Modified extension-block K-wire fixation technique for the treatment of bony mallet finger.

This article describes the treatment of a bony mallet finger deformity using 2 extension-block Kirschner wires (K-wires) with a transarticular K-wire fixation technique for precise alignment of the terminal extensor tendon-bone relationship and effective immobilization of the distal interphalangeal joint. Twenty-nine patients (33 fingers) with a bony mallet finger deformity and fracture fragment involving more than one-third of the articular surface were treated surgically. The fracture fragment was fixed and the mallet finger deformity was corrected in all patients using modified extension-bl

A biomechanical comparison of three sternotomy closure techniques.

BACKGROUND: A biomechanical study of three sternotomy closure techniques (figure-of-eight stainless-steel wires, Pectofix Dynamic Sternal Fixation [DSF] stainless-steel plates, and figure-of-eight stainless-steel cables) was conducted to compare strength and stiffness variables in three clinically relevant loading modes (anterior-posterior shear, longitudinal shear, and lateral distraction). METHODS: All tests were conducted on polyurethane foam sternal models that simulate the properties of cancellous bone. Each model was divided longitudinally and reconstructed using one of the sternotomy c

Factors affecting healing following percutaneous intramedullary fixation of metacarpal fractures.

Although percutaneous intramedullary nailing of metacarpal fractures is a straightforward and reliable technique, it is not without complications, and patients experience different outcomes. This study analyzed factors affecting fracture healing time and complication rates in patients who underwent percutaneous intramedullary fixation of metacarpal fractures. This study was a retrospective review of the 25 patients who underwent retrograde percutaneous Kirschner wire (K-wire) nailing for fracture of the metacarpal shaft or neck at a military hospital between May 2016 and October 2018. Treatment of nasoethmoid-orbital fractures using Kirschner wire fixation of the nasal septum.

As a postoperative complication in nasoethmoid-orbital fractures, nasal depression often occurs. The cause of this is an overlooked left nasal septum fracture. By reducing and fixing the nasal septum, nasal depression could have been avoided. Kirschner wire is useful for the fixation of the nasal septum, and it is fixed with zygomatic bone or/and frontal process of the maxillary bone. Three cases of nasoethmoid-orbital fractures were treated with this method. Kirschner wire fixation of the nasal septum is a simple and an easy method and can avoid unnecessary bone graft.

Salvage of delayed union of a phalangeal fracture with "a hairpin wire".

The advantages of using multiple joint sparing antegrade intramedullary wiring for displaced, transverse and short oblique fractures of the small long bones of the hand has been well documented. This technique provides stable three-point fixation and permits early range of motion exercises to prevent hand stiffness. But in some revision cases where there is a breach or deficiency of subchondral bone due to previous intramedullary wiring or in old neglected juxta articular fractures of the metacarpal, proximal phalanx or middle phalanx with disuse osteopenia, there is a high chance of inadverte

A comparative study of wire fixation and screw fixation in arthrodesis for the correction of hallux rigidus using an in vitro biomechanical model.

BACKGROUND: Arthrodesis of the great toe metatarsophalangeal joint for osteoarthritis usually involves internal fixation. Ideally, the fixation method should be reproducible, leading to a high rate of fusion and minimal complications. METHODS: This biomechanical study compares circumferential wire and screw fixation methods of arthrodesis. RESULTS: The two arthrodesis models have remarkably similar limits of elastic and plastic deformation. However, in the screw, failure by permanent deformation occurs at a load six times higher than the wire. CONCLUSIONS: The use of circumferential wire fi

Medial malleolus fractures: A biomechanical comparison of tension band wiring fixation methods.

BACKGROUND: This study compared the biomechanical properties of three different fixation methods of tension band wirings, used in the treatment of medial malleolus fractures. The first method used an innovative "handmade bent pin" for cerclage fixation. The second method used a U-shaped K-wire to attach the cerclage to the medial tibial cortex. These two novel fixation techniques were compared with the traditional tension band wiring technique which is fixated to the bone by a screw. HYPOTHESIS: Novel fixation techniques of tension band wiring provide stable fixation. MATERIAL AND METHODS: I

Modified tension band wiring technique by safely inserting K-wires in olecranon fracture osteosynthesis.

AIM: The present study presents the results of a modified tension band technique by surgically inserting K-wires to treat olecranon fractures. MATERIALS AND METHODS: The modification includes inserting the K-wires from the olecranon's upper tip and directing them to the ulna's dorsal surface. Twelve patients (three males and nine females) from 35 to 87 years of age were operated for olecranon fracture. After the standard approach, the olecranon was reduced and fixed with two K-wires from the tip to the dorsal ulnar cortex. Then the standard tension band technique was carried out. RESULTS: Th

Aneurysmal bone cyst of the fifth metacarpal.

Aneurysmal bone cyst is a rare, rapidly growing, and destructive benign bone tumor that even more rarely involves the bones of the hand. Various treatment options for aneurysmal bone cyst have been reported in the literature, but controversy exists regarding optimal treatment. Due to its rarity in the hand, no evidence-based treatment regimen has been established. A 21-year-old man presented with a history of pain and local swelling over his fifth metacarpal of 5 months' duration. Physical and radiographic examination of the hand was consistent with aneurysmal bone cyst. After biopsy, patholog

K-wire Migration Following Acromioclavicular Joint Fixation: A Case Report.

Kirschner wire (K-wire) is commonly used for temporary fixation of bone fractures, particularly in pediatric patients, but it poses risks such as breakage and migration. We present the case of a 67-year-old patient who underwent acromioclavicular joint fixation with K-wires after an injury. Nine months post-surgery, a wire fragment was found to have broken and migrated to the neck area. The migration led to the development of a pseudoaneurysm, which required surgical intervention for removal. The wire fragment was successfully removed, and the aneurysm was repaired after rupture. This case hig

Biomechanical Evaluation of Interosseous-to-Interosseous Wiring Technique (IIWT) and Interosseous-to-Interdental Wiring Technique (IIDWT) to Repair Simulated Transverse Feline Caudal Mandibular Fractures.

Mandibular fractures in cats are relatively common and likely to cause malocclusion. Maxillomandibular fixation and internal fixation with plates and screws are among the described repair methods in the literature. Interosseous wiring and interdental splints are surgical techniques indicated for simple and stable fractures. The purpose of this study was to compare the mechanical stability of 2 caudal mandibular transverse fracture repair techniques using interosseous wires and acrylic interdental splints. Technique 1-Interosseous-to-Interosseous Wire Technique (IIWT) involved fracture stabiliz

Reduction of comminuted fractures of the anterior wall of the frontal sinus using threaded Kirschner wires and a small eyebrow incision.

BACKGROUND: Frontal sinus fractures are relatively rare. Their surgical management significantly differs depending on whether the posterior wall is invaded and the clinical features vary. A bicoronal incision or endoscopic approach can be used. However, the minimally invasive approach has been attracting attention, leading us to introduce a simple and effective surgical method using multiple-threaded Kirschner wires. METHODS: All patients had isolated anterior wall fractures without nasofrontal duct impairment. The depth from the skin to the posterior wall was measured using computed tomograp

[New principles of using biodegradable implants in the treatment of fractures].

So far the operative treatment of bone fractures has resolved itself to immobilisation and bone fragment joining with the help of metal implants such as nails, screws, plates and wires. Once the bone had healed, they had to be removed. This action bears some risk, e.g. extremity disfunction, wound infection. This requires re-hospitalization and increases social cost of the treatment. These reasons gave rise to research on new, biodegradable materials. One of them is "Biofix" manufactures as threads or nails, employed when immobilising bone fragments. Application of "Biofix" allows early rehabi

Biomechanical comparison of different implants for PIP arthrodesis.

BACKGROUND: Surgical correction of hammertoe deformities with arthrodesis of the proximal interphalangeal joint (PIP) is one of the most frequent forefoot procedures. Recently, new intramedullary fixation devices for PIP arthrodesis have been introduced. The aim of this study was to compare a newly developed absorbable intramedullary implant made of magnesium (mm.PIP), an already available intramedullary implant made of titanium (PipTree), and the classical Kirschner-wire (K-wire). METHODS: The three intramedullary devices (mm.PIP, PipTree, and K-wire) for PIP arthrodesis were compared. A cla

Transscaphoid perilunate fracture/dislocations--results of surgical treatment.

Sixteen cases of dorsal transscaphoid perilunate fracture/dislocations in 15 patients were treated by open reduction and internal fixation of the scaphoid with a Herbert screw and/or Kirschner wires. All patients were male, with a mean age of 31 years. The average follow-up period was 3 years. These perilunate dislocations were transscaphoid in 13 cases, and transstyloid and transscaphoid in 3 cases. There was one case of median nerve deficit preoperatively. Open reduction was performed through a volar approach in all cases. Herbert screw fixation of the scaphoid was performed on 13 cases, of

Occipital-axis posterior wiring and fusion for atlantoaxial dislocation associated with occipitalization of the atlas. Technical note.

The authors present their technique of occipital-axis posterior wiring and fusion for atlantoaxial dislocation associated with an occipitalized atlas. The technique consists of drilling a 3 x 1-cm horizontal groove in the occipital bone 1 cm posterior to the foramen magnum and building up a bony bridge along the posterior margin of the foramen magnum. This bony bridge is referred to as an "artificial atlas." Conventional wiring and fusion is performed between the artificial atlas and the C-2 lamina, interposing a strut bone graft. Since the compression force on tightening the wire is vertical,

Fixation and Grafting After Limited Debridement of Scaphoid Nonunions.

PURPOSE: To evaluate a surgical technique of treating nondisplaced waist and proximal pole scaphoid nonunions without avascular necrosis (AVN). METHODS: We performed a retrospective review of all patients with nondisplaced, scaphoid waist or proximal pole nonunions without AVN treated with the following technique. Two K-wires are positioned along the scaphoid axis to stabilize the proximal and distal poles. Debridement with a curette or burr is performed parallel to the nonunion site until the K-wires are visualized and punctate bleeding of the proximal and distal fragments is encountered. Th

Management of infected defect nonunion of the metacarpals.

A 30-year-old man sustained a displaced closed fracture of the fourth metacarpal bone, which was treated by open reduction and internal fixation with Kirschner wires. Severe postoperative infection led to a segmental defect with shortening of the fourth metacarpal bone and infected defect nonunion of the fifth metacarpal bone. After serial debridements and intravenous antibiotics, the infection was controlled. An AO external minifixator was applied to restore the length of the fourth metacarpal bone and to stabilize the fourth and fifth metacarpals, and iliac bone grafting was performed, leadi

Inferomedially impacted zygomatic fracture reduction by reverse vector using an intraoral approach with Kirschner wire.

BACKGROUND: In inferomedially rotated zygomatic fractures sticking in the maxillary sinus, it is often difficult to achieve complete reduction only by conventional intraoral reduction. We present a new intraoral reduction technique using a Kirschner wire and its clinical outcome. METHODS: Among 39 inferomedially impacted zygomatic fractures incompletely reduced by a simple intraoral reduction trial with a bone elevator, a Kirschner wire (1.5 mm) was vertically inserted from the zygomatic body to the lateral orbital rim in 17 inferior-dominant rotation fractures and horizontally inserted to th
