Anorectal angle at rest predicting successful sacral nerve stimulation in idiopathic fecal incontinence-a cohort analysis.

PURPOSE: Sacral nerve stimulation is an effective treatment for patients suffering from fecal incontinence. However, less is known about predictors of success before stimulation. The purpose of this study was to identify predictors of successful sacral nerve stimulation in patients with idiopathic fecal incontinence. METHODS: Consecutive female patients, receiving peripheral nerve evaluation and sacral nerve stimulation between September 2008 and October 2014, suffering from idiopathic fecal incontinence were included in this study. Preoperative patient's characteristics, anal manometry, and

[Functional evaluation of the anorectal region].

Defecography is a method allowing the morphodynamic evaluation of the anorectal region. The technique we employed needs two complementary times: "phase" defecography and dynamic defecography. In our series of patients affected with severe constipation, 2 groups could be identified. Group A included those patients (mean age: 38.7 years) in whom no significant changes were observed in anorectal angle and in the distance of anorectal angle from pubococcygeal line in comparison with normal subjects (Student's t-test). Group B included those patients (mean age: 63.3 years) in whom significant reduc

ESR Essentials: pelvic floor imaging-practice recommendations by the European Society of Urogenital Radiology.

Pelvic floor dysfunction is a common and underdiagnosed condition affecting urinary, gastrointestinal, and sexual function. Imaging plays a pivotal role in diagnosis and treatment planning, yet approaches to technique, interpretation, and reporting remain inconsistent. This article, authored by members of the ESUR Pelvic Floor Working Group, provides practical recommendations for the use of fluoroscopic defecography, MR defecography, and pelvic floor ultrasound, including standardized protocols, key measurements, and a structured, compartment-based interpretation strategy. We detail the imagin

Internal intussusception of the rectum: a changing perspective.

Internal intussusception of the rectum is the funnel-shaped infolding of the rectum during straining to defecate. Patients present with multiple symptoms; most commonly rectal pressure and pain in association with constipation and straining. Defecography (videofluoroscopy during defecation) is the method of choice for diagnosing this problem. Rectal mobility from the sacrum, infolding of the rectum and sphincter relaxation can be used to grade the findings on defecography (Grade I-IV). The optimal therapy for internal intussusception of the rectum is unknown because the cause of the problem is

Patients' perception of tests in the assessment of faecal incontinence.

The objective of this study was to evaluate patient perception of endoanal MRI compared with defecography and anorectal functional testing in the workup of patients with faecal incontinence. Consenting consecutive patients underwent a standard testing protocol consisting of endoanal MRI, defecography and anorectal function combination. Patient experience was evaluated with a self-administered questionnaire, addressing anxiety, embarrassment, pain and discomfort, each measured on a 1 (none) to 5 (extreme) point-scale. Patients were also asked to rank the three tests from least to most inconveni

Magnetic Resonance Imaging Template to Standardize Reporting of Evacuation Disorders.

Magnetic resonance (MR) defecography, including both static and dynamic phases, is frequently requested by gastroenterologists and colorectal surgeons for planning the treatment of obstructive defecation syndrome and pelvic organ prolapse. However, reports often lack key information needed to guide treatment strategies, making management challenging and, at times, controversial. It has been hypothesized that using structured radiology reports could reduce missing information. In this paper, we present a structured MR defecography template report that includes nine key descriptors of rectal eva

Enterocele associated with rectocele revealed by dynamic pelvic CT.

Enterocele is often associated with other pelvic floor disorders but it is not always possible to detect by clinical examination. Defecography with peritoneography and/or barium meal intake has recently been developed as a new method to identify enterocele, but this method is an invasive procedure. Multislice computed tomography was performed at rest and during simulated defecation to evaluate an 80-year-old female patient who had a defecation disorder and was diagnosed as having rectocele based on results from defecography and clinical findings. Multiplanar reconstruction images were generate

Changing concepts in the pathogenesis, evaluation, and management of solitary rectal ulcer syndrome.

The purpose of this study was to assess the impact of new technology on both the understanding of the underlying pathophysiology and treatment of solitary rectal ulcer syndrome (SRUS). This study is a retrospective review of patients with a histologic diagnosis of SRUS (1993 to 2007) complimented with a prospective database of those patients studied with defecography and dynamic pelvic MRI. Thirty patients were available for evaluation. A polyp or mass was present in 74 per cent. Ulcers were found in only 23 per cent. All 12 patients undergoing defecography demonstrated rectorectal intussuscep

Multimedia article. External pelvic rectal suspension (the express procedure) for internal rectal prolapse, with or without concomitant rectocele repair: a video demonstration.

PURPOSE: Internal rectal prolapse has been proposed as a cause of symptomatic rectal evacuatory dysfunction. Abdominal rectopexy, the standard surgical approach, has significant attendant risk and does not address any concomitant rectocele. This video was designed to demonstrate a novel surgical method that uses porcine collagen implants (Permacol), designed to correct internal rectal prolapse, with or without rectocele. INCLUSION CRITERIA: severe rectal evacuatory dysfunction refractory to maximal conservative therapy and full-thickness internal rectal prolapse impeding rectal emptying on de

[MR Imaging Diagnostics in Proctology].

MR Imaging Diagnostics in Proctology Abstract. MR imaging is excellent for 1) the staging of rectal cancer 2) the assessment of chronic inflammatory bowel disease and 3) the performance of defecography. 1) MRI accuracy for the classification of the T- and N-stage is 75 - 95 % and 71 - 85 %, respectively. An infiltration of the mesorectal fat is easily detectable. 2) Due to the high soft-tissue contrast MRI depicts perirectal and perianal abscesses and fistulas, demonstrates the anatomical correlation to the anal sphincters and monitors the therapeutic response and the activity of the underlyin

Anal endosonographic findings in patients with obstructed defecation.

Anal endosonography, including measurements of anal sphincter size, was performed in 16 patients with obstructed defecation. The findings were compared with those at defecography and anal manometry. Patients with rectocele and intussusception had a normal endosonographic appearance. One patient with puborectalic spasm had normal sonography. There was no correlation between sphincter size and anal manometry. The external sphincter muscle was thicker and the cross-sectional area larger in patients with obstructed defecation than in healthy controls (p < 0.05). Two patients with sphincter spasm a

Rectal diverticulum: a new complication of STARR procedure for obstructed defecation.

We report a case of rectal diverticulum developed after stapled transanal rectal resection (STARR) procedure for obstructed defecation. A 21-year-old woman with chronic constipation was diagnosed with a rectocele at defecography. The patient underwent STARR procedure. Six months later, she presented with severe constipation requiring enemas and a worse condition than that preoperatively. Defecography and rectoscopy revealed a rectal wall diverticulum cavity with incomplete elimination of barium enema. The patient underwent transanal diverticulectomy and direct rectal wall repair. Anorectal motility in children with complete rectal prolapse.

Anorectal manometry, defecography, and ultrasonographic study were performed in 36 children with complete rectal prolapse and 45 age- and sex-matched controls. Anorectal manometry disclosed that there was no significant difference in Pr, Pac, or length of HPZ between patients and controls. The rate of BRC of the smooth muscle of the anal canal and RC were significantly lower in patients. Rectoanal reflex was present in all patients and controls. Defecography and ultrasonographic examination confirmed the hypothesis that rectal prolapse starts initially as an intussusception of the rectum, then

Partial Division of Puborectalis Muscle with Lateral Internal Sphincterotomy: A Novel Surgical Technique for Management of Anal Hypertonia-Associated Anismus.

BACKGROUND: Treatment of anismus usually starts with biofeedback therapy and injection of botulinum toxin in the puborectalis muscle (PRM). Patients refractory to conservative treatment may require surgery. The present cohort study aimed to assess a combined technique of partial division of PRM and tailored lateral internal sphincterotomy (LIS) in treatment of anismus. METHODS: Patients with anismus who failed conservative treatments were assessed clinically and with high-resolution anal manometry (HRAM), EMG, defecography, and underwent combined partial division of PRM on one side and tailor

Fecal incontinence successfully managed by antegrade continence enema in children: a report of two cases.

Two children with intractable fecal incontinence after correction of high anorectal malformations were successfully managed by the daily administration of a glycerin enema into the cecum via an appendicocecostomy or tubularized cecostomy, according to the method of Malone's antegrade continence enema (ACE). Fluoroscopic defecography performed during this procedure in each patient disclosed that the glycerin enema promptly evoked cecal peristalsis, which was transmitted to the distal colon and rectum, and squeezed out almost all the fecal matter, evacuating it from the anus. However, two enemas
