Comparison of Audiological Results Between a Transcutaneous and a Percutaneous Bone Conduction Instrument in Conductive Hearing Loss.

OBJECTIVES: In conductive, mixed hearing losses and single-sided-deafness bone-anchored hearing aids are a well-established treatment. The transcutaneous transmission across the intact skin avoids the percutaneous abutment of a bone-anchored device with the usual risk of infections and requires less care.In this study, the audiological results of the Bonebridge transcutaneous bone conduction implant (MED-EL) are compared to the generally used percutaneous device BP100 (Cochlear Ltd., Sydney, Australia). METHODS: Ten patients implanted with the transcutaneous hearing implant were compared to 1

Bilateral bone conduction devices: improved hearing ability in children with bilateral conductive hearing loss.

OBJECTIVES: The aim of the study was to investigate whether children with bilateral conductive hearing loss benefit from their second device (i.e., the bilateral bone conduction device [BCD]). DESIGN: Speech recognition in noise was assessed in 10 children fitted with bilateral BCDs during childhood. Speech recognition was measured in 2 conditions with both BCDs active. Spatial resolution was tested with the Minimum Audible Angle test in the bilateral and monaural listening conditions. RESULTS: Children demonstrated an improvement in speech recognition when speech was presented from the fron

The bone conduction threshold pattern may help to estimate the pathology underlying conductive hearing loss.

Air conduction and bone conduction thresholds in audiograms provide clinically useful information. However, whether bone conduction audiograms can help distinguish between different conductive hearing disorders remains unestablished. This retrospective case-control study included patients with hearing loss (surgically-confirmed ossicular sclerosis, ossicular discontinuity or tympanic membrane perforation) and a group of people with normal hearing (Jikei University School of Medicine, Tokyo, Japan). Data were collected between November 30, 2017 and December 30, 2022. Bone conduction threshold p

The Use of a Novel, Nonsurgical Bone Conduction Hearing Aid System for the Treatment of Conductive Hearing Loss.

OBJECTIVE: Conventional bone conduction hearing aid solutions often require a cumbersome retention system such as a headband or cap. This study aims to determine if a novel, nonsurgical bone conduction aid utilizing an adhesive attachment over the mastoid is equivalent to the conventional bone conduction hearing aid (BCHA) for the management of conductive hearing loss. STUDY DESIGN: Prospective, single-subject randomized, crossover trial. SETTING: Tertiary referral center. PATIENTS: Eleven adults and 1 child between 11 and 70 years of age with unilateral conductive hearing loss were enrolle

A new adhesive bone conduction hearing system effectively treats conductive hearing loss in children.

OBJECTIVES: Bone conduction hearing devices integrated in softbands (BCDSs) are frequently not well accepted by children with conductive hearing loss due to pressure on the head, sweating, or cosmetic stigma. A non-surgical hearing system (ADHEAR) uses a new bone conduction concept consisting of an audio processor connected to an adhesive adapter fixed behind the ear. This study is the first to evaluate the audiological and clinical outcome of this novel system, comparing it with conventional BCDSs in a short- and mid-term follow-up in children under 10 years of age. METHODS: The ADHEAR was c

Vestibular evoked myogenic potential induced by bone-conducted stimuli in patients with conductive hearing loss.

CONCLUSIONS: Bone-conducted vestibular evoked myogenic potentials (B-VEMPs) showed high specificity for the presence of vertigo in patients with unilateral chronic otitis media (COM). These results suggest that vestibular function can be evaluated with B-VEMPs, even in patients with conductive hearing loss. OBJECTIVE: The purpose of this study was to investigate the VEMPs induced by bone-conducted stimuli (B-VEMPs) in patients with conductive hearing loss due to perforated COM. SUBJECTS AND METHODS: The subjects were 48 patients with unilateral perforated COM and conductive hearing loss. The

An examination of the effects of broadband air-conduction masker on the speech intelligibility of speech-modulated bone-conduction ultrasound.

Ultrasound can be heard by bone-conduction, and speech-modulated bone-conducted ultrasound (BCU) delivers the speech information to the human ear. One of the recognition mechanisms is the demodulation of the signals. Because some of the profoundly deaf can also hear speech-modulated BCU, another mechanism may also contribution to the recognition of speech-modulated BCU. In this study, eight volunteers with normal hearing participated. The intelligibilities of speech-modulated BCU were measured using a numeral word list under masking conditions. Because the masker can mask the demodulated sound

Models of bone-conducted sound in the chinchilla external ear.

Models and previously reported data are used to describe two hypothetical bone-conduction (BC) induced ear-canal sound-pressure (PEC) sources in chinchillas. Sound-pressure measurements and a middle-ear model are combined with new micro-computed tomography ear-canal measurements. The ear-canal models contain one ideal volume-velocity source in the cartilaginous canal walls and a second in the bony walls. Analyses suggest the following: (i) Vibrations of both the cartilaginous and bony walls contribute to BC-induced PEC. (ii) With an open ear canal, the BC-induced PEC is dominated by cartilagin

Outcomes after Application of Active Bone Conducting Implants.

BACKGROUND: A bone conducting implant is a treatment option for individuals with conductive or mixed hearing loss (CHL, MHL) who do not tolerate regular hearing aids, and for individuals with single-sided deafness (SSD). An active bone conducting implant (ABCI) was introduced in 2012 with indication in CHL, MHL, and SSD, and it is still the only ABCI available. With complete implantation of the active transducer and consequent intact skin, a decrease in infections, skin overgrowth, and implant losses, all common disadvantages with earlier passive bone conducting implants, could be expected. The importance of the hook region of the cochlea for bone-conduction hearing.

For the most part, the coiled shape of the cochlea has been shown to have only minor importance for air-conducted hearing. It is hypothesized, however, that this coiled shape may play a more significant role for the bone-conducted (BC) route of hearing, through inertial forces exerted by the middle ear and cochlear fluid, and that this can be tested by comparing the results of applying BC stimuli in a variety of different directions. A three-dimensional finite element model of a human middle ear coupled to the inner ear was formulated. BC excitations were simulated by applying rigid-body vibra

Binaural interaction of bone-conducted auditory brainstem responses.

Bone-conducted auditory brainstem responses (ABRs) elicited by monoaural stimulation are very useful for evaluating hearing in children with congenital atresia of both ears. In a previous study of sound lateralization in children with congenital atresia of both ears, using bilateral bone-conducted stimuli, we found that most of the children could sufficiently retain binaural hearing ability in terms of both intensity and time differences. In this study we attempted to record bilateral bone-conducted ABRs in normal subjects in order to explore binaural interaction objectively. The study reveale

Frequent failure of percutaneous bone conduction implants in a low-resource setting.

There are few data on outcomes of bone conduction implant outcomes in low-resource settings, where a potentially higher risk of infection means that percutaneous implants may have different outcomes. Of the first five consecutive percutaneous bone implants performed in our hospital in Cambodia, three have failed, leading to our centre abandoning this procedure. Percutaneous implantation should be undertaken with caution in low-resource settings, but more data are needed.

Experience With the New Active Transcutaneous Bone-Conduction Implant With Smaller Dimensions.

OBJECTIVE: Recently, the Bonebridge 602 implant was introduced. Its smaller dimensions facilitate implantation even in surgically demanding cases. However, in extreme anatomical conditions, implant lifts are still required. We intended to report on the medical and audiological outcomes of all patients implanted with the implant with a secondary focus on the safety, efficacy, and feasibility of the use of 1-mm lifts. STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary academic center. METHODS: We retrospectively analyzed all patients implanted with the Bonebridge 602. Patient demogra
