The activity of ceftazidime, other beta-lactams, and aminoglycosides against Pseudomonas aeruginosa.

The inhibitory and bactericidal activities of ceftazidime, cefoperazone, ceftriaxone, piperacillin, and five aminoglycosides were determined against 50 tobramycin-susceptible and 25 multidrug-resistant isolates of Pseudomonas aeruginosa. Ceftazidime was the most active beta-lactam and tobramycin the most active aminoglycoside. The combination of piperacillin and tobramycin was synergistic in most cases. The combination of cephalosporin and tobramycin showed mostly addition or indifference, as did combination of two beta-lactams. No antagonism was observed.

In vitro evaluation of using ceftazidime/avibactam against carbapenem-resistant Acinetobacter baumannii.

OBJECTIVE: Carbapenem-resistant Acinetobacter baumannii (CRAB) is a global concern as effective treatments are very limited. We previously used a modified susceptibility testing approach to predict growth suppression in carbapenem-resistant Enterobacterales, but there are uncertainties about the generalizability of the model. The objective of this study is to verify if a similar approach can be extended to CRAB. METHOD: A clinical isolate of CRAB resistant to ceftazidime/avibactam (CAZ/AVI, MIC = 32/4 mg/L) was examined. CAZ susceptibility was determined using increasing concentrations of AVI

OXA-48-Mediated Ceftazidime-Avibactam Resistance Is Associated with Evolutionary Trade-Offs.

Infections due to carbapenemase-producing Gram-negative pathogens are associated with limited treatment options and consequently lead to increased mortality and morbidity. In response, combinations of existing β-lactams and novel β-lactamase inhibitors, such as ceftazidime-avibactam (CAZ-AVI), have been developed as alternative treatment options. To understand the development of resistance and evolutionary trajectories under CAZ-AVI exposure, we studied the effects of ceftazidime (CAZ) and CAZ-AVI on the carbapenemase OXA-48 and the epidemic OXA-48 plasmid in Escherichia coli Exposure of CAZ a

Ceftazidime/Avibactam: Who Says You Can't Teach an Old Drug New Tricks?

PURPOSE: Gram-negative resistance continues to rise with treatment options becoming more limited. Ceftazidime/avibactam was recently approved in the United States and Europe, which combines an established third-generation cephalosporin with a new, unique, non-β-lactam β-lactamase inhibitor. This review conducts a thorough examination of structure, pharmacology, spectrum of activity, pharmacokinetics/pharmacodynamics, in vitro and clinical efficacy and safety/tolerability of ceftazidime/avibactam, as well as detailed future directions for the agent. METHODS: Pubmed and clinicaltrials.gov searc

Activity of a novel combination against multidrug-resistant nonfermenters: ceftazidime plus NXL104.

AmpC overexpression appears to be a significant mechanism of β-lactam resistance in Pseudomonas aeruginosa and other nonfermenter Gram-negatives. The article under review describes the in vitro activity of a novel combination ceftazidime plus NXL104, against ceftazidime-resistant Pseudomonas aeruginosa, Burkholderia cepacia complex and OXA carbapenemase-producing Acinetobacter baumannii. The study results show that NXL104 overcomes the ceftazidime resistance engendered by AmpC enzymes, even when these are completely derepressed. NXL104 has shown good activity against ESBL PER-1 in P. aeruginos

Ceftazidime-hydrolysing CTX-M-15 extended-spectrum beta-lactamase (ESBL) in Poland.

The CTX-M-15 extended-spectrum beta-lactamase (ESBL) was recently identified in Enterobacteriaceae isolates in India, and demonstrated significant hydrolytic activity against ceftazidime, in contrast to the majority of CTX-M enzymes. CTX-M-15 differs from CTX-M-3, which is one of the most prevalent ESBLs in Poland, by only a single amino acid change (Asp-240-->Gly). Three cefotaxime- and ceftazidime-resistant Enterobacteriaceae isolates, recovered during 1998-2000 in two Polish hospitals, were found to produce CTX-M-15. Similar to those from India, the isolates contained the ISEcp1 insertion s

Ceftazidime- and imipenem-induced endotoxin release during treatment of gram-negative infections.

To determine whether ceftazidime and imipenem, which target two different penicillin-binding proteins, result in different amounts of endotoxin and cytokine release in patients with gram-negative infection, plasma endotoxin, interleukin-6, and tumor necrosis factor alpha were measured during the first 24 h of antibiotic therapy in 27 patients with gram-negative infection who had been randomized to receive either ceftazidime 2 g t.i.d. (n=12) or imipenem/cilastatin 1 g t.i.d. (n=15). The source of infection was the digestive tract (n=13), the urinary tract (n=5), the respiratory tract (n=2), so

Aztreonam plus ceftazidime-avibactam for post-neurosurgical meningitis due to Stenotrophomonas maltophilia.

Clinical evidence is lacking regarding the efficacy of aztreonam-avibactam in severe infections involving Stenotrophomonas maltophilia. We report a case of post-neurosurgical meningitis due to S. maltophilia (minimal inhibitory concentration of aztreonam plus avibactam, 4/4 µg/mL) clinically and microbiologically cured with a combination of aztreonam and ceftazidime-avibactam (2 g and 2 g/500 mg q8h, 3 h simultaneous infusions). Therapeutic drug monitoring showed adequate concentrations of aztreonam and avibactam in the cerebrospinal fluid at each time point over the 14-day treatment duration.

Chitosan biological molecule improves bactericidal competence of ceftazidime against Burkholderia pseudomallei biofilms.

Biofilm-associated Burkholderia pseudomallei infections (melioidosis) are problematic because of reduced sensitivity to antibiotics and high frequency of relapse. Biofilm dispersal agents are essential to liberate the biofilm-encased cells, which then become planktonic and are more susceptible to antibiotics. This study aimed to evaluate the ability of deacetylated chitosan (dCS), an antimicrobial and antibiofilm biological macromolecule, to disrupt established biofilms, thus enabling ceftazidime (CAZ) to kill biofilm-embedded B. pseudomallei. We combined dCS with CAZ using a mechanical stirri

Myoclonus from Antibiotic Therapy (Ceftazidime-induced Neurotoxicity): A Case Report and Review.

A 78-year-old Chinese man with a history of end-stage renal disease (ESRD) presented with fever of one-day duration. He was treated for catheter-related sepsis with intravenous piperacillin and tazobactam, which was later switched to vancomycin and ceftazidime secondary to persistent fever with negative cultures. On the fifth day of treatment with vancomycin and ceftazidime, he developed new-onset upper limb myoclonus which progressed to bilateral upper limb ataxia. A provisional diagnosis of myoclonus and ataxia secondary to neurotoxicity related to ceftazidime was made and the ceftazidime wa

Stability of Two Reserve Antibiotics in Elastomeric Pumps: Ceftazidime-Avibactam and Ceftolozane-Tazobactam.

Background/Objectives: Parenteral antibiotic therapy is indispensable in the treatment of several infections. The parenteral administration often leads to the need for prolonged hospitalization. Therefore, interest in outpatient parenteral antimicrobial therapy (OPAT) is growing. OPAT is typically administered in elastomeric devices, which release an infusion solution over a 24 h period and are meanwhile worn close to the body. This work aimed to evaluate the stability of the reserve antibiotics ceftazidime-avibactam and ceftolozane-tazobactam for OPAT use. Methods: Elastomeric pumps were prep

Global epidemiology and resistance-related mutations of ceftazidime-avibactam-resistant Klebsiella pneumoniae strains.

OBJECTIVE: Carbapenem-resistant Klebsiella pneumoniae (CRKP) infection has become a global public health problem in recent years. However, ceftazidime-avibactam-resistant (CAZ/AVI-R) Klebsiella pneumoniae (K. pneumoniae) has emerged during treatment for CRKP infection. Therefore, understanding the molecular epidemiological characteristics and resistance-related mutations of global CAZ/AVI-R K. pneumoniae strains is crucial for guiding the rational use of CAZ/AVI and for implementing the control measures to prevent the spread of CAZ/AVI-R K. pneumoniae. METHODS: Non-repetitive K. pneumoniae st

Role of Ceftazidime-Avibactam in Urinary Tract Infections Caused by Carbapenem-Resistant Enterobacterales.

Introduction Urinary tract infections (UTIs) are one of the most common bacterial infections encountered in community and healthcare settings. Increasing antimicrobial resistance patterns worldwide have limited the treatment options available. Overuse of carbapenems which were considered as the last resort for multi-drug resistant UTIs over the past decade has led to the emergence of carbapenem-resistant Enterobacterales (CRE). Ceftazidime-avibactam is a novel beta-lactam inhibitor combination drug indicated for the treatment of complicated UTIs caused by CRE. Materials and methods This was a

d-Methionine-induced DNases disperse established Burkholderia pseudomallei biofilms and promotes ceftazidime susceptibility.

Burkholderia pseudomallei biofilm is correlated with pathogenesis, antibiotic resistance, and relapsing cases of melioidosis, leading to challenges in clinical management. There is increasing interest in employing biofilm dispersal agents as adjunctive treatments for biofilm-associated infections. Methionine (Met) has shown promise as an anti-biofilm agent by inducing bacterial DNase production, resulting in the degradation of extracellular DNA (eDNA) and dispersion of bacterial biofilm. In this study, we investigated the impact of 0.05-50 μM D-Met and L-Met on the 24-h established biofilm of
