Opioid toxicity: histamine, hypersensitivity, and MRGPRX2.

Insights into the pathophysiology of many non-immune-mediated drug reactions referred to as toxicities, sensitivities, intolerances, or pseudoallergies have resulted from research identifying the mastocyte-related G-protein-coupled receptor (GPCR) member X2 (MRGPRX2), a human mast cell receptor mediating adverse reactions without the involvement of antibody priming. Opioid-induced degranulation of mast cells, particularly morphine, provoking release of histamine and other preformed mediators and causing hemodynamic and cutaneous changes seen as flushing, headache and wheal and flare reactions

[Diagnostic possibilities in drug allergies].

Drug allergies can be subclassified into three subgroups, which differ in their pathophysiology and require different diagnostic steps: (1.) classical drug allergies, which are directed to the drug itself, a reactive compound of the drug, or some contamination of it; (2.) pseudo-allergic reactions, which are caused by non-immune mediated degranulation of mast cells and basophils, and (3.) autoimmune reactions, in which the drug elicits an immune reaction to autologous structures. A very detailed (criminalistic) history has the highest priority for clarification of a suspected drug allergy. In

Evaluation of recombinant MGL_1304 produced by Pichia pastoris for clinical application to sweat allergy.

BACKGROUND: We previously identified MGL_1304 secreted by Malassezia globosa as a sweat antigen for patients with atopic dermatitis (AD) and cholinergic urticaria (ChU). However, purifying native MGL_1304 from human sweat or culture supernatant of M. globosa (sup-MGL_1304) is costly and time-consuming. Moreover, recombinant MGL_1304 expressed by using Escherichia coli (TF-rMGL_1304) needs a large chaperon protein and lacks the original glycosylation of yeasts. Thus, we generated a recombinant MGL_1304 by Pichia pastoris (P-rMGL_1304) and investigated its characteristic features. METHODS: Reco

Antigen-specific reduced IgG inhibits cellular response in rat basophilic leukemia cells activated with multivalent antigen.

OBJECTIVE: The aim of this study was to investigate whether antigen-specific reduced IgG inhibits mast cell activation evoked by the aggregation of the high-affinity IgE receptor (FcεRI) in a manner similar to intact IgG. MATERIALS: Rat basophilic leukemia (RBL-2H3) cells are used for a mast cell model. TREATMENT: Monovalent mouse anti-trinitrophenyl (TNP) IgG1 (75 kDa) was prepared by reducing the disulfide bond between the heavy chains using cysteamine hydrochloride. IgE-sensitized RBL-2H3 cells were stimulated with TNP-OVA, and reduced IgG1 was added 5 min after stimulation. METHODS: Deg

Hypersensitivity to antipyretics: pathogenesis, diagnosis, and management.

Antipyretics are commonly prescribed drugs and hypersensitivity occurs at rates of 0.01% to 0.3%. Hypersensitivity can be due to immune mechanisms that include type I to IV hypersensitivity. Type I hypersensitivity results from specific immunoglobulin E production following sensitisation on first exposure. Subsequent exposures elicit degranulation of mast cells, culminating an immediate reaction. Non-type I hypersensitivity is a delayed reaction that involves various effector cells, resulting in maculopapular rash, fixed drug eruptions, drug reaction with eosinophilia and systemic symptoms, an

Anaphylaxis to general anesthetics.

The incidence of hypersensitivity reactions to anesthetics is estimated 1 in 13,000 anesthetics up to 1 in 3,180. The rate of mortality ranges between 3 and 9%. 90% of reactions appear at anesthesia induction. Cardiovascular collapse and bronchospasm are more frequent in IgE-dependent reactions. The leading causes are neuromuscular blocking agents (50-70% of cases). IgE-dependent reactions are predominant. Previous sensitization by other compounds containing quaternary ions is suspected. Cross-reactions are frequent. Latex allergy is the second cause, followed by antibiotics and beta-lactams i

Inhibition of allergen-specific IgE reactivity by a human Ig Fcgamma-Fcepsilon bifunctional fusion protein.

BACKGROUND: Coaggregating FcepsilonRI with FcgammaRII receptors holds great potential for treatment of IgE-mediated disease by inhibiting FcepsilonRI signaling. We have previously shown that an Fcgamma-Fcepsilon fusion protein, human IgG-IgE Fc fusion protein (GE2), could inhibit FcepsilonRI-mediated mediator releases in vitro and in vivo. OBJECTIVE: We sought to test whether GE2 was capable of blocking mediator release from FcepsilonRI cells sensitized with IgE in vivo or in vitro before exposure to GE2, a critical feature for GE2 to be clinically applicable. METHODS: GE2 was tested for its

[A familial case of metabisulfite intolerance. Clinical and pharmacological study].

Within a context of a familial case of severe respiratory signs, with intolerance of sulfites and aspirin, we decided to study the clinical reliability of "in vitro" activation of human basophils in the presence of sulfites. Positivity of these tests for the three patients in the study (60, 73, and 66%) is confirmed by passive sensitization of donor basophils, which was destroyed when the serum was heated. This argument in favour of IgE dependence was strengthened by observation of an "in vitro" desensitization by an anti-IgE and not by an anti-IgG. These "in vitro" data are in agreement with

Fc-mediated immune stimulating, pro-inflammatory and antitumor effects of anti-HER2 IgE against HER2-expressing and trastuzumab-resistant tumors.

BACKGROUND: Anti-human epidermal growth factor receptor 2 (HER2) IgG1-based antibody therapies significantly improve cancer prognosis, yet intrinsic or acquired resistance to fragment antigen-binding (Fab)-mediated direct effects commonly occurs. Most resistant tumors retain antigen expression and therefore remain potentially targetable with anti-HER2 therapies that promote immune-mediated responses. Tumor-antigen-specific IgE class antibodies can mediate powerful immune cell-mediated effects against different cancers and have been shown to activate IgE Fc receptor-expressing monocytes. We pre

Physiochemical and functional evaluation of the first-in-class anti-cancer IgE antibody drug, MOv18, through process development and good manufacturing practice production.

Antibodies used for cancer therapy are monoclonal IgGs, but tumor-targeting IgE antibodies have shown enhanced effector cell potency against cancer in preclinical models. Research-grade recombinant IgE antibodies have been generated and studied for several decades. The recent Phase 1 clinical trial of the first-in-class MOv18 IgE, however, necessitated the inaugural process development and scaled manufacture of a recombinant IgE to clinical quality standards. During the process development and scaled Good Manufacturing Practice production, we demonstrate the retention of glycosylation state, b

Differentiating the cellular and humoral components of neuromuscular blocking agent-induced anaphylactic reactions in patients undergoing anaesthesia.

BACKGROUND: The significance of IgE antibodies to neuromuscular blocking agent (NMBA)-induced anaphylactic reactions during anaesthesia is unclear. We investigated the relevance of IgE to rocuronium using an in vitro technique. METHODS: Serum samples from 61 patients with anaphylactic reactions during anaesthesia were investigated. On the basis of clinical history, allergy to NMBA was considered likely in 48 patients, further assessed using intradermal skin tests for several commonly used NMBAs, including rocuronium, vecuronium, and succinylcholine. To determine the presence of rocuronium IgE

Anti-allergic activity of principles from the roots and heartwood of Caesalpinia sappan on antigen-induced beta-hexosaminidase release.

The dichloromethane extract of the roots and heartwood of Caesalpinia sappan exhibited potent inhibitory activity against beta-hexosaminidase release as marker of degranulation in rat basophilic leukemic (RBL-2H3) cells, with inhibition of 98.7% and 87.5% at concentration of 100 microg/ml, respectively. These extracts were further separated by chromatographic techniques to give two chalcones and seven homoisoflavones. Among the compounds tested, sappanchalcone (2) possessed the most potent effect against allergic reaction in RBL-2H3 cells with an inhibitory concentration (IC50) value of 7.6 mi

Atomic force microscopy to study direct neurite-mast cell (RBL) communication in vitro.

Communication between nerves and mast cells is a prototypic demonstration of neuroimmune interaction. We used an in vitro co-culture approach comprising cultured murine superior cervical ganglia (SCG) and rat basophilic leukemia (RBL-2H3) cells. Atomic force microscopy (AFM) showed how neurites attached to a pseudopodium or a cell body of an RBL cell. After stimulation of SCG neurites with bradykinin or scorpion venom, RBL cells attached to neurites spread and flattened, and several discharged granules (0. 5-1.0 microm in diameter) were found on the surface of the RBL cells. A neurokinin (NK)-

A recombinant Sal k 1 isoform as an alternative to the polymorphic allergen from Salsola kali pollen for allergy diagnosis.

BACKGROUND: The incidence of Amaranthaceae pollen allergy has increased due to the desertification occurring in many countries. In some regions of Spain, Salsola kali is the main cause of pollinosis, at almost the same level as olive and grass pollen. Sal k 1 - the sensitization marker of S. kali pollinosis - is used in clinical diagnosis, but is purified at a low yield from pollen. We aimed to produce a recombinant (r)Sal k 1 able to span the structural and immunological properties of the natural isoforms from pollen, and validate its potential use for diagnosis. METHODS: Specific cDNA was a

[Value of in-vitro diagnostic tools after anaphylaxis].

Anaphylaxis is a potentially life-threatening reaction and should be treated immediately. Diagnosis is made based on history and clinical features. However, measurement of sequential serum tryptase can provide additional clues and should be performed soon as possible. Referral to specialist allergy service is believed to reduce rate of recurrence. New specific in vitro techniques to indentify the triggering agent help optimize secondary preventive measures. Thus in vitro diagnosis plays an important, albeit complementary, role both in the acute and secondary phases facilitating a safe diagnosi

Simultaneous occurrence of chronic autoimmune urticaria and non-allergic asthma: a common mechanism?

Chronic urticaria is now considered as an autoimmune disorder due to histamine-releasing autoantibodies in 40-50% of cases. These histamine releasing-autoantibodies directed against the high affinity IgE receptor or against IgE can be detected in vivo by autologous serum skin test (ASST) or in vitro by a functional assay employing basophils. ASST positivity has been found also in patients with non-allergic asthma, but its relevance to the disease mechanism remains to be defined. Here, we report two women aged 43 and 75 years who complained simultaneous occurrence of chronic urticaria and asthm

Detection of circulating histamine releasing autoantibodies with functional properties of anti-IgE in chronic urticaria.

Circulating histamine releasing factor(s) have been demonstrated previously in chronic urticaria by an immediate weal-and-flare response to intradermal autologous serum injection. We have studied 25 chronic urticaria patients by in vivo skin testing with autologous sera and an in vitro histamine release assay using mixed leukocytes of healthy donors, to define the nature and functional properties of the serum factor(s). Twenty showed a weal response to autologous serum (mean +/- s.e.m., 37.3 +/- 6.8 mm3). Weal formation was confined to ultrafiltered serum fractions greater than 100 kD in nine

Basophil activation test for the diagnosis of hymenoptera venom allergy in childhood: a pilot study.

INTRODUCTION: Cellular in vitro tests such as the CD63-based basophil activation test (BAT) have been successfully used to diagnose hymenoptera venom sensitization in adult patients while this has not been investigated in children so far. METHODS: 15 children (9 male, 6 female; 12.7±3.5 years) with suspected allergy to vespula (VE) or honey bee (HB) venom entered this study. Besides serum tryptase (ST) levels, sensitisation against VE and HB was assessed by titrated skin testing and determination of venom-specific serum IgE (sIgE) in all patients. After stimulation with 50 ng of insect venom,

Clinical utility analysis of the Hoxb8 mast cell activation test for the diagnosis of peanut allergy.

BACKGROUND: Peanut allergy is among the most severe and common food allergies. The diagnosis has a significant impact on the quality of life for patients and their families. An effective management approach depends on accurate, safe, and easily implementable diagnostic methods. We previously developed a cell-based assay using Hoxb8 mast cells (Hoxb8 MCs) aimed at improving clinical allergy diagnosis. In this study, we assessed its diagnostic performance by measuring blinded sera from a prospectively enrolled and pre-validated peanut allergy cohort. METHODS: Hoxb8 MCs were passively sensitized

Monitoring of basophil activation using CD63 and CCR3 in allergy to muscle relaxant drugs.

Allergic or pseudoallergic reactions that occur during anesthesia have been increasing for the last few years. To date, the diagnosis of allergy to muscle relaxants remains difficult. In this respect, we developed a flow cytometric method for the study of drug-induced basophil degranulation using CD63 and CCR3. Fifty patients who developed clinical features evocative of allergic reactions immediately after induction of anesthesia were included and classified into two groups. Group 1 (n = 39) comprised true allergic patients, who developed typical signs of shock associated to positive skin test
