[Current progress on characteristics of intracranial electrophysiology related to prolonged disorders of consciousness].

Prolonged disorders of consciousness (pDOC) are pathological conditions of alterations in consciousness caused by various severe brain injuries, profoundly affecting patients' life ability and leading to a huge burden for both the family and society. Exploring the mechanisms underlying pDOC and accurately assessing the level of consciousness in the patients with pDOC provide the basis of developing therapeutic strategies. Research of non-invasive functional neuroimaging technologies, such as functional magnetic resonance (fMRI) and scalp electroencephalography (EEG), have demonstrated that the

Modulation of Corticospinal Excitability during Action Observation in Patients with Disorders of Consciousness.

Brain imaging studies have recently provided some evidence in favor of covert cognitive processes that are ongoing in patients with disorders of consciousness (DoC) (e.g., a minimally conscious state and vegetative state/unresponsive wakefulness syndrome) when engaged in passive sensory stimulation or active tasks such as motor imagery. In this exploratory study, we used transcranial magnetic stimulation (TMS) of the motor cortex to assess modulations of corticospinal excitability induced by action observation in eleven patients with DoC. Action observation is known to facilitate corticospinal

Transcutaneous auricular vagus nerve stimulation in disorders of consciousness: A mini-narrative review.

In this mini review, 6 studies that investigated the effects of transcutaneous auricular vagus nerve stimulation (taVNS) in patients with disorders of consciousness (DOC) were reviewed. Generally, the application of taVNS in patients with DOC appears to be effective (positive results in 5 of 6 studies) and safe. Furthermore, 4 studies that evaluated changes in the brain following taVNS reported positive results (2 studies, functional magnetic resonance imaging and 2 studies, electroencephalography). Based on our review of the 6 studies, we believe that research and clinical application of taVN

Attitudes towards end-of-life issues in disorders of consciousness: a European survey.

Previous European surveys showed the support of healthcare professionals for treatment withdrawal [i.e., artificial nutrition and hydration (ANH) in chronic vegetative state (VS) patients]. The recent definition of minimally conscious state (MCS), and possibly research advances (e.g., functional neuroimaging), may have lead to uncertainty regarding potential residual perception and may have influenced opinions of healthcare professionals. The aim of the study was to update the end-of-life attitudes towards VS and to determine the end-of-life attitudes towards MCS. A 16-item questionnaire relat

Information processing in severe disorders of consciousness: vegetative state and minimally conscious state.

OBJECTIVE: To study the presence of electrophysiological indicators of remaining cortical functions in patients with persistent vegetative state (PVS) and minimally conscious state (MCS). Previous electrophysiological and PET data indicated that some PVS patients have partially intact cortical processing functions. However, it remains unclear whether the reported patients were representative for PVS population or just some exceptional cases. METHODS: Event-related brain responses to stimuli of different complexity levels, recorded in 98 patients with extremely severe diffuse brain injuries, 5

Residual Language Abilities Correlate With Diagnosis and Prognosis in Patients With Disorders of Consciousness.

BACKGROUND: Evaluating residual consciousness and predicting outcomes in unresponsive patients pose significant clinical challenges. Language assessment is crucial for these patients, and language-based EEG paradigms have shown promise in aiding diagnosis and prognosis in patients with disorders of consciousness (DoC). However, current language-based EEG paradigms either require higher-order cognitive functions, thereby misidentifying some conscious patients as unconscious, or rely on automatic processing, which fails to capture residual consciousness in DoC. METHODS: Newly developed, simple

Effects of General Anaesthesia on the Sleep Structure of Patients With Prolonged Disorders of Consciousness.

Sleep disturbances are common in patients with prolonged disorders of consciousness (pDoC) and are closely correlated with clinical outcomes. With an increasing number of pDoC patients undergoing surgery under general anaesthesia (GA), this study characterized sleep patterns in pDoC patients and evaluated the effects of GA on consciousness and nocturnal sleep. We prospectively enrolled 30 pDoC patients (14 in vegetative state [VS] and 16 in minimally conscious state [MCS]) undergoing spinal cord stimulation and 12 healthy controls. Among pDoC patients, 19 (10 VS, 9 MCS) with complete pre- and

From detection to decision: a conceptual framework of uncertainty migration in disorders of consciousness.

Accurate assessment of consciousness in patients with disorders of consciousness (DoC) remains a longstanding challenge in neurocritical care and neurorehabilitation. Although standardized behavioral assessments continue to serve as the clinical foundation for diagnosis, their reliance on overt behavioral responses may underestimate residual consciousness in a subset of patients. Recent advances in multimodal neuroimaging and neurophysiological techniques have substantially expanded the capacity to detect covert consciousness, reducing some of the inherent limitations of behavioral assessment

Medical AI, inductive risk and the communication of uncertainty: the case of disorders of consciousness.

Some patients, following brain injury, do not outwardly respond to spoken commands, yet show patterns of brain activity that indicate responsiveness. This is 'cognitive-motor dissociation' (CMD). Recent research has used machine learning to diagnose CMD from electroencephalogram recordings. These techniques have high false discovery rates, raising a serious problem of inductive risk. It is no solution to communicate the false discovery rates directly to the patient's family, because this information may confuse, alarm and mislead. Instead, we need a procedure for generating case-specific proba

The role of stimulus-induced skin conductance in disorder of consciousness: A systematic review.

Disorders of consciousness (DoC) are characterized by severe impairments in arousal or awareness and remain challenging to assess accurately with standard behavioral tools alone. Skin conductance (SC), as an index of autonomic arousal, may offer a complementary, motor-independent marker of residual responsiveness. This systematic review examined studies combining sensory stimulation paradigms with SC recordings in patients with DoC, aiming to identify which stimuli are most frequently employed and how they relate to clinical and neurophysiological outcomes. A comprehensive search of PubMed, Sc

Participatory Development of an International Information Brochure on the Multimodal Assessment of Disorders of Consciousness.

BACKGROUND: Disorders of consciousness (DoC) refers to a group of clinical conditions of altered consciousness. To improve their diagnosis and prognosis, multimodal assessment can be of great importance. Informal caregivers of people with DoC who are confronted with new technologies as such can benefit from interventions to expand their health literacy, i.e., the ability to use information to make health decisions for oneself and others. METHODS: We developed an information brochure on multimodal assessment for DoC in a participatory process, with decisions made by a steering group. The proce

Network Mapping of Connectivity Alterations in Disorder of Consciousness: Towards Targeted Neuromodulation.

Disorder of consciousness (DoC) refers to a group of clinical conditions that may emerge after brain injury, characterized by a varying decrease in the level of consciousness that can last from days to years. An understanding of its neural correlates is crucial for the conceptualization and application of effective therapeutic interventions. Here we propose a quantitative meta-analysis of the neural substrate of DoC emerging from functional magnetic resonance (fMRI) and positron emission tomography (PET) studies. We also map the relevant networks of resulting areas to highlight similarities wi

Effect of acoustic stimuli in patients with disorders of consciousness: a quantitative electroencephalography study.

Auditory stimuli are proposed as beneficial neurorehabilitation methods in patients with disorders of consciousness. However, precise and accurate quantitative indices to estimate their potential effect remain scarce. Fourteen patients were recruited from the Neuro-Rehabilitation Unit of Hangzhou Hospital of Zhejiang Armed Police Corps of China. Altogether, there were seven cases of unresponsive wakefulness syndrome (five males and two females, aged 45.7 ± 16.8 years) and seven cases of minimally conscious state (six males and one female, aged 42.3 ± 20.8 years). Simultaneously, fourteen healt
