OBJECTIVE: Aggressive surgery seems mandatory for poor-grade middle cerebral artery (MCA) aneurysm with associated intracerebral hemorrhage (ICH). However, primary decompressive craniectomy (DC) is controversial. We performed a case control study to define the role of primary DC.
MATERIALS AND METHODS: We analyzed data from the two cohorts: a multicenter prospective poor-grade aSAH registry study (AMPAS); and the National Clinical Research Center for Neurological Diseases (NCRC-ND) database of poor-grade patients. Outcome was assessed by modified Rankin Scale (mRS) and was dichotomized into f

UNLABELLED: Decompressive craniectomy (DC) is the surgical management of increasing intracranial pressure (ICP) following a severe traumatic brain injury (TBI) as well as severe cerebrovascular insult. Established protocols for the use of DC include monitoring ICP and going through non-surgical methods to reduce ICP before DC which is the last resort. ICP monitors and facilities to render patients hypothermic and for inducing barbiturate coma are not readily available in resource depleted facilities, hence the need for a timely DC. DC when timely done saves lives and improves chances of surviv

BACKGROUND: Decompressive craniectomy (DC) has been sporadically used in cases of infectious encephalitis with brain herniation. Like for other indications of DC, evidence is lacking regarding the beneficial or detrimental effects for this pathology.
METHODS: We reviewed all the cases of viral and bacterial encephalitis treated with decompressive craniectomy reported in the literature. We also present one case from our institution. These data were analyzed to determine the relation between clinical and epidemiological variables and outcome in surgically treated patients.

INTRODUCTION AND OBJECTIVE: Intracranial hypertension (ICH) is the main cause of morbidity and mortality in patients with severe traumatic head injuries. Decompressive craniectomy (DC) is a surgical technique that allows to reduce intracranial pressure (ICP) and to improve cerebral blood flow.
OBJECTIVE: To present our experience on DC for the treatment of ICH.
PATIENTS AND METHODS: Retrospective review of patients admitted from January 2005 to December 2008 who had a traumatic brain injury (TBI) and uncontrollable intracranial hypertension despite optimal medical treatment and who needed DC

BACKGROUND: Meningoencephalitis may sometimes cause medically refractory intracranial hypertension and brain herniation. In such patients death is common. There are a limited number of reports on the use of decompressive craniectomy as a life saving measure in these circumstances with some good results. The aim of the study was to report experience in three further patients.
MATERIALS AND METHODS: In a 15-month period, three patients affected by acute meningoencephalitis were surgically treated by decompressive craniectomy at the Department of Neurosurgery of the Polytechnic University of Anc

BACKGROUND: High intracranial pressure (ICP) is the most frequent cause of death and disability after severe traumatic brain injury (TBI). High ICP is treated by general maneuvers (normothermia, sedation etc) and a set of first line therapeutic measures (moderate hypocapnia, mannitol etc). When these measures fail to control high ICP, second line therapies are started. Among these, second line therapies such as barbiturates, hyperventilation, moderate hypothermia or removal of a variable amount of skull bone (known as decompressive craniectomy) are used.

INTRODUCTION: The management of refractory post-traumatic cerebral oedema remains a frustrating endeavor for the neurosurgeon and the intensivist. Mortality and morbidity rates remain high, despite refinements in medical and pharmacological means of controlling intracranial hypertension.
METHOD AND MATERIAL: In this retrospective study we have evaluated the efficacy of decompressive craniectomy as a last resort therapy, from the data of nine patients with severe brain injury and delayed cerebral oedema (diffuse injury type III), treated between January 1997 and September 1999. The following p

OBJECTIVE: To investigate the frequency and risk factors of contralateral epidural hematoma (CEDH) following decompressive craniectomy (DC) in patients with calvarial skull fracture contralateral to the craniectomy site.
METHODS: After reviewing the medical records and radiographs over a 5-year period, 72 patients with calvarial fracture contralateral to the craniectomy site were enrolled to determine the risk factors for the development of CEDH following DC.
RESULTS: Among 13 patients with CEDH following DC, all but 1 patient were younger than 60 years of age. In 10 patients (77%) with CEDH

BACKGROUND: Cerebral venous sinus thrombosis (CVT) is a rare condition that, in severe cases, can cause refractory intracranial hypertension. Despite limited evidence, decompressive craniectomy (DC) is endorsed as a rescue treatment. We aimed to describe indications, surgical characteristics, and outcomes of DC for severe CVT in a near-nationwide Swedish cohort.
METHODS: This multicentre retrospective case-series included all CVT patients treated with DC (n = 13) at five Swedish neurosurgical centres between 2008-2022. Demographic, clinical, radiological, and surgical variables, and six-month

BACKGROUND: Decompressive craniectomy (DC) is a last-resort treatment for severe traumatic brain injury (TBI) with refractory intracranial hypertension. Randomized controlled trials (RCTs) report mixed and sometimes conflicting results, leaving uncertainties regarding indications, timing, and long-term benefits. This study explored DC practices and outcomes in a contemporary Swedish setting contextualised in modern RCT evidence.
METHODS: This retrospective multicentre study included 299 TBI patients who underwent DC between 2008 and 2022 across four Swedish neurosurgical centres. Clinical, ra

BACKGROUND AND OBJECTIVES: Post-traumatic hydrocephalus (PTH) is a prominent complication that arises after decompressive craniectomy(DC) for traumatic brain injury(1), characterized by a multifaceted pathogenesis and substantial inter-individual variability. This study aims to identify potential risk factors to provide clinical support for the diagnosis and treatment of PTH.
METHODS: This study retrospectively analyzed the clinical data of 457 patients who underwent decompressive craniectomy for traumatic brain injury at the Department of Neurosurgery, Sir Run Run Shaw Hospital, between Janu

BACKGROUND: Severe community-acquired central nervous system (CNS) infections may, in rare cases, cause life-threatening cerebral oedema. This study aimed to investigate the utilisation, indications, and outcomes of decompressive craniectomy (DC) as a last-tier intervention for intracranial hypertension for these conditions in a Swedish multicentre setting.
METHODS: In this retrospective multicentre case series, we included all patients who underwent DC for severe CNS infection between 2008 and 2022 (only 2008-2018 at one centre) at five neurosurgical departments, covering approximately 80% o

BACKGROUND: Ruptured middle cerebral artery aneurysm (MCAa) with intraparenchymal hematoma (IPH) can benefit at the same time from evacuation of the hematoma and exclusion of the aneurysm of a decompressive craniectomy (DC). To date, there are no clear recommendations for performing a DC in such cases.
METHODS: We retrospectively collected data from nine French neurosurgical units from January 1, 2013 to December 31, 2020. All MCAa patients with IPH requiring evacuation of the IPH were included in this study. Poor outcomes were defined by an mRs score of 3-6 at 6 months. Propensity score matc

UNLABELLED: Although the first publication devoted to early decompressive craniectomy in children with TBI and refractory intracranial hypertension occurred more than two decades ago, the issue of effectiveness and functional outcomes continues to be discussed. Variability of data and methods for assessing the effectiveness of decompressive craniectomy in children with severe TBI does not allow for convincing recommendations.
OBJECTIVE: To identify the risk factors of unfavorable outcomes after decompressive craniectomy in children with severe TBI.

BACKGROUND: Decompressive craniectomy (DC) seemed to reduce the risk of death or profound disability (modified Rankin Scale score, 5-6) after deep intracerebral hemorrhage (ICH) by an absolute 13% (95% CI, 0%-26%) in the SWITCH trial (Swiss Trial of Decompressive Craniectomy versus Best Medical Treatment of Spontaneous Supratentorial Intracerebral Hemorrhage). Whether the effect of DC differs by ICH location is unknown.
METHODS: Post hoc analysis of participants with supratentorial severe deep ICH from the intention-to-treat population of the SWITCH randomized controlled trial. We categorized

INTRODUCTION AND IMPORTANCE: Intracerebral haemorrhage (ICH) secondary to use of anti-coagulants is one of the fearsome complications. Haemorrhage within the intracranial space due to such anti-coagulants results in expansion of the intracranial bleeding despite the reversal of anti-coagulants. Hence, timely surgical intervention can be lifesaving.
CASE SUMMARY: An elderly female who was undergoing management for her deranged coagulation parameters was found to be in a state of features suggestive of stroke. The patient was on regular anti-coagulants medication for her cardiology issues. Comp

BACKGROUND: This study aimed to compare the clinical outcomes of decompressive craniectomy (DC) and craniotomy in treating acute subdural hematoma (ASDH) to provide a more precise assessment of the procedures' outcomes.
METHODS: We searched for relevant articles in PubMed, Web of Science, Embase, Scopus, and Cochrane till August 2023, including cohort studies and randomized controlled trials comparing craniotomy and DC for ASDH. The analysis was conducted using "Review Manager" software, using the risk ratio along with a 95% confidence interval (CI) for categorical data, whereas continuous da

Decompressive craniectomy (DC) has been the main surgical treatment of refractory high intracranial pressure (ICP) in traumatic brain injury (TBI) for decades. Basal cisternostomy emerged as a novel option in brain trauma, with results still unclear. We performed a systematic review and meta-analysis comparing cisternostomy added to conventional DC versus DC alone for the treatment of TBI. This study was designed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guideline. Three databases were systematically searched for studies comparing cister

BACKGROUND AND PURPOSE: Malignant cerebral edema (MCE) secondary to ischemic stroke is a highly morbid condition. Decompressive craniectomy (DC) is the only treatment for MCE that has been shown to reduce mortality. We examined whether early infarction and/or hypoperfusion in specific topographic regions was predictive of the need for later DC.
METHODS: A retrospective database of patients evaluated for large vessel occlusion (LVO) stroke at Stanford between 2010 and 2019 was used. Thirty patients with LVO and baseline perfusion MRI who underwent DC were evaluated. Propensity matching based o
