Anatomical study for the treatment of proximal humeral fracture through the medial approach.

BACKGROUND: The treatment of complex 3- and 4-part proximal humeral fractures has been controversial due to numerous postoperative complications. With the further study of medial support and blood supply of humeral head, new techniques and conception are developing. The study aims to illustrate the medial approach of the proximal humeral fracture through cadaver autopsy. METHOD: Upper limbs from 19 cadavers have been dissected to expose the shoulder joint. We selected the coracoid process as the bony reference. Vernier caliper will be used to measure the following data, including distance fro

Nerve supply of the subscapularis during anterior shoulder surgery: definition of a potential risk area.

PURPOSE: The purpose of the study was to evaluate the position of the subscapular nerves relative to surgical landmarks during exposure and to analyze the pattern of innervation of the subscapularis to avoid injury during anterior shoulder surgery. METHODS: 20 embalmed human cadaveric shoulder specimens were used in the study. The muscular insertions of the subscapular nerves were marked and their closest branches to the musculotendinous junction and the coracoid process were measured in horizontal and vertical distances. In addition, the innervation pattern of each specimen was documented.

Monocortical fixation of the coracoid in the Latarjet procedure is significantly weaker than bicortical fixation.

PURPOSE: A crucial step of the Latarjet procedure is the fixation of the coracoid process onto the glenoid. Multiple problems associated with the fixation have been described, including lesions of the suprascapular nerve due to prominence of the screw or bicortical drilling. The purpose of the present study was to evaluate whether monocortical fixation, without perforating the posterior glenoid cortex, would provide sufficient graft stability. METHODS: Coracoid transfer was performed in 14 scapula models (Sawbones®, Composite Scapula, 4th generation). Two groups were assigned: in one group, f

Treatment of avulsed clavicle and recurrent subluxations of the ipsilateral shoulder by dynamic fixation.

A new operative approach is presented for treatment of fractured distal end of clavicle associated with recurrent anterior shoulder subluxations. One case is presented in which open reduction of the fractured clavicle and stabilizing the anterior shoulder joint were accomplished by transfer of the coracoid process with its attached muscles under the subcapularis muscle onto the clavicle. The operation was followed by complete reduction of the fracture and a stable shoulder joint.

Open-Guided Latarjet With Suture-Button Fixation: Early Experience.

Recurrent anterior shoulder instability associated with glenoid bone loss poses a significant surgical challenge. The open Latarjet procedure with screw fixation remains a reliable option. However, screw-related complications, such as hardware irritation and graft fracture, have prompted the exploration of alternative fixation systems. This article describes a guided open Latarjet technique using dual suture-button constructs for coracoid fixation and reports early clinical and radiographic outcomes. Five male patients (median age, 27.5 y) with recurrent anterior instability and glenoid bone l

Chronic unreduced anterior shoulder dislocation: application of anatomy to forensic identification.

The uniqueness of anatomic structures and their variations provide the basis for forensic identification. During the identification process of 1991-1995 Croatian war victims, forensic pathologists and forensic anthropologists were faced with the problem of incomplete skeletal remains and insufficient premortem medical data. Here we present a case of anterior shoulder dislocation, which was a key confirmation in the identification of a war victim. In our case, anterior shoulder dislocation was not medically treated for 15 years, and the changes in the glenoid cavity and coracoid process indicat

Open Shoulder Stabilization for Anterior Glenohumeral Instability: The Latarjet Procedure.

BACKGROUND: Traumatic anterior glenohumeral instability is a commonly encountered shoulder pathology among young active patients. Nonoperative management and arthroscopic techniques are associated with high rates of reoccurrence, particularly in patient populations with known major risk factors. INDICATIONS: The coracoid process transfer (Latarjet procedure) is typically reserved for cases of anterior glenohumeral instability where glenoid deficiency exceeds 20% to 25% or when soft tissue stabilization techniques have failed. TECHNIQUE DESCRIPTION: We use a standard open deltopectoral approa

Predictors of coracoid graft osteolysis after the Latarjet procedure: A systematic review.

BACKGROUND: Coracoid graft osteolysis is a known complication following the Latarjet procedure and may influence postoperative shoulder stability. While prior studies have reported variable rates of graft osteolysis, the risk factors that influence this process are not well defined. Therefore, the purpose of this systematic review was to investigate the risk factors associated with coracoid graft osteolysis after the Latarjet procedure. We hypothesized that the location of coracoid graft placement on the glenoid would influence graft osteolysis. METHODS: This review followed PRISMA guidelines

Stress distribution patterns at the coracoacromial arch in rotator cuff tear measured by computed tomography osteoabsorptiometry.

When a rotator cuff tear occurs, forces compressing the humeral head toward the glenoid are disturbed, and the kinematics of the glenohumeral joint change. Therefore, stress distributions at the coracoacromial arch in cuff tear shoulders should differ from those in normal shoulders. To investigate this hypothesis, we studied stress distribution patterns at the coracoacromial arch in normal and cuff tear shoulders using a computed tomography (CT) osteoabsorptiometry method, in which bone density correlates directly with long-term physiologic loading. Eight normal subjects and 11 patients with c

Anatomical study of the musculocutaneous nerve branching pattern: application for selective neurectomy in the treatment of elbow flexors spasticity.

PURPOSE: Spastic flexion deformity of the elbow is mainly mediated by the biceps brachii and the brachialis muscles, innervated by the musculocutaneous nerve. Selective neurectomy of the musculocutaneous nerve showed promising results to relieve excessive spasticity in the long term but lacks of a consensual surgical strategy. The aim of the study was to describe the distal branching pattern of the motor branches of the musculocutaneous nerve in an attempt to develop guidelines for surgery. METHODS: Sixteen arms of fresh cadaver specimen were dissected. We recorded the site of each primary an

Arthroscopic, Implant-Free Bone-Grafting for Shoulder Instability with Glenoid Bone Loss.

UNLABELLED: Anterior shoulder instability is commonly associated with injuries to the labrocapsular structures of the glenohumeral joint and with osseous glenoid defects of varying degree. If the bone defect exceeds a clinically relevant threshold, mere soft-tissue repairs result in high rates of recurrent instability and the glenoid needs to be reconstructed with osseous augmentation. The most popular techniques for glenoid reconstruction include variations of coracoid transfer or bone-block procedures; however, these procedures are not free from risks and complications, which are often relat

[A modified Putti-Platt surgical method in the treatment of habitual luxation of the anterior shoulder: our experience].

Fifty cases of recurrent dislocation of the shoulder operated according to the Putti-Platt method modified by lateralization of the insertion of the coracoid processes are reported. Excellent results were obtained in 86% of the cases. There was neither incidence of recurrence, nor degenerative joint alteration. Mild limitation in extrarotation (less than 10 degrees) was observed in 7 cases.

'Superior biceps aponeurosis'-Morphological characteristics of the origin of the short head of the biceps brachii muscle.

PURPOSE: Our aim was to characterize the morphology of the proximal attachment of the biceps brachii short head. We hypothesize that it has an aponeurotic component that may affect shoulder joint biomechanics. METHODS: The coracoacromial region and the biceps brachii muscle were dissected in 30 cadaveric shoulders. The course and dimensions of the tendon and aponeurosis were evaluated. The cross-sectional area of the belly of the short head and the length of the whole muscle were measured. Correlations between the aponeurosis and dimensions of the muscle were tested with the Spearman's rank c

Full exposure of the axillary and suprascapular nerves.

We describe a method of exposing the whole length of the axillary and suprascapular nerves through a sabre-cut incision. The coracoid process is osteotomised and part of trapezius is detached from its insertions. The posterior deltoid is freed from its scapular origin to expose infraspinatus. We have used this approach to explore combined injuries of the axillary and suprascapular nerves with good results, and no serious complications. Its success depends on a meticulous surgical technique.

[Enthesopathy of the coraco-acromial ligament and rupture of the rotator cuff].

Ossification of the coracoacromial ligament was looked for routinely on the roentgenograms of 50 patients with arthrographically-documented rotator cuff tear and of 25 controls free of clinical evidence of shoulder disease. Ossification of the coracoacromial ligament was seen in none of the 25 controls versus 12 of the 50 patients. The ossification consistently arose from the acromial attachment site. A few patients developed ossification of the entire ligament bridging the gap between the acromion and coracoid process.

Technical tip: tightrope fixation of neer type II distal clavicle fracture supported by a case series.

BACKGROUND: We present a case series of 3 patients who underwent a novel technique of tight rope fixation for Neer type II distal clavicle fracture. MATERIAL AND METHODS: 2-3 cm incision was made lateral to the fracture site moving inferomedially. Part of the distal end of clavicle was exposed close to fracture site and further dissection was carried out to reveal the coracoid process. Tight rope fixation of the distal ends of clavicle and coracoid was performed to achieve satisfactory fracture reduction on x-ray. RESULTS: 4 weeks of sling with gentle pendulum movement were followed by activ

Local Subcutaneous Fat Depth Is Associated With Increased Complications After Total Shoulder Arthroplasty.

BACKGROUND: Increased body mass index (BMI) is a well-documented risk factor for complications after lower extremity arthroplasty. The literature in total shoulder arthroplasty (TSA) displays notable variation. The utility of BMI in predicting complications in TSA may be limited because of differences in weight distribution. Prior studies in spine surgery have demonstrated increased complications based on subcutaneous fat depth at the incision site. The purpose of this study was to determine whether tissue depth may be associated with an increased risk of complication after primary TSA. METHO

A novel infraclavicular brachial plexus block: the lateral and sagittal technique, developed by magnetic resonance imaging studies.

UNLABELLED: A new infraclavicular brachial plexus block method has the patient supine with an adducted arm. The target is any of the three cords behind the pectoralis minor muscle. The point of needle insertion is the intersection between the clavicle and the coracoid process. The needle is advanced 0 degrees -30 degrees posterior, always strictly in the sagittal plane next to the coracoid process while abutting the antero-inferior edge of the clavicle. We tested the new method using magnetic resonance imaging (MRI) in 20 adult volunteers, without inserting a needle. Combining 2 simulated need

The musculocutaneous nerve and its branches to the biceps and brachialis muscles.

The musculocutaneous nerve and its motor branches to the biceps and brachialis were dissected and studied under the operating microscope in 24 fresh-frozen cadaveric specimens. The motor branch to the biceps exits from the musculocutaneous nerve at 119 mm distal to the coracoid process. Anatomic variations were seen in the innervation of the two heads of the biceps. A common primary motor branch that bifurcates to supply the two heads was seen in 20 specimens (type I). Two specimens were observed to have two separate primary branches originating from the main musculocutaneous nerve trunk to in
