A minimally invasive incision and loop drainage technique for the treatment of lower limb Morel-Lavallée lesions: Nose ring drainage technique.

Morel-Lavallée lesions (MLLs) are shearing injuries resulting in separation of the skin and subcutaneous tissue from the underlying fascia. They are closed internal degloving injuries. Classical sites of the lesions are around the greater trochanter, pelvis, thigh, knee joint, and on the head, in decreasing order of frequency. This injury is often delayed or misdiagnosed when patients present with soft tissue injury alone or when more obvious injuries distract from its presence in polytrauma patients. There is currently no universally accepted treatment for these lesions. Conservative manageme

Free serratus anterior fascia flap for reconstruction of hand and finger defects.

INTRODUCTION: Reconstruction of the dorsal surface of hand defects requires thin, pliable, well vascularized tissue with a gliding surface for the extensor tendon course. Also defects of the palmar hand and degloved fingers need non-bulky soft tissue for reconstruction. We present a retrospective analysis of nine patients with free serratus anterior fascia flaps used to cover defects of the palmar and dorsal hand and of degloved fingers. METHODS: Three of the patients presented limited range of hand motion due to full-thickness burns; one patient had defects of the dorsum of one hand after ac

A Morel-Lavallée lesion of the proximal calf in a young trauma patient: A case report.

RATIONALE: The Morel-Lavallée lesion (MLL) is a closed degloving injury developing when shear force acts between the muscle fascia and the subcutaneous layer. MLLs develop principally in the trochanteric area or the pelvis; lesions in the proximal calf are rare. Acute lesions can be treated conservatively, but chronic lesions are best treated surgically because of a high rate of recurrence. To the best of our knowledge, this is a rare case of successful treatment of an MLL in the proximal calf associated with tibio-fibular shaft fracture. PATIENT CONCERNS: A 14-year-old male visited our emerg

Lower Limb Morel-Lavallée Lesion Treated With Short-Stretch Compression Bandaging.

Morel-Lavallée lesions (MLLs) classically occur in the greater trochanteric region, lateral thigh, buttocks, and back. A high percentage of large MLLs require surgical intervention, which comes with an increased risk of skin necrosis and infection. We report a rare case of a large MLL that was successfully treated with compression. The lesion was created when the patient, a 66-year-old man, sustained a low-velocity crush injury. Extending from the medial distal thigh to the proximal medial calf, the MLL was nonoperatively treated with the short-stretch compression bandaging that is used in lym

Traumatic degloving lesion of male external genitalia.

Injury to the male external genitalia is relatively uncommon when compared with other parts of the human body. Trauma to external genitalia could result from accidents involving motor vehicles, power farm machinery, gun shot and stab injuries. We present a rare case of traumatic degloving injury of male external genitalia associated with partial penile amputation and loss of both testes sparing the internal abdominal and pelvic organs, resulting from run over by a heavy vehicle tyre.

Transfer of vessels in the management of thumb and ring avulsion injuries.

The main problem in avulsion or degloving injury is the extensive damage to long segments of vessels which makes direct suture of the structures difficult. Various techniques have been proposed over the years to bypass the segment of an injured vessel. Many of these are difficult and often require the execution of several vascular anastomoses. The transfer of vessels from adjacent fingers, as proposed by Doi, is a valid alternative. During the 5-year period from 1988 to 1993, vessel transfer from adjacent fingers was carried out on 15 patients. Three patients suffered incomplete amputation and

Repurposing Avulsed Skin as a Primary Full Thickness Autograft in Extensive Lower Limb Degloving: A Case Report.

INTRODUCTION: Degloving injuries involve the traumatic separation of skin or underlying tissues from their respective bone structures, typically resulting from high-energy mechanisms such as road traffic accidents. The hallmark of these lesions is the avulsion of septal and muscular perforators, which devascularizes the skin envelope and may lead to delayed full-thickness necrosis or limb loss if mismanaged. Historically, these injuries require staged procedures to replace the skin; however, this case report highlights the role of primary full-thickness skin grafting (FTSG) using repurposed ti

Negative pressure wound therapy incorporating early exercise therapy in hand surgery: bag-type negative pressure wound therapy.

In the postoperative treatment of hand surgery, it is important to start exercise therapy as early as possible. In conventional negative pressure wound therapy, the fingers are immobilized by the film dressing covering the wound and hand, thereby preventing sufficient rehabilitation. Here, we devised a bag-type negative pressure wound therapy that makes it possible to start finger exercises almost immediately, and we applied it to 4 patients:one each with hand burns, symmetrical peripheral gangrene, a crush injury accompanied by extensive skin defects and a fingertip degloving injury. The dura

Adolescent Morel-Lavallée Lesions: A Case Report of Challenges in Diagnosis and Management in Atypical Locations.

BACKGROUND Morel-Lavallee lesions (MLLs) are uncommon, closed soft-tissue degloving injuries caused by high-energy trauma that are often missed due to their rarity and delayed presentation, resulting in serious complications. MLLs are particularly missed and underreported in pediatric and adolescent patients. We describe the case of an adolescent MLL occurring in an atypical lesion site at the calf to increase awareness of this diagnosis and associated presentation in this patient group, which can differ from adult presentation and contribute to diagnostic uncertainty that consequently impacts

Primary Cutaneous Mucormycosis in a Trauma Patient with Morel-Lavallée Lesion.

Mucormycosis is an aggressive fungal infection caused by zygomycetes from the order of Mucorales. Immunocompromised patients or patients with comorbidities are susceptible to this infection. There are many forms of mucormycosis such as rhino-orbito-cerebral, cutaneous, gastrointestinal, and pulmonary. Cutaneous mucormycosis is rare in trauma patients with no comorbidities. Morel-Lavallée lesions are rare degloving injuries in trauma patients. We report a case of cutaneous mucormycosis in a trauma patient with the Morel-Lavallée lesions.

The management of severe fractures of the lower extremities.

This review is designed to address the problem of severe fractures of the lower extremities that often accompany the patient who is polytraumatized. Although open fractures occupy a predominant place in the subsequent discussion, closed fractures also may be associated with a significant crush component, a vascular lesion, a degloving injury, or a compartment syndrome. It is of paramount importance that the treating physician understand the implications of the potential or actual soft tissue damage to maximize the outcome after these devastating injuries.

Analysis of Surgical Approaches to Skull Base Tumors Involving the Pterygopalatine and Infratemporal Fossa.

Selecting an appropriate surgical approach for resection of huge skull base tumors involving pterygopalatine and infratemporal fossa is challenging because of their rarity and high possibility of vital anatomical structure injuries. To suggest the guidance of selecting the appropriate approach by analyzing outcomes and satisfactions of known surgical approaches with our previous experience, the authors retrospectively analyzed skull base tumor cases experienced for 24 years, and condensed to 4 well-known surgical approaches: maxillary swing, infratemporal fossa type C, transzygomatic, and a co

A Case of Arterial and Venous Tear during Single Lead Extraction.

Transcutaneous lead extraction can be associated with significant morbidity and mortality. The risk of causing concomitant arterial and venous injury is rare. We report a case of marginal artery rupture with coronary sinus rupture after a CS lead extraction. A 71-year-old male was admitted for extraction of a 6-year-old implantable cardioverter-defibrillator lead due to fracture from insulation break. During the lead extraction, blood pressure fell precipitously and echocardiographic findings were consistent with pericardial effusion. After unsuccessful pericardiocentesis, open chest sternotom

Associations Between Fracture Characteristics and Surgical Outcomes in Distal Digit Replantation of Tamai Zones 1-2.

BACKGROUND: Although numerous risk factors were reported for the failure of digit replantation, there was a lack of studies investigating the association between replantation outcomes and fracture characteristics. The purpose of this study was to evaluate the survival rate and functional outcomes of distal digit replantation based on the fracture patterns. METHODS: We retrospectively reviewed patients with Tamai zones I-II amputated digits who underwent replantation between 2018 and 2022. The fracture characteristics of the distal phalanx were compared to outcomes, including the survival rate
