluminal therapy. Post-operative complication rates vary widely based on the complication of interest, and have been reported to be as high as 68% following adjustable gastric banding. Similarly, there is a wide range of presenting symptoms for post-operative bariatric complications, including abdominal pain, nausea and vomiting, dysphagia, gastrointestinal hemorrhage, and weight regain, all of which may provoke an endoscopic assessment. Bleeding and anastomotic leak ar

[Routine fluoroscopic investigations after primary bariatric surgery].

BACKGROUND: Staple line and anastomotic leakages are life-threatening complications after bariatric surgery. Upper gastrointestinal (GI) tract X-ray examination with oral administration of a water-soluble contrast agent can be used to detect leaks. The aim of this study was to evaluate the impact of routine upper GI tract fluoroscopy after primary bariatric surgery. METHODS: Between January 2009 and December 2014 a total of 658 bariatric interventions were carried out of which 442 were primary bariatric operations. Included in this single center study were 307 sleeve gastrectomies and 135 Rou

Gastric cancer following bariatric surgery: a review.

BACKGROUND: Bariatric procedures can induce a massive weight loss that lasts for >15 years after surgery; in addition, they achieve important metabolic effects including diabetes resolution in the majority of morbidly obese patients. However, some bariatric interventions may cause gastroesophageal reflux disease and other serious complications. The aim of our study is to evaluate the risk of cancer after bariatric surgery. METHODS: We conducted a review of the literature about the cases of gastric cancer arising after any bariatric procedure, including a case of adenocarcinoma incidentally di

Obesity: long-term management after bariatric surgery.

For many patients, bariatric surgery results in enduring weight loss and comorbidity resolution, but it is not without risk of complications. Complications necessitating possible surgical intervention include small bowel obstruction, internal hernia, intussusception, anastomotic ulcer, and anastomotic leak. Medical complications include dumping syndrome; osteoporosis; substance abuse; nephrolithiasis; high suicide rates; and nutritional deficiencies, some of which can result in neurologic disorders. Patients need to take daily multivitamins with minerals (containing iron, copper thiamine, and

Complications from micronutrient deficiency following bariatric surgery.

We report a case of clinically significant micronutrient deficiencies following biliary pancreatic diversion (BPD) surgery. Our patient was admitted to hospital six years after BPD surgery following a low impact humeral fracture complicated by postoperative wound infection. On presentation she complained of a widespread rash and loss of night vision. Laboratory testing confirmed hypoalbuminaemia, deficiencies of vitamins A, E and D and of the trace elements copper, zinc and selenium. Bone densitometry confirmed osteoporosis. The skin rash was thought to be due to zinc deficiency and improved w
