Multiply resistant mutants of Enterobacter cloacae selected by beta-lactam antibiotics.

Mutants of Enterobacter cloacae, selected in vitro with ceftriaxone, ceftazidime, carumonam, or aztreonam, fell into several distinct classes. Three mutants highly resistant to nearly all beta-lactam antibiotics were stably derepressed for beta-lactamase production. Although no other changes could be detected, virulence in a mouse septicemia model was decreased in two of these mutants. One mutant, 908-Ssi, showed selectively decreased susceptibility to ampicillin and cefotetan. A change in beta-lactamase expression was thought to be responsible for this. Alterations in the production of two ou

[Two cases of neonatal meningitis caused by "Enterobacter cloacae" (author's transl)].

Two cases are reported of neonatal meningitis caused by Enterobacter cloacae. The two patients are alive; one is perfectly well, the other resulted in hydrocephalus. Both children came from the same neonatal Unit where the were given phototherapy. The importance of nosocomial infections is outlined and different aspects related to the etiology, the environment and the host (particularly immunologic deficits) are briefly reviewed.

Cloaca reconstruction: a new algorithm which considers the role of urethral length in determining surgical planning.

BACKGROUND: Cloacal malformations represent a uniquely complex challenge for surgeons. The surgical approach to date has been based on the common channel (CC) length with two patient groups considered: less than or greater than 3cm, which we believe is an oversimplification. We reviewed 19 patients, referred after surgery done elsewhere. Eight had postoperative urinary complications, 3 had constant urinary leakage and had been left after surgery with a urethra <1cm, .5 with an original 3 to 5cm common channel, who had undergone total urogenital mobilization (TUM), experienced peri-operative ur

Morphology of the cloaca in the estuarine crocodile, Crocodylus porosus, and its plastic response to salinity.

The cloacal complex of Crocodylus porosus is composed of three chambers (proctodaeum, urodaeum, and coprodaeum) separated by tight, muscular sphincters. The proctodaeum is proximal to the cloacal vent and houses the genitalia. The urodaeum is the largest chamber, is capable of storing large quantities of urine, and is lined with an epithelium with the capacity for transepithelial water and ion exchange. The coprodaeum, the most orad cloacal chamber, is a small, only marginally expandable chamber that has an epithelium composed almost entirely of mucus-secreting cells. The coprodaeum and lower

Abdominoperineal Reconstruction of the High Vagina in Cloaca and Urogenital Sinus - A Report in 25 Girls.

AIM: To evaluate the management of high vagina in cloaca and non-congenital adrenal hyperplasia (non-CAH) urogenital sinus (UGS) using a staged, need-based, nontraditional surgical approach. MATERIALS AND METHODS: A retrospective review (2010-2024) was conducted of 25 girls who underwent reconstruction of a high vagina. Management consisted of delayed, need-based vaginoplasty rather than total correction in infancy, using a top-down abdominal approach instead of total urogenital mobilization or posterior sagittal anorectovaginourethroplasty . Clinical presentation, associated anomalies, opera

The incomplete cloaca and Bardet-Biedl syndrome: A remarkable association with broader implications on patient's care.

BACKGROUND: Incomplete cloaca is a rare cloacal variant that refers to a persistent urogenital sinus in girls with the bowel opening anteriorly in the perineum or the vestibule (summary figure). Most of these girls present primarily to pediatric surgeons/urologists as neonatal hydrocolpos. This study outlines the diagnosis and management of this condition while highlighting its association with Bardet-Biedl syndrome (BBS). The latter is a genetically heterogenous group of autosomal recessive disorders characterised by multisystem affection: pigmentary retinopathy, obesity, mental defect, polyd

Short-term "double natural orifice catheterization": Nonoperative management of hydrocolpos in persistent cloaca patients - case series.

PURPOSE: Management of hydrocolpos in cloaca patients is of clinical importance. We report a nonoperative method for the management of hydrocolpos in the form of initial catheter decompression, followed by an endoscopy with catheters placement into both the bladder and vagina, and leaving these catheters for 9 and 14days respectively. METHODS: The medical records of six cloaca patients with hydrocolpos in the last 12years were reviewed. The outcomes measured were the renal function, bladder emptying, the presence/resolution of hydronephrosis, and the recurrence of hydrocolpos. RESULTS: Compl

Fibronectin glomerulopathy complicated with persistent cloaca and congenital esophageal atresia: a case report and literature review.

BACKGROUND: Fibronectin glomerulopathy is a rare, inherited, autosomal dominant, glomerular disease characterized by proteinuria, microscopic hematuria, hypertension, massive glomerular deposits of fibronectin, and slow progression to end-stage renal failure. Because the incident of fibronectin glomerulopathy is extremely low, the pathophysiology, genetic abnormalities, epidemiology, and mechanisms remain to be elucidated. CASE PRESENTATION: We report a 21-year-old woman with fibronectin glomerulopathy, who had been diagnosed with persistent cloaca and congenital esophageal atresia at birth.

Bladder after total urogenital mobilization for congenital adrenal hyperplasia and cloaca--does it behave the same?

PURPOSE: Followup of total urogenital mobilization for persistent urogenital sinus is well established anatomically and functionally. Nevertheless, studies comparing bladder function in different subsets of patients with urogenital sinus, such as congenital adrenal hyperplasia and cloaca, are scant. MATERIALS AND METHODS: We reviewed the records of patients with congenital adrenal hyperplasia and cloaca who underwent total urogenital mobilization and urodynamics in the last 10 years. Those with a short urogenital sinus (less than 2.5 cm) not requiring an abdominal approach and without spinal

The surgical management of persistent cloaca: results in 54 patients treated with a posterior sagittal approach.

This report describes the author's personal experience in the surgical treatment of 54 patients. The approach is called posterior sagittal ano recto vagino urethroplasty (PSARVUP). Forty patients underwent a primary procedure, and 14 a secondary operation. The anatomic variations found were multiple, integrating a wide spectrum of defects. The posterior sagittal approach proved to be a good initial approach, and permitted complete repair of the defect in 47 patients. Seven patients required a laparotomy in addition. The length of the common channel varied from 0.5 to 7 cm. Common channels long

Temocillin efficacy and emergence of resistance in Enterobacter cloacae Complex in a murine peritonitis model.

BACKGROUND: Temocillin (TEM) is a seducing alternative to treat multidrug-resistant Enterobacterales thanks to a narrow spectrum, resistance to hydrolysis by ESBLs and some carbapenemases and limited impact on the gut microbiota. However, its efficacy on Enterobacter cloacae complex (ECC) is unclear. We evaluated TEM efficacy against ECC, in vitro and in a high-inoculum murine peritonitis model. METHODS: Five TEM-susceptible E. xiangfangensis UTI strains and an isogenic pair of E. asburiae clinical isolates-E. asburiae TEM_S and E. asburiae TEM_R-were studied. MICs, bactericidal kinetics and

Molecular dynamic simulations to assess the structural variability of ClpV from Enterobacter cloacae.

The Enterobacter cloacae complex (ECC) consists of six Enterobacter species (E. cloacae, hormaechei, kobei, ludwigii, nimipressuralis and asburiae) that have emerged as nosocomial pathogens of interest, with E. cloacae and Enterobacter hormachei being the most frequently isolated ECC species in human clinical specimens and intensive care unit (ICU) patients. Many nosocomial outbreaks of E. cloacae have been related to transmission through contaminated surgical equipment and operative cleaning solutions. As this pathogen evades the action of antibiotics, it is important to find alternative targ

Outcomes of Enterobacter cloacae-Associated Periprosthetic Joint Infection Following Hip Arthroplasties.

BACKGROUND: Periprosthetic joint infections (PJIs) represent a serious complication following total hip arthroplasty (THA) and are associated with significant morbidity. While recent data suggest that Enterobacter cloacae is an emerging source of PJI, characteristics and outcomes of E. cloacae-associated infections are rarely described. The study aimed to present and describe the findings and outcomes of E. cloacae-associated PJI in our department. METHODS: This is a retrospective descriptive study of patients who underwent revision THA for E. cloacae-associated PJI between 2011 and 2020 and

Multi-resistant Enterobacter cloacae dacryocystitis and preseptal cellulitis: case and review of literature.

A 61-year-old man with no significant past medical history presented to a quaternary ophthalmic referral center with acute right-sided medial canthal pain, periorbital edema, and erythema despite oral antibiotic therapy. CT imaging confirmed the presence of right preseptal cellulitis and lacrimal sac aspiration identified Enterobacter cloacae. A diagnosis of acute right-sided dacryocystitis with preseptal cellulitis was made. He was successfully treated with broadening of antibiotic therapy to intravenous meropenem. Symptomatic resolution was noted on follow-up without evidence of disease recu

Treatment of bone and joint infections caused by Enterobacter cloacae with a fluoroquinolone-cotrimoxazole combination.

Although the frequency of bone and joint infections caused by Enterobacter spp. is increasing, studies regarding the optimal antibiotic therapy are scarce. The objective of this retrospective study was to assess the clinical outcomes and safety of a fluoroquinolone-cotrimoxazole combination for the treatment of bone and joint infections caused by Enterobacter cloacae. Between 2010 and 2017, 30 patients with bone and joint infections caused by E. cloacae were treated with a fluoroquinolone-cotrimoxazole combination for 8-12 weeks. There were 26 cases (87%) of infection of an internal fixation d
