Oral lesions due to child abuse. Identification of the problem and role of the professional.

Violence in the family setting is a serious social problem of obliged concern to all health professionals. The victims are mainly children, women and the elderly. Due to the defenselessness characterizing childhood and the requirements of Spanish law, the dentist must be aware of the magnitude of the situation in order to act in favor of the affected child. The problems encountered in this aspect comprise physical abuse, sexual abuse, physical neglect, emotional abuse and emotional neglect. Any of these problems can have irreversible consequences for the psychological and physical development

'Just' prevention of child sexual abuse: A critique and call to action.

The UN Sustainable Development Goals advocate for the protection of all children from sexual abuse. This inevitably links the safety of children with social justice principles, emphasizing the need to apply an equality lens to child sexual abuse prevention. This commentary paper offers a critical discussion that advances the field by reformulating the 'just sustainabilities' framework from urban and environmental planning, to guide a 'just' child sexual abuse prevention agenda. This reformulated framework is used to critique current prevention practice, utilizing Australian and international i

Longitudinal genetic analysis of problem behaviors in biologically related and unrelated adoptees.

The genetic and environmental influences on problem behaviors at two assessment points, three years apart, and their stability were studied in a sample of international adoptees, initially aged 10 to 15 years. Parents of 111 pairs of adopted biological siblings, 221 pairs of adopted nonbiological siblings and 1484 adopted singletons completed the Child Behavior Checklist (75 pairs, 154 pairs and 1080 singletons respectively at second assessment). At first assessment, genetic factors accounted for more than 50% of the variance in the Externalizing, Aggressive Behavior, Attention Problems and So

Conflict resolution dynamics with stable caregivers confer resilience for youth exposed to early caregiving-related adversity.

Contingent responses in which caregiver and child build on each other's positive behavior may attenuate the deleterious effects of early adversity on youth mental health and neuroendocrine functioning. 159 caregiver-child dyads (child age: 6-16 years; 50.9% male; 44.6% adversity-exposed in stable arrangements with adoptive caregivers) participated in a 6-min conflict resolution task, which was coded for second-by-second changes in caregivers' and children's behavior (κ's >0.78). Caregivers reported on their child's mental health problems; youth hair cortisol concentration was obtained. Caregiv

The management of genetic origins: secrecy and openness in donor assisted conception in Israel and elsewhere.

Donor assisted conception provides new opportunities for achieving parenthood but at the same time raises issues of secrecy, anonymity, and the management of the offspring's genetic origins. As with adoption, the child's right to a genetic identity is at stake. The first part of this paper examines bio-psychosocial evidence indicating that maintaining secrecy and anonymity regarding genetic parents may be detrimental to the well-being of the donor offspring, the donor, and the nurturing, social parents. The second part discusses different approaches to the disclosure of genetic origins. The th

Exploring adverse parent-child relationships from the perspective of convicted child murderers: A South African qualitative study.

Child homicide is the most extreme form of violence against children. Within South Africa, children face the highest risk of homicide by parents/caregivers. It is suggested that prolonged exposure to adverse relationships with one's own parents may be linked to committing child homicide as it may lead to psychological damage and disturb neurological functioning. This paper explores the adverse parent-child relationships of 22 men and women incarcerated for the murder of either a biological child, a stepchild or a child in their care and draws on 49 in-depth interviews with these participants.

Adopted children with cleft lip and/or palate: a unique and growing population.

BACKGROUND: Standard clinical pathways are well established for children with cleft lip and/or palate. Treatment of internationally adopted children differs because of the late age at presentation, a newly evolving child-family relationship, and variable extent and quality of previous treatment. METHODS: The authors characterized the presentation and treatment patterns of all internationally adopted children with clefts at their institution between 1997 and 2011. RESULTS: Among 1841 children with clefts, 216 (12 percent) were internationally adopted: 78 percent had cleft lip and palate, 18 p

Trends in U.S. State Alcohol and Other Drug Use During Pregnancy Policies from 2016 to 2020: Policymaking in the Comprehensive Addiction and Recovery Act Era.

Background: Prior to the U.S. Comprehensive Addiction and Reovery Act of 2016 (CARA) , policymaking on alcohol and drug use during pregnancy was more concentrated on alcohol than drug use policy - although the overlap between the two types of policy was high. Further, the highest levels of legislative activity were requirements to report pregnant women who used alcohol or other drugs and child abuse/neglect policy. Methods: This research brief uses rigorous legal epidemiology methodology to explore state policy activity on alcohol and other drug use during pregnancy after the U.S. Comprehensiv

Iatrogenic axillary artery injury from humeral neck fracture repair. A rare and unusual event in which and aggressive strategy was mandatory to save a child limb.

Humeral neck fracture is rarely associated with injury of the nearby axillary artery and in the English literature only 29 such cases have been reported. An injury of the distal axillary artery secondary to reduction and fixation of a humeral neck fracture is a very rare and unusual complication, to the best of our knowledge has never been reported in literature. In this paper we report the case of entrapment of the distal axillary artery in the humerus rhyme fracture in a 9-year-old child who suffered a nighttime car-accident with her father. At admission, the child presented a left humeral n

Informed Consent in Paediatric Telemedicine: Challenge or Opportunity? A Scoping Review.

The fundamental importance of informed consent as a prerequisite for the lawfulness of the medical act is an indisputable cornerstone of clinical practice. However, the provision of effective information and the collection of informed consent presents important critical issues in the underage patient, even considering that in general terms he or she does not have the power to directly express consent, which must be provided by parents or legal guardians. These critical issues are amplified in the context of telemedicine. The present study aims, through a scoping review of the literature of the

NGA Tukitanga Mai Koka Ki Tona Ira: Maori mothers and child to mother violence.

In common with other indigenous women Māori mothers risk illness, harm, and possible death when abused and intimidated by their children. Yet women suffering child to mother violence are silenced by their fear and shame, and endeavour to minimise the effects of this form of abuse. A qualitative descriptive research design using kaupapa Māori methodology was adopted to explore the experiences of Māori mothers who had been abused by a son or daughter. During semi-structured interviews with five Māori women experiences of abuse by a child, and its impact on the whānau/family were recorded. Interv

Type 1 diabetes mellitus, familial hypercholesterolemia, and elevated lipoprotein (a).

Risk factor screening of all youth, including those with high-risk medical conditions such as diabetes mellitus, is important to reduce premature morbidity and mortality attributable to atherosclerotic cardiovascular disease. In those found to have significant hypercholesterolemia and/or elevated levels of lipoprotein (a), reverse cascade screening (child to parent) is recommended. This case demonstrates the benefits of targeted lipid testing. Early detection may provide additional motivation for families to adopt healthier lifestyles and reduce future atherosclerotic cardiovascular disease ev

A taxonomy of duties to report child sexual abuse: Legal developments offer new ways to facilitate disclosure.

BACKGROUND: Child sexual abuse is undisclosed for many reasons that are resistant to change. Citizens can play an important role in disclosing cases of child sexual abuse to authorities. Professionals who deal with children also play a crucial role. Office-holders in organisations have a clear responsibility to prevent cover-ups of sexual abuse. Recently, some countries have created important new legal duties for adults to disclose child sexual abuse. OBJECTIVES: This article creates a contemporary taxonomy of duties to disclose cases of child sexual abuse, and explains their nature and justi

Adoption of contraception following a pregnancy loss in India.

OBJECTIVE: The present study was carried out to describe contraceptive adoption following pregnancy terminations that resulted in outcomes other than live birth. METHOD: Retrospective calendar data on 31486 women who had experienced a pregnancy loss within the last 60 months prior to the survey date were drawn from a nationally representative dataset. Logistic regression was employed to model the associated factors with contraceptive uptake. RESULTS: Overall, 57.8% reported not adopting any method following the end of the recent pregnancy. There was a significant association between the choi

Examining Research Domain Criteria (RDoC) constructs for anger expression and regulation in toddlers.

The present study is focused on anger expression and regulation within the National Institute of Mental Health (NIMH) Research Domain Criteria (RDoC) construct of Frustrative Nonreward. Although previous studies have examined associations between child anger regulation and expression, these studies do not directly address the dynamic processes involved in Frustrative Nonreward using microlongitudinal methods. The current study used data from 561 adopted children, their adoptive parents, and birth parents and aimed to address gaps in the literature by examining: (a) temporal associations betwee

Healthy Families America: ruminations on implementing a home visitation program to prevent child maltreatment.

Following a 1990 federal report forecasting a national child abuse and neglect epidemic, Prevent Child Abuse America (PCA America) promoted a home visitation program known as Healthy Families America (HFA). HFA achieved rapid adoption and implementation across the nation going from 25 sites in 1992 to 430 in a decade. In this article, the authors describe PCA America's approach to develop, promote, oversee, and evaluate a national home visitation program. Despite its promising growth, HFA has been criticized for failing to achieve the goal of preventing child maltreatment. HFA's past and prese

State-of-the-art interventions for office-based parental tobacco control.

Parental tobacco use is a serious health issue for all family members. Child health care clinicians are in a unique and important position to address parental smoking because of the regular, multiple contacts with parents and the harmful health consequences to their patients. This article synthesizes the current evidence-based interventions for treatment of adults and applies them to the problem of addressing parental smoking in the context of the child health care setting. Brief interventions are effective, and complementary strategies such as quitlines will improve the chances of parental sm

The ongoing legacy of Indigenous family separation: Long-term outcomes of child welfare involvement among American Indian and First Nations youth.

BACKGROUND: Despite policies designed to preserve family structures, Indigenous children in the U.S. and Canada remain disproportionately removed from their homes and communities. This systemic removal represents an ongoing form of historical trauma with intergenerational consequences, yet research on its long-term psychosocial effects remains limited. OBJECTIVES: To examine the prevalence of foster care and adoption among a sample of Indigenous youth and their associations with psychosocial outcomes in early adulthood. PARTICIPANTS AND SETTING: Data come from a community-based participatory

Some factors affecting mother-child relationships following low birthweight delivery in a Nigerian sociocultural environment.

Twenty mothers with low birthweight babies, cared for in neonatal unit and kept in incubators (as index group) and 20 mothers with full-term babies (as control group) were studied to determine the extent of maternal separation and deprivation following delivery and the effects of these on mother-child relationships. The population was prospectively sampled and an interview method using a structured questionnaire, was adopted to obtain the information for the study. The result showed a delayed post-partum contact, negative reactions and some delays in maternal attachment in mothers of premature
