Serum parathyroid hormone and alkaline phosphatase as predictors of calcium requirements after total parathyroidectomy for hypocalcemia in secondary hyperparathyroidism.

BACKGROUND: To prevent hypocalcemia, this study examined the efficiency of a calcium supplement formula guided by predicted calcium requirement in patients with renal failure after parathyroidectomy with autotransplantation. METHODS: In the first phase, a protocol was followed whereby intravenous calcium gluconate was repeatedly titrated based on serum calcium levels in 22 patients with parathyroidectomy with autotransplantation. In the second phase, the first equation protocol was applied in 74 patients with parathyroidectomy with autotransplantation and revised. RESULTS: There is a signifi

Severe hypocalcemia after denosumab in a patient with acquired Fanconi syndrome.

We report the case of a 48-year-old man with acquired Fanconi syndrome due to IgG-kappa monoclonal gammopathy, who received a single dose of denosumab 60 mg for secondary prevention of skeletal fractures, in conjunction with oral calcium and vitamin D supplementation. The treatment was complicated with a severe, symptomatic hypocalcemia occurring 1 month after the injection and necessitating 4 weeks of intravenous calcium gluconate therapy. Similarly to bisphosphonates, inhibitors of the receptor activator of nuclear factor kappa-B ligand may not be appropriate for the treatment of acquired Fa

Herpes zoster developing after a spider bite.

A patient was bitten on the hand by a spider and subsequently hemorrhagic herpes zoster developed at the site of the bite as well as over most of the area distal to the elbow (innervated by cutaneous nerves C5-C8). Mild systemic symptoms of nausea, abdominal cramping, and visual disturbances responded to the intravenous administration of calcium gluconate. There was residual paresthesia of the C5-C8 dermatome but no atrophy. This is the first reported case of herpes zoster precipitated by a spider bite.

Treatment of digital hydrofluoric acid burns.

BACKGROUND: Hydrofluoric acid (HF) is one of the strongest inorganic acids and is used widely in industry. It differs from other acids in the mechanism of injury. The hydrogen ion readily penetrates the skin and causes destruction of deep tissue layers and even bone. METHODS: If HF burns to the digital skin, keep washing with large amounts of tap water for a minimum of 15 minutes. Then topically apply 2.5% calcium gluconate gel and massage for at least 40 minutes. Locally inject with 10% calcium gluconate solution if the pain persists. We studied the cases with HF finger burns in our hospital

Eleven-Year-Old Girl With Bilateral Leg Pain: A Case Report on Recognizing Hypoparathyroidism in Pediatric Practice.

Hypoparathyroidism is a rare disorder that presents with a low total serum or ionized calcium, characterized by insufficient release of parathyroid hormone (PTH) from the parathyroid glands to maintain serum calcium. Patients with hypoparathyroidism may present with acute symptoms of muscle cramps, paresthesia, numbness, tetany, muscle pain in the calves, and carpopedal spasms. This case study reports an 11-year-old girl with 3 weeks of bilateral, painful leg, occasional leg paresthesia, and fatigue. Initial investigations in the emergency department revealed an elevated creatinine kinase of 9

Successful treatment of severe iatrogenic calcinosis cutis with intravenous sodium thiosulfate in a child affected by T-acute lymphoblastic leukemia.

Sodium thiosulfate has been successfully used to treat calcyphilaxis in adults and children, but its effect on iatrogenic calcinosis cutis secondary to extravasation of calcium solutions is less known. We describe a 5-year-old boy with acute lymphoblastic leukemia who developed severe calcinosis cutis in the right forearm and hand, and in the left leg and foot after extravasation of calcium gluconate during treatment for tumor-lysis-syndrome-related hypocalcaemia. Surgical debridement, local wound care, hyperbaric oxygen therapy, and sodium thiosulfate infusion achieved a complete healing of a

Iron and Folic Acid Supplementation Affects Mineral Status in Female Rats with a Deficiency of These Micronutrients.

Supplementation with iron and folic acid is widely recommended in women of childbearing age and during pregnancy; however, the effect of such supplementation on mineral status is not well-known. The aim of this study was to determine the effects of oral iron and folic acid, administered together and separately, on copper, zinc, calcium, and magnesium concentrations in the tissues of rats with a deficiency of both these micronutrients. The experiment was performed on 8-week-old female Wistar rats. In the first stage of the experiment, the animals were randomly assigned to a control group of rat

[Cardiac surgery of tetralogy of Fallot associated with DiGeorge syndrome: a case report].

A case of 7-year-old boy with tetralogy of Fallot associated with DiGeorge syndrome was reported. He had been diagnosed as cardio-facial syndrome with hypoparathyroidism because of hypocalcemia, convulsion, cardiac defect and the characteristic face. But immunologic study showed hypofunction of cellular immunity and cardiac catheterization and angiogram revealed the Tetralogy of Fallot. So we diagnosed as DiGeorge syndrome and performed operation. The thymus was not found and we close the ventricular defect with patch and reconstructed right ventricular outflow tract with transannular patch. W

[A new treatment method for hydrochloric acid burns of the hand].

The progression of hydrofluoric acid corrosion on the hands, which cannot be arrested by other means and leads to crippling or amputation, can be stopped by one single or several repeated intra-arterial perfusions of 10 ml 20% calcium-gluconate solution in 40 ml normal saline over 4 hours as close to the defect as possible via the brachial artery or one of the forearm arteries. In 4 patients, with corrosion of a severity judged to be between first and third degree, spontaneous healing resulted after this treatment. Based on our experience we advise the use of intra-arterial perfusion and show

Hypomagnesaemia with secondary hypocalcaemia due to a novel TRPM6 mutation.

Hypomagnesaemia with secondary hypocalcaemia is a rare disorder characterised by severe hypomagnesaemia with moderate to severe hypocalcaemia caused by mutations in TRPM6. We report an infant born to a consanguineous marriage with multiple episodes of seizures. Investigations showed low total calcium and ionised calcium, hypomagnesaemia, normal vitamin D3 and parathyroid hormone. Fractional excretion of magnesium was 2.1%, pointing to an intestinal cause. Exome sequencing revealed a novel homozygous in-frame deletion mutation, c.1550_1552delTAG, p.(Val517del), in exon 14 of the TRPM6 gene. San

Calcium supplementation in clinical practice: a review of forms, doses, and indications.

Most Americans do not meet the adequate intake (AI) for calcium; calcium supplements can help meet requirements. Calcium supplementation has been found to be beneficial for bone health in children, young adults, and menopausal women. In addition to calcium, vitamin D is necessary for bone health and is generally deficient in the industrialized world. Calcium from carbonate and citrate are the most common forms of calcium supplements. Calcium carbonate, the most cost-effective form, should be taken with a meal to ensure optimal absorption. Calcium citrate can be taken without food and is the su

Calcium salt administration for circulatory shock due to severe hyperkalemia.

Patients with severe hyperkalemia may present hemodynamic instability. The use of intravenous (IV) calcium for the treatment of hyperkalemia is based on sparse evidence. We hypothesized that the administration of calcium salts would decrease mortality in patients with severe hyperkalemia and circulatory shock. We report a case of a 56-year-old female who presented to an academic emergency department with acute confusion, lethargic mental status, and circulatory shock. Venous blood gas showed a potassium concentration of 7.9 mmol/L. The patient was given 2 g of IV calcium gluconate. The patient

Acid-Base and Electrolyte Disorders: Calcium Disorders.

Calcium is an essential element for normal physiologic function. Normal serum calcium is maintained within a close range of 8.4 to 10.4 mg/dL, and acute or severe changes can result in serious cardiac, musculoskeletal, and neurologic consequences. Normal calcium levels are maintained by interactions of parathyroid hormone and vitamin D. Parathyroid hormone regulates calcium levels via bone resorption, renal calcium reabsorption, and enhanced calcium absorption in the small intestine. Causes of hypocalcemia include hypoparathyroidism (often postsurgical), severe chronic kidney disease, and vita

[Review of latrodectism and Malmignatta sting (Latrodectus tredecimguttatus) in Italy].

The latrodectism is a rare poisoning rather unknown in Italy and some times fatal. The Authors in this latrodectism review intend to provide information about the Malmignatta spider Latrodectus tredecimguttatus: biology, Italian distribution and peculiarity of its venom. Here are reported the clinical picture of the toxic syndrome, with first principles of differential diagnosis. Afterwards, they face the treatment, illustrating the cases where they can use the antivenom, calcium gluconate or methocarbamol, with modalities, doses and administration stages.

Transient neonatal hypocalcaemia caused by maternal hyperparathyroidism.

Hypocalcaemia in neonates can range from asymptomatic to a potentially life-threatening condition. We present a case of a 36 weeks gestational age boy, admitted to our neonatal intensive care unit for jitteriness, mild hypotonia and breastfeeding difficulties. By the ninth day of life, he presented with late-onset hypocalcaemia, hypomagnesaemia, low 25-OH-vitamin D and inappropriately normal parathyroid hormone. Further investigation revealed maternal hypercalcaemia with high parathyroid hormone. Maternal asymptomatic hyperparathyroidism was diagnosed and admitted as the cause of neonatal hypo
