The effects of pneumothorax on defibrillation thresholds during pectoral implantation of an active can implantable cardioverter defibrillator.

Pneumothorax has previously been reported to increase defibrillation thresholds and lead impedance in monophasic systems. This article demonstrates a case using an active can system in which the lead impedance between the right ventricular defibrillation coil and the device improved from 70 omega with pneumothorax to 48 omega after resolution. The defibrillation threshold also improved from > 30 J to < or = 10 J after the pneumothorax had resolved. We conclude that acute pneumothorax can increase the transthoracic lead impedance and defibrillation thresholds; however, with resolution both para

Initial clinical experience with endocardial defibrillation using an implantable cardioverter/defibrillator with a triple-electrode system.

We evaluated the early clinical performance of an implantable cardioverter/defibrillator with a nonepicardial lead system in patients with refractory ventricular tachycardia or ventricular fibrillation. Ten patients, mean age 67 years, mean left ventricular ejection fraction 35%, refractory to 5 +/- 2 antiarrhythmic drugs and with a history of prior cardiac surgery (7 patients), severe lung disease (2 patients), or renal failure (1 patient) underwent device and lead system implant. A tripolar electrode catheter with one sensing electrode and two defibrillating electrodes was placed in the righ

Automated External Defibrillators for In-Hospital Cardiac Arrest: A Systematic Review.

BACKGROUND: Automated external defibrillators (AEDs) improve survival after out-of-hospital cardiac arrest, but their role in in-hospital cardiac arrest (IHCA) remains uncertain. This systematic review evaluates whether ward-accessible AED capability prior to code blue team arrival improves survival and neurologic outcomes compared with standard resuscitation systems. METHODS: Six databases were searched. Eligible studies compared AED use to manual defibrillation in IHCA outside specialized areas. Primary outcome was survival with favorable neurological status (CPC 1-2, mRS 0-3, or no deterio

Cost-effectiveness of drone-delivered automated external defibrillators for cardiac arrest.

BACKGROUND: Out-of-hospital cardiac arrest (OHCA) is a significant cause of mortality and morbidity in North America, for which timely defibrillation of shockable rhythms is essential. Drones have been proposed as an intervention to improve response time and are being implemented in practice. AIM: To determine the cost-effectiveness of drone-delivered automated external defibrillators (AEDs) for OHCAs. METHODS: Using data from 22,017 OHCAs in Ontario, Canada over 10 years, we developed a comprehensive computational framework combining machine learning, optimization and a Markov microsimulati

Subcutaneous Implantable Defibrillators in Young Patients: Arrhythmias, Complications, and Physical Activity.

BACKGROUND: The safety of subcutaneous implantable cardioverter defibrillator (S-ICD) recipients who lead active lifestyles and engage in recreational sports is unknown. We aimed to evaluate the association between lifestyle and recreational sports and the occurrence of arrhythmia- and device-related complications, appropriate and inappropriate shocks in S-ICD recipients. METHODS: We assessed a cohort of young-adult (15-65 years) S-ICD patients, evaluated their physical activity with IPAQ (International Physical Activity Questionnaire), and assessed the association between lifestyle and recre

Long-term low-voltage impedance measurements in subcutaneous implantable cardioverter-defibrillators.

BACKGROUND: High-voltage impedance (HVI), measured during subcutaneous implantable cardioverter-defibrillator (S-ICD) implantation via defibrillation testing or a synchronized shock, is associated with defibrillation efficacy. Recently, S-ICD systems have been upgraded to automatically perform long-term measurements of low-voltage impedance (LVI) using a 1-V subthreshold pulse. OBJECTIVE: This study evaluated LVI as a surrogate for HVI and described its long-term trends in S-ICD recipients. METHODS: We analyzed data from 1226 patients who underwent de novo S-ICD implantation across 15 Italia

Polarity-Dependent, Reduced or No-Output Shocks in Implantable Cardioverter-Defibrillators.

Recently, polarity-dependent shock failures were reported in implantable cardioverter-defibrillators caused by structural failure in the high-voltage feedthrough. Short circuits may occur when the right ventricular coil is cathodal for phase 1 of biphasic shocks (cathodal shock). This viewpoint proposes a mechanism for observed polarity dependence and considers whether the same mechanism may apply in other shock-induced, short circuits. Implantable cardioverter-defibrillator connections to the lead traverse feedthroughs into the hermetically sealed housing ("Can"). The feedthrough comprises 2

Mechanisms and management of inappropriate therapy in subcutaneous implantable cardioverter defibrillators.

Subcutaneous implantable cardioverter defibrillators (S-ICDs) provide reliable defibrillation and have enhanced supraventricular tachycardia discrimination and fewer infection rates compared with traditional transvenous systems. However, inappropriate shocks remain a frequent problem. Herein, we review the various mechanisms of these inappropriate therapies, some of which are unique to S-ICDs, and propose an algorithm for preventing recurrences. Proper screening of preimplants is essential to help minimize inappropriate therapies, but patients with hypertrophic cardiomyopathy, Brugada syndrome

Arrhythmic outcome of arrhythmogenic right ventricular cardiomyopathy patients without implantable defibrillators.

BACKGROUND: Implantable defibrillators (ICD) are an important therapy for arrhythmogenic right ventricular cardiomyopathy (ARVC) patients at high risk of sudden death. Given the high appropriate ICD therapy rate, some have argued that the mere act of implanting an ICD inflates the malignant arrhythmia rate in ARVC. OBJECTIVE: To report the arrhythmic course of ARVC patients without ICDs at the fulfillment of the 2010 Task Force Criteria and explore predictors of malignant ventricular arrhythmias. METHODS: We included 131 definite ARVC patients (age 32 ± 15 years, male 39%, proband 50%) eithe

Performance of automated external defibrillators under conditions of in-flight turbulence.

INTRODUCTION: Modern automated external defibrillators (AEDs) are designed to prevent shock delivery when excessive motion produces rhythm disturbances mimicking ventricular fibrillation (VF). This has been reported as a safety issue in airline operations, where turbulent motion is commonplace. We aimed to evaluate whether all seven AEDs can deliver shock appropriately in a flight simulator under turbulent conditions. METHODS: The study was performed in a Boeing 747-400 full motion flight simulator in Hong Kong. An advanced life support manikin and arrhythmia generator were used to produce si

Clinical experience with wearable cardioverter defibrillators at a tertiary electrophysiology program.

BACKGROUND: A wearable cardioverter defibrillator (WCD) is an alternative for patients who are not immediate candidates for an implantable cardioverter defibrillator (ICD). METHODS: Retrospective analysis of 242 patients prescribed a WCD from January 2014 through March 2015 with 1-year follow-up. RESULTS: A WCD was prescribed to 242 patients during the 15-month period. Forty-one patients were excluded due to lack of complete follow-up. The remaining 201 patients received a WCD for primary or secondary prevention of SCD. Seventy-nine percent of WCDs were prescribed by nonelectrophysiologists.

Eligibility of cardiac resynchronization therapy patients for subcutaneous implantable cardioverter defibrillators.

BACKGROUND: Before subcutaneous implantable cardioverter defibrillator (S-ICD) implantation, the adequacy of sensing is required to be verified through surface ECG screening. Our objective was to determine whether S-ICD can be considered as a supplementary therapy in patients who are receiving biventricular (BIV) pacing. METHODS: We evaluated 48 patients with BIV devices to determine S-ICD candidacy during BIV, left ventricular (LV), right ventricular (RV) pacing, and intrinsic conduction (left bundle branch block-LBBB) by using an automated screening tool. Eligibility was defined by the pres

Experiences of Young Adults With Pacemakers and/or Implantable Cardioverter Defibrillators.

BACKGROUND: The number of implanted devices (pacemakers [PMs] and implantable cardioverter defibrillators [ICDs]) is increasing. Because of technological advances, these devices are used in greater numbers in children and adolescents. Challenges of having these devices have been well studied in adults, but little is known about the lived experiences from the perspective of young adults. OBJECTIVE: This study explored the lived experiences, coping behaviors, and challenges of young adults who had cardiac devices implanted during childhood. METHODS: A descriptive qualitative study design was u

Surviving out-of-hospital cardiac arrest: just a matter of defibrillators?

Out-of-hospital sudden cardiac arrest (OHCA) is a leading cause of death all over the world. Although the outcome of OHCA resulting from 'nonshockable' rhythms (asystole and pulseless electrical activity) is poor regardless of resuscitation efforts, 'shockable' rhythms such as ventricular tachycardia or fibrillation may carry a good prognosis if early defibrillation is performed. At present, simplified cardiopulmonary resuscitation techniques (hands-only cardiopulmonary resuscitation) and automated external defibrillators (AEDs) offer lay people the possibility to provide lifesaving treatment

Recommendations for the programming of implantable cardioverter-defibrillators in New Zealand.

AIM: Our manuscript proposes recommendations to standardise implantable cardioverter-defibrillator (ICD) programming practice in New Zealand (NZ) and provides a review of the literature behind such an initiative. BACKGROUND: Shocks have traditionally been the mainstay of ICD therapy for ventricular tachyarrhythmia. There is substantial evidence to suggest shocks may cause psychological harm, are often painful and may increase hospitalisation for heart failure. Shock therapy may be reduced by avoiding unnecessary therapy of non-sustained arrhythmia, utilising anti-tachycardia pacing to termina
