On donkeys, ingratiates, thinkers and...clinical development units.

Clinical Development Units celebrate nurses as thinkers and nursing as a highly skilled professional activity. They are typified by the exploration and development of therapeutic, consumer-focused and research-based practice and participatory governance. Establishing and maintaining them, therefore, can be problematic in health care facilities typified by centralism, autocracy and nurses' disempowerment. In this type of facility nurses are treated like donkeys and/or ingratiates. What CDUs are, the standards nurses in them strategically and systematically pursue and their achievements to date

Resource allocation decisions faced by health care providers during the COVID-19 pandemic in a community intensive care unit in Ontario, Canada: a qualitative study.

PURPOSE: Insufficient resources and dynamic infection control policies during the COVID-19 pandemic necessitated that frontline health care providers (HCPs) make frequent ethical decisions about the allocation of scarce supplies, space, and clinician time to patient care activities. We aimed to understand the process, nature, and impact of these resource allocation decisions. METHODS: Using a qualitative case study approach, we developed an understanding of the decision-making of frontline intensive care unit (ICU) HCPs during the pandemic. We conducted 25 semistructured interviews with multi

Explicating the process of moral courage in clinical nurses: a grounded theory study.

Moral courage in nurses is the product of a complex process and can be enhanced by identifying the steps through which nurses make morally courageous decisions. This qualitative study aimed to explicate the process of moral courage in clinical nurses and present a descriptive model. The study was conducted in Shahrekord University of Medical Sciences in 2022 using the grounded theory approach. Twenty-one clinical nurses were selected through purposive and theoretical sampling. Data were analyzed using Strauss and Corbin's 2015 approach and data management was supported by MAXQDA software (vers

Healthcare providers perspective of chronic pain management in persons with traumatic spinal cord injury accessing the public system in a region of South Africa.

PURPOSE: Persons with traumatic spinal cord injury (PWTSCI) have expressed a lack of education from healthcare providers and poor shared-decision making between providers and clients. The aim was to explore the healthcare providers' perspective on factors influencing the optimal management of chronic pain. METHODS: Healthcare providers were recruited from two institutions at tertiary healthcare level. Interviews explored current chronic pain management practices, influencing factors and recommendations for improvement. Data saturation occurred after interviewing 11 participants. Thematic anal

Digital clinical decision support to save costs: a qualitative evaluation of the views of frontline healthcare professionals.

BACKGROUND: Digital clinical decision support (CDS) can improve clinical decision-making, prescribing, diagnostic accuracy, and guideline adherence. Its potential to reduce healthcare costs has received less attention, and the mechanisms by which cost savings might occur remain poorly understood. This study explored resident doctors' experiences of using CDS to improve care and reduce costs. METHODS: A qualitative study was carried out. Individual semi-structured interviews were conducted by videoconference with resident doctors who were users of a digital CDS resource. Interviews explored ex

Gaps, tensions, and conflicts in the FDA approval process: implications for clinical practice.

Despite many successes, drug approval at the Food and Drug Administration (FDA) is subject to gaps, internal tensions, and conflicts of interest. Recalls of drugs and devices and studies demonstrating advantages of older drugs over newer ones highlight the importance of these limitations. The FDA does not compare competing drugs and rarely requires tests of clinical efficacy for new devices. It does not review advertisements before use, assess cost-effectiveness, or regulate surgery (except for devices). Many believe postmarketing surveillance of drugs and devices is inadequate. A source of te

Job context, selection decision outcome, and the perceived fairness of selection tests: biodata as an illustrative case.

S. W. Gilliland (1993) has proposed a model of perceived selection system fairness to help understand applicants' prehire and posthire behavior. The present study aimed to verify and extend his framework by investigating the role of job context in the formation of fairness perceptions of biodata. A sample of 255 students (108 men, 147 women) completed an operational biodata instrument, believing that it would be used to hire persons for either international, local, or unspecified entry-level managerial positions. Participants were then presented with outcome information (selected or rejected f

Whose voice counts? Family caregivers' roles in shared decision making for chronic illness in urban South Korea: A qualitative study.

BACKGROUND: Shared decision making (SDM) is fundamental to patient-centered care, yet dominant frameworks have been developed within dyadic patient-provider models grounded in Western individualistic assumptions. Prior research from East Asian, Sub-Saharan African, and Latin American contexts has documented that medical decision making is frequently family-centered and that caregivers function as recognized actors in clinical encounters. Building on this evidence base, this study examines how caregiver participation is enacted and constrained within the Korean chronic care context, contributin

A transformational shift toward a just culture.

A health system implemented a comprehensive just culture initiative to strengthen patient safety and improve staff confidence in reporting errors and near misses. Guided by stakeholder engagement, multidisciplinary collaboration, leadership support, staff education, champion development, and the introduction of a just culture decision-making algorithm, the organization sought to foster accountability, learning, and psychological safety. A pilot implementation was followed by systemwide adoption across 10 hospitals. Evaluation demonstrated significant increases in incident reporting rates and i

[Not Available].

During the 2nd interregional CIPOMO conference (April 2026), Sicilian and Calabrian oncologists discussed healthcare issues, focusing on clinicians' needs and digital transition. A survey revealed that while physicians recognize the potential of AI and digital tools to improve clinical safety and continuity of care, major structural and organizational weaknesses persist. Indeed, 68.4% feel their departments cannot integrate digital tools without increasing workloads. Respondents demand less bureaucracy, better hospital-territory continuity, and strategic recognition. Crucially, 100% agree that

Managing a Private Radiology Practice: Business, Legal, and Organizational Perspectives.

ABSTRACT: Outpatient radiology is an indispensable part of patient-centered healthcare in Germany. However, successfully managing a radiology practice requires far more than medical expertise. Radiologists in private practice must navigate a complex landscape comprised of patient-centered care, legal requirements, economic constraints, and organizational challenges. This article outlines the essential framework conditions for entrepreneurial action in outpatient radiology: from the legal foundations of establishing a practice to tax planning opportunities and business management tools, as well

[Effectiveness of the contingency plan of the internal inflammatory disease unit before Covid-19 infection.].

OBJECTIVE: Decision making in hospitals, and especially in their own healthcare services, is hardly referenced in the literature. During the pandemic period, healthcare services have put in place contingency plans to minimize the consequences of the coronavirus on professionals and patients. However, the deployment of contingency plans and results are hardly shared, depriving other services of references to refute, compare or emulate the aforementioned plans. The objective of this work was the description of the implementation and evaluation of Contingency Plans in the Covid-19 pandemic in a u

Patient-centered care's relationship with substance use disorder treatment utilization.

BACKGROUND: Calls for more patient-centered care are growing in the substance use disorder (SUD) treatment field. However, evidence is sparse regarding whether patient-centered care improves access to, or utilization of, effective treatment services. METHODS: Using nationally representative survey data from SUD treatment clinics in the United States, we examine the association between patient-centered clinical care and the utilization of six services: methadone, buprenorphine, behavioral treatment, routine medical care, HIV testing, and suicide prevention counseling. We measured clinics' prac

Using Community Feedback to Guide the COVID-19 Response in Sub-Saharan Africa: Red Cross and Red Crescent Approach and Lessons Learned from Ebola.

Risk communication and community engagement are critical elements of epidemic response. Despite progress made in this area, few examples of regional feedback mechanisms in Africa provide information on community concerns and perceptions in real time. To enable humanitarian responders to move beyond disseminating messages, work in partnership with communities, listen to their ideas, identify community-led solutions, and support implementation of solutions systems need to be in place for documenting, analyzing, and acting on community feedback. This article describes how the International Federa

[Special medical problems in end-of-life care : Crisis at the end of life - Which therapy is adequate and when is redefining treatment goals appropriate?].

Crisis at the end of life are exceptional challenges for patients, relatives and therapists. With respect to the individual treatment goals and the patients' autonomy, therapeutic action should be commenced in an adequate manner in order to preserve the patients' quality of life as much as possible. Advance care planning for specific critical scenarios may be helpful in order to treat patients according to their wishes and values even if they are not capable to express themselves. Furthermore, a crisis plan can define the scope of action for therapists in emergency situations. Dyspnea, pain an

Optimal incentives for allocating HIV/AIDS prevention resources among multiple populations.

Many agencies, such as the United Nations Program on HIV/AIDS (UNAIDS), the World Health Organization (WHO), the World Bank, the U.S. President's Emergency Plan for AIDS Relief (PEPFAR), and the Global Fund to Fight AIDS, Tuberculosis and Malaria, provide funding to prevent HIV/AIDS infections worldwide. These funds are allocated at multiple levels, resulting in a highly complicated distribution process. An oversight agency allocates funds to various national-level decision-makers who then allocate funds to regional-level decision-makers who in turn distribute the monies to local organizations

From consultation to choice: gynecologic cancer patients' perspectives on shared decision making in clinical practice - findings from a cross-sectional observational qualitative interview study.

BACKGROUND: Shared decision making (SDM) is a cornerstone of patient-centered care in oncology, yet its implementation remains inconsistent across clinical contexts. Particularly in gynecologic oncology, where treatment choices often carry profound implications for identity, fertility and quality of life, understanding how patients engage in SDM is crucial. Patient autonomy, understood as the capacity and right to participate meaningfully in medical decisions, is central to this process, yet remains complex and context-dependent. OBJECTIVE: This study explores how women with gynecologic cancer

Barriers and Facilitators to Intensive Care Unit Nurses' Participation in End-of-Life Decision-Making: A Scoping Review.

Intensive care unit (ICU) nurses are widely regarded as essential contributors to end-of-life (EOL) decision-making, given their continuous bedside presence, close engagement with patients and families, and central role in interprofessional communication. However, a persistent gap remains between the conceptual recognition of this role and nurses' actual participation in formal decision-making. This scoping review summarizes current evidence on the barriers and facilitators shaping ICU nurses' participation in EOL decision-making and on the present state of their involvement in clinical practi

The impact of case factors on the initial screening decision in child welfare investigations in Norway.

BACKGROUND: When a child welfare service agency receives a report of concern, there is an initial screening to decide whether an investigation needs to be initiated. In addition to the decision maker, case factors, external factors, and organizational factors have an impact on decision making in Child Welfare Services (CWS). Few recent studies have considered the impact of case factors on the initial screening. OBJECTIVE: This study examined case factors that have an impact on the decision to investigate in the Norwegian CWS. PARTICIPANTS AND SETTING: Participants included randomly drawn sam
