Effects of isoflurane and sevoflurane alone and in combination with butorphanol or medetomidine on the bispectral index in chickens.

BACKGROUND: The bispectral index (BIS) is an anaesthesia monitoring technique able to assess the level of central nervous system depression in humans and various animal species. In birds, it has been validated in chickens undergoing isoflurane anaesthesia. The aim of this study was to evaluate in an avian species the influence of isoflurane and sevoflurane on BIS, each at different minimum anaesthetic concentrations (MAC) multiples, alone or combined with butorphanol or medetomidine. Ten chickens (5 males and 5 females) underwent general anaesthesia with isoflurane or sevoflurane alone, and co

Suxamethonium induces a prompt increase in the bispectral index.

Upon inducting general anesthesia in the operating room, we have observed a prompt increase in the bispectral index (BIS) after the intravenous injection of suxamethonium. We hypothesized that the cause of this BIS increase is muscle hyperactivity owing to fasciculation. However, no reports have been published regarding this abrupt increase in the BIS upon the induction of general anesthesia by suxamethonium. To investigate the degree of change in the BIS in patients receiving anesthesia with suxamethonium, we performed a prospective observational study of 63 participants who underwent closed

Automation of anaesthesia: a review on multivariable control.

Anaesthesia is a multivariable problem where a combination of drugs are used to induce desired hypnotic, analgesia and immobility states. The automation of anaesthesia may improve the safety and cost-effectiveness of anaesthesia. However, the realization of a safe and reliable multivariable closed-loop control of anaesthesia is yet to be achieved due to a manifold of challenges. In this paper, several significant challenges in automation of anaesthesia are discussed, namely model uncertainty, controlled variables, closed-loop application and dependability. The increasingly reliable measurement

Simultaneous bispectral index and near-infrared spectroscopy monitoring to evaluate two induction protocols in elective cardiovascular surgery.

BACKGROUND: Laryngoscopy and intubation provoke strong sympathetic activation and may cause hemodynamic instability in cardiovascular surgery patients. Maintaining adequate hypnotic depth and cerebral oxygenation during induction is essential. Although BIS and NIRS are widely used, their simultaneous behavior during induction in a homogeneous cardiovascular surgery population is not well established. OBJECTIVES: To compare thiopental–low-dose fentanyl and midazolam–high-dose fentanyl induction protocols regarding their effects on BIS, regional cerebral oxygen saturation (rSO₂), and hemodynamic

Awareness and bispectral index (BIS) monitoring in mechanically ventilated patients in the emergency department and intensive care unit: a systematic review protocol.

INTRODUCTION: Accidental awareness with recall is one of the most feared complications for patients undergoing general anaesthesia and can lead to post-traumatic stress disorder in up to 70% of patients experiencing it. To reduce the incidence of awareness with recall, the bispectral index monitor is recommended for patients receiving total intravenous anaesthetics, especially those receiving neuromuscular blockers. While extensive investigation into awareness and bispectral index monitoring has occurred for operating room patients, this has not extended to other clinical arenas where sedated

Anaesthesia for electroconvulsive therapy - new tricks for old drugs: a systematic review.

OBJECTIVE: The objective of this review is to investigate existing literature in order to delineate whether the use of anaesthesia and timing of seizure induction in a new and optimised way may improve the efficacy of electroconvulsive therapy (ECT). METHODS: PubMed/MEDLINE was searched for existing literature, last search on 24 June 2015. Relevant clinical studies on human subjects involving choice of anaesthetic, ventilation and bispectral index (BIS) monitoring in the ECT setting were considered. The references of relevant studies were likewise considered. RESULTS: Propofol yields the sho

The effects of atracurium on bispectral index (BIS) values in dogs anaesthetized with isoflurane.

Balanced anaesthesia techniques employ specific drugs in addition to general anaesthetics. The potential effect of some of these drugs on the bispectral index (BIS) remains unclear. BIS seems to be unaffected by the administration of neuromuscular blockers while opioids may block nociceptive-induced BIS increases in anaesthetized humans. In this study, the effect of a single dose of atracurium on BIS values was assessed in dogs premedicated with morphine and anaesthetized with isoflurane (1 MAC). Fifteen female dogs undergoing elective ovariohysterectomy were employed. BIS values were recorded

Use of bispectral index system (BIS) to Monitor Enteral Conscious (moderate) sedation during general dental procedures.

Although dental board regulations for the provision of in-office enteral conscious (oral) sedation vary widely with respect to training and pharmacologic strategies, they agree on the use of drugs that are inherently safe, the use of pulse oximetry and the availability of emergency equipment, including pharmacologic antagonists. Patient safety is of greatest concern and is best addressed by appropriate selection of patients, adequate training of personnel and appropriate monitoring of patients. Readings from bispectral index system (BIS) monitors, which use electroencephalographic signals, cor

EEG signature of near-death-like experiences during syncope-induced periods of unresponsiveness.

During fainting, disconnected consciousness may emerge in the form of dream-like experiences. Characterized by extra-ordinary and mystical features, these subjective experiences have been associated to near-death-like experiences (NDEs-like). We here aim to assess brain activity during syncope-induced disconnected consciousness by means of high-density EEG monitoring. Transient loss of consciousness and unresponsiveness were induced in 27 healthy volunteers through hyperventilation, orthostasis, and Valsalva maneuvers. Upon awakening, subjects were asked to report memories, if any. The Greyson

[Ventricular fibrillation following hyperventilation and apneic underwater swimming].

This is a case study of an 18 years old boy who lost consciousness during apneic underwater swimming. When cardiopulmonary resuscitation was initiated ventricular fibrillation was seen on cardiac monitoring. Bradycardia, atrial and ventricular premature beats are a known response to hyperventilation and apneic underwater diving. This case is the first documentation of ventricular fibritllation as a cause of sudden cardiac death during apneic underwater swimming.

Striatal neurones expressing D1 dopamine receptors modulate consciousness in sevoflurane but not propofol anaesthesia in mice.

BACKGROUND: Sevoflurane and propofol are the most widely used inhaled and i.v. general anaesthetics, respectively. The mechanisms by which sevoflurane and propofol induce loss of consciousness (LOC) remain unclear. Recent studies implicate the brain dopaminergic circuit in anaesthetic-induced LOC and the cortical-striatal-thalamic-cortical loop in decoding consciousness. We investigated the contribution of the dorsal striatum, which is a critical interface between the dopaminergic circuit and the cortical-striatal-thalamic-cortical loop, in sevoflurane and propofol anaesthesia. METHODS: Elect

[Flash-evoked visual potentials in the early diagnosis of optic nerve injury due to craniofacial fractures].

Impairment or loss of vision due to optic nerve injury occurs in about 10% of patients with cranio-facial fractures. The assessment of optic nerve function is important for decisions regarding optic nerve decompression. But examination of vision and pupillary reflexes may be difficult, especially in uncooperative patients with reduced consciousness and primary disturbances of pupillary functions. In these cases, optic nerve function can be monitored by means of flash-evoked visual potentials elicited by use of a LED-goggle stimulator. VEPs were recorded in ten patients with head injuries compr

Functional near-infrared spectroscopy for the assessment and treatment of patients with disorders of consciousness.

BACKGROUND: Advances in neuroimaging have significantly enhanced our understanding of brain function, providing critical insights into the diagnosis and management of disorders of consciousness (DoC). Functional near-infrared spectroscopy (fNIRS), with its real-time, portable, and noninvasive imaging capabilities, has emerged as a promising tool for evaluating functional brain activity and nonrecovery potential in DoC patients. This review explores the current applications of fNIRS in DoC research, identifies its limitations, and proposes future directions to optimize its clinical utility. AI

Bradycardia in a newborn with accidental severe hypothermia: treat or don't touch? A case report.

BACKGROUND: Hypothermia significantly affects mortality and morbidity of newborns. Literature about severe accidental hypothermia in neonates is limited. We report a case of a neonate suffering from severe accidental hypothermia. An understanding of the physiology of neonatal thermoregulation and hypothermia is important to decide on treatment. CASE PRESENTATION: A low-birth-weight newborn was found with severe accidental hypothermia (rectal temperature 25.7 °C) due to prolonged exposure to low ambient temperature. The newborn presented bradycardic, bradypnoeic, lethargic, pale and cold. Brad

Perservative paroxysmal atrioventricular block : Cardiac syncope misdiagnosed as anxiety for more than 20 years.

A 42-year-old woman was referred for cardiac diagnostic work-up of loss of recurrent consciousness over the past 25 years. She received medication with an anxiolytic, an antidepressant, and a neuroleptic drug. After a normal resting ECG, there were 112 episodes of paroxysmal atrioventricular block III° in her 24 h Holter recording with asystole for up to 27 s. The patient was symptomatic only once with dizziness due to an asystole of 8.8 s while she was awake in the early morning. After DDDR pacemaker implantation, the patient was asymptomatic during the following 2 years. This case illustrate

Syncope: a videometric analysis of 56 episodes of transient cerebral hypoxia.

To investigate the clinical features of transient cerebral hypoxia, syncope was induced in 56 of 59 healthy volunteers through a sequence of hyperventilation, orthostasis, and Valsalva maneuver. All events were monitored on video by two cameras. Complete syncope with falling and loss of consciousness was observed in 42 subjects, lasting 12.1 +/- 4.4 seconds. Myoclonic activity occurred in 38 of these 42 episodes (90%). The predominant movement pattern consisted of multifocal arrhythmic jerks both in proximal and distal muscles. Superposition of generalized myoclonus was common. Additional move

Photo-evoked eyelid microvibration in neonates with hyperosmolality.

We studied neonates with hyperosmolar syndrome using photo-evoked eyelid microvibration, a blink reflex elicited by photic stimuli. Patients with hyperosmolality showed disturbance of consciousness, convulsive seizures, hyperexcitability, and tachypnea. The latency of photo-evoked eyelid microvibration tended to be prolonged with an elevation of the plasma osmolality. Photo-evoked eyelid microvibration is believed to be a suitable method for monitoring brain disturbance due to hyperosmolality.

Spatiospectral dynamics of electroencephalography patterns during propofol-induced alterations of consciousness states.

Altered consciousness induced by anesthetics is characterized by distinct spatial and spectral neural dynamics that are readily apparent in the human electroencephalogram. Despite considerable study, we remain uncertain which brain regions and neural oscillations are involved, as well as how they are impacted when consciousness is disrupted. The experimental data was obtained from the open-access dataset, which contains pre-processed EEG data recorded from 20 healthy participants during propofol sedation. Using unsupervised machine learning methods (i.e., non-negative matrix factorization, NMF
