The Impact of Shared Decision-Making on the Quality of Decision Making in Aortic Dissection: A before-and-after Comparison Study.

BACKGROUND: Complex surgical plans and consideration of risks and benefits often cause decisional conflicts for decision-makers in aortic dissection (AD) surgery, resulting in decision delay. Shared decision-making (SDM) improves decision readiness and reduces decisional conflicts. The purpose of this study was to investigate the impact of SDM on decision quality in AD. METHODS: One hundred and sixty AD decision-makers were divided into two groups: control (n = 80) and intervention (n = 80). The surgical plan for the intervention group was determined using patient decision aids. The primary o

Tracing the dynamics of treatment decision-making: A qualitative study about older patients' treatment decision-making and retrospective evaluation.

INTRODUCTION: Treatment decisions are influenced by disease and patient related factors, yet little is known about how older patients evaluate these decisions after treatment. This study explores how older adults with cancer reflect on the treatment decision-making (TDM) process and whether they considered the treatment 'worth it', capturing perspectives before treatment and three months after TDM. The findings aim to inform person-centered treatment decision-making in geriatric oncology. MATERIALS AND METHODS: We conducted a qualitative study using semi-structured interviews at two timepoint

Hierarchical Decision-Making Theory: A Cognitive-Affective Framework for Explaining Decision Quality.

Drawing on the trade-off patterns of "smaller-sooner-loss for larger-later-gain" (SSL-LLG) and "smaller-sooner-gain for larger-later-loss" (SSG-LLL), this study proposes a hierarchical decision-making theory from a dual-system (cognitive-affective) perspective. The theory posits that decision quality arises from the synergy between cognitive maturity-the capacity to weigh long-term interests-and emotional maturity-the ability to maintain affective independence. This yields a four-type, three-level taxonomy of decision makers. We tested our hypotheses across behavioral experiments, eye-tracking

[When AI is involved in making decisions. Ethical considerations on shared decision-making in psychotherapeutic-psychiatric treatment].

BACKGROUND: Artificial intelligence (AI) is on the rise in the treatment of persons with psychiatric disorders. It can integrate diverse data points, analyze patterns and thereby generate clinically relevant insights that can influence the shared decision-making (SDM) process between patient and practitioner. OBJECTIVE: This article aims to examine how AI can influence the SDM process in psychiatric and psychotherapeutic care. Furthermore, ethical and clinical considerations relevant to an appropriate and justified implementation are addressed. METHODS: The potential effects on SDM are analy

Decision regret and shared decision-making in patients undergoing hyperbaric oxygen therapy.

INTRODUCTION: Hyperbaric oxygen therapy (HBOT) is used for various medical conditions. HBOT is demanding and varies in effectiveness. This intensity combined with logistical challenges may lead to patients regretting undergoing HBOT. Therefore, shared decision-making (SDM) seems applicable when considering HBOT. The goal of this study was to assess the level of SDM as perceived by HBOT patients and relate this to post-hoc regret about choosing HBOT. METHODS: Patients referred for ≥ 10 sessions HBOT, were recruited for this prospective cohort study. Participants completed the SDM-Q-9 and SDM-K

The elusiveness of context effects in decision making.

Contextual features influence human and non-human decision making, giving rise to preference reversals. Decades of research have documented the species and situations in which these effects are observed. More recently, however, researchers have focused on boundary conditions, that is, settings in which established effects disappear or reverse. This work is scattered across academic disciplines and some results appear to contradict each other. We synthesize recent findings and resolve apparent contradictions by considering them in terms of three core categories of decision context: spatial arra

Decision-making in the diagnosis of tuberculous meningitis.

Tuberculous meningitis (TBM) is the most devastating form of tuberculosis (TB) but diagnosis is difficult and delays in initiating therapy increase mortality. All currently available tests are imperfect; culture of Mycobacterium tuberculosis from the cerebrospinal fluid (CSF) is considered the most accurate test but is often negative, even when disease is present, and takes too long to be useful for immediate decision making. Rapid tests that are frequently used are conventional Ziehl-Neelsen staining and nucleic acid amplification tests such as Xpert MTB/RIF and Xpert MTB/RIF Ultra. While pos

To tilt or not to tilt; the decision-making process.

PURPOSE: This study's aim was to explore the complexities of how power tilt use is integrated within the context of daily life. Other studies have used mobility-type metrics of frequency, duration, and amplitude as measures of power tilt use in the context of daily life but results lack depth in explaining the complex interplay of participating in daily activities and power tilt use. METHODS: This grounded theory study explored how power tilt was used in daily life from the perspectives of people experienced with using power tilt and therapists experienced with prescribing power tilt. The psychopharmacology of violence: making sensible decisions.

Violent behavior associated with mental disorders is a common reason for admission to a psychiatric inpatient unit. Once hospitalized, patients may continue to be intermittently agitated and have persistent aggressive behaviors, preventing their discharge back into the community. Managing agitation quickly with effective pharmacological agents can avoid further escalation to aggression and violence. In the acute setting, this usually involves the parenteral use of antipsychotics, with or without benzodiazepines. Within the past decade, short-acting intramuscular formulations of second-generati

Before you make that big decision..

When an executive makes a big bet, he or she typically relies on the judgment of a team that has put together a proposal for a strategic course of action. After all, the team will have delved into the pros and cons much more deeply than the executive has time to do. The problem is, biases invariably creep into any team's reasoning-and often dangerously distort its thinking. A team that has fallen in love with its recommendation, for instance, may subconsciously dismiss evidence that contradicts its theories, give far too much weight to one piece of data, or make faulty comparisons to another b

Bulimia nervosa: emotions and making decisions.

OBJECTIVE: The purpose of this article is to add to the knowledge of the neurobiology and aetiopathogenesis of bulimia nervosa, a common disorder in routine clinical practice. To do this, we will study decision making in low risk and low uncertainty patients. MATERIAL AND METHODS: The study consisted of 19 females who fulfilled the diagnostic criteria of bulimia nervosa according to the DSM-IV and a control group of 28 healthy female students from the Faculty of Psychology. They performed two tasks: one a "cups task" risk and the other an "ambiguity task", both associated with emotions and de

Decision making in the treatment of hallux valgus.

The surgeon must determine the pathologic elements that need correction. Close observation for an increased HV angle, increased IM angle, pronation of the first toe, increased DMAA, enlarged medial eminence, and subluxation of the sesamoids must be performed. While there are a large number of procedures available for the management of HV, no one method sufficiently corrects all HV deformities. The upper limits of deformity correction for each procedure vary with the surgeon and their familiarity with each procedure. Ultimately, the surgeon must attempt to maintain a flexible first MTP joint an

Nutrition support. Making the difficult decisions.

Weight loss and cachexia are common characteristics associated with the cancer patient. Although the wasted appearance seems the same in each person, the causes are varied. Studying a patient's history and identifying surgical causes to weight loss or weight loss as a result of treatment complications assists in the consideration of nutritional support. Nutritional parameters combined with the oncology nurse's knowledge of the patient, disease process, and treatment side effects place the nurse in the position to help identify options for nutritional support. The oncology nurse's expertise ass

Is Shared Decision Making for End-of-Life Decisions Associated With Better Outcomes as Compared to Other Forms of Decision Making? A Systematic Literature Review.

Background: Whether shared decision making (SDM) has been evaluated for end-of-life (EOL) decisions as compared to other forms of decision making has not been studied. Purpose: To summarize the evidence on SDM being associated with better outcomes for EOL decision making, as compared to other forms of decision making. Data Sources: PubMed, Web of Science, Cochrane Central Register of Controlled Trials, EMBASE, PsycINFO, and CINAHL databases were searched through April 2014. Study Selection: Studies were selected that evaluated SDM, compared to any other decision making style, for an EOL decisi

Decision-making and acute behavioural disturbance (ABD): a qualitative thematic analysis of perspectives on decision-making by UK ambulance paramedics.

BACKGROUND: Incidents involving severe agitation are complex emergencies which occur infrequently in the community but have high stakes for patients and responders. Acute behavioural disturbance (ABD), an umbrella term used to describe severe agitation, sometimes known as excited delirium, affects patients who typically present to police and ambulance paramedics and may require restraint. However, little is known about how paramedics make these restraint decisions. This research aimed to explore the decisions made by paramedics when managing restraint in the context of ABD in the UK ambulance

An examination of decision making in bulimia nervosa.

BACKGROUND: Patients with eating disorders favor immediate gratification and ignore long-term negative consequences. This study investigated decision making in bulimia nervosa using the Iowa Gambling Task (IGT) and skin conductance responses (SCR). METHOD: A total of 26 bulimia nervosa patients and 51 healthy controls took part in this study; 29 patients with anorexia nervosa were included for comparison. RESULTS: Bulimia nervosa patients performed poorly in the IGT, but showed no decrease in anticipatory SCR, whereas a markedly diminished anticipatory SCR was seen in the anorexia nervosa gr
