Cannula-associated Ocular Injuries during Cataract Surgery: A Preventable Intraoperative Complication?

Although rare, inadvertently dislodged cannula can occur during cataract surgery. We report two cases of cannula-associated ocular injury during stromal hydration of the main corneal incision despite the use of Luer-lock syringes. Case 1 suffered from an initially occult intraocular injury which led to a delayed presentation of vitreous prolapsing into the anterior chamber, presumed posterior capsular rupture, vitreous hemorrhage, and multiple retinal tears, which required a three-port pars plana vitrectomy and cryotherapy. Case 2 sustained an iris laceration, anterior capsular tear, and posto

High-flow oxygen through nasal cannula in acute hypoxemic respiratory failure.

BACKGROUND: Whether noninvasive ventilation should be administered in patients with acute hypoxemic respiratory failure is debated. Therapy with high-flow oxygen through a nasal cannula may offer an alternative in patients with hypoxemia. METHODS: We performed a multicenter, open-label trial in which we randomly assigned patients without hypercapnia who had acute hypoxemic respiratory failure and a ratio of the partial pressure of arterial oxygen to the fraction of inspired oxygen of 300 mm Hg or less to high-flow oxygen therapy, standard oxygen therapy delivered through a face mask, or nonin

High-flow nasal cannula oxygen in adult patients: a narrative review.

High-flow nasal cannula oxygen (HFNC) is a relatively new therapeutic innovation being used in adults with severe respiratory disease. It delivers heated humidified oxygen through short nasal prongs and supplies much higher flow rates than traditional nasal cannula systems. These higher flows match patient flow demands better, reduce anatomic dead space and provide a slightly positive pressure in the upper airway. Randomized trials, nonrandomized prospective trials and case series using HFNC in adults were identified in the PubMed, Google Scholar and Cochrane databases for the period of June 1

Ultrasmall Gauge Biopsy of Small Choroidal Melanomas with a Subretinal Cannula.

PURPOSE: To report a novel technique for biopsying small choroidal tumors for molecular prognostication utilizing a 38-gauge subretinal cannula. METHODS: Through a transvitreal approach, a subretinal cannula enters the eye and penetrates the tumor in an area devoid of blood vessels. Manual aspiration is conducted with the aid of a surgical assistant. The cannula's transparency allows for visualization of biopsy depth and tissue aspiration. The cannula is withdrawn, and the sample is manually injected into the specimen vial. RESULTS: Sixteen patients underwent biopsy using this technique for

CT-guided robotic placement of multiple convective delivery cannulas.

OBJECTIVE: Emerging data indicate that the use of cranial robotics with CT guidance can provide an efficient and effective alternative for the simultaneous placement of multiple convection-enhanced delivery (CED) infusion cannulas to targeted regions in the brain. To assess the feasibility, efficiency, and accuracy of robot-assisted, CT-guided infusion cannula placement, this study simultaneously placed multiple bilateral CED cannulas to clinically relevant targets in cranial phantoms. METHODS: Simultaneous bilateral CT-guided robot-assisted infusion cannula placement was performed in 3D-prin

Cannula tip separation and configuration as determinants of recirculation in venovenous ECMO.

BACKGROUND: Recirculation during venovenous extracorporeal membrane oxygenation (VV ECMO) is an important phenomenon that limits the efficiency of support. Femoro-jugular (FJ) cannulation may be associated with lower recirculation than femoro-femoral (FF) configurations. Local practice is to routinely overlap cannula in the FJ configuration; this approach remains poorly described, and its impact on recirculation is not understood. METHODS: The saturation-based formula was used to assess recirculation fraction amongst 24 patients receiving VV ECMO between July 2018 and March 2025 in this singl

Is the Laminar Flow From the Vitrectomy Infusion Cannula Potentially Harmful?

PURPOSE: To model the jet flow of a 25G vitrectomy infusion cannula onto the opposite retina in a balanced salt solution-filled eye, calculate pressure and shear stress at the retina, and evaluate the biologic response to the localized increase of pressure on an ex vivo pig retina model. METHODS: Fluidics, pressure, and shear stress were calculated using computational fluid dynamics. Ten pig eyes within 4 hours of slaughter were prepared to receive a 10 mL/min flow for 1 or 3 minutes after removing the vitreous and inserting a 25G infusion cannula in the usual fashion. Prevalence and impact of ECMO cannula colonization: a single center study.

During ECMO therapy, infections are common and have substantial impact on treatment outcomes. However, data on the prevalence and clinical impact of microbial ECMO cannula colonization are scarce. Between October 2020 and May 2022, we evaluated 112 ECMO cannulas from 58 patients for microbial colonization using sonication fluid culture & 16S-rRNA-PCR (n = 105), or roll plate method (n = 7). 38 (33.9%) cannulas from 30 (51.7%) patients showed evidence for bacterial colonization with the predominance of coagulase-negative staphylococci (67.4%). High Flow Nasal Cannula in Patients With Cancer at the End of Life.

CONTEXT: High flow nasal cannula (HFNC) is frequently used to manage dyspnea in patients with cancer near the end of life. Because HFNC is restricted to the in-patient setting, patients on HFNC need to be liberated from it to be discharged from the hospital. OBJECTIVES: The purpose of this study is to assess the rate of successful liberation from HFNC in a palliative and supportive care unit (PSCU). METHODS: The study is a retrospective chart review of all 374 adult patients with cancer on HFNC admitted to a palliative and supportive care unit at a tertiary medical center from January 1, 201

Reducing Overutilization of High-flow Nasal Cannula in Children with Bronchiolitis.

BACKGROUND: Bronchiolitis is a leading cause of pediatric hospitalizations. A high-flow nasal cannula (HFNC) does not significantly improve clinical outcomes and is associated with increased costs and intensive care unit (ICU) utilization. Despite this, hospitals continue to overuse HFNC in children with bronchiolitis. We aimed to reduce HFNC initiation in children hospitalized with bronchiolitis by 20 percentage points within 6 months. METHODS: This study included patients aged 1 month to 2 years diagnosed with bronchiolitis, excluding patients with prematurity less than 32 weeks or preexist
