The effect of interpolating low amplitude leads on the inverse reconstruction of cardiac electrical activity.

Electrocardiographic imaging is an imaging modality that has been introduced recently to help in visualizing the electrical activity of the heart and consequently guide the ablation therapy for ventricular arrhythmias. One of the main challenges of this modality is that the electrocardiographic signals recorded at the torso surface are contaminated with noise from different sources. Low amplitude leads are more affected by noise due to their low peak-to-peak amplitude. In this paper, we have studied 6 datasets from two torso tank experiments (Bordeaux and Utah experiments) to investigate the i

Zero-Fluoroscopy Left Bundle Branch Pacing in Pregnancy Using Electroanatomic Mapping.

BACKGROUND: Left bundle branch pacing (LBBP) offers physiological ventricular activation and has been proposed as an effective pacing strategy in young patients, particularly to prevent pacing-induced cardiomyopathy. In vulnerable populations, such as pregnant women, eliminating fluoroscopy is paramount to avoid fetal radiation exposure. CASE SUMMARY: We describe a 23-year-old pregnant woman (20 weeks gestation) presenting with symptomatic complete atrioventricular (AV) block. A dual-chamber pacemaker with LBBP was implanted using a three-dimensional (3D) electroanatomic mapping (EAM) system,

Rapid 12-lead automated localization method: Comparison to electrocardiographic imaging (ECGI) in patient-specific geometry.

BACKGROUND: Rapid accurate localization of the site of ventricular activation origin during catheter ablation for ventricular arrhythmias could facilitate the procedure. Electrocardiographic imaging (ECGI) using large lead sets can localize the origin of ventricular activation. We have developed an automated method to identify sites of early ventricular activation in real time using the 12-lead ECG. We aim to compare the localization accuracy of ECGI and the automated method, identifying pacing sites/VT exit based on a patient-specific model. METHODS: A patient undergoing ablation of VT on th

[Distribution of extrema on body surface isointegral maps in RBBB].

The investigations were undertaken in order to assess the characteristic features of the isointegral maps obtained from the patients demonstrating on the standard ECG complete and incomplete right bundle branch block (RBBB). The body surface potential mapping (BSPM) procedure was performed using Fukuda Denshi system enabling to record electrocardiographic signals simultaneously from the 87 leads arranged over anterior and posterior thoracic surfaces. The isointegral maps of QRST, QRS, early portion of QRS complex (from QRS onset to R wave peak), and of late portion of QRS (from R wave peak to

Electroanatomic mapping of right atrial activation in patients with and without paroxysmal atrial fibrillation.

Inter-atrial conduction delay in patients with atrial fibrillation (AF) has been reported. However, the area of this conduction delay has not been well identified. The activation time and conduction velocity over the right atrial endocardium were evaluated during sinus rhythm using the CARTO mapping technique in 6 patients with paroxysmal AF (AF group) and 11 patients without history of AF (control group). No significant differences were observed between the 2 groups in the mean activation times and conduction velocities from the earliest activation site to the superior septum, His bundle area

Successful radiofrequency current catheter ablation of accessory atrioventricular pathway in Ebstein's anomaly using electroanatomic mapping.

Ebstein's anomaly is highly associated with atrioventricular reciprocating tachycardia (AVRT) due to an atrioventricular accessory pathway (AP). This case report describes a case of a 30-year-old man with Ebstein's anomaly who had been suffering from recurrent palpitation caused by AVRT due to the right-sided AP. Conventional mapping technique failed to ablate his AP successfully. In baseline electrophysiological study, orthodromic AVRT with a right posterior AP were induced. The AP was mapped using an electroanatomic mapping system. RF current was successfully delivered at the posterior site.

QRST isointegral map reflects the sudden reduction of right ventricular pressure after balloon pulmonary valvuloplasty.

To elucidate the mechanism of electrocardiographic changes due to right ventricular pressure overload, serial changes on body surface maps recorded in two children with valvular pulmonary stenosis before and after successful reduction of right ventricular pressure by balloon valvuloplasty (BVP) were studied. Body surface maps and 12-lead electrocardiograms were simultaneously recorded before and 1-1.5 hours, 8 hours, and 5 days after BVP. Before BVP, QRST isointegral maps showed either two maxima or an expanded single maximum on the right anterior chest. These abnormal characteristics become n

Effects of hemodialysis on ventricular activation time in children with end-stage renal disease.

Patients with end-stage renal disease are affected by cardiovascular complications, including disturbances of the heart intraventricular conduction. Body surface potential mapping is a non-invasive electrocardiographic detection method of initial disturbances in heart activation propagation. A goal of the study was to analyze the effects of single hemodialysis (HD) session on ventricular activation time (VAT) maps obtained from hemodialyzed children. The study group consisted of 13 hemodialyzed children (age: 6-18 years). The control group is composed of 26 healthy subjects. In each HD patient

3D ECG: a new simplified view.

BACKGROUND: The standard 12‑lead electrocardiogram (ECG) represents cardiac electrical activity through two-dimensional projections, requiring clinicians to mentally reconstruct the three-dimensional behavior of the cardiac vector. This process is highly dependent on experience and may limit interpretation accuracy. OBJECTIVE: To introduce and demonstrate a novel qualitative method for three-dimensional (3D) ECG visualization based on orthogonal leads, designed to enhance spatial and temporal interpretation of cardiac electrical activity. METHODS: Orthogonal X, Y, and Z leads from the Frank

Pulsed field or cryoballoon ablation for paroxysmal atrial fibrillation-insights from acute and chronic electroanatomic remapping in the randomized SINGLE-SHOT CHAMPION trial.

AIMS: The SINGLE SHOT CHAMPION multicentre trial compared the effectiveness of pulsed-field ablation (PFA) and cryoballoon ablation (CBA) for pulmonary-vein isolation (PVI) in patients with paroxysmal atrial fibrillation (AF). In a prespecified substudy, post-ablation three-dimensional electroanatomic mapping (3D-EAM) was performed, and repeat-procedure mapping data were analysed. METHODS AND RESULTS: Patients were randomized 1:1 to fluoroscopy-guided PVI with PFA or CBA. The first 25 patients in each group underwent high-density 3D-EAM immediately after ablation. Acute PV isolation and lesio

Ablation of persistent atrial fibrillation based on atrial electrogram duration map: methodology and clinical outcomes from the AEDUM pilot study.

BACKGROUND: Catheter ablation of persistent atrial fibrillation (PsAF) represents a challenge for the electrophysiologist and there are still divergences regarding the best ablative approach to adopt. Create a new map of the duration of atrial bipolar electrograms (Atrial Electrogram DUration Map, AEDUM) to recognize a functional substrate during sinus rhythm and guide a patient-tailored ablative strategy for PsAF. METHODS: Forty PsAF subjects were assigned in a 1:1 ratio to either for PVI alone (Group B1) or PVI+AEDUM areas ablation (Group B2). A cohort of 15 patients without AF history unde

The combination of electroanatomic mapping and minielectrodes in a series of cases of redo procedures.

BACKGROUND: In patients with supraventricular tachycardia, catheter ablation is an important treatment option. However, approximately one quarter of these patients remain symptomatic, so sustainable strategies for the treatment of those patients who do not benefit from the first catheter ablation are required. METHODS: In a series of redo procedures, we investigated the combined use of an electro-anatomic mapping system and an ablation catheter with mini-electrodes. RESULTS: Catheter ablation was successful in two patients with recurrent common type atrial flutter and one patient with recurr

Substrate mapping for unstable ventricular tachycardia.

The primary goal of catheter ablation of scar-related ventricular tachycardia (VT) is the interruption of critical areas of slow conduction responsible for the development and maintenance of the reentrant VT circuit. Most patients with scar-related VT present with unstable arrhythmias that are not amenable to interrogation from multiple sites to define the VT circuit based on the intracardiac activation sequence and the response to entrainment mapping. In order to effectively target unstable VTs, a number of ablation approaches have been described with the aim of targeting the abnormal substra
