Cardiovascular fingolimod effects on rapid baroreceptor unloading are counterbalanced by baroreflex resetting.

BACKGROUND AND PURPOSE: Initial cardiovascular fingolimod effects might compromise baroreflex responses to rapid blood pressure (BP) changes during common Valsalva-like maneuvers. This study evaluated cardiovascular responses to Valsalva maneuver (VM)-induced baroreceptor unloading and loading upon fingolimod initiation. PATIENTS AND METHODS: Twenty-one patients with relapsing-remitting multiple sclerosis performed VMs before and 0.5, 1, 2, 3, 4, 5, and 6 hours after fingolimod initiation. We recorded heart rate (HR) as RR intervals (RRI), systolic and diastolic BP (BPsys, BPdia) during VM ph

Noiseless Variable-Pressure Neck Chamber Device to Assess the Carotid Baroreflex Function.

Background: The blood pressure responses to baroreflex perturbations can be assessed only using the variable-pressure neck chamber technique. However, the application of this approach in hospital environments is limited owing to the loud noise emitted during its operation. This study was aimed at developing a noiseless neck suction chamber device (NCD) that could stimulate the baroreceptors located in the carotid sinus in humans. Methods: A non-invasive device was developed to pressurize the carotid arteries externally. A microcontroller with a computer interface and neck chamber (3D-printed)

Standardized Autonomic Testing in Patients With Probable Radiation-Induced Afferent Baroreflex Failure.

Injury of the afferent limb of the baroreflex from neck radiation causes radiation-induced afferent baroreflex failure (R-ABF). Identification and management of R-ABF is challenging. We aimed to investigate the pattern of autonomic dysfunction on standardized autonomic testing in patients with probable R-ABF. We retrospectively analyzed all autonomic reflex screens performed at Mayo Clinic in Rochester, MN, between 2000 and 2020 in patients with probable R-ABF. Additional tests reviewed included ambulatory blood pressure monitoring, plasma norepinephrine, and thermoregulatory sweat test. We id

Tracking autonomic balance using an open-loop model of the arterial baroreflex.

Blood pressure control is vital for maintaining adequate perfusion of the brain and other organs in the body across varying physiological demands, and the arterial baroreceptor reflex (baroreflex) is the major short-term blood pressure control loop mediated by the autonomic nervous system (ANS). Accurate quantitative models of the baroreflex would provide physiological insight and could allow for real-time tracking of ANS activity in clinical settings. In this work, we formulate a causal, parametric beat-to-beat model, relating systolic blood pressure (input) to heart rate (output). Model stru

Baroreflex control of renal sympathetic nerve activity in mice with cardiac hypertrophy.

Altered renal sympathetic nerve activity (RSNA) plays a major role in the progression of cardiac hypertrophy. We aimed to evaluate the baroreflex control of RSNA in mice with cardiac hypertrophy. Swiss Webster mice were treated with isoproterenol (15 μg/g/day, s.c.) or vehicle and the baroreflex evaluation was performed by measuring changes in RSNA in response to changes in arterial pressure. The maximal gain of the reflex changes in RSNA was reduced in isoproterenol-treated animals (1.39 ± 0.08%/mm Hg) in comparison with vehicle-treated animals (1.77 ± 0.10%/mm Hg). Therefore, we can conclude

Baroreflex failure syndrome: a rare complication of bilateral carotid body tumor excision.

Baroreflex failure syndrome is a rare disorder seen after bilateral carotid body tumor resection. Iatrogenic injuries to the baroreceptor reflex arc cause fluctuations in blood pressure with hypertensive attacks or hypotensive episodes. A 43-year-old woman underwent bilateral carotid body tumor resection with one-week interval for a hypervascular tumor, 78 x 50 x 45 mm in size, at the right carotid artery bifurcation and a smaller tumor (50 x 30 x 20 mm) in the contralateral neck. Blood pressure of the patient became significantly unstable after excision of the second tumor, with hypertensive

Radiation-induced cranial neuropathies manifesting as baroreflex failure and progressive bulbar impairment.

INTRODUCTION: Baroreflex failure and bulbar palsy have been rarely reported as potential complications of radiotherapy for head and neck tumors. However, the occurrence of both disorders in the same patient has not been well described. We present a unique patient with radiation-induced cranial neuropathies, resulting in bulbar palsy and baroreflex failure. CASE REPORT: A 67-year-old man underwent surgical resection of tonsillar squamous cell carcinoma and postoperative radiation therapy. Six years later, he developed severe orthostatic hypotension with syncope, followed later by progressive b

Reflex control of sympathetic outflow and depressed baroreflex sensitivity following myocardial infarction.

Reflex control of heart rate is frequently impaired following myocardial infarction. This is referred to as depressed baroreflex sensitivity. The aim of these experiments was to assess the function of other autonomic reflexes in dogs with depressed baroreflex sensitivity. Comparisons were made to dogs in whom baroreflex sensitivity was preserved or unchanged after myocardial infarction. Under chloralose-barbiturate anesthesia, reflex control of sympathetic outflow by the sinoaortic baroreceptors was determined by measurement of changes in systolic arterial pressure and efferent renal sympathet

Modulation of arterial baroreflex dynamic response during mild orthostatic stress in humans.

We tested the hypothesis that in humans, carotid-baroreflex dynamic responses (evaluated by examining the time course of the carotid-baroreflex-induced alterations in muscle sympathetic nerve activity (MSNA), mean arterial blood pressure (MAP) and heart rate (HR)) would be altered during mild orthostatic stress in ways that serve to limit orthostatic hypotension. In 12 healthy subjects (10 male, 2 female), 5-s periods of neck pressure (NP) (50 mmHg) and neck suction (NS) (-60 mmHg) were used to evaluate carotid baroreflex function at rest (CON) and during lower body negative pressure (LBNP) (-

Baroreflex control of muscle sympathetic nerve activity after carotid body tumor resection.

Bilateral carotid body tumor resection causes a permanent attenuation of vagal baroreflex sensitivity. We retrospectively examined the effects of bilateral carotid body tumor resection on the baroreflex control of sympathetic nerve traffic. Muscle sympathetic nerve activity was recorded in 5 patients after bilateral carotid body tumor resection (1 man and 4 women, 51+/-11 years) and 6 healthy control subjects (2 men and 4 women, 50+/-7 years). Baroreflex sensitivity was calculated from changes in R-R interval and muscle sympathetic nerve activity in response to bolus injections of phenylephrin

Effects of repeated Valsalva maneuver straining on cardiac and vasoconstrictive baroreflex responses.

INTRODUCTION: We hypothesized that repeated respiratory straining maneuvers (repeated SM) designed to elevate arterial BPs (arterial baroreceptor loading) would acutely increase baroreflex responses. METHODS: We tested this hypothesis by measuring cardiac baroreflex responses to carotid baroreceptor stimulation (neck pressures), and changes in heart rate and diastolic BP after reductions in BP induced by a 15-s Valsalva maneuver in 10 female and 10 male subjects at 1, 3, 6, and 24 h after performing repeated SM. Baroreflex responses were also measured in each subject at 1, 3, 6, and 24 h at t

Reduced cardiovagal baroreflex gain in visceral obesity: implications for the metabolic syndrome.

The influence of excess total and abdominal adiposity on cardiovagal baroreflex gain remains unclear. We tested the hypotheses that cardiovagal baroreflex gain would be reduced in men with 1) higher [higher fat (HF), mass >20 kg, n = 11] compared with lower [lower fat (LF), mass <20 kg, n = 10] levels of total body and abdominal fat and 2) higher abdominal visceral fat (HAVF; n = 10) compared with total body weight- and subcutaneous fat-matched peers with lower abdominal visceral fat (LAVF; n = 7) levels. To accomplish this, we measured cardiovagal baroreflex gain (modified Oxford technique),

Spontaneous baroreflex sensitivity: toward an ideal index of cardiovascular risk in hypertension?

OBJECTIVE: Estimating the risk entailed by classical risk factors like blood pressure (BP) or serum cholesterol may be difficult because of their variability and the often unknown duration of exposure. Having variables integrating the impact of those classical risk factors on the cardiovascular system would probably aid the prediction of cardiovascular events. The present study aimed at determining whether cardiac baroreflex sensitivity (BRS), correlates with several risk factors and thus is a good candidate for being such an integrative variable. As a comparison, left ventricular mass (LVM),

Influence of rostral ventrolateral medulla on renal sympathetic baroreflex in conscious rabbits.

Previous studies with anesthetized animals have shown that the pressor region of the rostral ventrolateral medulla (RVLM) is a critical site in vasomotor control. The aim of this study was to develop, in conscious rabbits, a technique for microinjecting into the RVLM and to determine the influence of this area on renal sympathetic nerve activity (RSNA) and arterial pressure (AP) using local injections of glutamate, rilmenidine, ANG II and sarile. Rabbits were implanted with guide cannulas for bilateral microinjections into the RVLM (n = 7) or into the intermediate ventrolateral medulla (IVLM,

Cardiac sympathetic overactivity and decreased baroreflex sensitivity in L-NAME hypertensive rats.

The present study evaluated the possible changes in the autonomic control of heart rate in the hypertensive model induced by the inhibition of nitric oxide synthase. Rats were treated with N(G)-nitro-L-arginine methyl ester (L-NAME group) in the drinking water during 7 days, whereas control groups were treated with tap water (control group) or with the N(G)-nitro-D-arginine methyl ester (D-NAME group), an inactive isomer of the L-NAME molecule. The L-NAME group developed hypertension and tachycardia. The sequential blockade of the autonomic influences with propranolol and methylatropine indica

Head-down bed rest impairs vagal baroreflex responses and provokes orthostatic hypotension.

We studied vagally mediated carotid baroreceptor-cardiac reflexes in 11 healthy men before, during, and after 30 days of 6 degrees head-down bed rest to test the hypothesis that baroreflex malfunction contributes to orthostatic hypotension in this model of simulated microgravity. Sigmoidal baroreflex response relationships were provoked with ramped neck pressure-suction sequences comprising pressure elevations to 40 mmHg followed by serial R-wave-triggered 15-mmHg reductions to -65 mmHg. Each R-R interval was plotted as a function of systolic pressure minus the neck chamber pressure applied du

Cocaine inhibits baroreflex control of blood pressure by actions at arterial baroreceptors.

Blood pressure and heart rate often increase during cocaine intoxication, but the mechanisms of these cardiovascular responses are poorly understood. The most often suggested theories are central nervous system mechanisms involving the blockade of neuronal transmitter uptake. Cocaine also has potent local anesthetic properties, and in this study we tested the possible role of peripheral actions of cocaine at baroreceptor afferents. Single fiber baroreceptors were recorded using an in vitro preparation of the rat aortic arch. Diameter, pressure, and baroreceptor discharge were recorded. Cocaine

Baroreflex effects of althesin infusions to supplement nitrous oxide anaesthesia in man.

The effects on baroreflex control of heart rate, of two infusion rates of Althesin (18 and 36 micrograms kg-1 min-1 as total steroid) to supplement 66% nitrous oxide in oxygen anaesthesia, have been studied in six younger (mean age 35.5 yr) and six older (mean age 61.0 yr) patients. Steady-state anaesthesia at both infusion rates was associated with diminished baroreflex sensitivity and resetting of the reflex to allow lower arterial pressures to be maintained without tachycardia.

The effects of limb suction during arterial occlusion on arterial baroreflex in humans.

Peripheral venous distension evokes a reflex sympathetic activation [venous distension reflex (VDR)]. Previous study showed that venous distension in a forearm with a volume infusion raised sympathetic baroreflex sensitivity (SBRS). However, the effects of venous distension on baroreflex sensitivity (BRS) in the lower limb have not been examined. We hypothesized that the venous distension in an upper or lower limb by the negative pressure would alter BRS. In 20 healthy subjects, one arm or one leg of the subjects was placed into an airtight pressure tank, although a cuff was placed proximally
