Mode of delivery after obstetric anal sphincter injury and the risk of long-term anal incontinence.

BACKGROUND: Primiparous women have an increased risk of obstetric anal sphincter injury; because most of these patients deliver again, there are major concerns about mode of delivery: the risk of recurrent obstetric anal sphincter injury and the risk of long-term symptoms of anal incontinence. Although an elective cesarean delivery protects against recurrent obstetric anal sphincter injury, it is uncertain how the second delivery affects the risk of long-term anal incontinence. OBJECTIVE: The purpose of this study was to evaluate whether the mode of delivery for a second pregnancy, after a do

Obstetric anal sphincter injuries in vaginal delivery of twins: associated risk factors and comparison with singletons.

INTRODUCTION AND HYPOTHESIS: Risk factors related to obstetric anal sphincter injuries (OASIS) are known for singleton deliveries. No study to date has described the rate and risk factors involved in twin deliveries. METHODS: We conducted a retrospective cohort study (1985-2010) of all vaginal twin and singleton deliveries in a single tertiary center. The adjusted odds ratio (OR) for OASIS at delivery was estimated using the logistic regression model. RESULTS: The study comprised 1,538 and 91,312 patients with vaginal twin and singleton delivery, respectively. Twenty twin (1.27 %) and 2,331

Emergency caesarean delivery in prolonged obstructed labour as risk factor for obstetric fractures--a case series.

Birth fractures predominantly affect the clavicle, humerus or femur. Brachial plexus injury may co-exist with humeral or clavicular fractures. From January 2002 to December 2010, 8 neonates with fractures after caesarean section were treated under the supervision of the first author following obstructed labour and caesarean delivery. The most classical of the cases is a vertex-presenting neonate who was delivered by caesarean section for obstructed labour in a primipara in whom ipsilateral klumpke's palsy and fractures of the clavicle and humerus were confirmed. Literature review did not consi

Expected day of delivery from ultrasound dating versus last menstrual period--obstetric outcome when dates mismatch.

OBJECTIVE: To analyse the association between fetal size at time of dating ultrasound and risk for preterm delivery and small-for-gestational-age (SGA) birth and to evaluate if timing of ultrasound, that is before 14 weeks of gestation or after 16 weeks affects this association. DESIGN: Retrospective cohort study. SETTING: Ultrasound departments of Ultragyn, Stockholm, Sweden. POPULATION: A total of 28,776 singleton pregnancies dated between 1998 and 2004. METHODS: Obstetric outcome was assessed through linkage of the cohort to the Swedish Medical Birth Register. MAIN OUTCOME MEASURES: Ri

The role of nocturnal delivery and delivery during the holiday period in Finland on obstetric anal sphincter rupture rates- a population based observational study.

BACKGROUND: Obstetric anal sphincter rupture (OASR) is a serious complication of delivery, which frequently results in faecal incontinence despite primary repair and has serious implications for women's health. The objective of this study was to assess whether human factors, workload and staffing at night, at weekends and during holidays has an effect on the increasing OASR rates among all singleton vaginal deliveries (n = 514,741) having occurred between 1997 and 2007 in Finland. Women (n = 2,849) with OASR were compared in terms of possible risk factors to women without OASR using stepwise l

Enhancing Obstetric Decision-Making With AI: A Systematic Review of AI Models for Predicting Mode of Delivery.

Accurate prediction of the mode of delivery is critical for optimizing maternal and neonatal outcomes and reducing unnecessary cesarean sections. In recent years, AI has emerged as a promising tool for enhancing obstetric decision-making. This systematic review aimed to evaluate and synthesize existing evidence on AI models developed for predicting the mode of delivery, comparing their performance and clinical applicability across diverse settings. A comprehensive literature search was conducted to identify studies that developed and/or validated AI-based predictive models for mode of delivery

Association between obstetric comorbidity index and complications of vaginal delivery: a large cross-sectional study from China.

BACKGROUND: The incidence of vaginal delivery complications (VDCs) has been increasing globally in recent years, posing a significant threat to maternal and neonatal health. The obstetric comorbidity index (OB-CMI) is a widely recognized tool that quantifies the burden of maternal comorbidities and predicts severe maternal morbidity or mortality during delivery and puerperium. However, it remains unclear whether there is an association between OB-CMI and VDCs. OBJECTIVE: The purpose of this study was to investigate the prevalence and spectrum of VDCs, and to explore the association between OB-

Does structured obstetric management play a role in the delivery mode and neonatal outcome of twin pregnancies?

PURPOSE: While the optimal delivery method of twin pregnancies is debated, the rate of cesarean deliveries is increasing. This retrospective study evaluates delivery methods and neonatal outcome of twin pregnancies during two time periods and aims to identify predictive factors for the delivery outcome. METHODS: 553 twin pregnancies were identified in the institutional database of the University Women's Hospital Freiburg, Germany. 230 and 323 deliveries occurred in period I (2009-2014) and period II (2015-2021), respectively. Cesarean births due to non-vertex position of the first fetus were

Outcomes of Term Induction in Trial of Labor After Cesarean Delivery: Analysis of a Modern Obstetric Cohort.

OBJECTIVE: To evaluate outcomes of induction of labor, compared with expectant management, in women attempting trial of labor after cesarean delivery (TOLAC) in a large obstetric cohort. METHODS: We performed a secondary analysis of data from the Consortium on Safe Labor that included women with term (37 weeks of gestation or greater) singleton gestations and a history of one prior cesarean delivery who attempted TOLAC. Induction of labor was compared with expectant management by week of gestation from 37 to 40 weeks in both high- and low-risk cohorts. The primary outcome was failed TOLAC. Se

The incidence of obstetric anal sphincter rupture in primiparous women: a comparison between two European delivery settings.

BACKGROUND: During the last years, the incidence of obstetric anal sphincter rupture (OASR) has increased markedly in Sweden, and significantly less frequently in Italy. Our objective was to explore if different delivery management may explain the variation in OASR incidences. METHODS: In a retrospective study, data from 2,000 primiparous women in a Swedish and an Italian delivery unit were compared with respect to OASR, maternal age, gestational length, birth weight, labour induction, use of epidural analgesia (EDA) and oxytocin, vacuum extraction, episiotomy, and duration of the second stag

The presence of cervical and vaginal fetal fibronectin predicts preterm delivery in an inner-city obstetric population.

OBJECTIVE: It has previously been shown that fibronectin bearing a specific oncofetal domain is present at the chorionic-decidual interface and that its release into cervical and vaginal secretions accurately predicts preterm delivery in patients with uterine contractions. This study examines whether serial assessment of cervical and vaginal fetal fibronectin allows for the prediction of preterm delivery in symptom-free patients derived from an inner-city, general obstetric population. STUDY DESIGN: Cervical and vaginal samples were obtained from 429 consenting patients who received routine p

Validation of a new method to assess estimated blood loss in the obstetric population undergoing cesarean delivery.

BACKGROUND: Postpartum hemorrhage is the leading cause of maternal mortality in developing countries and the primary cause of one-quarter of all maternal deaths globally. Inaccuracy in estimating blood loss obscures the diagnosis of postpartum hemorrhage and its management. OBJECTIVE: Our objective was to compare assessment of blood loss using the quantitative Triton system (Gauss Surgical, Inc, Los Altos, CA) with other measures of blood loss in women undergoing cesarean delivery. STUDY DESIGN: Women scheduled for cesarean deliveries at our facility were included. Intraoperative blood loss

Ethical Considerations for the Delivery of Obstetric and Gynecologic Care During a Pandemic: ACOG Committee Statement No. 6.

Obstetrician-gynecologists (ob-gyns) are essential to providing high-quality health care, and this duty remains unchanged during pandemics. This Committee Statement discusses ethics related to the provision of obstetric and gynecologic care during a pandemic caused by a highly transmissible pathogen. As health care guidelines related to pandemics are created by institutions, ob-gyns have a responsibility to advocate for obstetric and gynecologic health priorities. Additionally, many clinical practice decisions made to reduce the spread of the infectious agent and maximize physicians' ability t
