Human immunodeficiency virus serodiscordant couples on highly active antiretroviral therapies with undetectable viral load: conception by unprotected sexual intercourse or by assisted reproduction techniques?

Until recently, only assisted reproduction was proposed to serodiscordant couples wishing to conceive. Nevertheless, recent publications have proposed unprotected sexual intercourse, targeting fertile days, for couples where antiretroviral treatment has lowered blood viral load to an undetectable level. Available data and the arguments for and against conception by safe sex versus the use of a strategy of unprotected sexual intercourse targeting fertile days are reviewed and analyzed. Although the rate of transmission of human immunodeficiency virus in serodiscordant couples in precise conditi

MoS2/Au-Ag@PEG nanosheets with plasmonic coupling effect-enhanced NIR-II photothermal therapy and silver ion release for combined treatment of MRSA infection.

The evolution of bacterial resistance to antibiotics has resulted in a global public health crisis, necessitating the development of novel antibiotic-independent antimicrobial strategies. In this study, MoS2/Au-Ag@PEG nanosheets (MAAP NSs) were prepared via sequential deposition of gold and silver nanoparticles onto MoS2 nanosheets (MoS2 NSs), which were then used for the treatment of methicillin-resistant Staphylococcus aureus (MRSA) infections. Compared to MoS2 NSs, MAAP NSs exhibit a significantly enhanced near-infrared region II (NIR-II) absorption at 1064 nm (a 7.51-fold increase), and th

Targeted Photodynamic Therapy using a Vectorized Photosensitizer coupled to Folic Acid Analog induces Ovarian Tumor Cell Death and inhibits IL-6-mediated Inflammation.

Ovarian cancer (OC) is one of the most lethal cancers among women. Frequent recurrence in the peritoneum due to the presence of microscopic tumor residues justifies the development of new therapies. Indeed, our main objective is to develop a targeted photodynamic therapy (PDT) treatment of peritoneal carcinomatosis from OC to improve the life expectancy of cancer patients. Herein, we propose a targeted-PDT using a vectorized photosensitizer (PS) coupled with a newly folic acid analog (FAA), named PSFAA, in order to target folate receptor alpha (FRα) overexpressed on peritoneal metastasis. This

Anticoagulation therapy for postoperative deep vein thrombosis coupled with pulmonary embolism: a potential trigger for delayed popliteal artery injury presentation after total knee arthroplasty.

OBJECTIVES: A case of delayed popliteal artery injury (PA-INJ) onset after total knee arthroplasty (TKA) in a patient under postoperative anticoagulation therapy is reported. The role of anticoagulation both in late PA-INJ presentation and in PA-INJ management is discussed. REPORT: An obese 76-year-old woman was presented with a common femoral vein thrombosis coupled with pulmonary embolism five days after TKA. She was immediately placed under anticoagulation therapy. Four days later, during physiotherapy-supervised mobilization, she developed a calf haematoma and large retro-articular pseudo

Association between different levels of suppressed viral load and the risk of sexual transmission of HIV among serodiscordant couples on antiretroviral therapy: a protocol for a two-step systematic review and individual participant data meta-analysis.

INTRODUCTION: HIV is a major global public health issue. The risk of sexual transmission of HIV in serodiscordant couples when the partner living with HIV maintains a suppressed viral load of <200 copies of HIV copies/mL has been found in systematic reviews to be negligible. A recent systematic review reported a similar risk of transmission for viral load<1000 copies/mL, but quantitative transmission risk estimates were not provided. Precise estimates of the risk of sexual transmission at sustained viral load levels between 200 copies/mL and 1000 copies/mL remain a significant gap in the liter

Scalp acupuncture coupled with speech-language therapy modulates the ventral-dorsal structural brain network in patients with post-stroke anomia: a randomized clinical and DTI neuroimaging trial.

BACKGROUND: Scalp acupuncture (SA) is used as adjunctive rehabilitation for post-stroke aphasia, but its effect on naming recovery and structural language-network changes remains unclear. We evaluated SA coupled with speech-language therapy (SLT) in patients with post-stroke anomia and explored diffusion tensor imaging markers of ventral-dorsal structural language networks. METHODS: This randomized controlled trial recruited 85 patients with post-stroke anomia and randomly assigned them to SA or control. Both groups received conventional stroke treatment and SLT; the SA group additionally rec

Longitudinal Coupling between Eating Disorder Psychopathology and Depression in Patients with Anorexia Nervosa and Bulimia Nervosa Treated with Enhanced Cognitive Behavior Therapy: A One-Year Follow-Up Study.

BACKGROUND: The relationship between eating disorder (ED) specific psychopathology and depressive symptomatology in EDs is often debated. The aim of this study was to provide an explicative model regarding the mechanisms by which enhanced cognitive-behavior therapy (CBT-E) might determine an amelioration of depressive symptoms in patients with anorexia nervosa (AN) or bulimia nervosa (BN). METHODS: A total of 157 women with AN or BN and no history of childhood trauma or bipolar disorder were evaluated before treatment and after 12 months of CBT-E. Self-administered questionnaires were used to

Effects of Synchronizing Foot Strike and Cardiac Phase on Exercise Hemodynamics in Patients With Cardiac Resynchronization Therapy: A Within-Subjects Pilot Study to Fine-Tune Cardio-Locomotor Coupling for Heart Failure.

BACKGROUND: Despite advances in medical and cardiac resynchronization therapy (CRT), individuals with chronic congestive heart failure (CHF) have persistent symptoms, including exercise intolerance. Optimizing cardio-locomotor coupling may increase stroke volume and skeletal muscle perfusion as previously shown in healthy runners. Therefore, we tested the hypothesis that exercise stroke volume and cardiac output would be higher during fixed-paced walking when steps were synchronized with the diastolic compared with systolic portion of the cardiac cycle in patients with CHF and CRT. METHODS: T

Portraying Mixed-Agenda Couples: Insights From Large-Scale Data Sets.

This study investigates "mixed-agenda" couples characterized by significant discrepancies in commitment and therapy goals, using both clinical and non-clinical data sets. While theoretical frameworks suggest these couples present unique challenges for treatment planning and therapy outcomes, empirical data on their prevalence have remained limited. We analyzed data from the National Couples' Health and Time Study (NCHAT) and the Marriage and Family Therapy Practice Research Network (MFT-PRN) to identify groups of couples that might practically be classified as mixed-agenda couples. Findings re

Does marital therapy enhance the effectiveness of treatment for sexual dysfunction?

A controlled treatment trial was conducted with couples in which the woman suffered from secondary forms of sexual dysfunction. To test whether marital therapy is helpful in enhancing the effectiveness of symptom-specific therapy for sexual dysfunction, two treatment conditions were compared to a waiting list control group. All couples received 12 sessions of therapy directed to the sexual symptom. One half of the couples received nine sessions of relaxation and information before sex therapy, and the other half, nine sessions of marital therapy before sex therapy. Whereas both treatment condi

Chronic β1-adrenergic blockade enhances myocardial β3-adrenergic coupling with nitric oxide-cGMP signaling in a canine model of chronic volume overload: new insight into mechanisms of cardiac benefit with selective β1-blocker therapy.

The β1-adrenergic antagonist metoprolol improves cardiac function in animals and patients with chronic heart failure, isolated mitral regurgitation (MR), and ischemic heart disease, though the molecular mechanisms remain incompletely understood. Metoprolol has been reported to upregulate cardiac expression of β3-adrenergic receptors (β3AR) in animal models. Myocardial β3AR signaling via neuronal nitric oxide synthase (nNOS) activation has recently emerged as a cardioprotective pathway. We tested whether chronic β1-adrenergic blockade with metoprolol enhances myocardial β3AR coupling with nitri

Persistence of ureaplasma urealyticum in the genital tract after antibiotic therapy.

Urine samples from 65 couples attending an infertility clinic were tested for the presence of Ureaplasma urealyticum. Of 36 couples found to harbor the organism, 35 were given antibiotic treatment in an attempt to eradicate these organisms from the genital tract. After the initial antibiotic therapy, tests of 12 of 27 couples (44%) still were Ureaplasma positive; after a second course of treatment, tests for 4 of 9 were positive. Two couples had Ureaplasma organisms that were resistant to all antibiotics tested. One couple remained positive after five courses of treatment, in spite of the demo
