Evaluation of Platelet-Rich Plasma as an Adjunct to Closed Reduction of Unilateral Condylar Fractures of Mandible.

BACKGROUND: Condylar fractures lack clear treatment guidelines and may have a lasting effect on temporomandibular joint function. Platelet-rich plasma (PRP), containing growth factors enhances healing in orthopedic fractures, but its role in condylar fractures remains unexplored. PURPOSE: The purpose of this study was to evaluate PRP's effect on pain, function, and signs and symptoms of anterior disc displacement (ADD) in treating unilateral condylar fractures. Study design, setting, and sample: The investigators implemented a double-blinded nonrandomized placebo-controlled trial enrolling su

[The Pencil Fulcrum Technique: A Simple Method for Closed Reduction of Extra-Octave Fractures in Children].

Extra-octave fractures of the fifth proximal phalanx are Salter-Harris type II physeal injuries resulting in ulnar deviation and dorsal angulation. These fractures, if inadequately reduced, may result in cosmetic deformity, malalignment, and functional impairment. Traditional reduction methods, such as the Jahss maneuver, can be technically demanding, forceful, and often require sedation or surgical assistance, which limits their use in younger children and in resource-constrained environments. We report our experience with the fulcrum-assisted "pencil technique" for closed reduction in four ch

DEVELOPMENT OF ORTHOPEDIC SIMULATOR TO PRACTICE CLOSED REDUCTION OF PEDIATRIC FRACTURES OF THE MIDDLE THIRD OF FOREARM.

UNLABELLED: In the current scenario of medical education, a trend of using models and simulators to train operational skills, especially in the practice of basic orthopedic techniques, is growing. This form of teaching allows academics to maximize learning opportunities and contributes to improving the quality of care for their future patients. However, the realistic simulation has high costs as a major limitation. OBJECTIVE: To develop a low-cost orthopedic simulator for practicing pediatric forearm reduction skills in the preclinical setting. METHODS: A model of an arm and forearm with a f

Peripheral nerve blocks for closed reduction of distal radius fractures-A protocol for a systematic review.

BACKGROUND: Current methods of anaesthesia used for closed reduction of distal radial fractures may be insufficient for pain relief and muscle relaxation, potentially compromising reduction quality and patient satisfaction. Peripheral nerve blocks have already been implemented for surgery of wrist fractures and may provide optimal conditions for closed reduction due to complete motor and sensory blockade of the involved nerves. However, existing literature on peripheral nerve blocks for closed reduction is sparse, and no updated systematic review or meta-analysis exists. AIMS: This protocol i

Displaced Tibia Fractures in Adolescents: Closed Reduction and Casting Versus Flexible Intramedullary Nails.

INTRODUCTION: Although there has been a recent trend towards the operative intervention of pediatric diaphyseal tibial fractures, there is sparse literature that supports this trend. This study compares the outcomes in children between 10 and 18 years of age with diaphyseal tibial fractures who undergo nonoperative treatment with closed reduction and casting (CRC) to those who undergo operative treatment with flexible intramedullary nailing. METHODS: A retrospective chart review was performed of all patients between 10 and 18 years of age who underwent treatment for tibia fractures at the aut

Surgical Technique: Closed Reduction and Percutaneous Pinning of Posterolaterally Displaced Supracondylar Humerus Fractures.

Gartland type III posterolateral (IIIB) supracondylar humerus fractures are common among the pediatric population and can lead to concomitant injury, including compromise of the brachial artery and median nerve and long-term deformity, such as cubitus varus. These fractures can be difficult to reduce, and there is little consensus regarding the optimal technique for closed reduction and percutaneous pinning. Here, we discuss the management of Gartland III posterolateral supracondylar humerus fractures, including an in-depth technical description of the methods of operative fixation. We describ

The Cost and Utility of Postreduction Radiographs After Closed Reduction of Pediatric Wrist and Forearm Fractures.

BACKGROUND: Formal radiographs are frequently obtained after reduction of closed pediatric wrist and forearm fracture performed under mini C-arm fluoroscopy. However, their utility has not been clearly demonstrated to justify the increased time, cost, and radiation exposure. We hypothesized that formal postreduction radiographs do not affect the rereduction rate of pediatric wrist and forearm fractures. We further sought to determine the time, monetary, and opportunity costs associated with obtaining these radiographs. METHODS: A total of 119 patients presented to our urban, level I pediatric

Pediatric Forearm Fractures Are Effectively Immobilized With a Sugar-Tong Splint Following Closed Reduction.

INTRODUCTION: Following closed reduction and initial casting of pediatric forearm fractures, loss of reduction (LOR) occurs in ∼5% to 75% of fractures. Sugar-tong splinting has been shown to maintain acceptable reduction in pediatric distal radius fractures while potentially avoiding issues associated with circumferential casting. We hypothesized that the sugar-tong splint would be an acceptable method for initial immobilization to prevent LOR in distal, mid-shaft, and proximal pediatric forearm fractures. METHODS: This is an IRB-approved, retrospective study. Inclusion criteria included pedi

A new approach to nasoseptal fractures: Submucosal endoscopically assisted septoplasty and closed nasal reduction.

BACKGROUND: Nasal bone fracture is the most common among facial bone fractures. The prevalence of concomitant septal and nasal bone fractures fluctuates between 34% and 96.2%. An adequate management of such fractures is essential to prevent complications such as post-traumatic nasal obstruction and nasoseptal deformities. The purpose of the present study is to introduce the submucosal endoscopically assisted septoplasty (SEAS) as an alternative approach for acute septal lesions and to report our experience and outcomes. METHODS: Retrospective review including patients with nasal fracture in a

Closed reduction and intramedullary pinning in the treatment of adult radial neck fractures: a case report.

Closed reduction and intramedullary pinning (CIMP) in pediatric radial neck fractureswas first reported by Metaizeau in 1980 andsatisfactory results have been published several times. The current literature did not encounter any publication related to the implementation of Metaizeau method to adult patients. We applied Metaizeau technique to an adult radial neck fracture and we have achieved satisfactory results. As this case report is single case of this method applied to an adult, we decided to present this case.

Closed reduction of mandibular condyle fractures using C-arm fluoroscopy: a technical note.

We describe a C-arm technique for mandibular condylar fractures in an anatomic study using a model skull and show its feasibility in a clinical case. The C-arm allowed posterior-anterior visualization of the condylar process. The X-ray axis was canted ∼15 degrees cranially to the Frankfort horizontal line. The skull's sagittal plane was rotated ∼15 degrees ipsilaterally to the X-ray axis. This technique facilitates clear visualization of the condylar neck with easy, flexible, and timely adjustments. In selected cases, this method would convert the clinical settings of the condylar fracture pat

Treatment of displaced intra-articular calcaneal fractures with closed reduction and percutaneous screw fixation.

BACKGROUND: Surgical treatment of displaced intra-articular fractures of the calcaneus is a standard procedure in many institutions. To avoid soft-tissue complications, several minimally invasive procedures have recently been introduced. The aim of this study was to assess the percutaneous treatment of displaced intra-articular calcaneal fractures with use of one of these techniques. METHODS: All patients who underwent percutaneous screw fixation according to the method of Forgon and Zadravecz between 1998 and 2006 were selected. Postoperative infections were recorded. During follow-up, pain,

Closed reduction and percutaneous cannulated screws fixation of displaced intra-articular calcaneus fractures.

BACKGROUND: Displaced intra-articular calcaneal fractures remain a therapeutic challenge due to fracture complexity and different treatment options. One of the adverse effects of operative treatment is secondary damage to soft tissues. To avoid soft tissue complications, several less invasive procedures have been introduced. The most frequently used minimally invasive technique is closed reduction of fracture and percutaneous cannulated screws fixation. METHOD: This study evaluates the medium-term outcome of a new technique of percutaneous treatment in 60 cases operated in Al-Razi orthopedic
