Development of a clinical decision tool to reduce diagnostic testing for primary aldosteronism in patients with difficult-to-control hypertension.

BACKGROUND: Satisfactory tools to preclude low-risk patients from intensive diagnostic testing for primary aldosteronism (PA) are lacking. Therefore, we aimed to develop a decision tool to determine which patients with difficult-to-control hypertension have a low probability of PA, thereby limiting the exposure to invasive testing while at the same time increasing the efficiency of testing in the remaining patients. METHODS: Data from consecutive patients with difficult-to-control hypertension, analysed through a standardized diagnostic protocol between January 2010 and October 2017 (n = 824)

Precision Assignment to Psychosocial Interventions for Late-Life Depression: An Automated Treatment Decision Rule.

IMPORTANCE: Most older adults with depression lack access to efficacious psychotherapies due to a critical clinician shortage. Even when treated, response rates are limited to approximately 50%. A treatment decision rule (TDR) may maximize treatment efficacy and resources by assigning patients to their optimal intervention. This is the first study to propose a TDR for late-life depression designed for community settings. OBJECTIVE: To develop a scalable TDR for assignment to a psychotherapy or usual care intervention for late-life depression that can be delivered easily in community settings.

Validation of the PediBIRN-7 clinical prediction rule for pediatric abusive head trauma.

BACKGROUND: The PediBIRN-7 clinical prediction rule incorporates the (positive or negative) predictive contributions of completed abuse evaluations to estimate abusive head trauma (AHT) probability after abuse evaluation. Applying definitional criteria as proxies for AHT and non-AHT ground truth, it performed with sensitivity 0.73 (95 % CI: 0.66-0.79), specificity 0.87 (95 % CI: 0.82-0.90), and ROC-AUC 0.88 (95 % CI: 0.85-0.92) in its derivation study. OBJECTIVE: To validate the PediBIRN-7's AHT prediction performance in a novel, equivalent, patient population. PARTICIPANTS AND SETTINGS: Con

Preferred Noninvasive Testing for Nonalcoholic Steatohepatitis.

BACKGROUND: There has been an increased interest in the use of noninvasive tests (NITs) to identify advanced liver fibrosis in patients with nonalcoholic fatty liver disease (NALFD). The aim of our study was to define the change in tests' characteristics (sensitivity and specificity) of different combinations of NITs to detect advanced fibrosis in NAFLD. METHODS: We stratified NITs into first and second tiers and compared two different strategies of combining NITs to screen for advanced fibrosis in patients with NAFLD. One strategy was using NITs in parallel, and the other was using NITs sequ

Validation of the "CHIIDA" and application for PICU triage in children with complicated mild traumatic brain injury.

BACKGROUND: The Children's Intracranial Injury Decision Aid (CHIIDA) was developed to predict which patients with complicated mild traumatic brain injury (cmTBI; GCS ≥13 with depressed skull fracture or intracranial injury) would achieve the composite outcome of neurosurgical intervention, intubation >24 h, or death. The study also explored the CHIIDA as a triage tool to determine need for PICU care. The purpose of this study is to externally validate the CHIIDA and assess its effects on PICU triage. METHODS: Retrospective cohort study (January 2016 to December 2017) to validate the CHIIDA to

International Framework for Red Flags for Potential Serious Spinal Pathologies.

The International Federation of Orthopaedic Manipulative Physical Therapists (IFOMPT) led the development of a framework to help clinicians assess and manage people who may have serious spinal pathology. While rare, serious spinal pathology can have devastating and life-changing or life-limiting consequences, and must be identified early and managed appropriately. Red flags (signs and symptoms that might raise suspicion of serious spinal pathology) have historically been used by clinicians to identify serious spinal pathology. Currently, there is an absence of high-quality evidence for the dia

A new diagnostic prediction model for infective endocarditis based on the 2023 duke-international society for cardiovascular infectious disease criteria: a multicenter observational study.

BACKGROUND: Infective endocarditis (IE) remains a diagnostically challenging disease with diverse symptoms. Whether IE is suspected frequently depends on clinical judgement. A reliable model to estimate the likelihood of IE at presentation across clinical departments is needed; however, no such model is currently available. We aimed to develop such a diagnostic prediction model using objective and reproducible variables and to validate its performance. METHODS: We included inpatients aged ≥ 20 years who had either a diagnosis code for IE or for undiagnosed fever during hospitalization. The mod

Functional ambulatory status as a potential adjunctive decision-making tool following wound, level of ischemia, and severity of foot infection assessment.

The Society for Vascular Surgery Lower Extremity Threatened Limb Classification System has been developed to stratify amputation risk on the basis of extent of the wound, level of ischemia, and severity of foot infection (WIfI). However, there are no currently validated metrics to assess, grade, and consider functional status, especially ambulatory status, as a major consideration during limb salvage efforts. Therefore, we propose an adjunct to the current WIfI system to include the patient's ambulatory functional status after initial assessment of limb threat. We propose a functional ambulato

A Prediction Model for Pediatric Radiographic Pneumonia.

BACKGROUND: Chest radiographs (CXRs) are frequently used in the diagnosis of community-acquired pneumonia (CAP). We sought to construct a predictive model for radiographic CAP based on clinical features to decrease CXR use. METHODS: We performed a single-center prospective study of patients 3 months to 18 years of age with signs of lower respiratory infection who received a CXR for suspicion of CAP. We used penalized multivariable logistic regression to develop a full model and bootstrapped backward selection models to develop a parsimonious reduced model. We evaluated model performance at di

External Validation of Brief Resolved Unexplained Events Prediction Rules for Serious Underlying Diagnosis.

IMPORTANCE: The American Academy of Pediatrics (AAP) higher-risk criteria for brief resolved unexplained events (BRUE) have a low positive predictive value (4.8%) and misclassify most infants as higher risk (>90%). New BRUE prediction rules from a US cohort of 3283 infants showed improved discrimination; however, these rules have not been validated in an external cohort. OBJECTIVE: To externally validate new BRUE prediction rules and compare them with the AAP higher-risk criteria. DESIGN, SETTING, AND PARTICIPANTS: This was a retrospective multicenter cohort study conducted from 2017 to 2021

Predicting Acute Cholecystitis on Final Pathology to Prioritize Surgical Urgency: An Evaluation of the Tokyo Criteria and Development of a Novel Predictive Score.

INTRODUCTION: Limited operating room access in publicly funded institutions, particularly for emergencies, highlights the need for improved prioritization rules for common pathologies, including acute cholecystitis (AC). We compared the performance of our institution's surgical prioritization rules to the Tokyo diagnostic criteria and to develop a novel decision rule to predict AC on surgical pathology. METHODS: All adult patients undergoing emergency cholecystectomy (EC) at a single academic institution (2017-2021) were reviewed. The primary outcome was the diagnosis of acute inflammation on

Prediction of blood test values under different lifestyle scenarios using time-series electronic health record.

Owing to increasing medical expenses, researchers have attempted to detect clinical signs and preventive measures of diseases using electronic health record (EHR). In particular, time-series EHRs collected by periodic medical check-up enable us to clarify the relevance among check-up results and individual environmental factors such as lifestyle. However, usually such time-series data have many missing observations and some results are strongly correlated to each other. These problems make the analysis difficult and there exists strong demand to detect clinical findings beyond them. We focus o
