Cryptorchidism and its long-term complications.

Cryptorchidism is the most frequent defect of the male urogenital tract at birth. It represents a risk factor for primitive testiculopathy associated with long-term complications (infertility, testicular neoplasia, and hormonal changes). An only consensus exists: "children with bilateral cryptorchidism who are not treated in early age are certainly set to become infertile". The majority of Authors agrees that the cryptorchid testicle will be in for structural and functional alterations and the rate of infertility is inversely proportional to the age at the time of orchidopexy. Cryptorchidism c

Management of cryptorchidism in children: guidelines.

QUESTION: To develop clinical guidelines for the management of cryptorchidism in pre-pubertal boys, from early diagnosis through therapy to long-term follow-up and prognosis. METHOD: Systematic review of articles from the medical literature, referenced since 1966, using validated search strategies through the following databases: Medline, Cochrane Database of Systematic Reviews, Cochrane Register of Controlled Trials, EMBASE, DARE, ACP Journal Club, National Guidelines Clearinghouse, Guidelines International Network. Relevant articles published after 1988 were taken as the basis for the state

[Cryptorchidism: update on diagnosis and therapy].

The incidence of cryptorchidism at 3 month-old boy is 0.8% which is close to that of an adult, and its diagnosis can be done at 3 months old. In case of cryptorchidism both MRI and USG are helpful to locate an undescended testis. If genitalia is pigmented too weakly, serum testosterone should be measured to rule out hypogonadism. Pathological changes of basement membrane and mitochondria in an undescended testis at 1 year-old boy suggests an earlier orchiopexy at least between 1 and 2 years at age, although if it is unilateral case an orchiopexy may be done by 4 years old. Fertility and malign

Cryptorchidism: an indicator of testicular dysgenesis?

Cryptorchidism is a common ailment of new-born boys, affecting 1-9% of full term boys at birth. Cryptorchidism has been associated with an increased risk of testicular cancer and reduced fertility. Aetiology of cryptorchidism remains obscure in most cases. Familial occurrence suggests a heritable susceptibility to cryptorchidism; however, seasonal variation in the incidence of cryptorchidism suggests that environmental factors also contribute. Testicular descent is characterised by androgen-dependent regression of cranial suspensory ligament and androgen + insulin-like hormone 3 (Ins l3)-depen

[Cryptorchidism in adults. About 81 cases].

Cryptorchidism is more often diagnosed and treated in pediatric age. Its occurence in adult provides opportunity to study its spontaneous course. This study has been realized on one series of 81 files of patients cryptorchides of age superior than 14 years. Cryptorchidism was unilateral in 87.6% of cases and bilateral in 12.4% of cases. The higher the location, the more important the glandular atrophy and histological lesions. Global rate of secretory azoospermia was 93% in relation with a seminiferous disertion and an intersitial fibrosis. The lesions are important when cryptorchidism is bila

Homeobox HOXA10 gene analysis in cryptorchidism.

BACKGROUND: In male mice, targeted disruption of the homeobox gene hoxa10 causes cryptorchidism and infertility. Genetic alterations in exon 1 of HOXA10 have been found in a high number of boys with cryptorchidism. AIM: To evaluate whether mutations of HOXA10 can be a common cause of cryptorchidism. PATIENTS AND METHODS: Genomic DNA was extracted from 18 patients with cryptorchidism (age 7-44 years; unilateral n = 13; no familial cases) and 28 healthy controls (age 9-39 years). HOXA10 was amplified by PCR and all coding sequences of exon 1 and 2 were sequenced. The PCR products were digested

[Expression of FasL in rat cryptorchidism].

OBJECTIVE: To investigate the expression of FasL in rat cryptorchidism and its significance. METHODS: Twenty-four male SD rats (22-day old) were randomly divided into two groups: unilateral cryptorchid group (n = 12) and pseudo-operation group (n = 12). When the rats were 110-day old, blood samples were taken and the rats were killed for analysis. Immunohistochemical method (SP) was used to detect FasL expression in testes and ELISA method to detect serum antisperm antibody (AsAb). RESULTS: The positive FasL expression rates in cryptorchid and contralateral testes were significantly higher t

Chromosomal anomalies in cryptorchidism and hypospadias.

PURPOSE: We studied the incidence of chromosomal anomalies in patients with cryptorchidism and hypospadias to determine the value of routine karyotyping in this population. MATERIALS AND METHODS: Blood samples from 984 patients with cryptorchidism and/or hypospadias were studied for chromosome analysis. RESULTS: Chromosomal anomalies were detected in 27 of the 916 patients (2.94%) with cryptorchidism and in 7 of the 100 (7%) with hypospadias. There were chromosomal aberrations in 13 of the 706 patients (1.84%) with isolated cryptorchidism (no additional congenital abnormalities) and in 14 of

[Study on risk factors of cryptorchidism].

OBJECTIVE: Cryptorchidism is the most common urogenital congenital malformations in males, which increases male infertility and testicular tumors. It is necessary to identify its risk factors. METHODS: A hospital-based case-control study was conducted. Cases were 99 boys with cryptorchidism with two matched controls per case were randomly selected. Both cases and controls were interviewed with a uniformed questionnaire by face to face. Conditional logistic regression model was used for univariable and multivariate analysis by SAS6.12 computer software was used to estimate odds ratios (OR) and

Cryptorchidism in mice mutant for Insl3.

Impaired testicular descent (cryptorchidism) is one of the most frequent congenital abnormalities in humans, involving 2% of male births. Cryptorchidism can result in infertility and increases risk for development of germ-cell tumours. Testicular descent from abdomen to scrotum occurs in two distinct phases: the trans-abdominal phase and the inguino-scrotal phase. Currently, little is known about the factors that regulate the trans-abdominal phase of testicular descent. Leydig insulin-like hormone (Insl3) is a member of the insulin hormone superfamily expressed in the developing testis. We sho

Postnatal testicular maldevelopment in unilateral cryptorchidism.

Histomorphometric analysis of semi-thin sections was performed on testicular biopsies of 232 unilaterally cryptorchid testes and 195 of their contralateral descended partners. The results demonstrated a decreased number of germ cells detectable from the first year of life. There was delayed and defective transformation of gonocytes to Ad spermatogonia, which normally is complete at age 6 months, delayed or failed transformation of Ad spermatogonia to primary spermatocytes, which normally commences at age 3 years and decreased numbers of Leydig cells. These abnormalities were present in the uni

[Endocrine studies in cryptorchidism (author's transl)].

The endocrinological study of the cryptorchid testicle was carried out using a HCG and LH-RH tests to stimulate the hypophysis. Stimulation with LH-RH demonstrated a low hypophysis reserve in some cases which made suspect the presence of a hypo-stimulated testicle. One is led to believe in the heterogeneous nature of the endocrinological response.

Iatrogenic cryptorchidism resulting from hernia repair.

Nine children have been examined for iatrogenic cryptorchidism during a four year period. Wound infection and testicular retractility appear to be factors predisposing to this complication. Routine testicular palpation for position at the completion of hernia repair and the use of orchidopexy in selected patients should aid in the prevention of this problem.
