[Overuse syndromes of the foot].

Overuse syndromes due to lifestyle problems or sporting activities commonly lead to foot abnormalities. The tendons of the long flexor and extensor muscles are specifically prone to degeneration. The various disorders may be classified by a grading system that includes peritendinous inflammation, degenerative tendon disease, and ruptures. Bone marrow edema is another typical manifestation of overuse. It may be differentiated from inflammatory or traumatic forms of edema by its anatomic distribution. Systematic pattern recognition is based on the concept of musculotendinous and osseous kinetic

Finger pain in rock climbers: reaching the right differential diagnosis and therapy.

Injuries and overuse syndromes of the fingers are the most common problems in rock climbers. While injuries to the finger flexor pulley system and tenosynovitis are well known to be frequent problems, other syndromes like the lumbrical shift syndrome or flexor tendon ganglions are rather unknown. The differential diagnosis of finger pain in rock climbers involves many differential diagnoses and can be quite difficult. The diagnostic and therapeutic procedures for the evaluation of finger pain in rock climbers are demonstrated. More than 20 different diagnoses need to be considered. After takin

Shoulder pain in water polo: a systematic review of the literature.

The main aim of this systematic review is to synthesize and critically evaluate literature on the incidence and clinical presentation of shoulder pain in water polo. A secondary aim is to examine the contributing factors to shoulder pain in water polo. Medline, Cinahl, Embase, Ausport, Ovid, Sports Discus, Pubmed and Google Scholar data bases were electronically searched. Data were extracted regarding research design, injuries, pain, incidence, interventions and therapy outcomes. Of an initial yield of 23 papers, 11 fulfilled the inclusion criteria and were categorized into studies on incidenc

Structure-function relationships of entheses in relation to mechanical load and exercise.

Entheses are regions of high-stress concentration that are commonly affected by overuse injuries in sport. This review summarizes current knowledge of their structure-function relationships - at the macroscopic, microscopic and molecular levels. Consideration is given to how stress concentration is reduced at fibrocartilaginous entheses by various adaptations which ensure that stress is dissipated away from the hard-soft tissue interface. The fundamental question of how a tendon or ligament is anchored to bone is addressed - particularly in relation to the paucity of compact bone at fibrocarti

Paratendinopathy.

Because most Achilles tendon injuries take place in sports and there has been a general increase in the popularity of sporting activities, the number and incidence of Achilles tendon overuse injuries have increased in the industrialized countries during the last few decades. The term "Achilles paratendinopathy" is used in clinical practice to describe activity-related Achilles pain combined with tenderness on palpation, providing that there is no suspicion of intratendinous pathology on the basis of patient history, clinical examination, or imaging examinations. This article discusses Achilles

[Femoroacetabular impingement. A common cause of hip complaints leading to arthrosis].

The exact cause of the idiopathic osteoarthritis of the hip has not been identified, although the cause of hip degeneration in developmental dysplasia can clearly be attributed to an excessive axial loading. Based on the development of a surgical technique for the safe surgical dislocation of the hip and the associated possibility of intraoperative joint evaluation, we have found motion-induced joint damage in many of these hips. This begins peripherally at the acetabular rim, progressing centrally. This so-called "femoroacetabular impingement" (FAI), leads, by an increased acetabular coverage

Do repetitive tasks give rise to musculoskeletal disorders?

Repetitive tasks can undoubtedly cause discomfort and pain, but whether they cause or worsen the pathology causing the pain is most uncertain. Research in this area is difficult as the 'work-related upper limb disorders' do not occur exclusively in workers and because there is no simple, reliable and reproducible test for most 'work-related upper limb disorders'. Furthermore many studies are difficult to interpret as they detect disease by the presence of symptoms: one would expect manual workers to complain of more symptoms than sedentary workers and symptom aggravation does not tell one anyt

Nerve injury in repetitive motion disorders.

Nerve compression in repetitive motion disorders is being recognized with increasing frequency. The pathophysiology of chronic nerve compression spans a broad spectrum beginning with subperineurial edema and progressing to axonal degeneration. These changes depend on the amount and duration of the compressive forces. Certain postures or positions in the upper extremity will increase pressures around certain nerves increasing pressure exposure. Evaluation of these patients with chronic nerve compression should include examination at all levels of potential entrapment in the upper extremity to i

Physeal growth arrest of the distal phalanx of the thumb in an adolescent pianist: a case report.

A case of premature closure of the distal phalanx growth plate of the thumb is reported in an adolescent, presumably caused by accumulated repetitive trauma incurred during years of piano playing. The injury may be analogous to overuse injury to certain growth plates reported in child and adolescent athletes. Upper extremity problems in musicians due to the unusual and excessive demands of playing an instrument are discussed.

Upper-extremity tendinitis and overuse syndromes in the athlete.

Overuse injuries are the result of repetitive microtrauma to the musculotendinous unit. Treatment protocols are based on the stage of the inflammatory process that is active at the time of diagnosis. Control of the inflammatory response with rest, elevation, and ice is the treatment objective during the inflammatory stage. Prevention of further injury is the primary treatment goal throughout the proliferative phase. Once the inflammatory process has reached the maturation stage, rehabilitation can begin with flexibility exercises, isometric contractions, and a slow return to strength training.

[Occupational environment and strain induced gout. A review of epidemiological studies of the connection between occupational environment and coxarthrosis].

Osteoarthrosis of the hip is a frequent complaint with multifactorial causal relations. The causes have hitherto not been associated with the work environment. Through a systematic literature search 15 epidemiological studies dealing with the relation between occupation or factors in the work environment and coxarthrosis were found. A critical evaluation finds that farmers and construction workers have an increased risk of contracting coxarthrosis. Work that is strenuous for the hip such as lifting and ladder climbing might also be risk factors. Prevention of heavy work involving the hip is pr

Prevention and rehabilitation of overuse injuries of the elbow.

Rehabilitation from injury and prevention of injury are terms that describe differing aspects of the same challenge. The clinician must be continually aware of the principles of rehabilitation and their order of priority (prevent further damage, restore motion, restore strength, reestablish coordination) while supervising patients' progress toward their goals. If patients are made aware of these principles at an early stage, and taught to take as much responsibility as possible for their own rehabilitative course, they will be more likely to reach their own ideal compromise of safety and speed
