PURPOSE: With pain treatment shifting away from opioids, alternative analgesics-such as gabapentinoids-are increasingly being used. This study investigated the prevalence and indications for gabapentinoid use among geriatric outpatients, a population particularly vulnerable to central nervous system (CNS)-active drugs.
METHODS: A retrospective cross-sectional chart review was conducted among patients visiting the geriatric falls clinic at Aalborg University Hospital, Denmark, in 2013-14, 2018-19 and 2023. Demographic data, comorbidities, medication status, fall frequency, in-clinic measuremen

AIMS: Systolic blood pressure (SBP) drops recorded by 24-h ambulatory blood pressure (BP) monitoring (ABPM) identify patients with susceptibility to reflex syncope and orthostatic intolerance. We tested the hypothesis that treatments aimed to increase BP (reassurance, education, and lifestyle measures plus pharmacological strategies) can reduce SBP drops.
METHODS AND RESULTS: This was a multicentre, observational proof-of-concept study performed in patients with reflex syncope and/or orthostatic intolerance and with SBP drops on a screening ABPM. Among 144 eligible patients, 111 underwent a s

CONTEXT: The management of behavioral symptoms and rigidity in patients with dementia constitutes a significant challenge. Short-term studies suggest an interest in the use of medical cannabis, but long-term data are lacking.
OBJECTIVES: The objective of this study was to investigate the feasibility and long-term safety of administering tetrahydrocannabinol/cannabidiol (THC/CBD) treatment as an additional drug to a poly medicated population with severe dementia, evaluate clinical improvements, and collect information on the pharmacokinetics of cannabinoids and possible drug-drug interactions.

OBJECTIVE: Polypharmacy and overprescription of off-label medications are common in patients with borderline personality disorder (BPD). The aim of the present naturalistic study was to explore whether the skills training module of dialectical-behavioural therapy (DBT) can reduce polypharmacy in these patients in routine clinical practice.
METHODS: Retrospective, observational study of 377 patients with a primary diagnosis of BPD consecutively admitted to the BPD outpatient unit from 2010 through 2020. All patients were invited to participate in the DBT skills training module (DBT-ST). DBT-ST

BACKGROUND: Older adults are at higher risk for cardiovascular disease and functional decline, often leading to deterioration and dependency. Cardiac rehabilitation (CR) provides opportunity to improve clinical and functional recovery, yet participation in CR decreases with age. Modified Application of CR in Older Adults (MACRO) is a National Institute on Aging (NIA)-funded pragmatic trial that responds to this gap by aiming to increase enrollment of older adults into CR and improving functional outcomes. This article describes the methodology and novel features of the MACRO trial.

OBJECTIVE: Older people approaching the end of life are at a high risk for adverse drug reactions. Approaching the end of life should change the therapeutic aims, triggering a reduction in the number of drugs. The main aim of this study is to describe the preventive and symptomatic drug treatments prescribed to patients discharged with a limited life expectancy from internal medicine and geriatric wards. The secondary aim was to describe the potentially severe drug-drug interactions (DDI).

BACKGROUND: Clinical champions are a known strategy for implementing evidence-based practices; however, their application in de-implementing potentially inappropriate medications (PIMs) among persons with dementia is underexplored. We conducted a pragmatic cluster-randomized clinical trial of a champion-led deprescribing intervention in accountable care organization (ACO) primary care settings.
OBJECTIVE: To (1) understand clinical champions' perspectives of their deprescribing projects and (2) identify key contextual factors that influenced champions' deprescribing PIMs for persons with deme

BACKGROUND: Fibromyalgia in older adults is frequently underrecognized or misdiagnosed because of overlapping symptoms with affective disorders, neurodegenerative disease, and adverse drug reactions related to polypharmacy. These factors may obscure diagnosis and contribute to persistent symptoms and functional decline.
CASE PRESENTATION: We describe an older woman who presented with chronic widespread pain, fatigue, sleep disturbance, and low mood in the context of multimorbidity and extensive pharmacotherapy. She was initially treated for depression, during which her symptoms worsened along

BACKGROUND: Benzodiazepines and z-drugs increase the fall risk in older adults. There is a lack of real-world data examining the association between falls and deprescribing medications.
OBJECTIVE: In a retrospective cohort study of older adults with chronic benzodiazepine or z-drug use receiving care at an academic health system from January 2017 to December 2020, we explored the association between medication discontinuation and falls.
METHODS: Chronic use was defined as ≥ 3 medication dispensings and cumulative days' supply ≥ 45 days within 100 days in 2018. Discontinuation was defined as

BACKGROUND: The effectiveness of psychotropic medication on behavioral and psychological symptoms of dementia (BPSD) is limited, while associated with a higher risk of adverse events. Non-pharmacological treatment of BPSD is advocated as treatment of first choice. However, many general practitioners (GPs) find it difficult to initiate deprescribing, and when attempting to discontinue psychotropic medication in nursing home residents, they face many barriers. Therefore, we hypothesize that an intervention aimed at improving communication with and involvement of nursing home staff, relatives, an

Sorafenib is a multi-kinase inhibitor approved for the treatment of renal cell and hepatocellular carcinoma. Adverse cutaneous reactions are a very common side effect of the medication. We report the development of hidradenitis suppurativa (HS) in a patient after initiation of treatment with sorafenib. HS is marked by recurrent deep painful nodules, fluctuant abscesses, and draining sinus tracts most frequently occurring in the groin and axilla. To our knowledge, sorafenib-induced HS in the axillary and inguinal skin folds has not been previously reported.

INTRODUCTION: In the DeprescrIPP-DAM trial, health insurance representatives (HIRs) conducted visits to general practitioners (GPs) to raise awareness on the need to deprescribe proton pump inhibitors (PPIs) and to present strategies for engaging in the deprescribing process. This study aimed to explore HIRs’ interactions and experiences during these interventions.
METHODS: A qualitative study was conducted using semi-structured interviews with 12 voluntary HIRs. Data were analyzed using a grounded theory approach.
RESULTS: HIRs demonstrated the ability to balance the objectives of

OBJECTIVE: To determine whether a letter-based intervention to patients taking benzodiazepines reduces their consumption.
METHOD: A before-after study without control group performed in primary care in May 2016. 1582 patients who took a single daily dose of benzodiazepines as hypnotic for more than 3 months were sent a personalized letter from their family doctor. The letter contained information about the risks of long-term benzodiazepine consumption and the recommendation to discontinue them with a gradual drop protocol (Benzoletter). The percentage of patients who stopped or decreased the

BACKGROUND: Benzodiazepines (BZDs) are commonly used in older populations despite their association with multiple adverse outcomes. However, the impact of continuing versus deprescribing BZDs during hospitalization on short-term mortality after discharge remains uncertain. This study aims to evaluate the association of BZD-use at hospital discharge with 6-month mortality and explore whether in-hospital deprescribing modifies mortality risk.
METHODS: This is a retrospective analysis of prospectively collected data from patients (aged 66-101 years) consecutively admitted to an Acute Geriatric U

Bariatric and metabolic surgery (BMS) is the most effective treatment for severe obesity and its related comorbidities. Rapid metabolic improvement following surgery frequently leads to the reduction or discontinuation of medications for obesity-related comorbidities. However, there are no established guidelines regarding timing or criteria for dose adjustment. A narrative review examined major clinical trials, meta-analyses, and society guidelines relevant to BMS. Evidence from previous and recent studies was synthesized to summarize pharmacokinetic alterations, disease-specific medication ad
