Atrioventricular Junction Ablation for Atrial Fibrillation.

Atrioventricular junction (AVJ) ablation is an effective therapy in patients with symptomatic atrial fibrillation who are intolerant to or unsuccessfully managed with rhythm control or medical rate control strategies. A drawback is that the procedure mandates a pacing system. Overall, the safety and efficacy of AVJ ablation is high with a majority of the patients reporting significant improvement in symptoms and quality-of-life measures. Risk of sudden cardiac death after device implantation is low, especially with an appropriate postprocedure pacing rate. Mortality benefit with AVJ ablation h

The "Block the Main Way" Technique for Left Ventricular Lead Implantation in Cardiac Resynchronization Therapy: A Case Report.

This case details the successful application of the Block the Main Way (BMW) technique to implant a left ventricular (LV) lead into an anomalously opening lateral vein during Left Bundle Branch-Optimized Cardiac Resynchronization Therapy Defibrillator (LOT-CRTD) implantation. We innovatively deployed a balloon to occlude the distal great cardiac vein adjacent to the target vessel ostium. This approach created a unidirectional pathway that provided both route guidance and enhanced support for LV lead wire manipulation, thereby reducing procedural complexity and shortening operation time. Challenging coronary sinus lead placement for CRT: A modified "Anchoring balloon" technique.

An 80-year-old man with a history of complete heart block underwent dual chamber pacemaker implantation about a year ago. He returned to the hospital due to de novo heart failure caused by pacing-induced cardiomyopathy; hence, we planned to upgrade his pacemaker to a biventricular device. The initial strategy was to perform left bundle branch area pacing-optimized cardiac resynchronization therapy (LOT-CRT) with left bundle branch area pacing (LBBAP) combined with a coronary sinus (CS) lead. In this challenging case, the successful placement of a CS lead was hindered by a complicated combinati

How to maximize QRS narrowing.

BACKGROUND: Cardiac resynchronization therapy (CRT) resulting in maximal QRS narrowing may be associated with improved outcomes. METHODS: Various atrioventricular (AV) delay settings, including the new SyncAV™ algorithm (St. Jude Medical/Abbott, St. Paul, MN, USA), aimed at maximal QRS narrowing were tested in an 81-year old CRT recipient. RESULTS: Maximal QRS narrowing from 160 to 100 ms was achieved with a manually programmed value of SyncAV™ -30 ms. At 2 months, the patient proved to be a CRT super-responder. CONCLUSION: SyncAV™ algorithm is a new way for effective QRS narrowing with pot

Direct left ventricular endocardial pacing: an alternative when traditional resynchronization via coronary sinus is not feasible or effective.

BACKGROUND: Biventricular pacing through the coronary sinus (CS) is effective for the treatment of patients with heart failure and left bundle-branch block. However, this approach is not always feasible. Although surgical epicardial lead implantation is an alternative, the technique may be deleterious in some patients. Thus, direct left ventricular (LV) endocardial pacing under local anesthesia may be an option. OBJECTIVE: We describe our technique and analyze the results of direct LV endocardial pacing. METHOD: Fourteen patients with failed resynchronization via CS (April 2006-September 201

Potential role of biventricular pacing beyond advanced systolic heart failure.

Cardiac resynchronization therapy (CRT) is an effective therapy for advanced heart failure (HF) patients. The indications are well defined in recent guidelines and broadly indicate that CRT is suitable for chronic HF patients with left ventricular ejection fraction (EF) ≤35% and in NYHA class III or IV (Class I), and those with prolonged QRS duration ≥120 ms with left bundle branch block (LBBB) QRS morphology, or QRS duration ≥150 ms irrespective of QRS morphology (Class IIa). For patients with NYHA class II symptoms, CRT is recommended for patients with EF ≤30% and QRS duration ≥130 ms with L

Complications arising from cardiac implantable electrophysiological devices: review of epidemiology, pathogenesis and prevention for the clinician.

Cardiac implantable electronic devices (CIEDs) which include devices such as permanent pacemakers, implantable cardioverter-defibrillators and cardiac resynchronization therapy devices are well established therapies that have been shown to improve quality of life and prolong survival in appropriately selected patients, and are being increasing utilized across the world. However, CIED implantations are invasive procedures and a thorough understanding of the potential complications is required to prevent deleterious patient outcomes. In this review, we will discuss frequent complications (infect

Clinical implications of long-term ECG monitoring: from loop recorder to devices remote control.

Long-term ECG monitoring has become nowadays an indispensable technology for prevention, management and treatment of many cardiac issues. Today long-term ECG monitoring is achievable through loop recorders and cardiac devices like pacemaker, implantable cardioverter-defibrillators and cardiac resynchronization therapy device-defibrillators, all of which associated with remote monitoring. Targeted long-term ECG monitoring patients who have recurrent non-documented episodes of palpitations, unexplained episodes of syncope, cryptogenic stroke or patients with a cardiomyopathy needing a risk strat

Improved relationship between left and right ventricular electrical activation after cardiac resynchronization therapy in heart failure patients can be quantified by body surface potential mapping.

OBJECTIVES: Few studies have evaluated cardiac electrical activation dynamics after cardiac resynchronization therapy. Although this procedure reduces morbidity and mortality in heart failure patients, many approaches attempting to identify the responders have shown that 30% of patients do not attain clinical or functional improvement. This study sought to quantify and characterize the effect of resynchronization therapy on the ventricular electrical activation of patients using body surface potential mapping, a noninvasive tool. METHODS: This retrospective study included 91 resynchronization

Contemporary management of heart failure patients with reduced ejection fraction: the role of implantable devices and catheter ablation.

Heart failure (HF) is a complex clinical syndrome characterised by significant morbidity and mortality worldwide. Evidence-based therapies for the management of HF include several well-established neurohormonal antagonists and antiarrhythmic drug therapy to mitigate the onset of cardiac arrhythmia. However, the degree of rate and rhythm control achieved is often suboptimal and mortality rates continue to remain high. Implantable cardioverter-defibrillators (ICDs), cardiac resynchronization (CRT), and combined (CRT-D) therapies have emerged as integral and rapidly expanding technologies in the

Devices in the management of advanced, chronic heart failure.

Heart failure (HF) is a global phenomenon, and the overall incidence and prevalence of the condition are steadily increasing. Medical therapies have proven efficacious, but only a small number of pharmacological options are in development. When patients cease to respond adequately to optimal medical therapy, cardiac resynchronization therapy has been shown to improve symptoms, reduce hospitalizations, promote reverse remodelling, and decrease mortality. However, challenges remain in identifying the ideal recipients for this therapy. The field of mechanical circulatory support has seen immense

[Anesthetic management of a hemodialysis-dependent patient implanted with a CRT-D device undergoing an operation for perforated peritonitis].

A 53-year-old man was admitted to our hospital for hematochezia, and an emergency operation was scheduled for his perforated sigmoid colon. He had received a CRT-D (cardiac resynchronization therapy with defibrillator) device for dilated cardiomyopathy two years before and had been receiving hemodialysis for the past year. Anesthesia was induced with midazolam and remifentanil and maintained with remifentanil and sevoflurane in oxygen. Before surgery, we disabled the defibrillation function of the CRT-D device and changed its pacing mode from VVI to VOO, and electrodes of an external defibrill

Use of Contemporary Imaging Techniques for Electrophysiological and Device Implantation Procedures.

Recent technological advances in cardiac imaging allow the visualization of anatomic details up to millimeter size in 3-dimensional format. Thus, it is not surprising that electrophysiologists increasingly rely upon cardiac imaging for the diagnosis, treatment, and subsequent management of patients affected by various arrhythmic disorders. Cardiac imaging methods reviewed in the present work involve: 1) the prediction of arrhythmic risk for sudden cardiac death in patients with heart disease; 2) catheter ablation of atrial fibrillation or ventricular tachycardia; and 3) cardiac resynchronizati

Veno-venous loop through coronary sinus for LV lead placement during cardiac resynchronization therapy.

Left ventricular lead placement in the appropriate branch of coronary sinus is the key to successful cardiac resynchronization therapy (CRT) and this step is technically challenging. We describe a case of non-ischemic cardiomyopathy with heart failure, taken up for cardiac resynchronization therapy with defibrillator (CRT-D) implantation. The quadripolar left ventricular lead was impossible to advance into the target lateral branch of the coronary sinus. We made a veno-venous loop, advancing the coronary guidewire through the middle cardiac vein to coronary sinus and then to superior vena cava
