Relationship between genotype and arcuate fasciculus morphology in six young children with global developmental delay: Preliminary DTI stuy.

PURPOSE: To investigate whether different genetic mutations observed in children with global developmental delay (GD) are associated with unique patterns of the arcuate fasciculus dysmorphology. MATERIALS AND METHODS: Six children with GD (age: 36.8 ± 14.1 months, 5 boys) having mutations in MID1, CDK4, SFRP1, EN2, RXRG-GLRB, or MECP2, and five children with typical development (TD, age: 38.5 ± 20.5 months, 4 boys) underwent a 3 Tesla MRI including diffusion weighted imaging (DWI). Five language pathway segments in the left hemisphere, "C1 : Broca's to Wernicke's area," "C2 : Broca's to premo

Neural regions essential for distinct cognitive processes underlying picture naming.

We hypothesized that distinct cognitive processes underlying oral and written picture naming depend on intact function of different, but overlapping, regions of the left hemisphere cortex, such that the distribution of tissue dysfunction in various areas can predict the component of the naming process that is disrupted. To test this hypothesis, we evaluated 116 individuals within 24 h of acute ischaemic stroke using a battery of oral and written naming and other lexical tests, and with magnetic resonance diffusion and perfusion imaging to identify the areas of tissue dysfunction. Discriminant

Evaluating evidence for the cortical localization for language: Systematic reviews in the 1860s and 1870s.

The specialization of cortical function for language was proposed by Paul Broca (1824-1880) in 1861 and further elaborated to include the principle of hemispheric lateralization in 1865. Broca and other French colleagues argued for and against these hypotheses, employing clinical and pathological observations of individuals with acquired language disorders as evidence. These ideas became a topic of widespread interest after the debates at the Paris Academy of Medicine in 1865 were reported internationally. During this period until the end of the decade, hundreds of publications appeared on the

Mapping language dominance through the lens of the Wada test.

The sodium amytal test, or Wada test, named after Juhn Wada, has remained a pillar of presurgical planning and is used to identify the laterality of the dominant language and memory areas in the brain. What is perhaps less well known is that the original intent of the test was to abort seizure activity from an affected hemisphere and also to protect that hemisphere from the effects of electroconvulsive treatment. Some 80 years after Paul Broca described the frontal operculum as an essential area of expressive language and well before the age of MRI, Wada used the test to determine language dom

Stereotaxic rTMS for the treatment of auditory hallucinations in schizophrenia.

Auditory verbal hallucinations in schizophrenia may be due to dysfunctional inner speech-related cortical areas. Repetitive transcranial magnetic stimulation (rTMS) has been reported to be an effective treatment of hallucinations. In a cross-over sham controlled study, we guided rTMS stereotactically to inner speech-related cortical areas in hallucinating patients. These areas were identified individually prior to rTMS using fMRI in a subgroup of our patients. Active stimulation was applied over Broca's area and over the superior temporal gyrus as determined by fMRI, or according to structural

Resting-state functional connectivity indexes reading competence in children and adults.

Task-based neuroimaging studies face the challenge of developing tasks capable of equivalently probing reading networks across different age groups. Resting-state fMRI, which requires no specific task, circumvents these difficulties. Here, in 25 children (8-14 years) and 25 adults (21-46 years), we examined the extent to which individual differences in reading competence can be related to resting-state functional connectivity (RSFC) of regions implicated in reading. In both age groups, reading standard scores correlated positively with RSFC between the left precentral gyrus and other motor reg

Clinical forms of aphasia.

A survey is given on the history of knowledge of aphasia and on the necessity and possibilities of classification of aphasic disability. Also the association of clinical syndromes of aphasia with particular damage to the brain is outlined. For Wernicke's aphasia (fluent aphasia with comprehension deficit) a superior posterior temporal lesion is obligatory. The persistent jargon aphasia is associated with a lesion of the supramarginal gyrus. Broca's aphasia is seen with posterior inferior frontal lesions, but additional central and subcortical components are involved in persisting deficit. The

Bihemispheric stimulation over left and right inferior frontal region enhances recovery from apraxia of speech in chronic aphasia.

Several studies have already shown that transcranial direct current stimulation (tDCS) is a useful tool for enhancing recovery in aphasia. However, all tDCS studies have previously investigated the effects using unihemisperic stimulation. No reports to date have examined the role of bihemispheric tDCS on aphasia recovery. Here, eight aphasic persons with apraxia of speech underwent intensive language therapy in two different conditions: real bihemispheric anodic ipsilesional stimulation over the left Broca's area and cathodic contralesional stimulation over the right homologue of Broca's area,

A functional neuroimaging description of two deep dyslexic patients.

Deep dyslexia is a striking reading disorder that results from left-hemisphere brain damage and is characterized by semantic errors in reading single words aloud (e.g., reading 'spirit' as 'whisky'). Two types of explanation for this syndrome have been advanced. One is that deep dyslexia results from a residual left-hemisphere reading system that has lost the ability to pronounce a printed word without reference to meaning. The second is that deep dyslexia reflects right-hemisphere word processing. Although previous attempts to adjudicate between these hypotheses have been inconclusive, the co

Suppressing contextually irrelevant meanings of homophonic versus heterophonic homographs: A tDCS study targeting LIFG.

Readers frequently encounter homographs (e.g., bank) whose resolution requires selection-suppression processes: selecting the contextually relevant meaning, while suppressing the irrelevant one. In two experiments, we investigated how these processes are modulated by the phonological status of the homograph (homographs with one vs. two possible pronunciations); and what is the involvement of the left inferior frontal gyrus (LIFG, including Broca's area) in these processes. To these ends, Experiment 1 utilized the context verification task with two types of Hebrew homographs: homophonic (e.g.,

Bilateral Prefrontal Cortex Blood Flow Dynamics during Silent and Oral Reading Using Near-Infrared Spectroscopy.

This study aimed to investigate blood flow dynamics in the bilateral prefrontal cortex during silent and oral reading using near-infrared spectroscopy (NIRS). The subjects were 40 right-handed university students (20.5±1.8 years old, 20 men and 20 women). After completing the NIRS measurements, the subjects were asked to rate their level of proficiency in silent and oral reading, using a 5-point Likert scale. During oral reading, the left lateral prefrontal cortex (Broca's area) was significantly more active than the right side. During silent reading, prefrontal cortex activity was lower than

Recurring utterances induced by local anesthetic administration to the left frontal lobe.

Recurring utterances (RUs) are a distinct language symptom observed in severe aphasia, known to be associated with global or Broca's aphasia, though their neural basis remains unclear. We present a case of RU induced by selective left frontal suppression using a novel technique named the super-selective Wada test (ssWada), which involves temporary anesthetization of specific brain regions through super-selective catheterization of cerebral arteries. This method allows for precise simulation of localized brain dysfunction. We applied this technique on a 49-year-old right-handed man with drug-re

From mouth to hand: gesture, speech, and the evolution of right-handedness.

The strong predominance of right-handedness appears to be a uniquely human characteristic, whereas the left-cerebral dominance for vocalization occurs in many species, including frogs, birds, and mammals. Right-handedness may have arisen because of an association between manual gestures and vocalization in the evolution of language. I argue that language evolved from manual gestures, gradually incorporating vocal elements. The transition may be traced through changes in the function of Broca's area. Its homologue in monkeys has nothing to do with vocal control, but contains the so-called "mirr

Projection of non-cholinergic basal forebrain neurons ensheathed with perineuronal nets to rat mesocortex.

The existence of non-cholinergic (GABAergic) components in the septo-hippocampal system but also in basal forebrain projections terminating in the olfactory bulb and certain cortical areas has been documented by several authors using retrograde and anterograde tracing techniques. On the other hand, the basal forebrain also contains a high number of mainly parvalbumin-positive neurons ensheathed by a lattice-like matrix of polyanionic proteoglycans forming so-called perineuronal nets of as yet unknown function. By a combination of retrograde tracing using Fluoro-Gold injection into mesocortical

Functional magnetic resonance imaging in adult craniopagus for presurgical evaluation.

Cranially conjoined twins are rare and pose unique challenges in the preoperative evaluation of cerebral language function. The authors report on their experience in the functional magnetic resonance (fMR) imaging evaluation of adult craniopagus (temporoparietooccipital fusion) to evaluate hemispheric language dominance and the eloquent language areas in the preoperative planning stages. Conventional clinical imaging hardware originally designed for individuals was adapted and tailored for use in the twins. They were assigned a selection of language tasks while undergoing fMR imaging. Signific

Hierarchical syntactic processing is beyond mere associating: Functional magnetic resonance imaging evidence from a novel artificial grammar.

Grammar is central to any natural language. In the past decades, the artificial grammar of the An Bn type in which a pair of associated elements can be nested in the other pair was considered as a desirable model to mimic human language syntax without semantic interference. However, such a grammar relies on mere associating mechanisms, thus insufficient to reflect the hierarchical nature of human syntax. Here, we test how the brain imposes syntactic hierarchies according to the category relations on linearized sequences by designing a novel artificial "Hierarchical syntactic structure-building

Striatum and language processing: Where do we stand?

More than a century ago, Broca (1861), Wernicke (1874) and Lichteim (1885) laid the foundations for the first anatomo-functional model of language, secondarily enriched by Geschwind (1967), leading to the Broca-Wernicke-Lichteim-Geschwind model. This model included the frontal, parietal, and temporal cortices as well as a subcortical structure, which could be the striatum, whose nature and role have remained unclear. Although the emergence of language deficits in patients with striatal injury has challenged the cortical language models developed over the past 30 years, the integration of the s

Comparing the production of complex sentences in Persian patients with post-stroke aphasia and non-damaged people with normal speaking.

BACKGROUND: Cerebrovascular disease leading to stroke is the most common cause of aphasia. Speakers with agrammatic non-fluent aphasia have difficulties in production of movement-derived sentences such as passive sentences, topicalized constituents, and Wh-questions. To assess the production of complex sentences, some passive, topicalized and focused sentences were designed for patients with non-fluent Persian aphasic. Afterwards, patients' performance in sentence production was tested and compared with healthy non-damaged subjects. METHODS: In this cross sectional study, a task was designed

Whole-Brain Dynamic Causal Tractography Atlas of Auditory and Visual Speech Networks Derived From Intracranial Neurophysiologic Data.

BACKGROUND AND OBJECTIVES: Since Penfield's era, invasive neurophysiology has laid a lasting foundation for functional neuroscience by elucidating brain regions necessary for speech. Yet, whole-brain studies have not distinguished the millisecond-scale dynamics and specific white matter pathways that support rapid naming in the auditory and visual domains. We developed a whole-brain dynamic causal tractography atlas of speech using intracranial neurophysiologic data. METHODS: This observational study analyzed intracranial EEG during naming tasks in patients with drug-resistant focal epilepsy
