Dementia Simulation for Family Caregivers of Persons With Dementia: A Scoping Review.

BACKGROUND: Alzheimer's disease and related dementias are becoming increasingly prevalent worldwide. Persons with dementia receive substantial assistance from family caregivers over extended periods of time. Family caregivers of persons with dementia are twice as likely to report physical, emotional, and financial challenges related to caregiving compared with other types of family caregivers. Empathy has been demonstrated to improve caregivers' quality of life and their relationships with care recipients. OBJECTIVE: This scoping review examines the current literature on the effects of dement

A Case of Reversible Dementia? Dementia vs Delirium in Lyme Disease.

Lyme disease is an uncommon cause of reversible dementia. A 75-year-old male patient, with a personal history of mild memory deficit, was admitted to Alzheimer's Disease Care Unit due to hallucinations, confusion and aggressive behavior unresponsive to antipsychotic therapy. A computed tomography (CT) scan of the brain was negative, while blood exams showed a rise in inflammatory parameters. A complete screening of infective diseases showed a positive serology for Borrelia burgdorferi, confirmed at Western blot. Even though the patient refused cerebrospinal fluid (CSF) exam, the brilliant clin

Dementia, a Polypharmaceutical Phenomenon: The Intimate Combinations of Dementia Drugs in Brazil.

It is commonplace to state that dementia is a complex condition. Such complexity involves the limits between pathological and normal aging, diagnosis with no simple organic causation, and the use of psychiatric medication that does not cure but generates hope to alleviate symptoms such as forgetfulness and delirium. Based on an ethnography of one year and a half (2017-2018) in a Brazilian metropolis, within a Public Geriatric center and the households of three families, I argue that dementia, more than a complex condition, is a generator of drug complexity. Following Stefan Ecks' reflections o

Depression in Dementia or Dementia in Depression? Systematic Review of Studies and Hypotheses.

The majority of research works to date suggest that Major Depressive Disorder (MDD) is a risk factor for dementia and may predispose to cognitive decline in both early and late onset variants. The presence of depression may not, however, reflect the cause, rather, an effect: it may be a response to cognitive impairment or alters the threshold at which cognitive impairment might manifest or be detected. An alternative hypothesis is that depression may be part of a prodrome to Alzheimer's Disease (AD), suggesting a neurobiological association rather than one of psychological response alone. Gene

Dementia and the gender trouble?: Theorising dementia, gendered subjectivity and embodiment.

Despite person-centred approaches increasingly focusing on looking at the person in dementia instead of the pathology, the role of gender in dementia has been little explored. This article discusses how pervasive discourses on a loss of self and dementia as abject are interwoven with a de-gendering of persons with dementia. The cultural anxiety that dementia evokes in terms of loss of bodily and cognitive control could also be linked to a failure to normatively and intelligibly express gender when living with dementia. As a way to sustain personhood for people with dementia and challenge disco

Are Parkinson's disease with dementia and dementia with Lewy bodies the same entity?

The diagnosis of Parkinson's disease with dementia (PDD) or dementia with Lewy bodies (DLB) is based on an arbitary distinction between the time of onset of motor and cognitive symptoms. These syndromes share many neurobiological similarities, but there are also differences. Deposition of beta-amyloid protein is more marked and more closely related to cognitive impairment in DLB than PDD, possibly contributing to dementia at onset. The relatively more severe executive impairment in DLB than PDD may relate to the loss of frontohippocampal projections in DLB. Visual hallucinations and delusions

Cognitive and motor symptoms in dementia: focus on dementia with Lewy bodies.

PURPOSE: To describe the clinical syndrome called dementia with Lewy bodies (DLB) and highlight its common and unique characteristics with respect to diagnosis and management. DATA SOURCES: Review of the scientific literature including psychiatric literature, reports of clinical trials, and clinical practice guidelines. CONCLUSIONS: DLB is a clinical and histopathologic disease, which is second only to Alzheimer's disease (AD) as a cause of dementia in older adults. The clinical syndrome of DLB includes cognitive and motor deterioration reminiscent of symptoms associated with AD and Parkinso

The tropicamide test in patients with dementia of Alzheimer type and frontotemporal dementia.

The tropicamide test was applied to 30 patients with probable Alzheimer's disease (pAD), 12 with frontotemporal dementia (FTD) and 46 healthy subjects. One drop of 0.01% tropicamide was instilled in one eye and one drop of saline solution in the other. The pupil diameter was measured with a Goldmann pupillometer in its basal condition and 10, 15, 20, 25, 30, 35, 45 and 55 minutes afterwards. The results do not show differences between pupil dilation observed in pAD and in FTD; in both groups, from the beginning of the test, the pupil dilated more than in healthy people. A high interindividual

Clinical diagnosis of dementia with Lewy bodies in a Japanese dementia registry.

ABO-incompatible (ABOi) living donor liver transplantation (LDLT) requires rituximab-based desensitization and intensified early immunosuppression, and whether this promotes hepatocellular carcinoma (HCC) recurrence remains controversial. We hypothesized that this effect depends on tumor burden, assessed morphologically (Milan criteria) and biologically (ADV score). In this single-center study, we analyzed 1,573 adults transplanted for HCC (ABOi, n = 316; ABO-compatible [ABOc], n = 1,257); the primary endpoint was recurrence-free survival (RFS). Crude recurrence was similar (21.8% vs. 20.7%; P

Neuropsychiatric aspects of multi-infarct dementia and dementia of the Alzheimer type.

Delusions, depression, and hallucinations were assessed in 30 patients with dementia of the Alzheimer type (DAT) and 15 with multi-infarct dementia (MID). The nature and prevalence of delusions did not distinguish DAT from MID: delusions were present at the time of examination in 30% of patients with DAT and in 40% of patients with MID, and had occurred at some time in the course of the illness in half the patients of each diagnostic group. Delusions were primarily paranoid in type and involved elementary misbeliefs concerning theft or infidelity. Depression was significantly more common in MI

Identifying pathways to the prevention of dementia: the Netherlands consortium of dementia cohorts.

BACKGROUND: Aggregation of cohort data increases precision for studying neurodegenerative disease pathways, but efforts to combine data and expertise are often hampered by infrastructural, ethical and legal considerations. We aimed to unite various cohort studies in the Netherlands to enhance research infrastructure and facilitate research on dementia etiology and its public health implications. METHODS: The Netherlands Consortium of Dementia Cohorts (NCDC) includes participants with initially no established cognitive impairment from 9 Dutch cohorts: the Amsterdam Dementia Cohort (ADC), Doeti

Comorbidity of Dementia: A Cross-Sectional Study of PUMCH Dementia Cohort.

BACKGROUND: Comorbidities reduce quality of life for people with dementia and caregivers. Some comorbidities share a genetic basis with dementia. OBJECTIVE: The objective of this study is to assess comorbidity in patients with different dementia subtypes in order to better understand the pathogenesis of dementias. METHODS: A total of 298 patients with dementia were included. We collected some common comorbidities. We analyzed the differences in comorbidities among patients with dementia according to clinical diagnosis, age of onset (early-onset: < 65 and late-onset: ≥65 years old) and apolip

Are dementia with Lewy bodies and Parkinson's disease dementia the same disease?

BACKGROUND: Dementia with Lewy bodies (DLB) and Parkinson's disease dementia (PDD), which share many clinical, neurochemical, and morphological features, have been incorporated into DSM-5 as two separate entities of major neurocognitive disorders with Lewy bodies. Despite clinical overlap, their diagnosis is based on an arbitrary distinction concerning the time of onset of motor and cognitive symptoms, namely as early cognitive impairment in DLB and later onset following that of motor symptoms in PDD. Their morphological hallmarks - cortical and subcortical α-synuclein/Lewy body plus β-amyloid

Differentiation of delirium, dementia and delirium superimposed on dementia in the older person.

Delirium is an acute clinical emergency that requires prompt clinical intervention. A predisposing factor for delirium is dementia, and delirium may highlight the vulnerability of a patient to developing dementia. However, delirium also occurs during an acute illness in patients diagnosed with dementia; this is classified as delirium superimposed on dementia. This complex interplay of both dementia syndromes and the condition of delirium has been extensively studied. However, delirium continues to be under-recognised in the acute setting, which impacts negatively on patient outcomes. Nurses ar

Clinical trials of dementia with Lewy bodies and Parkinson's disease dementia.

Despite the frequency and importance of dementia associated with Parkinson's disease (PDD) and dementia with Lewy bodies (DLB), there is relatively little evidence on which to base treatment. Evidence from meta-analysis suggests that rivastigmine can improve cognition and functioning in PDD and also reduce risk of falling. There is also evidence supporting its use in DLB. Recent evidence suggests that memantine may also be effective, particularly for PDD, although evidence is more conflicting. Memantine may also improve parkinsonism and dyskinesias. Few clinical trials of cognition in PD witho

Prevalence of treatable and reversible dementias: A study in a dementia outpatient clinic.

UNLABELLED: Dementia is a syndrome characterized by the development of multiple cognitive deficits and behavioral changes that leads to impairment of functional activities. There are primary degenerative, progressive and irreversible dementias, and other dementias resulting from a progressive but potentially reversible dementia of secondary origin. OBJECTIVE: To assess the prevalence of cases with a diagnosis of potentially reversible dementia at a Behavioral Neurology Outpatient Unit. METHODS: A retrospective study based on a review of 340 medical records of patients seen from 1999 to 2009

Rapidly progressive dementia: prion diseases and other rapid dementias.

Dementias that occur over weeks to months require a different differential than the slowly progressive dementias that occur over years. Because of the rapidity of decline in cognition, the evaluation of the patient is usually urgent and requires an extensive workup with multiple tests running in parallel. Creutzfeldt-Jakob disease, perhaps the prototypic rapidly progressive dementia (RPD), is often the first diagnosis many neurologists consider when faced with a patient with rapid cognitive decline. Many conditions other than prion disease, however, can present with RPD. Other etiologies inclu

The HIV Dementia Scale: predictive power in mild dementia and HAART.

BACKGROUND: HIV-associated dementia (HIV-D) is a subcortical dementia consisting of cognitive and motor symptoms that ultimately affects as many as 20% of patients with AIDS and is associated with significant morbidity and mortality. With the advent of highly active antiretroviral therapy (HAART), the use of sensitive and efficient screening tests for HIV-D continue to be needed for identifying individuals who develop this disorder. OBJECTIVE: The objective of this study was to compare the HIV Dementia Scale (HDS) with comprehensive neuropsychological procedures in detecting both minor cognit
