Human-to-human transmission of Brucella - a systematic review.

OBJECTIVE: The most common form of transmitting human brucellosis is through contaminated food or direct contact with infected animals. Human-to-human transmission (HHT) has been described as isolated case reports. The aim of this systematic review was to describe all cases of HHT of human brucellosis reported in the medical literature. METHODS: A literature search was conducted using PubMed, Scopus and Scielo databases using specific search terms published until March 2016. Two investigators independently determined study eligibility. All clinical data were evaluated to construct a table com

Bone status in juvenile systemic lupus erythematosus.

Osteoporosis is a well-recognized major health problem in adult patients with systemic lupus erythematosus (SLE). Children and adolescents with SLE, however are at even higher risk of developing osteoporosis later in life, since they develop the disease before achieving peak bone mass, which serves as a 'bone bank' for the rest of life. There is still a paucity of studies on bone mass in pediatric SLE, but those studies available provide evidence of reduced bone mass in this age group. A frequency of osteopenia of 40% measured by dual energy X-ray absorptiometry at one or more skeletal sites h

Disruption of the knee extensor apparatus complicating anterior cruciate ligament reconstruction.

Disruption of the knee extensor apparatus, after harvesting the central third of the patellar tendon for a bone-tendon-bone autograft, is a rare complication. We made 2215 reconstructions of the anterior cruciate ligament of the knee using bone-patellar tendon-bone technique, and 10 patients had fracture of the patella (0.45%), and fore patients had rupture of the patellar tendon (0.18%). The fracture of the patella in two patients was treated nonoperatively and 8 patients was treated with operative reduction and osteosynthesis. Reconstruction of the patellar ligament in four patients with a r

Experiences of the Dresdner Cord Blood Bank, supported by the Deutsche Knochenmarkspenderdatei.

Allogeneic bone marrow and peripheral blood stem cell transplantation is the treatment of choice for some malignant hematologic diseases, marrow failure syndromes, and severe congenital immunodeficiency states. Since Gluckman et al reported in 1988 the first successful human leukocyte antigen (HLA)-matched sibling umbilical cord blood stem cell transplantation, it has been known that cord blood is a valuable source of hematopoietic stem cells. The Cord Blood Bank at the University Hospital of Dresden was founded in 1997 and started collecting, processing, and cryoconserving umbilical cord bloo

Crushed cartilage in nasal reconstruction.

Experience when using crushed cartilage for restoration of the dorsal nasal contour is presented. The septal cartilage is used as fresh autogenous grafts and as preserved septal cartilage. Implantation of autogenous grafts is preferable. Bank cartilage, however, has the advantage of being easily available and of causing very little reaction in the host organism. Normally, it is absorbed very slowly and replaced by connective tissue, which retains most of the bulk and shape of the implant. The crushed material is pushed in over the undermined nasal dorsum by means of a syringe connected to a pl

A method of sealing perforated sinus membrane and histologic finding of bone substitutes: a case report.

To augment the atrophic posterior maxilla, a sinus bone graft has been widely used for sinus floor augmentation. Various bone substitutes have been developed and grafted in the maxillary sinus with and without membranes perforation, although autogenous bone is recommended as a gold standard of grafting materials. Membrane perforation is the most common complication associated with sinus bone graft. To repair a perforation, various methods have been developed. This case report is focused on histologic findings of 1 bovine hydroxyapatite (Bio-Oss; Geistlich Pharma AG, Wolhusen, Switzerland) and

The results of 39 fractures complicated by major segmental bone loss and/or leg length discrepancy.

Thirty-nine fractures with segmental bone loss and/or limb length discrepancy with defects of 1.8 to 12.0 cm were reviewed retrospectively. Most patients presented with an acute open fracture with segmental bone loss or a late reconstructive problem with a nonunion, frequently complicated by infection, segmental bone loss, and adjacent soft-tissue disruption. One hundred forty-two weeks was the average length of time of treatment before referral of a patient to us. Eighty-eight per cent of the patients received a bone graft that included either autologous cancellous bone, bank bone, a vascular

Identification of new HLA-C alleles by polymerase chain reaction-sequence specific oligonucleotide typing: Cw*0314 and Cw*1511*.

In this article, we report two new human leukocyte antigen-C (HLA-C) alleles, HLA-Cw*0314 and Cw*1511, which were identified during routine tissue typing of donors for the Australian Bone Marrow Donor Registry and Australian Cord Blood Bank. HLA-Cw*0314 shows six codon changes in exon 3 compared to Cw*030401 and shares some sequence homology with Cw*07 alleles. Cw*1511 has two nucleotide changes compared with Cw*150201 in exon 2, both resulting in amino acid changes in the protein sequence.

Cranioplasty with Autologous Bone Flaps Cryopreserved with Dimethylsulphoxide: Does Tissue Processing Matter.

OBJECTIVE: The aim of this article was to study the outcome of patients who underwent cranioplasty with cryopreserved autologous bone after decompressive craniectomy. METHODS: Data from 74 patients were retrospectively analyzed. They were divided into groups according to the storage time and the age at cranioplasty. To assess the predictive potential for complication, factors were related to successive stages (preoperative, craniectomy, tissue processing, cranioplasty, and postoperative). Cooling and warming rates applied on bone flap were calculated. The ability to inhibit microbial growth w

The preservation of tissues for transplantation.

This paper is a written version of a lecture given during the celebration of Professor Rudolf Klen's 90th birthday. Dr. Klen played by far the major part in the introduction and the development of Tissue Banking in Europe. His concept of a tissue bank envisaged the storage of all types of cell, tissue and organ that physicians and surgeons might need for the treatment of their patients. There has been much progress towards this goal, but still the final objective remains elusive. This review of the current position starts with the recognition that some tissues are required to comprise or inclu

New strategies for haploidentical transplantation.

Haploidentical transplantation in children opens the possibility to offer this treatment to every child with an otherwise incurable disease, such as some hematological or oncological malignancies, inborn or acquired bone marrow-failure syndromes, hemoglobinopathies, immunodeficiencies, or other genetic diseases. Although initial attempts at haploidentical transplantation were associated with a high transplant-related mortality, recent insights into the biology of haploidentical transplantation, the availability of effective in vivo large-scale graft-manipulation technology, and improved suppor

Microwave sterilization of femoral head allograft.

The potential shortage of allograft bone has led to the need to investigate other sources of bone for allografts. Some allograft bone donated from primary total hip arthroplasty recipients must be discarded or treated to become usable as a result of bacterial contamination. Femoral head allografts were contaminated with Staphylococcus aureus and Bacillus subtilis. A domestic microwave oven was used. The contaminated bone was exposed to microwave irradiation for different time periods. The samples were then cultured to attempt to grow the two bacterial species. The contaminated bone samples fai

Osteochondrodysplasias in South Africa.

Jürgen Spranger's visit to the University of Cape Town in 1974 provided impetus for the establishment of a bone dysplasia registry. By 1996 more than 2,500 affected persons had been documented and radiographs and DNA had been obtained in many instances. Of these disorders, about 1,500 fall into the category of "osteochondrodysplasias" as listed in the International Nomenclature [Spranger, 1992]. The numbers of affected persons with each of these disorders are presented in this article. Departmental or collaborative investigations on DNA banked in conjunction with the registry, has resulted in

[Application of bone graft in revision total hip arthroplasty].

OBJECTIVE: To compare the differences of bony union in autograft, allograft and their mixture-graft. METHODS: The postoperative X-ray films of revision total hip arthroplasties with acetabular bonegraft were observed in 41 patients. They were women, aged at operation from 44 to 86 years (average 65 years). Follow-up ranged from 12 to 72 months (average 29 months). At the operation, autogenous bone was taken from the iliac-crest and homologous bone was from banked bone i.e. femeral head taken previously. Of these, 18 were autografts, 8 allografts, and 15 mixture-grafts. Bone incorporation was

No increased risk of acute osteomyelitis associated with closed or open long bone shaft fracture.

OBJECTIVES: Osteomyelitis of the long bones can result from hematogenous spread, direct inoculation or from a contiguous focus of infection. The association of osteomyelitis after long bone fractures has widely been believed to be true by practicing surgeons since the 1950s, even though the evidence has been poor. We hypothesized that long bone shaft fracture and major bone surgery are independent risk factors for osteomyelitis in adult trauma patients. METHODS: The National Trauma Data Bank (NTDB) was queried between 2007 and 2015 for patients ≥18 years of age presenting after trauma. Patien

Acetabular Cup Revision Arthroplasty Using Morselized Impaction Allograft.

The rate of acetabular cup revision arthroplasty is gradually rising along with an increased risk of osteolysis and prosthesis loosening over time and an increase in life expectancy. The goals of revision total hip arthroplasty are: i) implant stability through reconstruction of large bone defects, ii) restoration of range of motion and biomechanics of the hip joint, and iii) normalization of uneven limb lengths. In acetabular cup revision arthroplasty, stable fixation of acetabular components is difficult in the presence of severe bone loss (e.g., evidence suggests that it is challenging to a

[One stage revision of infected total hip replacements with replacement of bone loss by allografts. Study of 90 cases of which 46 used bone allografts].

We usually manage infected total hip replacements by a one stage revision. We have reviewed 90 cases operated on before 1988. Notable bone loss was frequently encountered and reconstructed using allografts. In all cases complete surgical debridement was performed and a cemented total hip replacement implanted. Bone allografts were used in 46 cases. Systemic antibiotics and antibiotic loaded cement were utilised. There were 17 failures (19%); one patient died soon after operation, there were 8 obvious infections (9%), 1 hip was thought to be infected although the prosthesis was not loose, and 7

Culture-Based Microbiological Assessment of Dentin Biomaterials Prepared from Long-Term Refrigerated Human Teeth: A Pilot Study Toward Evidence-Based Tooth Banking.

Dentin derived from extracted human teeth has emerged as a promising autogenous biomaterial for bone regeneration owing to its structural and biological similarity to bone tissue. Growing clinical interest in dentin-derived graft materials has also stimulated the development of tooth-banking concepts, in which extracted teeth are preserved for future regenerative applications. However, microbiological contamination remains one of the principal concerns associated with long-term storage before clinical use. This in vitro study evaluated the microbiological status of dentin biomaterials prepared
