Hyperbaric medicine: state of the art, 1979.

An attempt has been made to determine the clinical usage of hyperbaric oxygen therapy at 83 North American hyperbaric treatment centers from 1971 to 1978. Questions were asked about the conditions or diseases treated, yearly case load for each condition, location of functional hyperbaric chambers, types of chambers used, operating costs, and personnel requirements. Commercial diving chambers that treat decompression sickness and air embolism from diving accidents were included in the last two years of the survey. Fifty-seven responses were received; 30 treatment centers had multiple chambers,

Optic neuropathy following an altitude exposure.

This case report describes a 20-yr-old man who presented with retro-orbital pain and blurred vision in his left eye 3 wk after an altitude exposure in a hypobaric chamber. He was found to have significant deficits in color vision and visual fields consistent with an optic neuropathy in his left eye. The patient was diagnosed with decompression sickness and treated with hyperbaric oxygen with a U.S. Navy Treatment Table VI. All signs and symptoms resolved with a single hyperbaric oxygen treatment but recurred. A head MRI revealed a left frontoethmoid sinus opacity. A concomitant sinusitis was d

Simple decompression or anterior subcutaneous transposition for ulnar neuropathy at the elbow: a cost-minimization analysis--Part 2.

OBJECTIVE: A randomized controlled trial was reported recently, in which simple decompression and anterior subcutaneous transposition were compared for treatment of ulnaropathy at the elbow. Clinically, both surgical options seem to be equally effective. The objective of this study is to compare the costs, from a societal standpoint, of simple decompression versus anterior subcutaneous transposition in Euros. METHODS: Published and unpublished data from a randomized controlled trial that compared simple decompression and anterior subcutaneous transposition were used. The investigators tabulat

Predicting Post-Dive Inert Gas Bubble Grades in Non-Decompression Scuba Diving with Air: Simplified Model for Enhanced Diver Safety.

BACKGROUND: Even well-planned no-decompression dives can still produce inert gas bubbles that increase decompression sickness risk. A previously proposed formula for predicting post-dive bubble grades integrates individual factors (age, breathing gas consumption) with dive parameters (maximum depth, surface interval). This study aimed to confirm the formula's validity in an independent dataset and to find out whether detailed dive profile data are of further relevance in predicting echocardiography-derived post-dive bubble grades. Additionally, we explored whether machine learning models lever

Medical aspects of air travel.

Air travel is convenient but can exacerbate underlying medical conditions. Most problems can be related to changes in atmospheric pressure, hypoxia, air sickness and jet lag. International travelers should be counseled about traveler's diarrhea and malaria if they are flying to endemic areas. Pregnancy is not in itself a contraindication to flying. Common-sense recommendations and reassurance can help assuage concerns about prolonged air travel.

Post-decompression bubble and inflammation interactions: a non-extensive dynamical system model.

Inert gas bubbles in tissues and in blood have been historically considered as the only triggering factors for DCS, but now many other factors are considered to affect the final outcome of a decompression profile for a certain individual. In this sense, inflammation seems to play a relevant role, not only due to the physical damage of tissues by the bubbles, but as a potentiator of the process as a whole. The present study aims to put forward a mathematical model of bubble formation associated with an inflammatory process related to decompression. The model comprises four state-variables (iner

Recreational scuba injuries.

Decompression illness, which includes decompression sickness and arterial gas embolism, can be a complex and difficult diagnostic dilemma. The signs and symptoms are not well known and mimic other illnesses which often results in delay when seeking medical evaluation. Additionally, most physicians are not exposed to diving medicine and may not recognize a decompression illness injury. The Divers Alert Network (DAN) offers information to the diving public and telephone consultation for physicians who require assistance in the evaluation of scuba divers and management of dive accident cases. Decompression strain in parachute jumpmasters during simulated high-altitude missions: a special reference to preoxygenation strategies.

PURPOSE: Military parachute operations are often executed at high altitude, from an unpressurized aircraft compartment. Parachute jumpmasters (JM) are thus regularly exposed to 29,500 ft for 60 min. The aim was to investigate the decompression strain during a simulated JM mission at high altitude and to compare two strategies of preoxygenation, conducted either at sea-level or below 10,000 ft, during ascent to mission altitude. METHODS: Ten JM completed, on separate occasions, a 45-min preoxygenation either at sea-level (normobaric: N) or 8200ft (hypobaric: H), followed by exposure to 28,000

Hyperbaric Oxygen Therapy and Radiation-Induced Injuries.

Initial clinical uses of hyperbaric oxygen (HBO2) capitalized upon physical effects to drive offending gases back into solution and deliver more oxygen to tissues in early treatments of decompression sickness. HBO2 has a myriad of other effects, including stimulating angiogenesis and new cellular in growth for healing.

Pulmonary barotrauma: assessment and investigation in divers.

Decompression illness (DCI) is an uncommon problem but can be significant in terms of morbidity and, very rarely, mortality. The mechanisms of DCI are pulmonary barotrauma and decompression sickness due to inert gas supersaturation. After the initial management phase, identification of predisposing factors is important to help advise divers regarding future risk and avoidance. Here we present four cases of DCI where pulmonary barotrauma was the likely causative mechanism. We highlight the important features in assessment for pulmonary barotrauma and advising divers on the risk of a recurrence.

Compressed air tunneling and caisson work decompression procedures: development, problems, and solutions.

Multinational experience over many years indicates that all current air decompression schedules for caisson and compressed air tunnel workers are inadequate. All of them, including the Occupational Safety and Health Administration tables, produce dysbaric osteonecrosis. The problem is compounded because decompression sickness (DCS) tends to be underreported. Permanent damage in the form of central nervous system or brain damage may occur in compressed air tunnel workers, as seen on magnetic resonance imaging, in addition to dysbaric osteonecrosis. Oxygen decompression seems to be the only viab

Risk mitigation in divers with persistent (patent) foramen ovale.

In this issue, Anderson and colleagues report follow-up of divers who were found to have a persistent (patent) foramen ovale (PFO) or, in eleven cases, an atrial septal defect (ASD). In most divers diagnosis followed an episode of decompression illness (DCI). The efficacy of closure of the PFO/ASD in preventing future DCI was compared with conservative diving. They reported that in the closure group the occurrence of confirmed DCI decreased significantly compared with pre-closure, but in the conservative group this reduction was not significant. It is believed there are three requirements for

DCS or DCI? The difference and why it matters.

There are few issues that generate as much confusion in diving medicine as the nomenclature of bubble-induced dysbaric disease. Prior to the late 1980s, the diagnosis 'decompression sickness' (DCS) was invoked for symptoms presumed to arise as a consequence of bubble formation from dissolved inert gas during or after decompression. These bubbles were known to form within tissues, and also to appear in the venous blood (presumably after forming in tissue capillaries). A second diagnosis, 'arterial gas embolism' (AGE) was invoked for symptoms presumed to arise when bubbles were introduced direct

A biophysical vascular bubble model for devising decompression procedures.

Vascular bubble models, which present a realistic biophysical approach, hold great promise for devising suitable diver decompression procedures. Nanobubbles were found to nucleate on a flat hydrophobic surface, expanding to form bubbles after decompression. Such active hydrophobic spots (AHS) were formed from lung surfactants on the luminal aspect of ovine blood vessels. Many of the phenomena observed in these bubbling vessels correlated with those known to occur in diving. On the basis of our previous studies, which proposed a new model for the formation of arterial bubbles, we now suggest th

Growth of a gas bubble in a perfused tissue in an unsteady pressure field with source or sink.

In the context of decompression sickness, this paper presents analytical formulae and explanations for growth of a gas bubble in blood and other tissues in an unsteady diffusion field with a source or a sink. The formulae are valid for variable (through decompression) and constant (concerning diving stops/at sea level) ambient pressure. Under a linear decompression regime for ambient pressure, the gas bubble growth is proportional to ascent rate, tissue diffusivity and initial tissue tension and inversely proportional to surface tension, initial ambient pressure and the strength of the source/

Principles for the Design, Validation, and Acceptance of Decompression Procedures.

INTRODUCTION: The 1987 UHMS Workshop on the Validation of Decompression Tables provided standards for the design, testing, and acceptance of decompression tables. Forty years later, research funding has become scarce, and diving companies no longer receive support from universities and Navy research centers. Companies build on their experience and safety culture to improve existing decompression procedures. We list principles and propose guidelines for these internal developments. PRINCIPLES: Editing decompression tables is a technical activity that aims to provide a practical solution to an

Physiological and clinical aspects of apnea diving.

Apnea diving is a fascinating example of applied physiology. The record for apnea diving as an extreme sport is 171 meters, 8:58 minutes. The short time beneath the surface induces profound cardiovascular and respiratory effects. Variations of blood-gas tensions result from the interaction of metabolism and the rapid sequence of compression and decompression. Decompression sickness is possible. Apnea divers can reach depths beyond the theoretic physiologic limit by using the lung-packing maneuver. Apnea divers exhibit a fall in heart rate, which can be trained and is an oxygen-conserving effec

Recent changes in hypoxia training at the Royal Air Force Centre of Aviation Medicine.

Hypoxia training at the Royal Air Force Centre of Aviation Medicine (RAF CAM) has traditionally involved the use of a hypobaric chamber to induce hypoxia. While giving the student experience of both hypoxia and decompression, hypobaric chamber training is not without risks such as decompression sickness and barotrauma. This article describes the new system for hypoxia training known as Scenario-Based Hypoxia Training (SBHT), which involves the subject sitting in an aircraft simulator and wearing a mask linked by hose to a Reduced Oxygen Breathing Device (ROBD). The occupational requirements to

Incidence of vestibular symptomatology in 2,500 U.S. Navy diving accidents (1933-1970).

The U.S. Navy diving accident records of 2,500 cases for the years 1933 to 1970 were analyzed and sorted into Type I and Type II decompression sickness. Type II was further sorted into "vestibular" and "other" categories. It was concluded that Type II symptoms accounted for 30% of the decompression accidents and it was estimated that the overall incidence of vestibular symptomatology was between 10 and 20%. Nearly 30% of the Type II cases were diagnosed as having vestibular involvement, although almost 60% of the cases contained a report of a symptom typically associated with the vestibular sy
