Pterional craniotomy for occlusion of a basal temporal arteriovenous fistula.

Atypical arteriovenous fistulas (AVFs) lack the parenchymal nidus observed in arteriovenous malformations (AVMs) and are not dural-based lesions supplied by meningeal arteries, unlike true dural AVFs. Atypical AVFs are parenchymally based with a nondural arterial supply. This video presents a man in his 50s with a remote carotid takedown for flow reduction of a described temporal AVM. Imaging showed a basal temporal AVF supplied by middle cerebral artery branches, coalescence of arterial feeders onto a large venous varix, and no intervening nidus. Dearterialization of the venous varix eliminat

Extent of the frontal sinuses in the aspect of neurosurgical craniotomy approaches.

BACKGROUND: Frontal sinuses within the frontal bone can be a potential obstacle in neurosurgical approach in this region. Their unintended opening during craniotomy is not beneficial due to the risk of brain infections by bacteria inhabiting the sinus mucosa. Therefore, such opening should be avoided whenever the surgical procedure does not involve the sinus itself. The aim of the study was the morphometric analysis of the frontal sinuses based on computed tomography imaging. MATERIALS AND METHODS: The width, height, projection surface area and location of the highest and most lateral points

Retrosigmoid Craniotomy for Resection of an Epidermoid Cyst of the Posterior Fossa.

This video illustrates the case of a 51-year-old woman who presented with sudden-onset headache, vertigo, and nausea. Imaging revealed an epidermoid cyst of the posterior fossa with mass effect upon the brainstem and displacement of the basilar artery. This lesion was approached using a left-sided keyhole retrosigmoid craniotomy with monitoring of the cranial nerves. This video illustrates the technique of internal debulking of the cyst contents with minimal manipulation of the cyst capsule, which is often densely adherent to the brainstem, cranial nerves, and vessels in the posterior fossa. Piezosurgery for Infra- and Supratentorial Craniotomies in Brain Tumor Surgery.

OBJECTIVE: Piezoelectric surgery represents an innovative technique to perform safe and effective osteotomies and is an alternative to traditional bony tissue management using rotating or perforating instruments. We evaluated the safety and feasibility of craniotomies using an ultrasonic device that allows the selective cut of mineralized structures, avoiding damages to the vascular, dural, and parenchymal structures. METHODS: We analyzed a series of 300 patients (age range, 1-81 years; SD ± 15.2) who underwent elective cranial surgery for brain tumors, in which the craniotomy was performed u

Neurophysiological Monitoring and Awake Craniotomy for Resection of Intracranial Gliomas.

Aggressive resection of intracranial gliomas has a positive impact on patients' prognosis, but is associated with a risk of neurological complications. For preservation of brain functions and avoidance of major postoperative morbidity various methods of intraoperative neurophysiological monitoring have been introduced into clinical practice. At present, somatosensory evoked potentials (SSEP), motor evoked potentials (MEP), visual evoked potentials (VEP), brainstem auditory evoked potentials (BAEP), and electrocorticography (ECoG) are used routinely during neurosurgical procedures. To maximize

Minipterional craniotomy in surgery for anterior circle of Willis aneurysms.

One of the significant events in aneurysm surgery was promotion of a microneurosurgical technique by G. Yasargil. Despite its versatility, pterional craniotomy is associated with extensive osteotomy and a significant incision of the skin and temporal muscle, which may lead to the adverse cosmetic effects, risk of temporomandibular joint dysfunction, injury to the frontal branch of the facial nerve, and facial and scalp numbness. We present our experience with minipterional craniotomy in surgery for anterior circle of Willis aneurysms in 40 patients. There were no serious complications or death

Left-sided retrosigmoid craniotomy for the resection of a vestibular schwannoma.

The cerebellopontine angle is the site for a wide-range of neoplastic and vascular pathologies. The retrosigmoid craniotomy remains the primary means by which to gain surgical access to this anatomically complex region. We present our standard technique for the completion of a retrosigmoid craniotomy and the resection of a left-sided vestibular schwannoma. Anatomy pertinent to the approach, including, the transverse and sigmoid sinuses, cranial nerves, and internal auditory canal (IAC) is displayed. Special emphasis is placed on patient positioning, adequate bone removal, and tumor resection.

Craniotomies without burr holes using an oscillating saw.

OBJECTIVE: The object of this study is to describe a new method for performing craniotomies which obviates the need for burr holes, improves bony reconstruction, and reduces post-operative cosmetic deformities. Moreover, this technique provides excellent exposure of skull base structures and dural venous sinuses. METHODS: Craniotomies in varied locations are created with the use of a micro-oscillating saw and chisel. No burr holes are used and reconstruction with plates and screws is unnecessary. RESULTS: We initially applied our technique to approaches to the anterior skull base with combin

Endoscope-assisted supraorbital craniotomy for lesions of the interpeduncular fossa.

OBJECTIVE: The supraorbital approach is well accepted for lesions in the anterior fossa, the sellar region, and the anterior circle of Willis. However, the usefulness of this approach has not yet been elucidated for lesions in the interpeduncular fossa. The technique of an endoscope-assisted, ipsi- and contralateral supraorbital approach to lesions within the interpeduncular fossa is described, and the initial results are reported. METHODS: A small supraorbital craniotomy, using an eyebrow incision, was performed in each of seven patients who were operated on for different types of lesions in

Evolution of craniotomy as a debridement technique for penetrating craniocerebral injuries.

A craniotomy debridement technique was recommended for penetrating craniocerebral injuries as early as 1940, in World War II. However, with due consideration for the bacterial contamination of penetrating injuries, the safety of this technique was questionable. The technique has been recommended in each succeeding war, but no data substantiating the safety or eventual sequelae have been available. Analysis of the data from the large Head Injury Registry of Vietnam casualties indicates that, in properly selected cases, debridement by craniotomy technique can be safe and efficacious.

Use of calcium phosphate bone cement in retrosigmoid craniotomies to reduce rates of pseudomeningocele and craniotomy-site CSF leakage: a cohort study and meta-analysis.

OBJECTIVE: Calcium phosphate cement (CPC) use has been previously described as a strategy to reduce the rates of pseudomeningocele and CSF leakage following skull base surgery. The authors aimed to present their institutional experience with CPC in retrosigmoid craniotomy and perform a systematic review of the literature to determine the effectiveness of CPC in reducing postoperative complications, including pseudomeningocele and craniotomy-site CSF leakage. METHODS: A retrospective single-institution cohort study of patients who underwent retrosigmoid craniotomy with and without CPC craniopl

Incidence of Surgical Site Infection Requiring Reoperation Following Awake Craniotomy.

BACKGROUND: An awake craniotomy (AC) is performed for cerebral tumors near or involving eloquent regions of the brain to maximize tumor resection whilst preserving neurological function. Reducing the risk of surgical site infections (SSIs) is vital, as infections can delay adjuvant therapy. However, data on SSI requiring reoperation following AC in the United Kingdom remain limited. METHODS: We performed a single-center retrospective case series of all AC procedures between January 2015 and April 2023. Data were collected from the electronic health record on surgical indication, demographics,

Immediate titanium mesh cranioplasty after debridement of post-craniotomy infection.

PURPOSE: For post-craniotomy surgical site infection (SSI) involving the bone, typical management involves removal of the bone flap and delayed cranioplasty. The disadvantages of delayed cranioplasty include cosmetic deformity, vulnerability of unprotected brain, skin contraction, syndrome of the trephined and the risks of further surgery. Second procedures also add to cost due to surgical time, hospital stay, and opportunity costs for patients from being away from work. METHODS: We retrospectively reviewed patients who underwent post-craniotomy bone flap removal due to SSI, with immediate ti

Trigeminocardiac reflex during supratentorial craniotomy for intraventricular meningioma: illustrative case.

BACKGROUND: Trigeminocardiac reflex (TCR) is a vagal reflex produced by trigeminal nerve stimulation that causes bradycardia, hypotension, and, occasionally, cardiac arrest. TCR usually occurs during surgical manipulation of the posterior fossa. In this report, the authors describe an extremely rare case of TCR that developed when a burr hole was drilled on the temporal bone during surgery for an intraventricular meningioma. OBSERVATIONS: The patient was referred to the authors' department after brain imaging revealed a mass lesion in the right trigone of the lateral ventricle. The surgery, w

Acute postoperative parotitis following frontal interhemispheric craniotomy: A case report.

BACKGROUND: Acute postoperative major salivary gland adenitis is a rare complication characterized by swelling of the salivary glands, most commonly associated with skull base neurosurgical procedures. Its occurrence following supratentorial non-skull base craniotomies is uncommon and may be overlooked. CASE DESCRIPTION: We report a 45-year-old man who developed ipsilateral parotid gland inflammation within 24 h after undergoing a frontal interhemispheric craniotomy for resection of a craniopharyngioma. The procedure was performed in the supine position with 90° head rotation and 45° neck fle

Chronic craniomandibular pain after craniotomy: A long-term clinical study.

OBJECTIVE: Chronic craniomandibular/cervical pain and temporomandibular disorders have not been studied in patients who had a craniotomy several years previously. The aim of the current clinical work was to address these issues. METHODS: A total group of 150 ambulant patients who had a previous craniotomy was subclassified according to whether or not the temporalis muscle was manipulated. RESULTS: The average incidence of multiple subsite regional head and neck pain was 69.3% a number of years after a craniotomy. Evidence of internal derangement of the temporomandibular joint was significant
