Development and challenges in setting up an international bone infection registry.

INTRODUCTION: Osteomyelitis is an increasing burden on the society especially due to the emergence of multiple drug-resistant organisms. The lack of a central registry that prospectively collects data on patient risk factors, laboratory test results, treatment modalities, serological analysis results, and outcomes has hampered the research effort that could have improved and provided guidelines for treatments of bone infections. The current manuscript describes the lessons learned in setting up a multi-continent registry. MATERIALS AND METHODS: This multicenter, international registry was con

Hydatid cyst of radial bone.

Hydatidosis is a parasitic disease caused by Echinococcus granulosus or, less frequently Echinococcus multilocularis. The liver and lungs are most often affected; hydatid disease of radius without involvement of other sites is extremely rare. Osseous hydatid disease should be included in the differential diagnosis for cystic lesions of bone. The diagnosis of bone hydatidosis is primarily based on radiological and histopathological findings. Here we present a case of hydatid cyst in the lower third of radius in a 34-year old woman. The cysts were removed with wide excision and post-operative al

Prevalence of low bone mass and changes in vitamin D levels in human immunodeficiency virus-infected adults unexposed to antiretrovirals.

INTRODUCTION: The prevalence of low bone mass is 3 times higher in people living with human immunodeficiency virus (PLWH) and using antiretrovirals than in the HIV-unaffected population. Changes in vitamin D levels is one of the factors associated with decreased bone mass. The objective of this study is to evaluate the low bone mass and altered vitamin D levels in PLWH who have not been exposed to antiretrovirals. METHODS: A cross-sectional study was carried out with HIV-infected individuals between the ages of 18 and 55 years immediately prior to the start of antiretroviral therapy in a spec

Facial swelling in a sickle cell patient.

Sickle cell disease (SCD) is characterized as a chronic hemolytic anemia with vaso-occlusive crises that result in multisystem organ damage. Bone marrow is one of the more common sites of these crises, presumably due to marrow hypercellularity that impairs blood flow- leading to regional hypoxia and subsequent infarction. Infarcts of facial bones are considered an uncommon complication of SCD. When infarcts occur in facial bones, the mandible and orbital bones are the most commonly affected. Overall, the clinical presentation of facial bone infarctions may mimic an infectious process, such as

Mycobacterial Musculoskeletal Infections.

Although less common as causes of musculoskeletal infection than pyogenic bacteria, both Mycobacterium tuberculosis and nontuberculous mycobacteria can infect bones and joints. Although tuberculous arthritis and osteomyelitis have been recognized for millennia, infections caused by nontuberculous mycobacteria are being identified more often, likely because of a more susceptible host population and improvements in diagnostic capabilities. Despite advances in modern medicine, mycobacterial infections of the musculoskeletal system remain particularly challenging to diagnose and manage. This artic

Neonatal osteomyelitis of the calcaneus following a heel pad puncture: a case report.

Osteomyelitis of the calcaneus developed in a 6-day-old girl subsequent to heel pad puncture for taking blood samples. A review of the literature indicates that as a complication, osteomyelitis is very rare, considering the thousands of punctures performed. There is no mention in the orthopedic literature of such cases being treated in neonatal units. The present case, like others reported thus far, is characterized by the benign course of the disease. Theoretically, it is possible that infectious process could spread from the heel primary site to other bones or joints. Some difficulty in earl

Hydatid cyst of bone.

Hydatid disease is an extremely uncommon differential diagnosis in the cystic/lytic lesions of bone. We present a rare case of echinococcal infection of femur which presented as pathological fracture in a middle-aged female and diagnosis was confirmed by histopathology. Hydatid disease of bone is often asymptomatic, and its diagnosis is usually made when lesions have become extensive.

Magnetic resonance imaging (MRI) is a powerful imaging modality in the evaluation of musculoskeletal (MSK) soft tissue, joint, and bone infections. It allows prompt diagnosis and assessment of the extent of disease, which permits timely treatment to optimize long-term clinical outcomes. MRI is highly sensitive and specific in detecting the common findings of MSK infections, such as superficial and deep soft tissue oedema, joint, bursal and tendon sheath effusions, lymphadenopathy, bone marrow oedema, erosive bone changes and periostitis, and bone and cartilage destruction and sequestration. Attempted reconstitution of a foal with primary severe combined immunodeficiency.

A foal with primary severe combined immunodeficiency, diagnosed within the first two weeks of life, was maintained with its dam in semi-isolation. The foal received continuous prophylactic antibiotic therapy, plasma from a sibling hyperimmunised with equine adenovirus vaccine, and intensive general nursing care. A full sibling female was selected as a bone marrow donor on the basis of red blood cell cross-matching and mixed lymphocyte reactions. Cyclophosphamide was given before two bone marrow transfusions at 35 and 73 days of age. To prevent graft versus host disease graft versus host diseas

Case Report: The First Case Report of Visceral Leishmaniasis in Cambodia.

Leishmaniasis is considered a neglected tropical disease that is commonly found in Asia, Africa, South America, and Mediterranean countries. Visceral leishmaniasis (VL) is the most severe form of the disease and is almost universally fatal if left untreated. The symptoms of VL overlap with many infectious diseases, malignancies, and other blood disorders. The most common findings include fever, cytopenias, and splenomegaly. Given the nonspecific symptoms, the diagnosis requires detailed laboratory investigations, including bone marrow examination, that can be challenging in low- and middle-inc

Historical review of thymosin α 1 in infectious diseases.

INTRODUCTION: Thymosin α 1 (Tα1) is a peptidic biological response modifier, which plays a significant role in activating and regulating various cells of the immune system. For the above-mentioned activities it is expected to exert a clinical benefit in the treatment of diseases where the immune system is altered. AREAS COVERED: Several clinical trials have been carried out with Tα1 for treatment or prevention of many different infectious diseases such as hepatitis B and C, sepsis and Aspergillosis in bone marrow-transplanted patients. Data available on the use of Tα1 in infectious disease as

Osteocyte dysfunction and renal osteodystrophy: not just calcium and phosphorus anymore.

In an important new cross-sectional analysis in this issue, Graciolli and colleagues present bone data from 148 adult patients across the spectrum of chronic kidney disease that confirm that disrupted osteocyte function and abnormal bone histology characterize all stages of chronic kidney disease and suggest that osteocytic Wnt signaling and osteocyte maturation may play a role in the pathogenesis of renal osteodystrophy. These concepts may alter how the skeletal, cardiovascular, and infectious complications of chronic kidney disease are managed.

Rapid bacterial evaluation beyond the colony forming unit in osteomyelitis.

Examination of bacteria/host cell interactions is important for understanding the aetiology of many infectious diseases. The colony forming unit (CFU) has been the standard for quantifying bacterial burden for the past century, however, this suffers from low sensitivity and is dependent on bacterial culturability in vitro. Our data demonstrate the discrepancy between the CFU and bacterial genome copy number in an osteomyelitis-relevant co-culture system and we confirm diagnosis and quantify bacterial load in clinical bone specimens. This study provides an improved workflow for the quantificati

[Leukocytopenia as an incidental finding. Finding the etiology].

Any leukopenia of less than 1000/microliter poses an acute threat to life, and mandates an immediate search for the underlying cause. An extensive history-taking (use of drugs? visits abroad? previous illnesses?) and physical examination (splenomegaly? exanthema? signs of hemorrhage?) are mandatory. In addition to a manual differential blood count, bone marrow aspiration for cytological and histological evaluation must be requested. In this overview, the major differential diagnoses, such as allergic agranulocytosis, leukemia, pernicious illnesses (e.g. malaria), hypersplenic syndrome and a nu

Genetic Vaccination as a Flexible Tool to Overcome the Immunological Complexity of Invasive Fungal Infections.

The COVID-19 pandemic has highlighted genetic vaccination as a powerful and cost-effective tool to counteract infectious diseases. Invasive fungal infections (IFI) remain a major challenge among immune compromised patients, particularly those undergoing allogeneic hematopoietic bone marrow transplantation (HSCT) or solid organ transplant (SOT) both presenting high morbidity and mortality rates. Candidiasis and Aspergillosis are the major fungal infections among these patients and the failure of current antifungal therapies call for new therapeutic aids. Vaccination represents a valid alternati

Femoral Head Stress Fracture in Children: A Case Report.

INTRODUCTION: The stress fracture of the femoral head rarely occurs; but, it is generally encountered in case of bone insufficiency, and it is exceptional in younger individuals. The main differential diagnosis may include several diseases, namely, slipped upper femoral epiphysis, septic arthritis, osteomyelitis, and Perthes' disease. Bone scintigraphy is very sensitive but lacks specificity. Nowadays, the magnetic resonance imaging (MRI) is the gold standard for an accurate diagnosis. CASE REPORT: We present the first description of this pathology in the pediatric population with the particu
