Determinants of Home Discharge Among Survivors of Necrotizing Fasciitis: A Population-Level Analysis.

BACKGROUND: The majority of patients with necrotizing fasciitis (NF) in the United Sates survive their illness, and there is increasing interest in addressing the ability of survivors to return to their community following hospitalization. However, there are no data on the factors affecting home discharge among survivors of NF. METHODS: We used the Texas Inpatient Public Use Data File to conduct a retrospective population-based examination of hospitalizations with NF aged 15 years or older between 2001 and 2010. Multivariate logistic regression modeling was used to explore predictors of home

"Just Google It": A Qualitative Study of Reproductive-Age Women's Stigmatizing Experiences When Accessing Buprenorphine for Opioid Use Disorder.

INTRODUCTION: The opioid crisis is increasingly impacting women, and access to buprenorphine to treat opioid use disorder (OUD) is limited by the number of providers authorized to dispense it. Stigma can represent an additional barrier to accessing medication for OUD. Qualitative data were analyzed from a randomized simulated patient field experiment of calls to outpatient buprenorphine-waivered providers. OBJECTIVES: Our primary objective was to analyze descriptions of barriers women encountered when seeking buprenorphine to treat OUD to account for differential experiences based on pregnanc

Empowering Latino Communities Through the Colorectal Health Research Champions Model: Enhancing Colorectal Cancer Awareness and Screening Advocacy.

Colorectal cancer (CRC) is the second leading cause of cancer mortality among Latinos who remain underrepresented in screening and are more often diagnosed at later stages, leading to poorer outcomes. Barriers include low health literacy, lack of insurance, limited access to screening, and cultural mistrust of healthcare and research. To implement the Colorectal Health Research Champion (CHRC) Model to address CRC screening disparities among immigrant Latinos, primarily from Central and South America, residing in Richmond, VA's urban Latino community. The CHRC model integrates evidence-based s

Suspicion of cancer clinic: Community-Based diagnostic system- lung cancer cohort analysis.

BACKGROUND: Delays in lung cancer diagnosis and treatment remain a persistent challenge in community health systems, where fragmented referral pathways and limited coordination may impede timely care. In response to observed delays in diagnostic workup, a community health system implemented a centralized Suspicion of Cancer (SOC) clinic designed to streamline evaluation of patients with suspected malignancy. METHODS: We conducted a descriptive observational study of consecutive patients referred to a community-based Suspicion of Cancer Clinic between January 2022 to March 2024. The SOC model

Strategies for Creating Multidisciplinary Community Care Teams to Address Barriers to Providing Family Based Treatment of Eating Disorders.

Eating disorders, especially anorexia nervosa, are complex and devastating illnesses. Although eating disorders have a high mortality rate and are relatively common, there are many barriers for those seeking treatment. Provider training and education, weight bias among health care providers, geographical and language barriers, and a lack of options because of insurance restrictions prevent many families from receiving appropriate care, especially in smaller or rural communities. In those areas, providers are left to piece together treatment using a small number of other providers from differen

A life course perspective on depressive symptoms in mid-pregnancy.

OBJECTIVE: To examine the prevalence of depressive symptoms in mid-pregnancy and their relation to life circumstances across the life course in a community-based sample. METHODS: The Pregnancy Outcomes and Community Health (POUCH) Study enrolled women at 16-26 weeks' gestation from 52 clinics in five Michigan communities. At enrollment women completed a screening instrument for depressive symptoms (CES-D) and questions about life circumstances covering three "periods," i.e. previous 6 months, adulthood, and childhood. Questions were grouped into sub-constructs (e.g., abuse, economic, substanc

Comparative effectiveness of team-based care with a clinical decision support system versus team-based care alone on cardiovascular risk reduction among patients with diabetes: Rationale and design of the D4C trial.

BACKGROUND: Diabetes has become a major public health challenge worldwide, especially in low- and middle-income countries (LMICs). Uncontrolled hyperglycemia, hypertension, and dyslipidemia major risk factors for all-cause mortality and cardiovascular disease (CVD) are common in patients with diabetes in China. We propose to compare the effectiveness of team-based care plus a clinical decision support system (CDSS) with team-based care alone on glycemic, blood pressure (BP), and lipid control, and clinical CVD reduction among patients with type-2 diabetes and at high risk for CVD. Lurking in plain sight: Hypertension awareness and treatment among New York City taxi/for-hire vehicle drivers.

Hypertension is a risk factor for cardiovascular disease, which is the leading cause of death in the United States. Taxi and for-hire vehicle (FHV) drivers, a largely male, immigrant and medically underserved population, are at increased risk of cardiovascular disease, in part due to the nature of their work. This study examined demographic and lifestyle predictors of hypertension diagnosis awareness, objectively measured blood pressure (hypertensive-range vs non-hypertensive-range readings), medication use, and hypertension control. A cross-sectional assessment was conducted with 983 male tax

Disparities in survival of hematologic malignancies in the context of social determinants of health: a systematic review.

Social determinants of health (SDHs) have been reported as relevant factors responsible for health inequity. We sought to assess clinical data from observational studies conducted in the United States evaluating the impact of SDHs on the outcomes of patients with hematologic malignancies. Thus, we performed a systematic review in 6 databases on 1 September 2021, in which paired reviewers independently screened studies and included data from 41 studies. We assessed the risk of bias using the Joanna Briggs Institute appraisal tools and analyzed the data using a descriptive synthesis. The most co

Rationale and design of the Clinic and Community Approaches to Healthy Weight Randomized Trial.

BACKGROUND: Recent studies have demonstrated the effectiveness of family-centered, pediatric weight management programs in reducing childhood obesity. Yet, programs to optimize the care of low-income children with obesity are needed. We sought to examine the comparative effectiveness of two, potentially scalable pediatric weight management programs delivered to low-income children in a clinical or community setting. MATERIALS AND METHODS: The Clinic and Community Approaches to Healthy Weight Trial is a randomized trial in two communities in Massachusetts that serve a large population of low-i

Distressed community index as a predictor of metabolic and bariatric surgery outcomes.

BACKGROUND: Obesity is associated with serious health and social consequences. Socioeconomically disadvantaged people are less likely to undergo metabolic and bariatric surgery (MBS) than their socioeconomically advantaged counterparts and experience increased complications or health care resource utilization, such as longer length of stay. The Economic Innovation Group's Distressed Communities Index (DCI) offers a metric to broadly assess socioeconomic distress. OBJECTIVES: This study investigated the relationship between community distress, as determined by DCI, and complications following

The Israeli eye care system through a public and global health lens.

Global eye health is increasingly recognized as integral to achieving universal health coverage (UHC), with equity, access, and service integration serving as key indicators of system performance. Israel provides a distinctive case study, having established UHC through the 1994 National Health Insurance Law, which guarantees essential services to all residents. This review assesses Israel’s eye care system through a public and global health lens, applying the WHO health system building blocks to examine its organization, strengths, and persistent challenges. Israel benefits from strong integra

Avoidable pitfalls on the path to health financing self-reliance in low-income and middle-income countries.

Low-income and middle-income countries (LMICs) are facing an urgent and complex challenge: how to transition to greater self-sustainability in health financing amid declining donor support. While this shift is inevitable, the policy responses it elicits carry significant implications for health system equity and access. This commentary highlights four policy choices increasingly observed in LMICs that we argue are unacceptable in the pursuit of sustainability. These include: (1) shifting the financial burden to out-of-pocket payments; (2) over-reliance on contributory health insurance schemes;

Implementation determinants of risk-stratified gestational diabetes mellitus screening in community-based women's peer groups in rural western Kenya.

BACKGROUND: Gestational Diabetes Mellitus (GDM) contributes to maternal and neonatal complications and is a growing concern in Kenya. The 2-hour oral glucose tolerance test (OGTT) is recommended to diagnose GDM between 24 and 28 weeks of pregnancy, but uptake remains low in settings experiencing financial and structural barriers to health care access. The Stratification Risk of Diabetes in Early pregnancy (STRiDE) tool is a risk-prediction model designed to identify women at elevated risk for GDM. The STRiDE-GDM screening approach integrates this tool within community-based peer groups to guid
