4-Year Pulp Survival in a Randomized Trial on Direct Pulp Capping.

INTRODUCTION: This study aimed to assess pulp survival in a randomized trial on pulp lavage in adult nonpainful posterior teeth with carious pulp exposure. The treatment included complete caries excavation, direct pulp capping with mineral trioxide aggregate, and immediate restoration with composite resin. METHODS: Fluid was collected from the pulp wound to assess matrix metalloproteinase-9 (MMP-9) and total protein values. Before pulp capping, cavities were randomly (block randomization, n = 48) washed with a physiological saline or a sodium hypochlorite solution (2.5% NaOCl). Treatment outc

Efficacy of calcium hydroxide and resin-modified calcium silicate as pulp-capping materials: a retrospective study.

The aim of this study was to compare the clinical success rates of light-cured, resin-modified calcium silicate (TheraCal LC) and calcium hydroxide (Dycal) as pulp-capping materials in permanent teeth. In this retrospective electronic record review of patients treated in the years 2012 through 2015, the postoperative data of 120 patients (n = 60 per material) were collected. In these 2 groups, 69 teeth were treated with calcium hydroxide (29 in men, 40 in women) and 79 with calcium silicate (37 in men, 42 in women). The following data were recorded: treated tooth; patient age and sex; reason f

Odontogenic effect of a fast-setting pozzolan-based pulp capping material.

INTRODUCTION: Mineral trioxide aggregate (MTA) is widely used as a pulp capping material. Recently, a MTA-derived fast-setting pozzolan cement (Endocem; Maruchi, Wonju, Korea) was introduced in the endodontic field. Our aim in this study was to investigate the odontogenic effects of this cement in vitro and in vivo. METHODS: Human dental pulp cells (hDPCs) were cultured, and the effects of Endocem and a previously marketed MTA (ProRoot; Dentsply, Tulsa, OK) on biocompatibility were evaluated by assessing cell morphology and performing a cell viability test. Chemical composition of each materi

Emerging trends of injectable hydrogels for vital pulp therapy: A comprehensive review.

BACKGROUND: Injectable hydrogels are widely used in biomedicine and tissue engineering. Recently, they have been introduced as promising capping materials for vital pulp therapy (VPT) owing to their biocompatibility, in situ adaptability, minimal immunogenicity, and ability to modify the cellular activities of dental pulp exposed to caries or accidental injuries. Although injectable hydrogel-based biomaterials facilitate pulp healing and reparative dentine formation, their clinical utility has not been fully explored. OBJECTIVES: This review highlights the gaps in current research, including

Management of the deep carious lesion and the vital pulp dentine complex.

This article describes the relationship between the carious process and pulp-dentine complex reactions. Where the balance between the two is in favour of the carious process and where conventional cavity preparation leads to a direct pulp exposure, the direct pulp cap technique is described. The success of the technique is addressed and more importantly an alternative technique for caries removal, namely stepwise excavation, is described which may lead to a reduced risk of carious exposure and the need for the direct pulp cap technique.

The treatment of deep dentine caries lesions.

UNLABELLED: This paper discusses the excavation of deep caries lesions, focusing on indirect pulp-capping and the stepwise excavation approach. The concept of a modified and less invasive stepwise excavation is presented, based on considerations of caries pathology. The aim of the first excavation is primarily to make a change within the cariogenic environment, and not to remove carious dentine close to the pulp because this risks an iatrogenic pulp exposure. Microbiological and clinical studies have shown that the number of bacteria decrease during stepwise excavation procedures, and that les

Pulp capping with hydroxyapatite ceramic in rat molars.

Direct pulp capping with a hydroxyapatite ceramic in rat molars induced the formation of reparative dentin in the exposure sites after 2 weeks. In addition, foci of acute and chronic pulpitis were detected. After 6-8 weeks of application of the material partial or total necrosis occurred. The use of hydroxyapatite as a possible pulp capping material is discussed.

Direct Pulp Capping with Advanced Platelet-Rich Fibrin: A Report of Two Cases.

This article aims to prove dentin bridge formation in two cases after direct pulp capping in reversible pulpitis using the platelet concentrate A-PRF+ and preservation of the vitality of the dental pulp. The hemostasis process for the pulp wound and cavity disinfection with gaseous ozone was performed under anesthesia. A large A-PRF+ membrane was prepared from blood plasma and applied to the pulp wound. After placing an MTA, the cavity was closed using glass-ionomer cement. Clinical and cone beam computed tomography findings demonstrated the formation of a dentin bridge in both cases. After th

Criteria for standardizing and increasing credibility of direct pulp capping studies.

There are many skeptics who condemn pulp capping but like to keep an eye on the research progress being made. Considerable literature emphasizes the negative aspects of vital pulp therapy and discourages its practice. Some clinicians and investigators continue to condemn pulp capping therapy for the same reasons reported in the literature 80 years ago despite the advances made in pulp biology. Clinicians are well aware of the immediate and long-term success rates after root canal therapy, but are less certain of the success of pulp capping. A number of nagging questions plague clinicians, when
