Simple conization and lymphadenectomy for the conservative treatment of stage IB1 cervical cancer. An Italian experience.

OBJECTIVES: Simple conization represents a plausible treatment scheme for managing stage IA1-2 tumors conservatively. However its curative potential has not been widely exploited as regards stage IB1 lesions. Recent studies suggest that, in selected circumstances, patients with stage IB1 disease undergoing radical hysterectomy could have been safely cured by simple hysterectomy and even by cervical conization. METHODS: Patients with stage IB1 cervical cancer desiring conservative management underwent simple conization and pelvic lymphadenectomy in three Italian institutes. RESULTS: Thirty-si

[Diagnosis and treatment of grade III cervical intraepithelial neoplasia by cervical conization in 98 patients].

OBJECTIVE: To explore the optimal diagnosis and treatment methods for grade III cervical intraepithelial neoplasia (CINIII). METHODS: Ninety-eight cases of CINIII initially diagnosed by colposcopical biopsy in our hospital from 2000 to 2008 were analyzed retrospectively in terms of the diagnosis, treatment and outcome. RESULTS: All the patients received cervical conization including 53 with cold knife conization and 45 with LEEP conization. Obvious differences were noted between cervical conization and colposcopical biopsy results in 15 cases (15.3%) diagnosed to be invasive carcinoma. Fifty

Role of frozen sections in the evaluation of moderate to severe dysplasia during uterine cervix conization.

BACKGROUND: The utility of frozen section examination (FSE) of cone specimens in evaluation of the resection margin status and in ruling out invasion in patients with high grade cervical intraepithelial neoplasia requires evaluation. METHODS: Twenty patients with high grade cervical intraepithelial neoplasia who underwent conization biopsy and frozen section examination were studied in a prospective trial from March 2008 through September 2009. The results with permanent paraffin sections were compared with those of FSE. RESULTS: Among the twenty cases, 15 (75%) had the same results in froze

Inflammatory cell infiltrate in the cervix as a predictor of residual cervical intraepithelial neoplasia after conization.

From January 1985 to January 1991, 112 patients underwent conization of the cervix for dysplasia. Twenty-one had incomplete resection, with cervical intraepithelial neoplasia (CIN) extending to the endocervical margins of resection. Only 10 had persistent CIN in follow-up. A decrease in the inflammatory cell infiltrate in the cervical stroma of the conization specimen was associated with persistent disease. A simple technique for quantitating this inflammatory cell infiltrate was used. Smoking was associated with a suppression of this local immune response in the cervix.

Conization for cervical dysplasia and carcinoma in situ: long term follow-up of 1013 women.

1013 women were treated with cold knife conization. Histopathological investigation of the cone biopsies revealed 169 with dysplasia, 718 with carcinoma in situ (CIS) and 26 with invasive carcinoma. Invasive carcinomas were found even in patients with the lowest degree of cytological abnormality. In 100 cases, no pathological changes were found. The majority of patients were followed for more than five years after conization. The longest period of follow-up was 17 years. In 98% of the women with various degrees of cervical dysplasia (including all severe dysplasias) the cytology returned to no

The Platelet-to-Lymphocyte Ratio (PLR) as a Non-Invasive Biomarker for Cervical Malignancy in Conization Patients.

BACKGROUND: Cervical cancer continues to pose a significant global health challenge, particularly in low-resource regions with limited access to advanced diagnostics. Cervical conization can occasionally uncover invasive carcinoma in patients initially suspected of having only pre-invasive lesions. This study assessed the platelet-to-lymphocyte ratio (PLR) as a potential predictive biomarker for identifying invasive disease in patients undergoing a loop electrosurgical excision procedure (LEEP). METHODS: A retrospective study was conducted on 371 patients who underwent LEEP conization for cer

3D ultrasound as a surgical quality control of conization in patients with severe dysplasia: a prospective study.

PURPOSE: To compare the techniques for cone measurement with ultrasound to determine the size of the resected tissue and to evaluate parameters which may be relevant for stratifying women at risk who need surveillance when pregnant. METHODS: The present study included women with a pathological cervical biopsy. Cervical length and volume were determined by transvaginal ultrasound prior to conization. The pathologist measured the volume of the removed tissue by the fluid displacement technique and using a ruler. A repeat transvaginal ultrasound was performed during a follow-up visit. Factors af

Postoperative cytological findings from the use of the Integran microfibrillar collagen hemostatic matrix in conization.

The Integran microfibrillar collagen hemostatic matrix is one form of microfibrillar collagen hemostat. This form has a sheet-type structure and has explicitly been used in Japan. In gynecology, this sheet-type matrix has helped effect uterine surface hemostasis, especially in myomectomy and cervical conization. However, cytotechnologists and pathologists have overlooked the foreign materials used for conization in postoperative cervical cytology. We report two cases describing the characteristic cervical cytology findings when Integran was used in conization. The first case was a 67-year-old

Outcome of pregnancy after laser conization: implications for infection as a causal link with preterm birth.

OBJECTIVE: To investigate a causal link between infection and preterm birth in women with a shortened cervix induced by prior laser conization. METHODS: We conducted a retrospective review of the outcomes of 47 singleton pregnancies with a history of laser conization. Cervical length was measured between 17 and 23 weeks of gestation. Receiver-operating characteristic curves were used to determine the best cut-off point for the cervical length for predicting spontaneous preterm birth. We measured infectious markers in 12 women who had experienced preterm labor. RESULTS: Nine women had had pre

Conization with a carbon dioxide laser as a uterine-preservation operation for patients with preinvasive cervical carcinoma.

Preinvasive cervical carcinoma (PCC) is a disease entity of the uterine cervix resulting from carcinoma in situ and various degrees of dysplasia. These cases are conventionally treated by a total abdominal hysterectomy. Effective management that can preserve the uterus is more desirable than hysterectomy. In this report, we present the results and complications of laser conization as a uterine-preservation treatment. At our dysplasia clinic, cases of reproductive age diagnosed with PCC can voluntarily undergo laser conization. From June 1985 to May 1988, there were 26 cases who received this t

Effect of CO2 laser conization of the uterine cervix on pathologic interpretation of cervical intraepithelial neoplasia.

The CO2 laser has become a valuable tool for the treatment of lower genital tract neoplasia. The records of 52 women who underwent CO2 laser cervical conization were analyzed retrospectively to evaluate the effect of tissue thermal damage on histopathologic interpretation. Lesion evaluability, defined by the ability to diagnose neoplasia accurately in the specimen, was satisfactory in only 26 cases; in the other 26, thermal damage was severe enough to preclude accurate diagnosis. A postoperative diagnosis of cervical intraepithelial neoplasia III was made in 17 instances, and microinvasion was
