Analyzing socioeconomic characteristics in brachial plexus birth injuries: A multicenter public health analysis.

PURPOSE: We aimed to examine and compare socioeconomic characteristics of infants with brachial plexus birth injury (BPBI) at two high-volume, distinct healthcare systems in the United States. METHODS: All patients diagnosed with non-transient BPBI presenting between November 2021-November 2023 at either institution, New York City ("NY") or Los Angeles ("LA"), were included. To stratify based on socioeconomic background, Area Deprivation Index (ADI) and Child Opportunity Index (COI) were utilized. ADIs were presented in national percentiles (1-100 %) or state-normed deciles (1-10), with highe

Head Injuries Related to Birth Trauma in Low Birthweight Neonates During Vacuum Extraction.

Introduction Preterm delivery (gestational age < 34 w) is a relative contraindication to vacuum extraction. Current data do not differentiate clearly between preterm delivery and low birthweight. We aimed to evaluate the impact of non-metal vacuum cup extraction on neonatal head injuries related to birth trauma in newborns with low birthweights (< 2500 g). Materials and Methods A retrospective cohort of 3377 singleton pregnancies delivered by vacuum extraction from 2014 to 2019. All were gestational age ≥ 34 w. We compared 206 (6.1%) neonates with low birthweights < 2500 g to 3171 (93.9%) neon

Correlation between histopathological nerve assessment and clinical recovery in brachial plexus birth injuries.

In the management of brachial plexus birth palsies, the quality of the roots eligible for reconstruction is thought to be a key issue. The aim of the present study was to evaluate the correlations between pathological root examination and motor recovery after brachial plexus reconstructions. Quantitative histopathological analysis of intraneural fibrosis was conducted on 72 nerve transections (40 roots, 18 trunks and 14 suprascapular nerves) in 20 patients. Clinical recovery of targeted muscles after surgery was assessed by standardized functional scores. After a mean follow-up of 32 months, p

Falls in children birth to 5 years: different mechanisms lead to different injuries.

BACKGROUND: Falls are the most common cause of injury-related hospitalization in children younger than 5 years old. Most anticipatory guidance surrounding falls is around falls from windows or stairs; however, falls from furniture also are important causes of morbidity. The purpose of this study was to compare the number of children injured, ages of injured children, and injuries sustained in falls from furniture and falls from stairs in hospitalized children younger than 5 years. METHODS: All records of individuals from 0 year through 4 years, hospitalized at our institution for a fall from

Fear causes tears - perineal injuries in home birth settings. A Swedish interview study.

BACKGROUND: Perineal injury is a serious complication of vaginal delivery that has a severe impact on the quality of life of healthy women. The prevalence of perineal injuries among women who give birth in hospital has increased over the last decade, while it is lower among women who give birth at home. The aim of this study was to describe the practice of midwives in home birth settings with the focus on the occurrence of perineal injuries. METHODS: Twenty midwives who had assisted home births for between one and 29 years were interviewed using an interview guide. The midwives also had exper

[Selection of newborns with birth injuries for after care in a special session].

Selection of birth-traumatical injured newborns for follow-up with special care.--Premature newborns most will be in a special care during infancy and childhood. For all the mature infants this way is to expensive. We publish a compilation of abnormal neurological findings suitable for selecting with high risk of birth traumatical induced brain damage. The risk-group contains about 5% of all mature newborns. This number is small enough for a high expense within the bounds of the common care for mother and child.

Development of an effective therapy and objective assessment for children with birth injuries.

Sitting in a Huple, a patented hemisphere-like tool, permanently stimulates the vestibular system, thus improving the balancing ability of children with movement disorders. The Gézengúz Foundation for Children with Birth Injuries has been successfully applying this tool in therapy. By attaching x-Inertial Measurement Unit, a wireless three-dimensional orientation sensor, to the Huple, it can serve as an input peripheral for simple PC games. Children are thus motivated; they willingly perform balance training as well as participate in the game-like test. This improves the accuracy and reproduci

[Upright birthing position--more birth canal injuries? Results of a retrospective comparative study].

The maternal birthing position is not only influenced by physical factors but also culture civilization. Nowadays more women prefer to give birth in an upright position (sit, squat, kneel) which is highly supported by some family practitioners. In this retrospective investigation we compared 3 different groups of maternal birthing positions (upright, lateral, mixed birthing position i.e. mainly on the back) concerning the fetal outcome and maternal perineal injury. There was no difference in the APGAR-values and umbilical cord pH. A higher incidence of intermediate and severe laceration as wel

[Birth injuries in calves and perinatal calf losses in a large dairy herd].

Post-mortem examinations were carried out on 65 calves which were stillborn or died within 24 hours of birth. All calves came from a large dairy unit with about one thousand cattle of the Schwarzbuntes Milchrind breed. To evaluate the perinatal losses, the farm records of parturitions from 1985-1990 were used. The calves had gross damage of the CNS, the musculature, subcutaneous tissue and internal organs such as lung and intestines. It may be assumed that a lesser degree of similar damage would also be present in the surviving calves. The level of perinatal losses is determined particularly b

[Birth injuries and congenital deviation of the nasal septum in the newborn infant].

Authors report an experience performed in a randomized group of newborns to evaluate the frequency of post-partum septal deviations. 349 subjects out of 1042 have been examined in a 7 months period. Newborns have been examined with an otoscope and 2 different kinds of nasal lesions have been considered: pyramid deformation with septum dislocation and columella deviation; deviation and sub-luxation of the septum. 250 newborns were born on natural delivery and 99 after Caesarean section. In the former group 8 pyramid deformation and 10 septal deviations have been found; in the latter only 4 sept

[Vitamin supply of premature infants with intracranial birth injuries fed in different ways].

A total of 136 premature infants with intracranial birth trauma were studied during the first 2 months of life for vitamin B, C, A and carotinoid supply with regard to their body mass at birth and type of feeding. Premature children with different patterns of birth trauma showed low supply with thiamine, riboflavin, niacin and vitamin A in the presence of breast feeding. Combined feeding raised the supply of the children under observation with thiamine and riboflavin. The data obtained attest to the necessity of further study in this direction with the purpose of developing pathogenetically va

Diagnostic accuracy of MRI for detecting nerve injury in brachial plexus birth injury.

OBJECTIVES: To determine the diagnostic accuracy of MRI for diagnosing nerve injury in brachial plexus birth injury (BPBI). METHODS: Electronic databases were searched from inception to February 15, 2023 for studies reporting the accuracy of MRI (index test) compared to surgical exploration (reference standard) in detecting the target conditions of: root avulsion, any nerve abnormality, and pseudomeningocele (as a marker of root avulsion) in children with BPBI. Meta-analysis using a bivariate model was performed where data allowed. RESULTS: Eight studies met the inclusion criteria. In total,

Clinical Classification Versus MRI Patterns of Injury in Brachial Plexus Birth Injury.

PURPOSE: The Narakas classification describes brachial plexus birth injury (BPBI) according to nerve root injury by the pattern of motor weakness on clinical examination. However, it is unknown whether the classification truly corresponds to the described nerve roots. The distribution of nerve root injuries on magnetic resonance imaging (MRI) in infants with BPBI was compared with the clinical classification. METHODS: Infants with BPBI were prospectively enrolled at three children's hospitals, and the Narakas group was determined by physical examination. Infants underwent MRI prior to age 16

Predicting obstetrical anal sphincter injuries in patients who undergo vaginal birth after cesarean delivery.

BACKGROUND: Women with a history of previous cesarean delivery must weigh the numerous potential risks and benefits of elective repeat cesarean delivery or trial of labor after cesarean delivery. Notably, 1 important risk of vaginal delivery is obstetrical anal sphincter injuries. Furthermore, the rate of obstetrical anal sphincter injuries is high among women undergoing vaginal birth after cesarean delivery. However, the risk of obstetrical anal sphincter injuries is not routinely included in the trial of labor after cesarean delivery counseling, and there is no tool available to risk stratif
