BackgroundThe atypical upper limb (UL) flexion pattern in children with unilateral cerebral palsy (UCP) debilitates their ability to experience natural weight bearing through arms and hands, leading to restricted mobility and reduced hand functions. The recommended therapeutic strategies to improve hand functions are not standardized, i.e., (varied protocols, treatment schedules, and frequency), though the benefits of UL weight-bearing on hand-opening and prehension skills are well-documented.ObjectiveTo study the efficacy of Static weight bearing (SWB) in Prone on UL with active elbow extensi

Constraint-induced movement therapy (CIMT) for the lower extremities CIMT (LE-CIMT) has been shown feasible and promising but the long-term outcomes remain uncertain. In this pilot study, we recruited eight participants with chronic stroke from our facility for persons with disabilities to determine changes in gait and balance throughout an extended treatment program based on the principles of LE-CIMT. The program consisted of a run-in phase (3 weeks), LE-CIMT phase (3 weeks), and maintenance phase (6 months). In the LE-CIMT phase (3.5 h/day, 5 days/week, 3 weeks), the participants received ta

The objective of this study was to test the feasibility of a treatment programme based on the elements of constraint-induced movement therapy (CIMT) to encourage use of the affected arm of a child with obstetric brachial plexus injury (OBP), as well as to document clinical changes observed with this intervention. A 2-year-old female child with Erb's palsy had 14 weeks of daily home-based treatment with 30-minute sessions planned according to the principles of CIMT. The child was assessed every 2 weeks with the Toddler Arm Use Test. Test scores throughout the intervention period demonstrated im

BACKGROUND: Constraint-induced movement therapy (CIMT) and transcranial direct current stimulation (tDCS) are used to reduce interhemispheric imbalance after stroke, which is why the combination of these therapies has been used for neurological recovery, but not in the acute phase.
OBJECTIVES: To evaluate the effectiveness of combining active or sham bihemispheric tDCS with modified CIMT (mCIMT) for the recovery of the Upper Limb (UL) in hospitalized patients with acute and subacute stroke.
METHODS: This randomized controlled, double-blind, placebo-controlled, parallel group clinical trial w

BackgroundThe upper extremity functional recovery remains a major challenge for specialists during post stroke rehabilitation, pertaining to adoption of various compensatory strategies by patients leading to motor redundancy and persistent functional limitations.ObjectivesThe study aimed to evaluate the effectiveness of Patient Tailored-Modified Constraint Induced Movement Therapy (mCIMT) combined with Trunk Restraint (TR) on Upper Extremity motor ability, balance, and health-related quality of life (HR-QOL) in chronic stroke survivors.MethodsIn this patient-blinded trial, 34 voluntary patient

INTRODUCTION: Children with congenital hemiplegia often present with limitations in using their impaired upper limb which impacts on independence in activities of daily living, societal participation and quality of life. Traditional therapy has adopted a bimanual training approach (BIM) and more recently, modified constraint induced movement therapy (mCIMT) has emerged as a promising unimanual approach. Evidence of enhanced neuroplasticity following mCIMT suggests that the sequential application of mCIMT followed by bimanual training may optimise outcomes (Hybrid CIMT). It remains unclear whet

BACKGROUND: Constraint-Induced Movement Therapy (CIMT) is a "rehabilitation strategy" supported by numerous basic studies. However, its clinical implementation is often limited by the poor ability of the affected limb.
OBJECTIVES: To combine mCIMT plus a SaeboGlove for persons who would not normally qualify for mCIMT training.
METHODS: Part A: mCIMT+Saebo group (n = 18) and mCIMT group (n = 19). Part B: mCIMT+Saebo group (n = 10) and control group (n = 9). Both parts were assessor-blinded, two-arm, parallel-group randomized controlled trials. In addition to conventional rehabilitation therap

BACKGROUND: Motor impairments contribute substantially to long-term disability following stroke. Studies of transcranial direct current stimulation (tDCS), combined with various rehabilitation therapies, have shown promising results in reducing motor impairment. We aimed to evaluate the safety and efficacy of three doses of tDCS in combination with modified constraint-induced movement therapy (mCIMT) in people who have had their first ischaemic stroke in the preceding 1-6 months.
METHODS: We conducted a phase 2, multicentre, randomised, triple-blind, sham-controlled study with a blinded centr

INTRODUCTION: Stroke remains a leading cause of death and long-term disability worldwide, with the majority of survivors experiencing functional impairments, particularly affecting the upper extremities (UEs). Although clinically widespread rehabilitation methods, such as physical and occupational therapy, provide some benefits, their efficacy in restoring UE function is often limited. Constraint-induced movement therapy (CIMT) is widely recognised as one of the most effective physical therapy interventions for improving motor outcomes in the paretic UE. Meanwhile, non-invasive brain stimulati

IMPORTANCE: Pediatric constraint-induced movement therapy (PCIMT) is increasing as a treatment method for children with unilateral hemiparesis, but limited evidence has been published on the outcomes of clinic-based PCIMT programs.
OBJECTIVE: To evaluate the outcomes of a clinic-based PCIMT program on upper extremity movement quality of children with unilateral hemiparesis.
DESIGN: Quantitative, retrospective study.
SETTING: A children's hospital in the southern United States.
PARTICIPANTS: Twenty-six children ages 2.5 to 8 yr with unilateral hemiparesis who previously completed a 3-wk PCI

OBJECTIVE: Upper-extremity functional improvements after constraint-induced movement therapy have not been documented beyond 2 years. This case report describes the long-term maintenance of the effects of change 4-5 years after an application of constraint-induced therapy.
METHOD: A 36-year-old female poststroke patient participated in constraint-induced therapy for 2 weeks. She was evaluated before and after treatment and again 4 and 5 years later. Primary outcome measures included the Wolf Motor Function Test, Stroke Impact Scale, and Motor Activity Log.
RESULTS: Improvements were maintain

CONTEXT: Rehabilitation for children with congenital hemiplegia to improve function in the impaired upper limb and enhance participation may be time-consuming and costly.

BACKGROUND: Children with unilateral cerebral palsy (UCP) learn not to use their affected upper limb, which is known as "developmental disregard". Consequently, early intensive training could improve hand function, increasing spontaneous use for the first 2 years of life, where there is a great window for brain changes due to neural plasticity.
OBJECTIVE: To study the effectiveness of home-based infant-intensive manual therapies in facilitating bimanual functional performance (BFP) compared to standard care in children with UCP aged 9-18 months.
METHODS: A randomized controlled trial with si

Brachial plexus birth injuries (BPBIs) can affect children's occupational participation. This review examined the efficacy of non-surgical interventions used by occupational therapy for increasing the range of motion (ROM) and improving motor skills of the affected upper extremities (UEs) of children with BPBIs. Non-surgical interventions are typically used as first-line treatment or in adjunct to surgery with the aim of optimizing recovery, decreasing risk of secondary impairment, and promoting occupational participation. We conducted a review to determine the effectiveness of interventions to improve occupational performance in people with motor impairments after stroke as part of the American Occupational Therapy Association's Evidence-Based Practice Project. One hundred forty-nine studies met inclusion criteria. Findings related to key outcomes from select interventions are presented. Results suggest that a variety of effective interventions are available to improve occupational performance after stroke. Evidence suggests that repetitive task practice, constraint-induced or modified constraint-induced moveme
