Chronic pain is associated with less grey matter volume in the anterior cingulum, anterior and posterior insula and hippocampus across three different chronic pain conditions.

BACKGROUND: Chronic pain of different aetiologies and localization has been associated with less grey matter volume (GMV) in several cortical and subcortical brain areas. Recent meta-analyses reported low reproducibility of GMV alterations between studies and pain syndromes. METHODS: To investigate GMV in common chronic pain conditions defined by body location (chronic back pain, n = 174; migraine, n = 92; craniomandibular disorder, n = 39) compared to controls (n = 296), we conducted voxel-based morphometry and determined GMV from high-resolution cranial MRIs obtained in an epidemiologic sur

[The contribution of standard radiographs in the evaluation of masticatory system dysfunction].

Conventional x-ray films (orthopantomogram, teleradiogram) visualize many signs of dysfunction of the manducatory apparatus (DAM). Progress in our knowledge of the pathophysiology of DAM has brought new light to these radiographic signs. The orthopantomogram allows an assessment of modifications in muscle insertion zones secondary to their dysfunction and, more difficulty, the articular remodeling of the TMJ, mainly of the mandibular condyle and the articular space. The lateral teleradiogram studied by architectural and structural analysis provides information on the biomechanical balance of t

A critical review of Dr. Charles S. Greene's article titled "Managing the Care of Patients with Temporomandibular Disorders: a new Guideline for Care" and a revision of the American Association for Dental Research's 1996 policy statement on temporomandibular disorders, approved by the AADR Council in March 2010, published in the Journal of the American Dental Association September 2010.

Dr. Charles Greene's article, "Managing the Care of Patients with TMDs A New Guideline for Care," and the American Association for Dental Research's (AADR) 2010 Policy Statement on Temporomandibular Disorders, published in the Journal of the American Dental Association (JADA) September 2010, are reviewed in detail. The concept that all temporomandibular disorders (TMDs) should be lumped into one policy statement for care is inappropriate. TMDs are a collection of disorders that are treated differently, and the concept that TMDs must only be managed within a biopsychosocial model of care is ina

[Stabilimetry and cranio-cervico-mandibular disorders].

BACKGROUND: Cranio-cervico-mandibular disorders can cause disturbances in posture. Stabilimetry measures spatio-temporal variations in the center of body pressure and evaluates the mechanisms of maintenance of balance. The study used a stabilimetric platform to evaluate posture changes in patients with cranio-cervico-mandibular disorders before and after treatment for malocclusion. METHODS: Between February 1998 and December 2000, 60 patients with cranio-cervico-mandibular disorders were recruited from the Dentistry Clinic of the University of Sassari. Each patient underwent two stabilimetric

Masticatory sensory-motor changes after an experimental chewing test influenced by pain catastrophizing and neck-pain-related disability in patients with headache attributed to temporomandibular disorders.

BACKGROUND: Recent research has shown a relationship of craniomandibular disability with neck-pain-related disability has been shown. However, there is still insufficient information demonstrating the influence of neck pain and disability in the sensory-motor activity in patients with headache attributed to temporomandibular disorders (TMD). The purpose of this study was to investigate the influence of neck-pain-related disability on masticatory sensory-motor variables. METHODS: An experimental case-control study investigated 83 patients with headache attributed to TMD and 39 healthy controls

Topical versus systemic diclofenac in the treatment of temporo-mandibular joint dysfunction symptoms.

The most frequent symptom of craniomandibular dysfunction is pain in the preauricular area or in the temporo-mandibular joint, usually localized at the level of the masticatory musculature. Patients sometimes also complain of reflect otalgia, headaches and facial pain. Osteoarthrosis is a frequent degenerative debilitating chronic disorder that can affect the temporomandibular joint. It causes pain and articular rigidity, a reduction in mobility, and radiological alterations are visible in stratigraphy. The aim of this study was to compare the efficacy of a topically applied non-steroid anti-i

A Phe377del mutation in ANK leads to impaired osteoblastogenesis and osteoclastogenesis in a mouse model for craniometaphyseal dysplasia (CMD).

Craniometaphyseal dysplasia (CMD) is a rare genetic disorder with hyperostosis of craniofacial bones and widened metaphyses in long bones. Patients often suffer from neurological symptoms due to obstruction of cranial foramina. No proven treatment is available and the pathophysiology is largely unknown. A Phe377 (TTC(1130-1132)) deletion in exon 9 of the pyrophosphate (PPi) transporter ANK leads to CMD-like features in an Ank(KI/KI) mouse model. Here, we investigated the effects of CMD-mutant ANK on mineralization and bone mass at a cellular level. Ank(KI/KI) osteoblast cultures showed decreas

Craniofacial approaches and reconstruction in skull base surgery: techniques for the oral and maxillofacial surgeon.

Skull base surgery (SBS) is considered the standard of care in treating benign and malignant lesions of the cranial base. SBS is a multidisciplinary team approach used to treat these complex lesions that may have intracranial extension. SBS can be broken down into 3 steps. Transfacial access is performed, followed by resection with sound oncologic principles, and then reconstruction of the cranial base and facial structures. Functional and esthetic concerns must be addressed by the surgeons. Oral and maxillofacial surgeons frequently perform elective facial osteotomies and treat victims of cra

Review and update on drugs related to the development of osteonecrosis of the jaw.

BACKGROUND: Medication-related osteonecrosis of the jaw (MRONJ) is a rare, but serious adverse effect of certain drugs, of which bisphosphonates are the most widely known. This pathology is also associated with other medications such as the biologic antiresorptive agent, denosumab and some antiangiogenics such as sunitinib, bevacizumab or aflibercept. Very recently, new medications have also been associated with osteonecrosis of the jaw (ONJ). The objectives were to update the list of medications associated with ONJ, to analyze the fundamental aspects of this list and to describe the level of

Atypical odontalgia.

Atypical odontalgia describes atypical facial pain in apparently normal teeth. Unfortunately, dentists usually consider this diagnosis only after the failure of invasive treatment. Atypical odontalgia patients are typified by women in their mid-40s who complain of persistent pain in one or more premolar or molar teeth. They associate pain with dental procedures or trauma to the region. While the cause of atypical odontalgia is uncertain, deafferentation pain appears to be a plausible mechanism. This article reviews relevant aspects of this perplexing pain problem. To help avert the untimely di

Angular and linear comparisons with unilateral mandibular asymmetry.

Twelve patients (mean age 24 years) presenting with facial asymmetry, thought to represent unilateral mandibular condylar hyperplasia, were included in this study. Two distinct patient groups were noted: (1) with apparent condylar hyperplasia and (2) with unilateral internal derangements. Six patients were found to have normal TMJs bilaterally and six had unilateral internal derangement occurring on the short side. Angular and linear measurements were compared using Student's t test for discrete variables between the two groups and ANOVA for continuous variables; however, they were unable to d

The role of functional occlusal relationships in temporomandibular disorders: a review.

This review highlights the consensus existing in past research on the role of functional occlusal factors in the pathophysiology of temporomandibular disorders (TMD). The functional occlusal relationships considered are balancing and working occlusal contacts, length and symmetry of retruded contact position-intercuspal position (RCP-ICP) slides, occlusal guidance patterns, parafunction, and dental attrition. Controlled studies fail to demonstrate any association between occlusal interferences and TMD signs or symptoms. Temporomandibular joint condylar autorepositioning secondary to intracapsu

Clinical implications of anomalous muscle insertion relative to jaw movement and mandibular dysfunction: the anterior belly of the digastric muscle in a cadaver.

This case report of an anomalous insertion of the anterior belly of the digastric muscle in a cadaver considers the components involved in mandibular movement as well as the radiologic changes that could be expected in asymmetric function of the temporomandibular joints. The occlusal scheme of the cadaver was noted and, after transcranial radiographs were taken, both joints were anatomically dissected. While the left condyle-disc relationship appeared normal and properly approximated, the right joint (which was assumed to be translating in an anterior, inferior, and medial direction on each op

Symptom characteristics in TMD patients reporting blunt trauma and/or whiplash injury.

Descriptive data are presented for 100 patients reporting facial pain and temporomandibular dysfunction (TMD) precipitated by (1) overt facial/head trauma, (2) "whiplash" injury, and (3) "whiplash" injury with overt trauma. Analysis of the data suggests that these TMD trauma subsets are significantly different in terms of total number of pain sites, presence of concomitant neck pain, range of opening, and report of sleep disturbance and involvement in litigation. They also varied with respect to reducing and nonreducing disc displacement. However, significant differences were not observed for

Distress, jaw habits, and connective tissue laxity as predisposing factors to TMJ sounds in adolescents.

Immediately before a craniomandibular examination and measurement of wrist laxity, 96 adolescents answered a questionnaire about jaw habits, jaw activities, symptoms of stress, facial trauma, and TMJ sounds. The sample was divided into five subgroups on the basis of history and stethoscopically detected TMJ sounds. Some groups with TMJ sounds had significantly more jaw habits, jaw activities, and symptoms of stress than the control group (without history and clinically detected TMJ sounds). No significant differences emerged in angular measurement of wrist laxity between groups with TMJ sounds
