Colorado Limb Donning-Timed Up and Go (COLD-TUG) Test in Lower-Extremity Amputation: Less Donning Time with Osseointegrated Bone-Anchored Prosthetic Limb.

BACKGROUND: Osseointegration of a bone-anchored limb (BAL) establishes a direct skeletal interface for prosthesis attachment, simplifying the donning/doffing process. The Timed Up and Go (TUG) test reliably assesses mobility in individuals with lower-extremity amputation who use socket prostheses, but it does not account for the time required to don a prosthetic limb. The aim of this study was to develop and examine the reliability and validity of the Colorado Limb Donning-Timed Up and Go (COLD-TUG) test. This test combines the time required for donning a prosthesis with the time to complete t

Critical appraisal of patient-reported and clinician-reported outcome measures assessing functional outcomes in lower extremity bone sarcoma patients-a COSMIN-based systematic review.

PURPOSE: Local therapy for bone sarcoma often significantly impacts patients' physical abilities and participation in daily life, which underscores the need for accurate assessment of these outcomes to guide optimal care. This study aims to 1) evaluate the measurement properties of eight patient- and clinician-reported outcome measures identified in a recent systematic review (2024) as being used more than once in this population, and 2) recommend the most suitable instruments for evaluation in clinic and research. METHODS: We searched MEDLINE and Embase in July 2024 for studies assessing the

Prognostic Factors After Bone Marrow-Derived Mononuclear Cell Implantation in No-Option Chronic Limb-Threatening Ischemia Patients with Atherosclerotic Lower Extremity Artery Disease.

Previous studies have reported the efficacy and safety of therapeutic angiogenesis through bone marrow-derived mononuclear cell (BM-MNC) implantation in patients with no-option chronic limb-threatening ischemia (CLTI) from atherosclerotic lower extremity artery disease (LEAD). However, uncertain clinical prognostic factors impact treatment outcomes. This study aimed to elucidate the long-term outcomes of patients with atherosclerotic LEAD-derived no-option CLTI after BM-MNC implantation and to identify prognostic factors. In this retrospective, single-center, observational study, the primary en

Risk-Stratified Predictive Analysis of Docking Site Outcomes in Lower Extremity Bone Transport: Identifying High-Risk and Low-Risk Zones for Large Segmental Defect Management.

Background: Reconstruction of limbs with extensive bone loss often requires complex surgical procedures, which can be technically demanding, time-consuming, and physically and psychologically burdensome for patients. Historically, the lack of alternatives for large bone defects often led to primary amputation. Modern musculoskeletal practice allows for reconstruction using autologous or allogeneic bone grafts, or through more complex procedures such as the Masquelet technique or distraction osteogenesis. However, these methods share a common challenge: the need for a docking site procedure in

How much is too much? A guide to appropriately bending ball tip guide wires when using intramedullary nails for the treatment of lower extremity long bone fractures.

INTRODUCTION: This study was to determine the angle of bend that can be placed in a ball-tip guide wire while still allowing passage of reamers and nails of several commonly utilized nailing systems. METHODS: Ball-tip guide wires, opening reamer tips, femoral nails, and tibial nails were collected from several manufacturers. Guide wires were incrementally bent 3 cm from the tip and passed through the reamer tip, tibial nail, and femoral nail until unable to pass. RESULTS: All systems tested demonstrated that the reamer, with its relative smaller diameter cannula as compared to the nails them

Is Rotationplasty Still a Reasonable Reconstruction Option for Patients With a Femoral Bone Sarcoma? A Comparative Study of Patients With a Minimum of 20 Years of Follow-up After Rotationplasty and Lower Extremity Amputation.

BACKGROUND: Rotationplasty was first introduced as an alternative to above-the-knee amputation after resection of bone sarcomas of the distal femur by Salzer in 1974. Although the procedure involves a substantial body image issue, it has many advantages such as durability of the reconstruction (compared with limb salvage procedures) and avoidance of phantom pain (compared with amputation). Although several reports have shown intermediate outcomes of rotationplasty, very long-term results in terms of function, activity levels, and quality of life (QoL) in comparison with above-the-knee amputati

Osteocutaneous superficial circumflex iliac perforator flap for the lower extremity bone and soft tissue reconstruction with perforator-to-perforator anastomosis after radical debridement of tibia osteomyelitis: A case report.

Reconstruction of soft tissue and bone defects in tibia chronic osteomyelitis is challenging and often managed by free flap with bone graft. However, the use of osteocutaneous free flap combined with perforator-to-perforator anastomosis has not been reported. We report the case of a 62-year-old man presenting with soft tissue and bone defects with right tibial chronic osteomyelitis, which was successfully treated with an osteocutaneous superficial circumflex iliac perforator (SCIP) flap with perforator-to-perforator anastomosis. After radical debridement and excision of the sequestrum, a 17 ×

Development of Explainable Machine Learning Models to Identify Patients at Risk for 1-Year Mortality and New Distant Metastases Postendoprosthetic Reconstruction for Lower Extremity Bone Tumors: A Secondary Analysis of the PARITY Trial.

BACKGROUND: Accurate prediction of postoperative metastasis and mortality risks in patients undergoing lower-limb oncological resection and endoprosthetic reconstruction is essential for guiding adjuvant therapies and managing patient expectations. Current prediction methods are limited by variability in patient-specific factors. This study aims to develop and internally validate explainable machine learning (ML) models to predict the 1-year risk of new distant metastases and mortality in these patients. METHODS: We performed a secondary analysis of data from the Prophylactic Antibiotic Regim

Does Augmenting Irradiated Autografts With Free Vascularized Fibula Graft in Patients With Bone Loss From a Malignant Tumor Achieve Union, Function, and Complication Rate Comparably to Patients Without Bone Loss and Augmentation When Reconstructing Intercalary Resections in the Lower Extremity?

BACKGROUND: Extracorporeally irradiated autografting is a recognized technique in reconstruction after intercalary resections, but it has drawbacks such as nonunion and graft fracture. Because sterilized autografts lose some of their mechanical properties due to involvement of the cortex with tumor, the curettage, and the adverse effects of irradiation or other sterilization techniques, some have proposed adding vascularized fibula to augment the autograft. Because this potentially adds morbidity, we sought to address the value of adding vascular fibular grafts to reconstruction with irradiate

Surgical Duration Is Independently Associated with an Increased Risk of Surgical Site Infection and May Not Be Mitigated by Prolonged Antibiotics: A Secondary Analysis of the PARITY Trial of Infection After Lower-Extremity Endoprosthetic Reconstruction for Bone Tumors.

BACKGROUND: Infection after tumor resection and endoprosthetic reconstruction is a potentially limb-threatening complication. The duration of surgery has been shown to be a risk factor for infection, but the ability of an extended duration of postoperative prophylactic antibiotics to potentially minimize that risk has not been specifically studied. The objective of this study was to determine whether extended postoperative antibiotic therapy protects against surgical site infection (SSI) in the setting of prolonged surgical duration in patients undergoing endoprosthetic reconstruction after tu

Bone mineral density and muscle strength of lower extremities after long-term strength training, subsequent knee ligament injury and rehabilitation: a unique 2-year follow-up of a 26-year-old female student.

Physical training is shown to have potential to increase the mass of healthy bones. In contrast, immobilization, used as a treatment of soft tissue and bone injuries, is shown to result in atrophy of these tissues. In this unique 26-month follow-up of a 26-year-old female student, we determined the effects of a 1-year strenuous unilateral lower limb strength training and subsequent anterior cruciate ligament (ACL) rupture on bone mineral density (BMD) of lumbar spine and lower extremities (femoral neck, distal femur, patella, proximal tibia, calcaneus), measured repeatedly (11 times) by dual-e

The effectiveness of ultrasound in the detection of fractures in adults with suspected upper or lower limb injury: a systematic review and subgroup meta-analysis.

BACKGROUND: The aim of the present review is to assess the effectiveness of ultrasound (US) in the detection of upper and lower limb bone fractures in adults compared to a diagnostic gold standard available in secondary and tertiary care centres (e.g. radiography, CT scan or MRI). METHODS: The review followed PRISMA guidelines and used a database-specific search strategy with Medline, EMBASE and The Cochrane Library plus secondary sources (see supplementary material for completed PRISMA checklist). Diagnostic performance of ultrasound was assessed with a qualitative synthesis and a meta-analy

Lower Limb Deformity Correction: Analysis and Preoperative Planning.

Lower limb deformities have a wide range of presentations and require significant preparation and planning from the surgeon to correct. It is important to provide a clear and systematic approach to preoperative planning for these cases and to detail three well-established correction techniques: external fixation, plate fixation, and intramedullary nail fixation. In using a simple mnemonic that lays out the systematic analysis of various axis lines and joint angles from proximal to distal in the lower extremity, deformities can be readily identified, and correction strategies can be effectively
