Cost-Effectiveness of Microbiota Restoration Therapies for Recurrent Clostridioides difficile Infection.

INTRODUCTION: Recurrent Clostridioides difficile infection (rCDI) is associated with substantial morbidity, healthcare utilization, and costs. US FDA-approved microbiota-based therapies, fecal microbiota spores, live-brpk (VOS) and fecal microbiota, live-jslm (RBL), reduce recurrence risk, but cost-effective placement within the rCDI treatment pathway remains uncertain. METHODS: We performed a US payer-perspective cost-effectiveness analysis using decision-tree modeling according to CHEERS 2022 guidelines. Adults with rCDI entered the model after the first or second recurrence and were follow

Decreasing treatment for postoperative Clostridioides difficile infection in Pediatric Surgery patients.

PURPOSE: Our hospital was an outlier for postoperative Clostridioides difficile infection (CDI) on the 01/2022 NSQIP-Pediatric Semiannual Report. We instituted a QI initiative to improve testing methods in our hospital and decrease the postoperative CDI treatment rate. METHODS: Surgical patients eligible for NSQIP-Pediatric sampling from 01/2022-07/2025 were analyzed to identify patients treated for CDI within 30 days of surgery. Prior to project initiation, screening tests were often used without confirmatory assays. In 01/2022, an initiative to increase confirmatory testing for patients scr

Case Commentary: Emergence of clinically relevant fidaxomicin-resistant Clostridioides difficile strains.

Clostridioides difficile infection (CDI) has limited antibiotic treatment options, with fidaxomicin being the guideline-preferred antibiotic. C. E. Kaple, S. N. Redmond, J. L. Cadum, B. Hausman, et al. (Antimicrob Agents Chemother 0:e00130-26, 2026, https://doi.org/10.1128/aac.00130-26) describe a case of clinical failure in an elderly patient with CDI treated with a pulsed-taper fidaxomicin regimen, in which the MIC increased from 0.05 μg/mL at baseline to 32 μg/mL in isolates recovered during relapse. Relapsing symptoms were eventually resolved only after switching to oral vancomycin. This c

Maternal Transmission of Symptomatic Clostridioides difficile Infection to Two Premature Neonates.

BACKGROUND: Clostridioides difficile infections (CDI) in neonates are challenging to diagnose due to high asymptomatic gut colonization rates with C. difficile in newborns and the difficulty of differentiating colonization and infection. OBJECTIVE: We describe a case of a severe CDI with a complicated course in a 48-year-old mother who had prior hospitalization and antibiotic treatment. She gave birth to a triplet at a gestational age of 31 weeks and transmitted toxin-producing C. difficile belonging to polymerase chain reaction ribotype 002 onto 3 newborns of which 2 developed diarrhea which

In vivo efficacy of fidaxomicin against rpoB mutant Clostridioides difficile infection.

OBJECTIVES: Clostridioides difficile infection (CDI) is a well-known healthcare-associated diarrheal disease. Fidaxomicin, a key antibiotic used to treat CDI, targets bacterial RNA polymerase. However, some clinical isolates have mutations in rpoB, which reduces their susceptibility to this antibiotic. In this study, the effects of rpoB mutations on the virulence of C. difficile and efficacy of fidaxomicin against CDI were evaluated in vivo. METHODS: An rpoB mutant strain (C. difficile G1073R-2024) with reduced fidaxomicin susceptibility was generated through spontaneous induction in a murine

Genotypic and phenotypic antimicrobial resistance of Irish Clostridioides difficile isolates, 2022.

OBJECTIVES: Infection with Clostridioides difficile usually occurs after antibiotic treatment for other infections and can cause gastro-intestinal disorders of variable severity. C. difficile can be resistant to a wide spectrum of antimicrobials. Detection of antimicrobial resistance (AMR) is important to direct optimal treatment and surveillance of AMR patterns in the overall population. Correlation between genotypic markers and phenotypic AMR is not yet well defined. The aim for this study is to assess whether and to what extent genotypic determinants of AMR correlate with phenotypic resista

Genomic study of European Clostridioides difficile ribotype 002/sequence type 8.

Clostridioides difficile has significant clinical importance as a leading cause of healthcare-associated infections, with symptoms ranging from mild diarrhoea to severe colitis, and possible life-threatening complications. C. difficile ribotype (RT) 002, mainly associated with MLST sequence type (ST) 8, is one of the most common RTs found in humans. This study aimed at investigating the genetic characteristics of 537 C. difficile genomes of ST8/RT002. To this end, we sequenced 298 C. difficile strains representing a new European genome collection, with strains from Germany, Denmark, France and

Budesonide, an anti-inflammatory drug, exacerbate clostridioides difficile colitis in mice.

BACKGROUND AND AIMS: Clostridioides difficile infection (CDI) induces intense acute inflammatory responses through toxin release. A combination of antibiotic and anti-inflammatory agents is sometimes recommended in severe, non-responsive cases, although clinical trials have been inconclusive, raising concerns about potential complications. This study aims to investigate the effect of budesonide and mesalamine in the treatment of CDI in a murine model, by evaluating the combination of fidaxomicin and these anti-inflammatory drugs. METHOD: C57BL/6 J female mice pretreated with an antimicrobial

Ridinilazole: a novel, narrow-spectrum antimicrobial agent targeting Clostridium (Clostridioides) difficile.

Clostridium (Clostridioides) difficile infection (CDI) remains an urgent threat to patients in health systems worldwide. Recurrent CDI occurs in up to 30% of cases due to sustained dysbiosis of the gut microbiota which normally protects against CDI. Associated costs of initial and recurrent episodes of CDI impose heavy financial burdens on health systems. Vancomycin and metronidazole have been the mainstay of therapy for CDI for many years; however, these agents continue to cause significant disruption to the gut microbiota and thus carry a high risk of recurrence for CDI patients. Treatment r

Glucosyltransferase-dependent and independent effects of Clostridioides difficile toxins during infection.

Clostridioides difficile infection (CDI) is the leading cause of nosocomial diarrhea and pseudomembranous colitis in the USA. In addition to these symptoms, patients with CDI can develop severe inflammation and tissue damage, resulting in life-threatening toxic megacolon. CDI is mediated by two large homologous protein toxins, TcdA and TcdB, that bind and hijack receptors to enter host cells where they use glucosyltransferase (GT) enzymes to inactivate Rho family GTPases. GT-dependent intoxication elicits cytopathic changes, cytokine production, and apoptosis. At higher concentrations TcdB ind

Pharmacokinetics of CamSA, a potential prophylactic compound against Clostridioides difficile infections.

Clostridioides difficile infections (CDI) are the leading cause of nosocomial antibiotic-associated diarrhea. C. difficile produces dormant spores that serve as infectious agents. Bile salts in the gastrointestinal tract signal spores to germinate into toxin-producing cells. As spore germination is required for CDI onset, anti-germination compounds may serve as prophylactics. CamSA, a synthetic bile salt, was previously shown to inhibit C. difficile spore germination in vitro and in vivo. Unexpectedly, a single dose of CamSA was sufficient to offer multi-day protection from CDI in mice without

Clostridioides difficile Spores: Bile Acid Sensors and Trojan Horses of Transmission.

The Gram-positive, spore-forming bacterium, Clostridioides difficile is the leading cause of healthcare-associated infections in the United States, although it also causes a significant number of community-acquired infections. C. difficile infections, which range in severity from mild diarrhea to toxic megacolon, cost more to treat than matched infections, with an annual treatment cost of approximately $6 billion for almost half-a-million infections. These high-treatment costs are due to the high rates of C. difficile disease recurrence (>20%) and necessity for special disinfection measures. T

Septic arthritis post anterior cruciate ligament reconstruction due to Clostridioides difficile.

Clostridioides difficile is an obligate anaerobe ubiquitous in the environment and is of particular interest in the healthcare setting as a cause of healthcare associated infection usually presenting with colitis. Extracolonic manifestations of C. difficile infection are less common with only rare reports of septic arthritis primarily in the setting of relative or overt immunocompromise. This report details the case of a 31-year-old immunocompetent male presenting with clinical features of septic arthritis, three weeks post right knee anterior cruciate ligament (ACL) reconstruction using a nat

Cephamycins inhibit pathogen sporulation and effectively treat recurrent Clostridioides difficile infection.

Spore-forming bacteria encompass a diverse range of genera and species, including important human and animal pathogens, and food contaminants. Clostridioides difficile is one such bacterium and is a global health threat because it is the leading cause of antibiotic-associated diarrhoea in hospitals. A crucial mediator of C. difficile disease initiation, dissemination and re-infection is the formation of spores that are resistant to current therapeutics, which do not target sporulation. Here, we show that cephamycin antibiotics inhibit C. difficile sporulation by targeting spore-specific penici

DAV131A Protects Hamsters from Lethal Clostridioides difficile Infection Induced by Fluoroquinolones.

Fluoroquinolone treatments induce dysbiosis of the intestinal microbiota, resulting in loss of resistance to colonization by exogenous bacteria such as Clostridioides difficile that may cause severe diarrhea in humans and lethal infection in hamsters. We show here that DAV131A, a charcoal-based adsorbent, decreases the intestinal levels of the fluoroquinolone antibiotics levofloxacin and ciprofloxacin in hamsters, protects their intestinal microbiota, and prevents lethal infection by C. difficile.

Clostridioides difficile infection and recurrence among 2622 solid organ transplant recipients.

BACKGROUND: Clostridioides difficile infection (CDI) is common after solid organ transplant (SOT) and is associated with high morbidity and mortality. METHODS: We assessed incidence, risk factors, and outcomes of CDI among SOT patients at a large multi-organ transplant center. Multivariable logistic regression was used to identify risk factors for initial and recurrent CDI. RESULTS: A total of 2622 SOT patients were included. 224 (8.5%) had CDI 1 year post-SOT. The highest incidence of CDI was among pancreas recipients (12.5%) followed by lung (11.7%), liver (11.0%), heart (10.8%), and kidne

Front-line education by infection preventionists helps reduce Clostridioides difficile infections.

When implementing the latest innovations to reduce health care-associated infections, it is important not to overlook basic infection prevention principles such as hand hygiene, isolation precautions, use of personal protective equipment, and cleaning and low-level disinfection. Like many facilities, we implemented a multifaceted approach to reduce hospital-onset Clostridioides difficile infections. In this paper, we share simple tools that we found helpful in improving infection prevention practices by addressing knowledge gaps among staff, visitors, and patients.

Impact of Gastric Acid Suppression on Clostridioides difficile Infection: A Systematic Review.

Clostridioides difficile infection (CDI) is a leading cause of healthcare-associated infections, posing significant morbidity, mortality, and financial burden. While antibiotic use remains the primary risk factor, increasing attention has been given to the role of gastric acid suppression-particularly proton pump inhibitors (PPIs) and histamine-2 receptor antagonists (H₂RAs)-in facilitating CDI. Gastric acid acts as a natural barrier against ingested pathogens; suppression of this barrier may allow C. difficile spores to survive and colonize the gut. The aim of this article is to systematicall

Auxotrophy from bioenergetic demand: the case of proline addiction in Clostridioides difficile.

Many microbial growth requirements are interpreted as auxotrophy arising from loss of biosynthetic capacity. Clostridioides difficile is widely considered to be proline auxotrophic because it fails to grow in defined media lacking proline, yet it harbors proline biosynthesis genes. Because C. difficile preferentially uses proline as an electron acceptor via D-proline reductase (Prd), we argue that this phenotype arises from a bioenergetic constraint rather than an inability to make proline. Consistent with this view, mutants lacking Prd gain the ability to grow without proline. Moreover, condi
